4生物可吸收依维莫司洗脱冠状动脉支架系统(ABSORB):2年临床结局和多种影像检查结果显示文摘背景药物洗脱金属冠状动脉支架易发生晚期支架血栓形成,阻止晚期血管腔扩大,妨碍外科血运重建,并且会影响多层CT的成像结果。作者评估了生物可吸收依维莫司洗脱支架(BVS)的安全性。方法对30例冠状动脉单个原发病变的患者进行为期2年的临床随访和多种成像方法随访,其中包括多层CT、血管造影、血管内超声、衍生的形态学指标(虚拟组织学、震动学参数和回声强度)以及光学相干断层成像(OCT)。结果2,L30例患者中获取了29例的临床资料。2年时,置入装置安全,未记录到心源性死亡、缺血驱动的靶病变血运重建或支架血栓形成,仅发生l例心肌梗死(非Q波性)。18个月时多层CT检查(共检查25例患者)显示平均直径狭窄为19%(S=9%)。2年时血管造影显示,支架内晚期丢失为0.48mm(s=0.28mm),直径狭窄为27%(s=11%),与6个月时的结果没有差别。OCT和血管内超声显示2年时管腔较6个月时扩张的原因是斑块体积下降,而血管大小并未改变。术后2年时.34.5%的支架置入部位用OCT已难以查见支架特征,证实回声强度和射频反向散射减少。尚存可见的支架放置良好,保持通畅。此外曾经的支架置入部位及其邻近的冠状动脉因响应血管活性药物而出现血管舒缩运动。结论置入2年后支架被生物吸收,重新出现血管运动,防止了再狭窄的发生,而且在临床上安全,提示未发生晚期支架血栓形成。晚期管腔扩大是否由斑块回缩造成(而无血管重构参与)尚需进一步证实。Patrick W Serruys John A Ormiston Yoshinobu Onuma Evelyn Regar Nieves Gonzalo Hector M Garcia-Garcia Koen Nieman Nico Bruining Cecile Dorange Karine Miquel-Hebert Susan Veldhof Mark Webster Leif Thuesen Dariusz Dudek 刘少伟(译) 2009世界临床医学2009,,9:1
