| 1 | Speech and language therapy for aphasia following subacute stroke显示文摘The aim of this study was to investigate the time window, duration and intensity of optimal speech and language therapy applied to aphasic patients with subacute stroke in our hospital. The study consisted of 33 patients being hospitalized for stroke rehabilitation in our hospital with first stroke but without previous history of speech and language therapy. Sixteen sessions of impairment-based speech and language therapy were applied to the patients, 30–60 minutes per day, 2 days a week, for 8 successive weeks. Aphasia assessment in stroke patients was performed with Gülhane Aphasia Test-2 before and after treatment. Compared with before treatment, fluency of speech, listening comprehension, reading comprehension, oral motor evaluation, automatic speech, repetition and naming were improved after treatment. The pathophysiology of IBD is probably the result of the complex interaction of genetic susceptibility and environmental influences. There is a well-known risk of thrombosis in patients with IBD. We present the case of a 53-year-old man with ulcerative colitis, who spontaneously developed intracranial sinus thrombosis that was treated with low molecular weight heparin. | 3 | Serum arylesterase and paraoxonase activity in patients with chronic hepatitis显示文摘AIM: To investigate the relationship between serum paraoxonase (PON1), AST, ALT, GGT, and arylesterase (AE) activity alterations and the degree of liver damage in patients with chronic hepatitis.METHODS: We studied 34 chronic hepatitis patients and 32 control subjects, aged between 35 and 65 years,in the Department of Infection and Clinical Microbiology at the Firat University School of Medicine. Blood samples were collected from subjects between 8:00 and 10:00 a.m. following a 12-h fast. Baseline and salt-stimulated PON1 activities were measured by the hydrolysis of paraoxon. Phenyl acetate was used as the substrate and formed phenol was measured spectrophotometrically at 270 nm after the addition of a 10-fold diluted serum sample in AE activity measurements.RESULTS: The results of this investigation revealed that the levels of AE activity decreased from 132±52 to 94±36 (29%), baseline PON1 activity from 452±112 to 164±67 (64%), salt-stimulated PON1 activity from 746±394 to 294±220 (61%), HDL from 58.4±5.1 to 47.2±5.6(20%), triglyceride from 133±51.2 to 86±34.0 (35%),while a slight increase in the level of LDL (from 163±54.1 to 177.3±56.0; 9%) and significant increases in the levels of AST (from 29±9.3 to 98±44), ALP (from 57.2±13.1 to 91±38.1), ALT (from 27.9±3.32 to 89±19.1), GGT (from 24.3±2.10 to 94±48.2), total bilirubin (from 0.74±0.02 to 1.36±0.06; 84%) and direct bilirubin (from 0.18±0.01 to 0.42±0.04; 133%) were detected.However, the levels of albumin, total protein, cholesterol,and uric acid were almost the same in chronic hepatitis and the control subjects.CONCLUSION: Low PON1 and AE activity may contribute to the increased liver dysfunction in chronic hepatitis patients by reducing the ability of HDL to retard LDL oxidation and might be clinically useful for monitoring the disease of chronic hepatitis. | Suleyman Sirri Kilic Suleyman Aydin Nermin Kilic Fazilet Erman Suna Aydin Ilhami Celik | 2005 | World Journal of Gastroenterology2005,11,46: | 4 |
| 4 | Agenesis of the gallbladder: A dangerously misdiagnosed malformation显示文摘Isolated agenesis of the gallbladder is a rare anomaly,often asymptomatic. However, one patient out of four presented with right upper abdominal pain, nausea, and fatty food intolerance. The condition is frequently mistaken with an excluded or sclero-atrophic gallbladder, regardless of the imaging modality used. Consequently, AG often leads to unnecessary and potentially dangerous laparoscopic surgery as described in a few case reports over the last10 years. The aim of this study is to clarify the diagnostic and therapeutic approach of this unusual pathology. Two cases seen in our institutions were retrospectively reviewed, together with a review of the American and European literature. During laparoscopy, the absence of normal anatomical structures and the impossibility of pulling on the gallbladder to expose and dissect the triangle of Callot increases the risk of iatrogenic injury to biliary or portal structures. Depending on the experiment of the surgeon in laparoscopic procedure, this has to be taken into account to decide a conversion to laparotomy.A high index of suspicion is necessary when interpreting the radiological images. In case of doubt, a MRJ-cholangiography is mandatory. | 8 | SOX9 in biliary atresia: New insight for fibrosis progression显示文摘Background:Liver fibrosis is a hallmark determinant of