12018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 5Modeling of degradation kinetics of Salvianolic acid B at different temperatures and pH values显示文摘In this work,the effects of degradation time,temperature,and pH value on the degradation of Salvianolic acid B in aqueous solution were determined.Higher pH values,higher extraction temperature,and longer extraction time led to more degradation of Salvianolic acid B.Danshensu concentration increased as Salvianolic acid B degraded.A mechanism model was developed considering the degradation of Salvianolic acid E and lithospermic acid,which were two degradation products of Salvianolic acid B.The reverse reactions of Salvianolic acid B degradation were also considered.Degradation kinetic constants were calibrated.The degradation kinetics of Salvianolic acid B,lithospermic acid,and Danshensu in a Salvia miltiorrhiza extract aqueous solution were predicted using the mechanism model.The predicted concentrations agreed well with the experimental results.This model was developed using degradation data obtained from simple composition systems,but it can be applied in a complex botanical mixture with high prediction accuracy.Xingchu Gong Shichao Huang Jianyang Pan Haibin Qu 2017Chinese Journal of Chemical Engineering2017,25,1:16
6A new criterion of coal burst proneness based on the residual elastic energy index显示文摘To evaluate the coal burst proneness more precisely,a new energy criterion namely the residual elastic energy index was proposed.This study begins by performing the single-cyclic loading-unloading uniaxial compression tests with five pre-peak unloading stress levels to explore the energy storage characteristics of coal.Five types of coals from different mines were tested,and the instantaneous destruction process of the coal specimens under compression loading was recorded using a high speed camera.The results showed a linear relationship between the elastic strain energy density and input energy density,which confirms the linear energy storage law of coal.Based on this linear energy storage law,the peak elastic strain energy density of each coal specimen was obtained precisely.Subsequently,a new energy criterion of coal burst proneness was established,which was called the residual elastic energy index(defined as the difference between the peak elastic strain energy density and post peak failure energy density).Considering the destruction process and actual failure characteristics of coal specimens,the accuracy of evaluating coal burst proneness based on the residual elastic energy index was examined.The results indicated that the residual elastic energy index enables reliable and precise evaluations of the coal burst proneness.Fengqiang Gong Yunliang Wang Zhiguo Wang Junfeng Pan Song Luo 2021International Journal of Mining Science and Technology2021,31,4:16
7Outcomes of preoperative chemoradiotherapy followed by surgery in patients with unresectable locally advanced sigmoid colon cancer显示文摘Background: Complete resection of locally advanced sigmoid colon cancer(LASCC) is sometimes difficult. Patients with LASCC have a dismal prognosis and poor quality of life, which has encouraged the evaluation of alternative multimodality treatments. This prospective study aimed to assess the feasibility and efficacy of neoadjuvant chemora?diotherapy(neo CRT) followed by surgery as treatment of selected patients with unresectable LASCC.Methods: We studied the patients with unresectable LASCC who received neo CRT followed by surgery between October 2010 and December 2012. The neoadjuvant regimen consisted of external?beam radiotherapy to 50 Gy and capecitabine?based chemotherapy every 3 weeks. Surgery was scheduled 6–8 weeks after radiotherapy.Results: Twenty?one patients were included in this study. The median follow?up was 42 months(range, 17–57 months). All patients completed neo CRT and surgery. Resection with microscopically negative margins(R0 resection) was achieved in 20 patients(95.2%). Pathologic complete response was observed in 8 patients(38.1%). Multivisceral resection was necessary in only 7 patients(33.3%). Two patients(9.5%) experienced grade 2 postopera?tive complications. No patients died within 30 days after surgery. For 18 patients with pathologic M0(yp M0) disease, the cumulative probability of 3?year local recurrence?free survival, disease?free survival and overall survival was 100.0%, 88.9% and 100.0%, respectively. 