| 4 | Clinical trial with traditional Chinese medicine intervention ''tonifying the kidney to promote liver regeneration and repair by affecting stem cells and their microenvironment'' for chronic hepatitis B-associated liver failure显示文摘AIM:To study the clinical efficacy of traditional Chinese medicine(TCM)intervention'tonifying the kidney to promote liver regeneration and repair by affecting stem cells and their microenvironment'('TTK')for treating liver failure due to chronic hepatitis B.METHODS:We designed the study as a randomized controlled clinical trial.Registration number of Chinese Clinical Trial Registry is Chi CTR-TRC-12002961.A total of 144 patients with liver failure due to infection with chronic hepatitis B virus were enrolled in this randomized controlled clinical study.Participants were randomly assigned to the following three groups:(1)a modern medicine control group(MMC group,36patients);(2)a'tonifying qi and detoxification'('TQD')group(72 patients);and(3)a'tonifying the kidney to promote liver regeneration and repair by affecting stem cells and their microenvironment'('TTK')group(36patients).Patients in the MMC group received general internal medicine treatment;patients in the'TQD'group were given a TCM formula'tonifying qi and detoxification'and general internal medicine treatment;patients in the'TTK'group were given a TCM formula of'TTK'and general internal medicine treatment.All participants were treated for 8 wk and then followed at 48 wk following their final treatment.The primaryefficacy end point was the patient fatality rate in each group.Measurements of various virological and biochemical indicators served as secondary endpoints.The one-way analysis of variance and the t-test were used to compare patient outcomes in the different treatment groups.RESULTS:At the 48-wk post-treatment time point,the patient fatality rates in the MMC,'TQD',and'TTK'groups were 51.61%,35.38%,and 16.67%,respectively,and the differences between groups were statistically significant(P<0.05).However,there were no significant differences in the levels of hepatitis B virus DNA or prothrombin activity among the three groups(P>0.05).Patients in the'TTK'group had significantly higher levels of serum total bilirubin compared to MMC subjects(339.40μmol/L±270.09μmol/L vs 176.13μmol/L±185.70μmol/L,P=0.014).Serum albumin levels were significantly increased in both the'TQD'group and'TTK'group as compared with the MMC group(31.30 g/L±4.77g/L,30.72 g/L±2.89 g/L vs 28.57 g/L±4.56 g/L,P<0.05).There were no significant differences in levels of alanine transaminase among the three groups(P>0.05).Safety data showed that there was one case of stomachache in the'TQD'group and one case of gastrointestinal side effect in the'TTK'group.CONCLUSION:Treatment with'TTK'improved the survival rates of patients with liver failure due to chronic hepatitis B.Additionally,liver tissue was regenerated and liver function was restored. | Han-Min Li Zhi-Hua Ye Jun Zhang Xiang Gao Yan-Ming Chen Xin Yao Jian-Xun Gu Lei Zhan Yang Ji Jian-Liang Xu Ying-He Zeng Fan Yang Lin Xiao Guo-Guang Sheng Wei Xin Qi Long Qing-Jing Zhu Zhao-Hong Shi Lian-Guo Ruan Jia-Yao Yang Chang-Chun Li Hong-Bin Wu Sheng-Duo Chen Xin-La Luo | 2014 | World Journal of Gastroenterology2014,20,48: | 20 |
| 5 | The enhanced X-ray Timing and Polarimetry mission—eXTP显示文摘In this paper we present the enhanced X-ray Timing and Polarimetry mission—eXTP. eXTP is a space science mission designed to study fundamental physics under extreme conditions of density, gravity and magnetism. The mission aims at determining the equation of state of matter at supra-nuclear density, measuring effects of QED, and understanding the dynamics of matter in strong-field gravity. In addition to investigating fundamental physics, eXTP will be a very powerful observatory for astrophysics that will provide observations of unprecedented quality on a variety of galactic and extragalactic objects. | 7 | Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. | 8 | Behavioral Abnormality along with NMDAR-related CREB Suppression in Rat Hippocampus after Shortwave Exposure显示文摘Objective To estimate the detrimental effects of shortwave exposure on rat hippocampal structure and function and explore the underlying mechanisms. Methods One hundred Wistar rats were randomly divided into four groups(25 rats per group) and exposed to 27 MHz continuous shortwave at a power density of 5, 10, or 30 m W/cm^2 for 6 min once only or underwent sham exposure for the control. The spatial learning and memory, electroencephalogram(EEG), hippocampal structure and Nissl bodies were analysed. Furthermore, the expressions of N-methyl-D-aspartate receptor(NMDAR) subunits(NR1, NR2 A, and NR2 B), c AMP responsive element-binding protein(CREB) and phosphorylated CREB(p-CREB) in hippocampal tissue were analysed on 1, 7, and 14 days after exposure. Results The rats in the 10 and 30 m W/cm^2 groups had poor learning and memory, disrupted EEG oscillations, and injured hippocampal structures, including hippocampal neurons degeneration, mitochondria cavitation and blood capillaries swelling. The Nissl body content was also reduced in the exposure groups. Moreover, the hippocampal tissue in the 30 m W/cm^2 group had increased expressions of NR2 A and NR2 B and decreased levels of CREB and p-CREB. Conclusion Shortwave exposure(27 MHz, with an average power