morbidity in biliary atresia(BA)even in successfully operated cases.Responsible factors for this rapid progression of fibrosis are not completely defined.Aberrant expression of the transcription factor SOX9 and hepatic progenitor cells(HPCs)proliferation have roles in fibrogenesis in cholestatic disorders.However,they were not investigated sufficiently in BA.We aimed to delineate the relation of SOX9 and HPCs to fibrosis and its progression in BA.Methods:Forty-eight patients with BA who underwent an initial diagnostic liver biopsy(LB)and consequent intraoperative LB were recruited and compared to 28 cases with non-BA cholestasis that had an LB in their diagnostic workup.Liver fibrosis,tissue SOX9 and HPC expressions were studied in both BA and non-BA-cholestasis cases.Liver fibrosis,SOX9,and HPCs’dynamic changes in BA cases were assessed.Relation of fibrosis and its progression to SOX9 and HPCs in BA was assessed.Results:SOX9 and HPCs in ductular reaction(DR)form were expressed in 100%of BA and their grades increased significantly in the second biopsy.The rapidly progressive fibrosis in BA,represented by fibrosis grade of the intraoperative LB,correlated significantly to SOX9-DR and HPC-DR at the diagnostic(r=0.420,P=0.003 and r=0.405,P=0.004,respectively)and the intraoperative(r=0.460,P=0.001 and r=0.467,P=0.001,respectively)biopsy.On the other hand,fibrosis,SOX9-DR,and HPC-DR were significantly lower in non-BA cases at a comparable age(P<0.001,P=0.006,and P=0.014,respectively).Conclusions:Fibrosis in BA is rapidly progressive within a short time and is significantly correlated to SOX9 and HPCs.Assessment of targeting SOX9 and HPCs on fibrosis progression is warranted. | Hanaa Ahmed El-Araby Magdy Anwar Saber Noha Mohamed Radwan Doha Maher Taie Nermin Mohamed Adawy Ahmad Mohamed Sira | 2021 | Hepatobiliary & Pancreatic Diseases International2021,20,2: | 3 |
| 9 | Hypoglycemic and antioxidant activities of Caesalpinia ferrea Martius leaf extract in streptozotocininduced diabetic rats显示文摘Objective: To evaluate the antidiabetic and antioxidant ef ects of aqueous ethanolic extract of Caesalpinia ferrea(C. ferrea) leaf in normal and streptozotocin(STZ) induced diabetic rats.Methods: Male Sprague-Dawley rats divided into 6 groups of 6 rats each were assigned into diabetic and non-diabetic groups. Diabetes was induced in rats by single intraperitoneal administration of STZ(65 mg/kg body weight). C. ferrea extract at the doses of 250 and 500 mg/kg body weight was orally administered to both diabetic and non-diabetic animals for a period of 30 days. After completion of experimental duration serum, liver and pancreas were used for evaluating biochemical and histopathological changes.Results: Oral administration of C. ferrea leaf extract significantly reduced elevated serum glucose, α-amylase, liver function levels and signii cantly increased serum insulin, total protein and body weight as well as improved lipid proi le due to diabetes. Furthermore, the treatment resulted in a marked increase in glutathione peroxidase, superoxide dismutase, catalase and reduced glutathione, and diminished levels of lipid peroxidation in liver and pancreas of diabetic rats. Histopathological studies demonstrated the reduction in the pancreas and liver damage and coni rmed the biochemical i ndings.Conclusions: From the present study, it can be concluded that the C. ferrea leaf extract ef ectively improved hyperglycaemia while inhibiting the progression of oxidative stress in STZ-induced diabetic rats. | 10 | Insulin-resistance in paediatric age:Its magnitude and implications显示文摘Insulin resistance(IR)is insulin failure in normal plasma levels to adequately stimulate glucose uptake by the peripheral tissues.IR is becoming more common in children and adolescents than before.There is a strong association between obesity in children and adolescents,IR,and the metabolic syndrome components.IR shows marked variation among different races,crucial to understanding the possible cardiovascular risk,specifically in high-risk races or ethnic groups.Genetic causes of IR include insulin receptor mutations,mutations that stimulate autoantibody production against insulin receptors,or mutations that induce the formation of abnormal glucose transporter 4 molecules or plasma cell membrane glycoprotein-1 molecules;all induce abnormal energy pathways and end with the development of IR.The parallel increase of IR syndrome with the dramatic increase in the rate of obesity among children in the last few decades indicates the importance of environmental factors in increasing the rate of IR.Most patients with IR do not develop diabetes mellitus(DM)type-II.However,IR is a crucial risk factor to develop DM type-II in children.Diagnostic