8Application of next-generation sequencing technology to precision medicine in cancer: joint consensus of the Tumor Biomarker Committee of the Chinese Society of Clinical Oncology显示文摘Next-generation sequencing(NGS) technology is capable of sequencing millions or billions of DNA molecules simultaneously.Therefore, it represents a promising tool for the analysis of molecular targets for the initial diagnosis of disease, monitoring of disease progression, and identifying the mechanism of drug resistance. On behalf of the Tumor Biomarker Committee of the Chinese Society of Clinical Oncology(CSCO) and the China Actionable Genome Consortium(CAGC), the present expert group hereby proposes advisory guidelines on clinical applications of NGS technology for the analysis of cancer driver genes for precision cancer therapy. This group comprises an assembly of laboratory cancer geneticists, clinical oncologists, bioinformaticians,pathologists, and other professionals. After multiple rounds of discussions and revisions, the expert group has reached a preliminary consensus on the need of NGS in clinical diagnosis, its regulation, and compliance standards in clinical sample collection. 9Performance analysis of ghost imaging lidar in background light environment显示文摘The effect of background light on the imaging quality of three typical ghost imaging(GI) lidar systems(namely narrow pulsed GI lidar, heterodyne GI lidar, and pulse-compression GI lidar via coherent detection) is investigated. By computing the signal-to-noise ratio(SNR) of fluctuation-correlation GI, our analytical results, which are backed up by numerical simulations, demonstrate that pulse-compression GI lidar via coherent detection has the strongest capacity against background light, whereas the reconstruction quality of narrow pulsed GI lidar is the most vulnerable to background light. 10Undifferentiated liver embryonal sarcoma in adults: A report of four cases and literature review显示文摘AIM: To evaluate the undifferentiated embryonal sarcoma of liver (UESL) in adults in order to improve its diagnosis and treatment. METHODS: Four primary and one recurrent cases of UESL were clinicopathologically evaluated and immunohistochemically investigated with a panel of antibodies using the EnVision+ system. Relevant literature about UESL in adults was reviewed. RESULTS: Three males and one female were enrolled in this study. Their chief complaints were abdominal pain, weight loss, or fever. Laboratory tests, imaging and pathological features of UESL in adults were similar to those in children. Immunohistochemistry showed evidence of widely divergent differentiation into mesenchymal and epithelial phenotypes. The survival time of patients who underwent complete tumor resection followed by adjuvant transcatheter arterial chemoembolization (TACE) was significantly longer than that of those who underwent surgical treatment alone.CONCLUSION: UESL in adults may undergo pluripotential differentiation and its diagnosis should be made based on its morphological and immunohistochemical features. 11A resistin binding peptide selected by phage display inhibits 3T3-L1 preadipocyte differentiation显示文摘Background Resistin, a newly discovered cysteine-rich hormone secreted mainly by adipose tissues, has been proposed to form a biochemical link between obesity and type 2 diabetes. However, the resistin receptor has not yet been identified. This study aimed to identify resistin binding proteins/receptor. Methods Three cDNA fragments with the same 11 bp 5’ sequence were found by screening a cDNA phage display library of rat multiple tissues. As the reading frames of the same 11 bp 5’ sequence were interrupted by a TGA stop codon, plaque lift assay was consequently used to prove the readthrough phenomenon. The stop codon in the same 11 bp 5’ sequence was replaced by tryptophan, and the binding activity of the coded peptide [AWIL, which was designated as resistin binding peptide (RBP)] with resistin was identified by the confocal microscopy technique and the affinity chromatography experiment. pDual GC-resistin and pDual GC-resistin binding peptide were co-transfected into 3T3-L1 cells to confirm the function of resistin binding peptide. Results Three cDNA fragments with the same 11 bp 5’ sequence were found. The TGA stop codon in reading frames of the same 11 bp 5’ sequence was proved to be readthroughed. The binding activity of RBP with resistin was consequently identified. The expression of the resistin binding peptide in 3T3-L1 preadipocytes expressing pDual GC-resistin significantly inhibited the adipogenic differentiation. 