| 1 | Effects of oral tacrolimus as a rapid induction therapy in ulcerative colitis显示文摘AIM:To determine the efficacy and safety of rapid induction therapy with oral tacrolimus without a meal in steroid-refractory ulcerative colitis(UC)patients.METHODS:This was a prospective,multicenter,observational study.Between May 2010 and August 2012,49 steroid-refractory UC patients(55 flare-ups)were consecutively enrolled.All patients were treated with oral tacrolimus without a meal at an initial dose of 0.1mg/kg per day.The dose was adjusted to maintain trough whole-blood levels of 10-15 ng/m L for the first 2 wk.Induction of remission at 2 and 4 wk after tacrolimus treatment initiation was evaluated using Lichtiger’s clinical activity index(CAI).RESULTS:The mean CAI was 12.6±3.6 at onset.Within the first 7 d,93.5%of patients maintained high trough levels(10-15 ng/m L).The CAI significantly decreased beginning 2 d after treatment initiation.At 2wk,73.1%of patients experienced clinical responses.After tacrolimus initiation,31.4%and 75.6%of patients achieved clinical remission at 2 and 4 wk,respectively.Treatment was well tolerated.CONCLUSION:Rapid induction therapy with oral tacrolimus shortened the time to achievement of appropriate trough levels and demonstrated a high remission rate 28 d after treatment initiation.Rapid induction therapy with oral tacrolimus appears to be a useful therapy for the treatment of refractory UC. | Ken Kawakami Takuya Inoue Mitsuyuki Murano Ken Narabayashi Sadaharu Nouda Kumi Ishida Yosuke Abe Koji Nogami Nobuyuki Hida Hirokazu Yamagami Kenji Watanabe Eiji Umegaki Shiro Nakamura Tetsuo Arakawa Kazuhide Higuchi | 2015 | World Journal of Gastroenterology2015,21,6: | 3 |

| 12 | Association between ADAMTS13 activity-VWF antigen imbalance and the therapeutic effect of HAIC in patients with hepatocellular carcinoma显示文摘BACKGROUND Prediction of HAIC treatment response is important for improving the prognosis in patients with hepatocellular carcinoma(HCC).The progression of HCC is related to hypercoagulability and angiogenesis.It is known that ADAMTS13 and von Willebrand factor(VWF)are related to hypercoagulability.In addition,previous study reported that the association between ADAMTS13 and VWF,and angiogenesis via vascular endothelial growth factor(VEGF).Recently,ADAMTS13 and VWF have been associated with the prognosis in patients with various kinds of cancer undergoing chemotherapy.AIM To investigate whether ADAMTS13 and VWF become useful biomarkers of treatment response in HCC patients before the initiation of HAIC treatment.METHODS Seventy-two patients were enrolled in this study.ADAMTS13 activity(ADAMTS13:AC),VWF antigen(VWF:Ag)and VEGF levels were determined via enzyme-linked immunosorbent assay.Univariable and multivariable analyses were performed to determine the predictive factors of treatment response in patients with HCC undergoing HAIC treatment.RESULTS ADAMTS13:AC levels in HCC patients with stable disease(SD)+partial response(PR)of HAIC treatment were significantly higher than those with progressive disease(PD)(P<0.05).In contrast,VWF:Ag/ADAMTS13:AC ratio and VEGF levels in HCC patients with SD+PR were significantly lower than those with PD(both P<0.05).Patients with high VWF:Ag/ADAMTS13:AC ratio(>2.7)had higher VEGF levels than those with low ratio(≤2.7).Multivariable analysis revealed that VWF:Ag/ADAMTS13:AC ratio was a predictive factor of HAIC treatment response.CONCLUSION VWF:Ag/ADAMTS13:AC ratio may become a useful biomarker of treatment response in HCC patients before the initiation of HAIC treatment. | Hiroaki Takaya Tadashi Namisaki Kei Moriya Naotaka Shimozato Kosuke Kaji Hiroyuki Ogawa Koji Ishida Yuki Tsuji Daisuke Kaya Hirotestu Takagi Yukihisa Fujinaga Norihisa Nishimura Yasuhiko Sawada Hideto Kawaratani Takemi Akahane Masanori Matsumoto Hitoshi Yoshiji | 2020 | World Journal of Gastroenterology2020,26,45: | 0 |
| 13 | Evaluation Method of the Gait Motion Based on Self-organizing Map Using the Gravity Center Fluctuation on the Sole显示文摘This paper describes the evaluation method of the gait motion in walk rehabilitation.We assume that the evaluation consists of the classification of the measured data and the prediction of the feature of the gait motion.The method may enable a doctor and a physical therapist to recognize the condition of the patients more easily,and increase the motivation of patient further for rehabilitation.However,it is difficult to divide the gait motion into discrete categories,since the gait motion continuously changes and does not have the clear boundaries.Therefore,the self-organizing map(SOM) that is able to arrange the continuous data on the almost continuous map is employed in order to classify them.And,the feature of the gait motion is predicted by the classification.In this study,we adopt the gravity-center fluctuation(GCF) on the sole as the measured data.First,it is shown that the pattern of the GCF that is obtained by our developed measurement system includes the feature of the gait motion.Secondly,the relation between the pattern of the GCF and the feature of the gait motion that the doctor and the physical therapist evaluate by visual inspection is considered using the SOM.Next,we describe the prediction of following features measured by numerical values: the length of stride,the velocity of walk and the difference of steps that are important for the doctor and the physical therapist to make a diagnosis of the condition of the gait motion in walk rehabilitation.Finally,it is investigated that the position of a new test data that is arranged on the map accords with the prediction.As a consequence,we confirm that the method using the SOM is often useful to classify and predict the condition of the patient. | Koji Makino Masahiro Nakamura Hidenori Omori Yoshinobu Hanagata Shohei Ueda Kyosuke Nakagawa Kazuyoshi Ishida Hidetsugu Terada | 2017 | International Journal of Automation and computing2017,14,5: | 0 |