standards for IR in children are not yet established due to various causes.Direct measures of insulin sensitivity include the hyperinsulinemia euglycemic glucose clamp and the insulin-suppression test.Minimal model analysis of frequently sampled intravenous glucose tolerance test and oral glucose tolerance test provide an indirect estimate of metabolic insulin sensitivity/resistance.The main aim of the treatment of IR in children is to prevent the progression of compensated IR to decompensated IR,enhance insulin sensitivity,and treat possible complications.There are three main lines for treatment:Lifestyle and behavior modification,pharmacotherapy,and surgery.This review will discuss the magnitude,implications,diagnosis,and treatment of IR in children。 | Mohammed Al-Beltagi Adel Salah Bediwy Nermin Kamal Saeed | 2022 | World Journal of Diabetes2022,13,4: | 2 |
| 11 | Stable gastric pentadecapeptide BPC 157 in the treatment of colitis and ischemia and reperfusion in rats: New insights显示文摘AIM To provide new insights in treatment of colitis and ischemia and reperfusion in rats using stable gastric pentadecapeptide BPC 157. METHODS Medication [BPC 157,L-NAME,L-arginine(alone/combined),saline] was bath at the blood deprived colon segment. During reperfusion,medication was BPC 157 or saline. We recorded(USB microscope camera) vessel presentation through next 15 min of ischemic colitis(ICrats) or reperfusion(removed ligations)(IC + RL-rats);oxidative stress as MDA(increased(IC-and IC + RLrats)) and NO levels(decreased(IC-rats);increased(IC + RL-rats)) in colon tissue. IC + OB-rats [IC-rats had additional colon obstruction(OB)] for 3 d(IC + OBrats),then received BPC 157 bath. RESULTS Commonly,in colon segment(25 mm,2 ligations on left colic artery and vein,3 arcade vessels within ligated segment),in IC-,IC + RL-,IC + OB-rats,BPC 157(10 μg/kg) bath(1 m L/rat) increased vessel presentation,inside/outside arcade interconnections quickly reappeared,mucosal folds were preserved and the pale areas were small and markedly reduced. BPC 157 counteracted worsening effects induced by L-NAME(5 mg) and L-arginine(100 mg). MDA-and NO-levels were normal in BPC 157 treated IC-rats and IC + RLrats. In addition,on day 10,BPC 157-treated IC + OBrats presented almost completely spared mucosa with very small pale areas and no gross mucosal defects;the treated colon segment was of normal diameter,and only small adhesions were present.CONCLUSION BPC 157 is a fundamental treatment that quickly restores blood supply to the ischemically injured area and rapidly activates collaterals. | 15 | Phytochemical and biological activities of Crataegus sinaica growing in Egypt显示文摘Objective:To evaluate the cardiac activity and hepatoprotection of Crataegus sinaica (C.sinaica).Methods:All the isolated compounds were isolated by open-column liquid chromatography(CC) using sephadex LH-20 as stationary phase.Elution of the column was performed with EtOH or MeOH.The phytochemical investigation of the young stem of C.sinaica for the first time together with the leaves and flowers lead to the isolation and identification of quercetin,hyperoside,vitexin-2'-0-rhamnoside,epicatechin,procyanidin B2 and procyanidins CI.Results:Rats treated with the low and high dose of C.sinaica leaves with flowers extract showed 15%and 17%reduction in the heart rate,and reduction in the STsegment by 107%and 57%;respectively.The T-amplitude was decreased by 59%of the high dose extract.On the other hand,the young steins and leaves with flowers extracts of C.sinaica on primary culture of rat hepatocytes monolayer indicated a hepatoprotection for the total extract,ethyl acetate,butanol,and chlor of orm fractions at 100μg/mL,75μg/mL,50μg/mL,and 25μg/mL; respectively.Conclusions:The results of these chemical and biological studies suggest the use of C.sinaica growing in Egypt as a preventive drug against cardiovascular and hepatic diseases. | 17 | Safety and efficacy of Hansenula-derived PEGylated-interferon alpha-2a and ribavirin combination in chronic hepatitis C Egyptian children显示文摘AIM:To investigate the safety and efficacy of a Hansenula-derived PEGylated(polyethylene glycol)interferon(IFN)-alpha-2a(Reiferon Retard)plus ribavirin customized regimen in treatment-na?ve and previously treated(non-responders and relapsers)Egyptian children with chronic hepatitis C infection.METHODS:Forty-six children with chronic hepatitis C virus(HCV)infection were selected from three tertiary pediatric hepatology centers.Clinical and laboratory evaluations were undertaken.Quantitative polymerase chain reaction(PCR)for HCV-RNA was performed before starting treatment,and again at 4,12,24,48,72wk during treatment and 6 mo after treatment cessation.All patients were assigned