density of 10 and 30 m W/cm^2) impaired rats' spatial learning and memory and caused a series of dose-dependent pathophysiological changes. | 10 | A combination of follicle stimulating hormone, estradiol and age is associated with the pregnancy outcome for women undergoing assisted reproduction: a retrospective cohort analysis显示文摘To investigate the effect of basal hormone levels including follicle stimulating hormone(FSH) and estradiol(E2) and age on the pregnancy outcomes in women undergoing in vitro fertilization and embryo transfer(IVF-ET), we designed a retrospective cohort analysis and collected information of patients undergoing IVF between January 2014 and April 2017. We found that young women had much better pregnancy outcomes than older women regardless of the hormone levels. The pregnancy outcomes declined in young women with high basal E2, even though they had normal FSH. Older women with higher FSH had a worse outcome of pregnancy, especially with increased E2 level. The results suggest that a combination of FSH, E2 and age could effectively predict the pregnancy outcome for women undergoing IVF-ET, and we should encourage infertile women with diminished ovarian reserve, especially young infertile women, to try the assisted reproduction as early as possible. | 11 | A novel satellite-equipped receiver for autonomous monitoring of GNSS navigation signal quality显示文摘Global navigation satellite system(GNSS) comes with potential unavoidable application risks such as the sudden distortion or failure of navigation signals because its satellites are generally operated until failure. In order to solve the problems associated with these risks, receiver autonomous integrity monitoring(RAIM) and ground-based signal quality monitoring stations are widely used. Although these technologies can protect the user from the risks, they are expensive and have limited region coverage. Autonomous monitoring of satellite signal quality is an effective method to eliminate these shortcomings of the RAIM and ground-based signal quality monitoring stations; thus, a new navigation signal quality monitoring receiver which can be equipped on the satellite platform of GNSS is proposed in this paper. Because this satellite-equipped receiver is tightly coupled with navigation payload, the system architecture and its preliminary design procedure are first introduced. In theory, code-tracking loop is able to provide accurate time delay estimation of received signals. However, because of the nonlinear characteristics of the navigation payload, the traditional code-tracking loop introduces errors. To eliminate these errors, the dummy massive parallel correlators(DMPC) technique is proposed. This technique can reconstruct the cross correlation function of a navigation signal with a high code phase resolution. Combining the DMPC and direct radio frequency(RF) sampling technology, the satellite-equipped receiver can calibrate the differential code bias(DCB) accurately. In the meantime, the abnormities and failures of navigation signal can also be monitored. Finally, the accuracy of DCB calibration and the performance of fault monitoring have been verified by practical test data and numerical simulation data, respectively. | 13 | Characteristics of Permafrost along Highway G214 in the Eastern Qinghai-Tibet Plateau显示文摘The characteristics of the permafrost along National Highway No. 214(G214) in Qinghai province(between kilometer markers K310 and K670),including the distribution patterns of permafrost and seasonally frozen ground(SFG), ground ice content and mean annual ground temperature(MAGT), were analyzed using a large quantity of drilling and measured ground temperature data. Three topographic units can be distinguished along the highway: the northern mountains, including Ela Mountain and Longstone Mountain; the medial alluvial plain and the southern Bayan Har Mountains.The horizontal distribution patterns of permafrost can be divided into four sections, from north to south: the northern continuous permafrost zone(K310-K460),the island permafrost zone(K460-K560), the southern continuous permafrost zone(K560-K630),and the discontinuous permafrost zone(K630-K670).Vertically, the permafrost lower limits(PLLs) of the discontinuous zone were 4200/4325 m, 4230/4350 m,and 4350/4450 m on the north-facing/south-facing slopes of Ela Mountain, Longstone Mountain and Bayan Har Mountains, respectively. The permafrost was generally warm, with MAGTs between-1.0°C and0°C in the northern continuous permafrost zone,approximately-0.5°C in the island permafrost zone,between-1.5°C and 0°C in the southern continuous permafrost zone, and higher than-0.5°C in the discontinuous permafrost zone. | 16 | Role of MR in the differentiation of IgG4-related from non-IgG4-related hepatic inflammatory pseudotumor显示文摘BACKGROUND:Hepatic inflammatory pseudotumor(IPT)is classified into 2 types based on IgG 4 stain:IgG 4-related and non-IgG4-related; the two types differ not only in their pathological characteristics, but also in the clinical features. This study aimed to investigate the MR character of hepatic IPT,and differentiate the IgG4-related IPT from the non-IgG4-related IPT.METHODS:Twenty-five patients with 27 histologically proven hepatic IPTs were retrospectively analyzed. Ten lesions were diagnosed as IgG4-related IPT, and the other 17 as non-IgG4-related IPT. The MR