| 1 | Risk factors for complications associated with upper gastrointestinal foreign bodies显示文摘AIM: To investigate predictive risk factors associated with complications in the endoscopic removal of foreign bodies from the upper gastrointestinal tract.METHODS: We retrospectively reviewed the medical records of 194 patients with a diagnosis of foreign body impaction in the upper gastrointestinal tract,confirmed by endoscopy,at two university hospital in South Korea.Patient demographic data,including age,gender,intention to ingestion,symptoms at admission,and comorbidities,were collected.Clinical features of the foreign bodies,such as type,size,sharpness of edges,number,and location,were analyzed.Endoscopic data those were analyzed included duration of foreign body impaction,duration of endoscopic performance,endoscopic device,days of hospitalization,complication rate,30-d mortality rate,and the number of operationsrelated to foreign body removal.RESULTS: The types of upper gastrointestinal foreign bodies included fish bones,drugs,shells,meat,metal,and animal bones.The locations of impacted foreign bodies were the upper esophagus(57.2%),mid esophagus(28.4%),stomach(10.8%),and lower esophagus(3.6%).The median size of the foreign bodies was 26.2 ± 16.7 mm.Among 194 patients,endoscopic removal was achieved in 189,and complications developed in 51 patients(26.9%).Significant complications associated with foreign body impaction and removal included deep lacerations with minor bleeding(n = 31,16%),ulcer(n = 11,5.7%),perforation(n = 3,1.5%),and abscess(n = 1,0.5%).Four patients underwent operations because of incomplete endoscopic foreign body extraction.In multivariate analyses,risk factors for endoscopic complications and failure were sharpness(HR = 2.48,95%CI: 1.07-5.72; P = 0.034) and a greater than 12-h duration of impaction(HR = 2.42,95%CI: 1.12-5.25,P = 0.025).CONCLUSION: In cases of longer than 12 h since foreign body ingestion or sharp-pointed objects,rapid endoscopic intervention should be provided in patients with ingested foreign bodies. | Kyong Hee Hong Yoon Jae Kim Jae Hak Kim Song Wook Chun Hee Man Kim Jae Hee Cho | 2015 | World Journal of Gastroenterology2015,21,26: | 35 |
| 2 | Pseudolymphoma (reactive lymphoid hyperplasia) of the liver:A clinical challenge显示文摘Reactive lymphoid hyperplasia(RLH), also known as pseudolymphoma or nodular lymphoid lesion of the liver is an extremely rare condition, and only 51 hepatic RLH cases have been described in the literature since the first case was described in 1981. The majority of these cases were asymptomatic and incidentally found through radiological imaging. The precise etiology of hepatic RLH is still unknown, but relative high prevalence of autoimmune disorder in these cases suggests an immune-based liver disorder. Imaging features of hepatic RLH often suggest malignant lesions such as hepatocellular carcinoma and cholangiocarcinoma. In this report, we discuss two cases of hepatic RLH in patients with autoimmune hepatitis. We also present pathologic and magnetic resonance imaging findings, including one case utilizing a hepatocellular contrast agent, Eovist. | 4 | Paeonol attenuates inflammation-mediated neurotoxicity and microglial activation显示文摘Chronic activation of microglial cells endangers neuronal survival through the release of various proinflammatory and neurotoxic factors. The root of Paeonia lactiflora Pall has been considered useful for the treatment of various disorders in traditional oriental medicine. Paeonol, found in the root of Paeonia lactiflora Pall, has a wide range of pharmacological functions, including anti-oxidative, anti-inflammatory and neuroprotective activities. The objective of this study was to examine the efficacy of paeonol in the repression of inflammation-induced neurotoxicity and microglial cell activation. Organotypic hippocampal slice cultures and primary microglial cells from rat brain were stimulated with bacterial lipopolysaccharide. Paeonol pretreatment was performed for 30 minutes prior to lipopolysaccharide addition. Cell viability and nitrite (the production of nitric oxide), tumor necrosis factor-alpha and interleukin-1beta products were measured after lipopolysaccharide treatment. In organotypic hippocampal slice cultures, paeonol blocked lipopolysaccharide-related hippocampal cell death and inhibited the release of nitrite and interleukin-1beta. Paeonol was effective in inhibiting nitric oxide release from primary microglial cells. It also reduced the lipopolysaccharide-stimulated release of tumor necrosis factor-alpha and interleukin-1β from microglial cells. | 6 | Improvement of diabetes and hypertension after gastrectomy: A nationwide cohort study显示文摘AIM: To evaluate the effect of gastrectomy on diabetes mellitus(DM) and hypertension(HTN) in non-obese gastric cancer patients. METHODS: A total of 100000 patients, diagnosed with