12Fatty Acid Synthase and Hormone-sensitive Lipase Expression in Liver Are Involved in Zinc-α2-glycoprotein-induced Body Fat Loss in Obese Mice显示文摘Objective To explore the effects of zinc-α2-glycoprotein(ZAG) on body weight and body fat in high-fat-diet(HFD)-induced obesity in mice and the possible mechanism.Methods Thirty-six male mice were fed with standard food(SF)(n=9) and HFD(n=27),respectively.Five weeks later,9 mice fed with HFD were subjected to ZAG expression plasmid DNA transfection by liposome transfection method,and another 9 mice to negative control plasmid transfection.Two weeks later,serum ZAG level in the mice was assayed by Western blot,and the effects of ZAG over-expression on body weight,body fat,serum biochemical indexes,and adipose tissue of obese mice were evaluated.The mRNA expressions of fatty acid synthase(FAS) and hormone-sensitive lipase(HSL) in liver tissue were determined by reverse transcription-polymerase chain reaction.Results Serum ZAG level significantly lowered in simple HFD-fed mice in comparison to SF-fed mice(0.51±0.10 AU vs.0.75±0.07 AU,P<0.01).Further statistical analysis demonstrated that ZAG level was negatively correlated with body weight(r =-0.56,P<0.001),epididymal fat mass(r=-0.67,P<0.001),percentage of epididymal fat(r=-0.65,P<0.001),and increased weight(r=-0.57,P<0.001) in simple SF-and HFD-fed mice.ZAG over-expression in obese mice reduced body weight and the percentage of epididymal fat.Furthermore,FAS mRNA expression decreased(P<0.01) and HSL mRNA expression increased(P<0.001) in the liver in ZAG over-expressing mice.Conclusions ZAG is closely related to obesity.Serum ZAG level is inversely correlated with body weight and percentage of body fat.The action of ZAG is associated with reduced FAS expression and increased HSL expression in the liver of obese mice.Feng-ying Gong Jie-ying Deng Hui-juan Zhu Hui Pan Lin-jie Wang Hong-bo Yang 2010Chinese Medical Sciences Journal2010,25,3:12
13Tensile behavior of Ti-6Al-4V alloy fabricated by selective laser melting: effects of microstructures and as-built surface quality显示文摘Selective laser melting(SLM) is a powerful additive manufacturing(AM) technology, of which the most prominent advantage is the ability to produce components with a complex geometry. The service performances of the SLM-processed components depend on the microstructure and surface quality. In this work, the microstructures, mechanical properties, and fracture behaviors of SLM-processed Ti-6 Al-4 V alloy under machined and as-built surfaces after annealing treatments and hot isostatic pressing(HIP) were investigated. The microstructures were analyzed by optical microscope(OM), scanning electron microscope(SEM) and transmission electron microscopy(TEM). The mechanical properties were measured by tensile testing at room temperature. The results indicate that the as-deposited microstructures are characterized by columnar grains and fine brittle martensite and the asdeposited properties present high strength, low ductility and obvious anisotropy. After annealing at 800-900°C for 2-4 h and HIP at 920°C/100 MPa for 2 h, the brittle martensite could be transformed into ductile lamellar(α+β) microstructure and the static tensile properties of SLM-processed Ti-6 Al-4 V alloys in the machined condition could be comparable to that of wrought materials. Even after HIP treatment, the as-built surfaces could decrease the ductility and reduction of area of SLM-processed Ti-6 Al-4 V alloys to 9.2% and 20%, respectively. The crack initiation could occur at the columnar grain boundaries or at the as-built surfaces. 14Prognostic value of preoperative prognostic nutritional index and its associations with systemic inflammatory response markers in patients with stage Ⅲ colon cancer显示文摘Background: The prognostic nutritional index(PNI) has been widely applied for predicting survival outcomes of patients with various malignant tumors. Although a low PNI predicts poor prognosis in patients with colorectal cancer after tumor resection, the prognostic value remains unknown in patients with stage Ⅲ colon cancer undergoing curative tumor resection followed by adjuvant chemotherapy. This study aimed to investigate the prognostic value of PNI in patients with stage III colon cancer.Methods: Medical records of 274 consecutive patients with stage Ⅲ colon cancer undergoing curative tumor resection followed by adjuvant