1Multidisciplinary management of nonfunctional neuroendocrine tumor of the pancreas显示文摘Pancreatic neuroendocrine tumors(PNETs) are a rare and diverse group of tumors; nonfunctional(NF) PNETs account for the majority of cases. Most patients with NF-PNETs have metastatic disease at the time of presentation. A variety of treatment modalities exist, including medical, liver directed, and surgical treatments. Aggressive surgical management is associated with prolonged survival, however available data are limited by selection bias and the frequent combination of PNETs with carcinoid tumors. 2Factor analysis identifies subgroups of constipation显示文摘AIM:To determine whether distinct symptom groupings exist in a constipated population and whether such grouping might correlate with quantifiable pathophysiological measures of colonic dysfunction.METHODS:One hundred and ninety-one patients presenting to a Gastroenterology clinic with constipation and 32 constipated patients responding to a newspaper advertisement completed a 53-item,wide-ranging selfreport questionnaire.One hundred of these patients had colonic transit measured scintigraphically.Factor analysis determined whether constipation-related symptoms grouped into distinct aspects of symptomatology.Cluster analysis was used to determine whether indi-vidual patients naturally group into distinct subtypes.RESULTS:Cluster analysis yielded a 4 cluster solution with the presence or absence of pain and laxative unresponsiveness providing the main descriptors.Amongst all clusters there was a considerable proportion of patients with demonstrable delayed colon transit,irritable bowel syndrome positive criteria and regular stool frequency.The majority of patients with these characteristics also reported regular laxative use.CONCLUSION:Factor analysis identified four constipation subgroups,based on severity and laxative unresponsiveness,in a constipated population.However,clear stratification into clinically identifiable groups remains imprecise.Philip G Dinning Mike Jones Linda Hunt Sergio E Fuentealba Jamshid Kalanter Denis W King David Z Lubowski Nicholas J Talley Ian J Cook 2011World Journal of Gastroenterology2011,17,11:3
3Using on-site liver 3-D reconstruction and volumetric calculations in split liver transplantation显示文摘BACKGROUND: Split liver transplantation increases the number of grafts available for transplantation. Pre-recovery assessment of liver graft volume is essential for selecting suitable recipients. The purpose of this study was to determine the ability and feasibility of constructing a 3-D model to aid in surgical planning and to predict graft weight prior to an in situ division of the donor liver. METHODS: Over 11 months, 3-D volumetric reconstruction of 4 deceased donors was performed using Pathfinder Scout~? liver volumetric software. Demographic, laboratory, operative, perioperative and survival data for these patients along with donor demographic data were collected prospectively and analyzed retrospectively.RESULTS: The average predicted weight of the grafts from the adult donors obtained from an in situ split procedure were 1130 g(930-1458 g) for the extended right lobe donors and 312 g(222-396 g) for left lateral segment grafts. Actual adult graft weight was 92% of the predicted weight for both the extended right grafts and the left lateral segment grafts. The predicted and actual graft weights for the pediatric donors were 176 g and 210 g for the left lateral segment grafts and 308 g and 280 g for the extended right lobe grafts, respectively. All grafts were transplanted except for the right lobe from the pediatric donors due to the small graft weight.CONCLUSIONS: On-site volumetric assessment of donors provides useful information for the planning of an in situ split and for selection of recipients. 12Cross-scale modeling of surface temperature and tree seedling establishment in mountain landscapes显示文摘Introduction:Estimating surface temperature from above-ground field measurements is important for understanding the complex landscape patterns of plant seedling survival and establishment,processes which occur at heights of only several centimeters.Currently,future climate models predict temperature at 2 m above ground,leaving ground-surface microclimate not well characterized.Methods:Using a network of field temperature sensors and climate models,a ground-surface temperature method was used to estimate microclimate variability of minimum and maximum temperature.Temperature lapse rates were derived from field temperature sensors and distributed across the landscape capturing differences in solar radiation and cold air drainages modeled at a 30-m spatial resolution.Results:The surface temperature estimation method used for this analysis successfully estimated minimum surface temperatures on north-facing,south-facing,valley,and ridgeline topographic settings,and when compared to measured temperatures yielded an R2 of 0.88,0.80,0.88,and 0.80,respectively.Maximum surface temperatures generally had slightly more spatial variability than minimum surface temperatures,resulting in R2 values of 0.86,0.77,0.72,and 0.79 for north-facing,south-facing,valley,and ridgeline topographic settings.Quasi-Poisson regressions predicting recruitment of Quercus kelloggii(black oak)seedlings from temperature variables were significantly improved using these estimates of surface temperature compared to air temperature modeled at 2 m.Conclusion:Predicting minimum and maximum ground-surface temperatures using a downscaled climate model coupled with temperature lapse rates estimated from field measurements provides a method for modeling temperature effects on plant recruitment.Such methods could be applied to improve projections of species’range shifts under climate change.Areas of complex topography can provide intricate microclimates that may allow species to redistribute locally as climate changes.John R Dingman Lynn C Sweet Ian McCullough Frank W Davis Alan Flint Janet Franklin Lorraine E Flint 2013Ecological Processes2013,2,1:1

15“Weighing the risk”: Obesity and outcomes following liver transplantation显示文摘Obesity is on the rise worldwide. As a result, unprecedented rates of patients are presenting with end stage liver disease in the setting of non-alcoholic fatty liver disease(NAFLD) and are requiring liver transplantation. There are significant concerns that the risk factors associated with obesity and the metabolic syndrome might have a detrimental effect on the long term outcomes following liver transplantation. In general, short term patient and graft outcomes for both obese and morbidly obese patients are comparable with that of non-obese patients, however, several studies report an increase in peri-operative morbidity and increased length of stay. Continued studies documenting the long-term outcomes from liver transplantation are needed to further examine the risk of recurrent disease(NAFLD) and also further define the role risk factors such cardiovascular disease might play long term.