either type 2 DM or HTN, were randomly selected from the 2004 Korean National Health Insurance System claims. Among them, 360 diabetes and 351 hypertensive patients with gastric cancer who had been regularly treated without chemotherapy from January 2005 to December 2010 were selected. They were divided into three groups according to their treatment methods: total gastrectomy(TG), subtotal gastrectomy(STG) and endoscopic resection(ER). RESULTS: The drug discontinuation rate of antidiabetic and anti-hypertensive agents after gastric cancer treatment was 9.7% and 11.1% respectively. DM appeared to be improved more frequently(22.8%) and earlier(mean ± SE 28.6 ± 1.8 mo) in TG group than in the two other groups [improved in 9.5% of ER group(37.4 ± 1.1 mo) and 6.4% of STG group(47.0 ± 0.8 mo)]. The proportion of patients treated with multiple drugs decreased more notably in TG group compared to others(P = 0.001 in DM, and P = 0.035 in HTN). | 7 | Electrolyte changes after bowel preparation for colonoscopy:A randomized controlled multicenter trial显示文摘AIM:To investigate the electrolyte changes between 2-L polyethylene glycol with ascorbic acid 20 g(PEGAsc) and 4-L PEG solutions.METHODS:From August 2012 to February 2013,a total of 226 patients were enrolled at four tertiary hospitals.All patients were randomly allocated to a PEG-Asc group or a 4-L PEG.Before colonoscopy,patients completed a questionnaire to assess bowel preparation-related symptoms,satisfaction,and willingness.Endoscopists assessed the bowel preparation using the Boston Bowel Preparation Scale(BBPS).In addition,blood tests,including serum electrolytes,serum osmolarity,and urine osmolarity were evaluated both before and after the procedure.RESULTS:A total of 226 patients were analyzed.BBPS scores were similar and the adequate bowel preparation rate(BBPS≥6)was not different between the two groups(PEG-Asc vs 4-L PEG,73.2%vs 76.3%,P=0.760).Bowel preparation-related symptoms also were not different between the two groups.The taste of PEG-Asc was better(41.1%vs 16.7%,P<0.001),and the willingness to undergo repeated bowel preparation was higher in the PEG-Asc group(73.2%vs 59.3%,P=0.027)than in 4-L PEG.There were no significant changes in serum electrolytes in either group.CONCLUSION:In this multicenter trial,bowel preparation with PEG-Asc was better than 4-L PEG in terms of patient satisfaction,with similar degrees of bowel preparation and electrolyte changes. | Kyong Joo Lee Hong Jun Park Hyun-Soo Kim Kwang Ho Baik Yeon Soo Kim Sung Chul Park Hyun Il Seo | 2015 | World Journal of Gastroenterology2015,21,10: | 3 |
| 8 | Berberine inhibits inflammatory activation of rat brain microglia显示文摘Chronic activation of microglial cells endangers neuronal survival through the release of various proinflammatory and neurotoxic factors.Berberine,the effective ingredient of Coptidis Rhizoma and Cortex Phellodendri,has a wide range of pharmacological functions,including anti-inflammatory,anti-atherosclerotic and anti-cancer effects.The neuroprotective potential of berberine has previously been demonstrated.The present study aimed to examine whether berberine could repress microglial activation and can be considered a potential therapeutic candidate to target neurodegenerative diseases.Primary microglial cells and BV2 microglial cells were cultured and stimulated with bacterial lipopolysaccharide (LPS).Berberine chloride was treated prior to LPS or simultaneously with LPS stimulation.Results revealed that berberine was effective at inhibiting nitric oxide release from primary microglial cells when cells were exposed to the compound prior to LPS or simultaneously with LPS.It also reduced the LPS-stimulated production of tumor necrosis factor-α,interleukin-1β,prostaglandin E2,and intracellular reactive oxygen species and nuclear factor-kappa activation.Additionally,berberine reduced nitric oxide release from microglia stimulated with interferon-γ and amyloid β.These results suggest that berberine provides neuroprotection by reducing the production of various neurotoxic molecules from activated microglia. | Kyong Nyon Nam Jae-Hong Kim Hoon-Ji Jung Jung-Mi Park Sang-Kwan Moon Young-Suk Kim Sun Yeou Kim Eunjoo H. | 9 | Fatty acid fouling of forward osmosis membrane:Effects of pH,calcium,membrane orientation,initial permeate flux and foulant composition显示文摘Octanoic acid(OA) was selected to represent fatty acids in effluent organic matter(EOM). The effects of feed solution(FS) properties, membrane orientation and initial permeate flux on OA fouling in forward osmosis(FO) were investigated. The undissociated OA formed a cake layer quickly and caused the water flux to decline significantly in the initial 0.5 hr at unadjusted p H 3.56; while the fully dissociated OA behaved as an anionic surfactant and promoted the water permeation at an elevated p H of 9.00. Moreover, except at the initial stage, the sudden