chemotherapy with oxaliplatin and capecitabine between December 2007 and December2013 were reviewed. The optimal PNI cutoff value was determined using receiver operating characteristic(ROC) curve analysis. The associations of PNI with systemic inflammatory response markers, including lymphocyte-to-monocyte ratio(LMR), neutrophil-to-lymphocyte ratio(NLR), platelet-to-lymphocyte ratio(PLR), and C-reactive protein(CRP)level, and clinicopathologic characteristics were assessed using the Chi square or Fisher's exact test. Correlation analysis was performed using Spearman's correlation coefficient. Disease-free survival(DFS) and overall survival(OS)stratified by PNI were analyzed using Kaplan-Meier method and log-rank test, and prognostic factors were identified by Cox regression analyses.Results: The preoperative PNI was positively correlated with LMR(r= 0.483, P < 0.001) and negatively correlated with NLR(r =-0.441, P < 0.001), PLR(r =-0.607, P < 0.001), and CRP level(r =-0.333, P < 0.001). A low PNI(≤49.22)was significantly associated with short OS and DFS in patients with stage ⅢC colon cancer but not in patients with stage ⅢA/ⅢB colon cancer.In addition, patients with a low PNI achieved a longer OS and DFS after being treated with6-8 cycles of adjuvant chemotherapy than did those with < 6 cycles. 15Double-balloon enteroscopy for mesenchymal tumors of small bowel:Nine years' experience显示文摘AIM:To assess the value of double-balloon enteroscopy(DBE) for the diagnosis of gastrointestinal mesenchymal tumors(GIMTs) in the small bowel and clarify their clinical and endoscopic characteristics.METHODS:A retrospective review in a total of 783 patients who underwent a DBE procedure from January 2003 to December 2011 was conducted.Data from patients with pathologically confirmed GIMTs were analyzed at a single tertiary center with nine years' experience.The primary outcomes assessed included characteristics of patients with GIMTs,indications for DBE,overall diagnostic yield of GIMTs,endoscopic morphology,positive biopsy,comparison of diagnosis with capsule endoscopy,and subsequent interventional management.RESULTS:GIMTs were identified and analyzed in 77 patients.The mean age was 47.74 ± 14.14 years(range:20-77 years),with 63.6% being males.The majority of individuals presented with gastrointestinal bleeding,accounting for 81.8%,followed by abdominal pain,accounting for 10.4%.Small bowel pathologies were found in 71 patients,the detection rate was 92.2%.The diagnostic yield of DBE for GIMTs was 88.3%.DBE was superior to capsule endoscopy in the diagnosis of GIMTs(P = 0.006;McNemar's χ2 test).Gastrointestinal stromal tumor was the most frequent and leiomyoma was the second frequent GIMT.Single and focal lesions were typical of GIMTs,and masses with smooth or unsmooth surface were the most common in the small bowel.GIMTs were removed from all the patients surgically except one patient treated with endoscopic resection.CONCLUSION:DBE is a safe and valuable procedure for patients with suspected GIMTs,and it provides an accurate position for subsequent surgical intervention.Qiong He Yang Bai Fa-Chao Zhi Wei Gong Hong-Xiang Gu Zhi-Min Xu Jian-Qun Cai De-Shou Pan Bo Jiang 2013World Journal of Gastroenterology2013,19,11:11
16Effects of recombinant human canstatin protein in the treatment of pancreatic cancer显示文摘AIM: To examine the effect of canstatin, a newly discovered endogenous inhibitor of angiogenesis, in the treatment of pancreatic cancer in vivo. METHODS: The canstatin cDNA fragment was synthesized and amplified from the total RNA extracted from human placenta tissues by RT-PCR. The resulting product was firstly cloned into pUCm-T vector, then into plasmid pET-22b (+) and transformed into E. coli BL21. Isopropyl-1-thio-b-Dgalactopyran-oside (IPTG) was used to induce the expression of canstatin protein and affinity chromatography was used to purify the protein. To determine the activity of purified recombinant human canstatin (rhCanstatin), orthotopic xenograft human pancreatic cancer models were established. Human pancreatic cancer cells (SW1990) were injected into the pancreas of BALB/c nude mice. Twenty-four nude mice with orthotopic xenograft tumor were randomly divided into 3 groups 10 d after the inoculation, and were treated with PBS 0.3 mL, or canstatin 5 mg/kg, or 10 mg/kg per day for 3 wk intraperitoneally. When the experiment was over, all tumors were resected and the effects of rhCanstatin on tumor growth, microvessel density (MVD) were analyzed. RESULTS: After IPTG induction, SDS-PAGE showed a new monomeric 24 kDa protein band. This protein was purified through affinity chromatography and refolded through dialysis with a final concentration of 60 mg/L. In orthotopic pancreatic cancer models, the final tumor volume in groups treated with PBS, canstatin 5 mg/ kg, 10 mg/kg were 355.21 ± 39.54 mm3, 112.73 ±10.47 mm3, and 61.75 ± 6.99 mm3 respectively. The immunohistochemical examination showed that the MVD in tumors treated with canstatin was significantly less than that in other group. 