signal features on T1,T2-weighted, dynamic-enhanced, and diffusion-weighted imaging were evaluated and compared. RESULTS:The dominant lesions were subcapsularly distributed(n=17, 63.0%) with clear boundary(n=20, 74.1%), and showed progressive enhancement pattern(n=21, 77.8%) with diffuse homogeneous(n=12, 44.4%) or heterogeneous(n=8,29.6%) hyperintensity, accompanied by delayed capsule-like enhancement(n=17, 63.0%) and central nonenhanced areas(n=18, 66.7%). Morphological features(P>0.05) were not sufficient to differentiate IgG4-related IPT from non-IgG4-related IPT; the wash-out pattern was only found in 2 IgG 4-related IPT, while the progressive enhancement pattern was more common in the non-IgG4-related lesions(n=16)(P=0.022).During portal and delayed phases, iso-/hypoenhanced lesions were only seen in 3 IgG4-related IPT, and circular-enhanced lesions(n=5) existed exceptionally in the non-IgG4-related group with significant differences(P=0.029 and 0.027). Most IgG4-related IPTs had lower apparent diffusion coefficient compared with the liver parenchyma(n=6), while most non-IgG4-related IPTs had higher apparent diffusion coefficient value(n=13)(P=0.046).CONCLUSIONS:Although MR images of hepatic IPT have certain characteristics, they are not enough to differentiate IgG4-related IPT from non-IgG4-related IPT. | 17 | Molecular Characteristics of Neisseria meningitidis Isolated during an Outbreak in a Jail: Association with the Spread and Distribution of ST-4821 Complex Serogroup C Clone in China显示文摘Objective To characterize the meningococcal strains isolated from cases and close contacts with meningococcal disease associated with an outbreak in a jail in May 2010 by investigating the national distribution of hyperinvasive ST-4821 serogroup C clone associated with this outbreak. Methods The cases were described based on the clinical symptoms and laboratory results. Pharyngeal swabs were cultured for N. meningitidis from men in the jail. Meningococcal isolates were identified by serogrouping, pulsed-field gel electrophoresis (PFGE) and multilocus sequence typing (MLST), respectively. Four hundred and sixteen serogroup C N. meningitidis strains were collected from 27 provinces between 2003 and 2010 for a nationwide survey and analyzed by PFGE and MLST. Results Three persons in a jail system were infected with invasive N. meningitidis serogroup C. All isolates tested had matching PFGE patterns and belonged to the multilocus sequence type (ST) 4821 clonal complex. All 47 N. meningitidis strains were identified from the pharyngeal swabs of 166 peoples in the jail, and 26 of them belonged to ST-4821 serogroup C clone, and 90.14% (375/416) serogroup C strains identified in the nationwide survey belonged to the ST-4821 complex. The ST-4821 serogroup C clone was spread nationwide, distributed in 24 provinces, especially in eastern provinces between 2003 and 2010. Conclusion Endemic transmission and carriage rate of ST-4821 serogroup C clone are high in this jail system. | 18 | Exposure Effects of Terahertz Waves on Primary Neurons and Neuron-like Cells Under Nonthermal Conditions显示文摘Objective This study aimed to explore the potential effects of terahertz(THz) waves on primary cultured neurons from 4 rat brain regions(hippocampus, cerebral cortex, cerebellum, and brainstem)and 3 kinds of neuron-like cells(MN9 D, PC12, and HT22 cells) under nonthermal conditions.Methods THz waves with an output power of 50(0.16 THz) and 10(0.17 THz) m W with exposure times of 6 and 60 min were used in this study. Analysis of temperature change, neurite growth, cell membrane roughness, micromorphology, neurotransmitters and synaptic-related proteins(SYN and PSD95) was used to evaluate the potential effects.Results Temperature increase caused by the THz wave was negligible. THz waves induced significant neurotransmitter changes in primary hippocampal, cerebellar, and brainstem neurons and in MN9 D and PC12 cells. THz wave downregulated SYN expression in primary hippocampal neurons and downregulated PSD95 expression in primary cortical neurons.Conclusion Different types of cells responded differently after THz wave exposure, and primary hippocampal and cortical neurons and MN9 D cells were relatively sensitive to the THz waves. | 20 | Recombinant scorpion insectotoxin AaIT kills specifically insect cells but not human cellss显示文摘The nucleotide sequence deduced from the amino acid sequence of the scorpion insectotoxin AaIT was chemically synthesized and was expressed in Escherichia coli. The authenticity of this in vitro expressed peptide was confirmed by N-terminal peptide sequencing. Two groups of bioassays, artificial diet incorporation assay and contact insecticidal effect assay, were carried out separately to verify the toxicity of this recombinant toxin. At the end of a 24 h experimental period, more than 60% of the testing diamondback moth (Plutella xylostella) larvae were killed in both groups with LCs0 value of 18.4 uM and 0.70 μM respectively. Cytotoxicity assay using cultured Sf9 insect cells and MCF-7 human cells demonstrated that the toxin AaIT had specific toxicity against insect cells but not human cells. Only 0.13 μM recombinant toxin was needed to kill 50% of cultured insect cells while as much as 1.3μM toxin had absolutely no effect on human cells. Insect cells produced obvious intrusions from their plasma membrane before broken up.