decline of water flux(meaning the occurrence of severe membrane fouling) occurred in two conditions: 1.0.5 mmol/L Ca2+, active layer facing draw solution(AL-DS) and 1.5 mol/L Na Cl(DS); 2. No Ca2+,active layer-facing FS(AL-FS) and 4 mol/L Na Cl(DS). This demonstrated that cake layer compaction or pore blocking occurred only when enough foulants were absorbed into the membrane surface, and the water permeation was high enough to compact the deposit inside the porous substrate. Furthermore, bovine serum albumin(BSA) was selected as a co-foulant.The water flux of both co-foulants was between the fluxes obtained separately for the two foulants at p H 3.56, and larger than the two values at p H 9.00. | 10 | Magnetic resonance imaging of the pancreas in streptozotocin-induced diabetic rats: Gadofluorine P and Gd-DOTA显示文摘AIM: To investigate the performance of Gadofluorine P-enhanced magnetic resonance imaging(MRI) on the diagnosis of diabetes in a streptozotocin(STZ)-induced diabetic rat model.METHODS: Fischer 344 rats were treated with STZ.Rats not treated with STZ served as controls. T1-weighted MRI was performed using a 3T scanner before and after the injection of Gd-DOTA or Gadofluorine P(6 diabetic rats, 5 controls). The normalized signal intensity(SI) and the enhancement ratio(ER) of the pancreas were measured at each time point, and the values were compared between the normal and diabetic rats using the Mann-Whitney test. In addition,the values were correlated with the mean islet number.Optimal cut-off values were calculated using a positive test based on receiver operating characteristics.Intrapancreatic Gd concentration after the injection of each contrast media was measured using laser ablation-inductively coupled plasma-mass spectrometry in a separate set of rats(4 diabetic rats, 4 controls for Gadofluorine P; 2, 2 for Gd-DOTA).RESULTS: The normalized SI and ER of the pancreas using Gd-DOTA were not significantly different between diabetic rats and controls. With Gadofluorine P, the values were significantly higher in the diabetic rats than in the control rats 30 min after injection(P < 0.05). The area under the receiver operating characteristic curve that differentiated diabetic rats from the control group was greater for Gadofluorine P than for Gd-DOTA(0.967 vs 0.667, P = 0.085). An increase in normalized SI 30 min after Gadofluorine P was correlated with a decrease in the mean number of islets(r 2 = 0.510, P = 0.014). Intra-pancreatic Gd was higher in rats with Gadofluorine P injection than Gd-DOTA injection(Gadofluorine P vs Gd-DOTA, 7.37 vs 0.00, P < 0.01). | 13 | Endoscopic submucosal dissection as alternative to surgery for complicated gastric heterotopic pancreas显示文摘BACKGROUND Gastric heterotopic pancreas(GHP)is generally asymptomatic and rarely features complications such as pancreatitis,pseudocysts,gastric outlet obstruction,bleeding,obstructive jaundice,or intussusception.However,the treatment of complicated GHP is challenging and often requires surgical resection.AIM To investigate the clinical outcomes of endoscopic submucosal dissection(ESD)as alternative to surgical resection for complicated GHP.METHODS This is a single-center,retrospective study.Between January 2013 and December 2017,a total of 5 patients underwent ESD for complicated GHP at Asan Medical Center.Patients who were diagnosed with complicated GHP were treated conservatively as with general practice for acute pancreatitis.After conservative management for resolving the acute phase of pancreatitis,ESD was performed as definitive treatment for complicated GHP.ESD was performed using the conventional method under conscious sedation.The clinical features of patients and tumors,procedure-related characteristics,and long-term outcomes were investigated.RESULTS The age of the 5 patients ranged from 28-43 years.Two of the patients were males.All lesions were located in the greater curvature of the antrum.On endoscopic ultrasonography during the pain episode,all lesions were located across the muscularis mucosa,submucosa,and proper muscle layers.The median lesion size was 20[interquartile range(IQR),18-35]during the pain episode at the time of the diagnosis of complicated GHP,and 15 mm(IQR,9-33)at the time of ESD after conservative treatment.The procedure time ranged from 15-120 min.There were no procedure-related adverse events such as perforation or bleeding.The length of hospital stay after the procedure ranged from 2-4 d.All patients were symptom free during the median follow-up period of 46.0 mo(IQR,39-60).CONCLUSION ESD appears to be a feasible and effective treatment option for complicated GHP based on the favorable clinical outcomes. | Jin Hee Noh Do Hoon Kim So-Woon Kim Young Soo Park Hee Kyong Na Ji Yong Ahn Kee Wook Jung Jeong Hoon Lee Kee Don Choi Ho June Song Gin Hyug Lee Hwoon-Yong Jung | 2020 | World Journal of Clinical Cases2020,8,20: | 2 |