to receive a weekly subcutaneous injection of PEG-IFN-alpha-2a plus daily oral ribavirin for 12 wk.Thirty-four patients were treatment-na?ve and 12 had a previous treatment trial.Patients were then divided according to PCR results into two groups.GroupⅠincluded patients who continued treatment on a weekly basis(7-d schedule),while groupⅡincluded patients who continued treatment on a 5-d schedule.Patients from either group who were PCR-negative at week 48,but had at least one PCRpositive test during therapy,were assigned to have an extended treatment course up to 72 wk.The occurrence of adverse effects was assessed during treatment and follow up.The study was registered at www.ClinicalTrials.gov(NCT02027493).RESULTS:Only 11 out of 46(23.9%)patients showed a sustained virological response(SVR),two patients were responders at the end of treatment;however,they were lost to follow up at 6 mo post treatment.Breakthrough was seen in 18(39.1%)patients,one patient(2.17%)showed relapse and 14(30.4%)were non-responders.Male gender,short duration of infection,low viral load,mild activity,and mild fibrosis were the factors related to a better response.On the other hand,patients with high viral load and absence of fibrosis failed to respond to treatment.Before treatment,liver transaminases were elevated.After commencing treatment,they were normalized in all patients at week 4 and were maintained normal in responders till the end of treatment,while they increased again significantly in non-responders(P=0.007 and 0.003 at week 24 and 72 respectively).The 5-d schedule did not affect the response rate(1/17 had SVR).Treatment duration(whether 48 wk or extended course to 72 wk)gave similar response rates(9/36 vs 2/8 respectively;P=0.49).Type of previous treatment(short acting IFN vs PEG-IFN)did not affect the response to retreatment.On the other hand,SVR was significantly higher in previous relapsers than in previous non-responders(P=0.039).Only mild reversible adverse effects were observed and children tolerated the treatment well.CONCLUSION:Reiferon Retard plus ribavirin combined therapy was safe.Our customized regimen did not influence SVR rates.Further trials on larger numbers of patients are warranted. | Suzan El Naghi Tawhida Y Abdel-Ghaffar Hanaa El-Karaksy Elham F Abdel-Aty Mona S El-Raziky Aleef A Allam Heba Helmy Hanaa A El-Araby Behairy E Behairy Mohamed A El-Guindi Hatem El-Sebaie Aisha Y Abdel-Ghaffar Nermin A Ehsan Ahmed M El-Hennawy Mostafa M Sira | 2014 | World Journal of Gastroenterology2014,20,16: | 2 |

| 19 | Potential effects of L-NAME on alcohol-induced oxidative stress显示文摘AIM: Nitric oxide (NO) is a highly reactive oxidant synthesized from L-arginine by nitric oxide synthase (NOS). NO may cause injury through the generation of potent radicals. Nwnitro-L-arginine methyl ester (L-NAME) is a non-selective inhibitor of NOS. We aimed to evaluate whether L-NAME treatment had protective effects against oxidative stress in rats intragastrically fed with ethanol during a 4 wk-period.METHODS: Thirty-six male Wistar rats were divided into 3 equal groups: group 1 (control group-isocaloric dextrose was given), group 2 (6 g/kg.d ethanol-induced group) and group 3 (both ethanol 6 g/kg.d and L-NAME 500 mg/L in drinking water-given group). Animals were sacrificed at the end of 4 wk-experimental period, and intracardiac blood and liver tissues were obtained. Biochemical measurements were performed both in plasma and in homogenized liver tissues. Alanine amino transferase (ALT), aspartate amino transferase (AST), malondialdehyde (MDA), NO, superoxide dismutase (SOD), catalase (CAT) and glutathione (GSH) levels were measured by spectrophotometry.RESULTS: ALT and AST in group 2 (62 U/L and 128 U/L,respectively) were higher than those in group 1 (24 U/L and 38 U/L) and group 3 (37 U/L and 81 U/L) (P<0.001 for both).Plasma and tissue levels of MDA in group 2 (4.66 μmol/L and 0.55 nmol/mg protein) were higher than in group 1 (2.65 μmol/L and 0.34 nmol/mg protein) and group 3 (3.43 μmol/L and 0.36 nmol/mg protein) (P<0.001 for both). Plasma and liver tissue levels of NO in group 2 (54.67 μmol/L and 586.50 nmol/mg protein) were higher than in group 1 (34.67 μmol/L and 435.33 nmol/mg protein)and group 3 (27.50 μmol/L and 412.75 nmol/mg protein )(P<0.001 for both). Plasma and liver tissue SOD activities in group 2 (15.25 U/mL and 5.38 U/ mg protein,respectively) were lower than in group 1 (20.00 U/mL and 8.13 U/mg protein) and group 3 (19.00 U/mL and 6.93 U/mg protein) (P<0.001 for both). Plasma and liver tissue CAT activities in group 2 (145 U/mL and 37 U/ mg protein,respectively) were lower than in group 1 (176 U/mL and 73 U/mg protein) and group 3 (167 U/mL and 61 U/mg protein) (P<0.001 for both).
