| 2 | Pancreatic intraepithelial neoplasia arising from an ectopic pancreas in the small bowel显示文摘BACKGROUND: Ectopic pancreatic tissue is relatively uncommon, and is characterized as pancreatic tissue with no contact with the normal pancreas, and with its own ductal system and blood supply. It is usually asymptomatic, and can be incidentally diagnosed by conventional imaging studies.METHOD: A 69-year-old woman with a prior history of bilateral breast carcinoma presented with ectopic pancreatic intraepithelial neoplasia (PanI N) that was identified incidentally in the small bowel during an oncological resection of a synchronous primary pancreatic adenocarcinoma, and renal cell carcinoma.RESULTS: The patient underwent subtotal pancreatectomy with splenectomy, regional lymphadenectomy, radical left nephrectomy, and small bowel resection with primary anastomosis of ectopic Pan IN-2. She had an uneventful hospitalization and was discharged home on postoperative day 7.CONCLUSIONS: The occurrence of ectopic PanI N is extremely unusual with only few cases previously reported in the literature. | 3 | Biliary-colonic fistula caused by cholecystectomy bile duct injury显示文摘Biliary-colonic fistula is a rare complication after laparoscopic cholecystectomy. We present a case of post-cholecystectomy iatrogenic biliary injury that resulted in a fistula between the common hepatic duct and large bowel. Magnetic resonance cholangiopancreatography provided good visualization of injury even with concurrent normal level of alkaline phosphatase. | 4 | Human enamel thickness and ENAM polymorphism显示文摘The tooth enamel development gene, enamelin(ENAM), showed evidence of positive selection during a genome-wide scan of human and primate DNA for signs of adaptive evolution. The current study examined the hypothesis that a single-nucleotide polymorphism(SNP) C14625T(rs7671281) in the ENAM gene identified in the genome-wide scan is associated with a change in enamel phenotype. African Americans were selected as the target population, as they have been reported to have a target SNP frequency of approximately 50%, whereas non-Africans are predicted to have a 96% SNP frequency. Digital radiographs and DNA samples from 244 teeth in 133 subjects were analysed, and enamel thickness was assessed in relation to SNP status,controlling for age, sex, tooth number and crown length. Crown length was found to increase with molar number, and females were found to have thicker enamel. Teeth with larger crowns also had thicker enamel, and older subjects had thinner enamel.Linear regression and generalized estimating equations were used to investigate the relationship between enamel thickness of the mandibular molars and ENAM SNP status; enamel in subjects with the derived allele was significantly thinner(P = 0.040)when the results were controlled for sex, age, tooth number and crown length. The derived allele demonstrated a recessive effect on the phenotype. The data indicate that thinner dental enamel is associated with the derived ENAM genotype. | 5 | Effects of Rhodiola rosea supplementation on mental performance,physical capacity,and oxidative stress biomarkers in healthy men显示文摘Purpose: The objective of this study was to investigate the effects of chronic Rhodiola rosea(R. rosea) supplementation on mental and physical performance, as well as hormonal and oxidative stress biomarkers.Methods: Twenty-six healthy male students received either R. rosea extract(600 mg/day; RR) or placebo(PL) in a randomized double-blind trial. Prior to supplementation(Term I) and following 4 weeks of supplementation(Term II), the students underwent psychomotor tests for simple and choice reaction time, included in the Vienna Test System. Also, the subjects performed VO2 peaktest. Blood samples were obtained before and after the test to measure the hormonal profile(cortisol, testosterone, and growth hormone), as well as the biomarkers of oxidative stress(lipid hydroperoxides, total antioxidant capacity, and superoxide dismutase) and muscle damage(creatine kinase).Results: R. rosea ingestion shortened reaction time and total response time. Moreover, a greater relative increase in the number of correct responses was observed in RR group as compared to the PL group. No changes in endurance exercise capacity and hormonal profile were observed after R. rosea ingestion. R. rosea ingestion raised plasma total antioxidant capacity. It did not, however, affect other measured parameters.Conclusion: Chronic R. rosea ingestion does not affect physical performance, but can improve the results of some psychomotor tests(simple and choice reaction time) in young, healthy, and physically active men. The improvements in mental performance, however, at least in our study,seem not to be related to changes in cortisol release or antioxidant activity of R. rosea extract. | 7 | Cecal bascule herniation into the lesser sac显示文摘Cecal bascule is a rare cause of bowel obstruction in which a mobile cecum folds anteriorly and superiorly over the ascending colon. Herein, we present the first case of internal herniation of a cecal bascule into the lesser sac through the foramen of winslow, aiming at discussing radiological findings, differential diagnosis,and surgical management of this uncommon condition.A 75-year-old female presented to the emergency room with an 18-h history of sudden onset sharp, progressively worsening abdominal pain associated with vomiting. Physical exam revealed abdominal distention and epigastric tenderness while initial laboratory tests were unremarkable. Computed tomography of her abdomen and pelvis showed a loop of distended colon within lesser sac without signs of bowel ischemia or perforation. On exploratory laparotomy, a cecal bascule was found herniating into lesser sac via foramen of winslow.Upon reduction, the cecum appeared viable therefore a cecopexy was performed without bowel resection.Unlike cecal volvulus, cecal bascule consists of no axial rotation of the bowel with no mesenteric vascular compromise and therefore ischemia would only occur from intraluminal tension or extraluminal compression from the borders of foramen of winslow.