17Neoadjuvant oxaliplatin and capecitabine combined with bevacizumab plus radiotherapy for locally advanced rectal cancer: results of a single-institute phase II study显示文摘Background:Neoadjuvant chemoradiotherapy followed by surgery is recommended as the standard of care for locally advanced rectal cancer,reducing local recurrence but not distant metastasis.Intensified systemic therapy is warranted to reduce the risk of distant metastasis.The present study aimed to evaluate the safety and efficacy of neo-adjuvant oxaliplatin and capecitabine(XELOX)combined with bevacizumab plus radiotherapy for locally advanced rectal cancer.Methods:Patients with stages II to III rectal cancer received one cycle of induction chemotherapy and concurrent chemoradiotherapy with XELOX plus bevacizumab.Surgery was performed 6-8 weeks after completion of radiotherapy,and postoperative chemotherapy with three cycles of XELOX and two cycles of capecitabine were given.The primary endpoints were pathologic complete response(pCR)rate and safety,and the secondary endpoints were 3-year overall survival and progression-free survival.Results:Forty-five patients were enrolled between February 2013 and April 2015.All completed the neoadjuvant therapy.Seven patients(15.6%)refused subsequent surgical therapy for personal reasons,and the other 38 patients received radical resection,with a sphincter preservation rate of 84.2%and a pCR rate of 39.5%.Toxicity was acceptable,with grades 3-4 hematological toxicity and diarrhea observed in six and two patients,respectively.Incidence of anastomotic leak that required surgical intervention was 13.3%.After a median follow-up period of 37 months,five patients developed disease progression and two died of cancer.The 3-year overall survival rate and 3-year progres-sion-free survival rate were 95.3%and 88.6%,respectively.Conclusions:The addition of bevacizumab to neoadjuvant chemoradiotherapy resulted in a satisfying pCR rate and 3-year survival,but also may increase the risk of anastomotic leak,thus this regimen is not suitable to be considered for regular recommendation for locally advanced rectal cancer.Xin Yu Qiao-xuan Wang Wei-wei Xiao Hui Chang Zhi-fan Zeng Zhen-hai Lu Xiao-jun Wu Gong Chen Zhi-zhong Pan De-sen Wan Pei-rong Ding Yuan-hong Gao 2018Cancer Communications2018,38,1:10
18Effect of thermal cycling on the mechanical properties of Zr_(41)Ti_(14)Cu_(12.5)Ni_(10)Be_(22.5) alloy显示文摘The effect of thermal cycling treatment on mechanical properties and thermal stability of a Zr41Ti14Cu12.5Ni10Be22.5 bulk metallic glass is investigated.The metallic glassy samples are sealed into quartz tubes under high vacuum condition,and liquid nitrogen together with electric furnace are used to control a periodical temperature variation between 196℃ and 150℃.The structure and properties of the tested samples for different thermal cycles have been examined by X-ray diffraction analysis,mechanical properties measurement and thermal analysis.It has been found that the structure and properties of the samples do not show a significant change even after 200 cycles,which suggests Zr41Ti14Cu12.5Ni10Be22.5 alloy as having potential in aerospace environment.WANG Xin SHAO Yang GONG Pan YAO KeFu 2012Science China(Physics,Mechanics & Astronomy)2012,55,12:10
19Enhanced aromatic selectivity by the sheet-like ZSM-5 in syngas conversion显示文摘Aromatics are important basic chemicals. However, direct conversion of syngas via the conventional Fischer-Tropsch synthesis produces little aromatics. We presented herein that a bifunctional composite of ZSM-5 in combination with Zn Cr Oxcatalyzes syngas conversion to aromatics. Particularly, ZSM-5 crystals with a sheet-like morphology can enhance significantly the aromatization activity. The lower length ratio of the b/a axes of the crystals, the more aromatics form but without influencing the selectivity of small molecules such as CH4 and C2–C4.
