| 1 | Cancer incidence,mortality,and burden in China:a time-trend analysis and comparison with the United States and United Kingdom based on the global epidemiological data released in 2020显示文摘Background:Cancer is one of the leading causes of death and a main economic burden in China.Investigating the differences in cancer patterns and control strategies between China and developed countries could provide reference for policy planning and contribute to improving cancer control measures.In this study,we reviewed the rates and trends of cancer incidence and mortality and disability-adjusted life year(DALY)burden in China,and compared them with those in the United States(US)and the United Kingdom(UK).Methods:Cancer incidence,mortality,and DALY data for China,US and UK were obtained fromtheGLOBOCAN2020 online database,Global Burden of Disease(GBD)2019 study,and Cancer Incidence in Five Continents plus database(CI5 plus).Trends of cancer incidence and mortality in China,US,and UK were analyzed using Joinpoint regression models to calculate annual percent changes(APCs)and identify the best-fitting joinpoints.Results:An estimated 4,568,754 newly diagnosed cancer cases and 3,002,899 cancer deaths occurred in China in 2020.Additionally,cancers resulted in 67,340,309 DALYs in China.Compared to the US and UK,China had lower cancer incidence but higher cancer mortality andDALY rates.Furthermore,the cancer spectrum of China was changing,with a rapid increase incidence and burden of lung,breast,colorectal,and prostate cancer in addition to a high incidence and heavy burden of liver,stomach,esophageal,and cervical cancer.Conclusions:The cancer spectrum of China is changing from a developing country to a developed country.Population aging and increase of unhealthy lifestyles would continue to increase the cancer burden of China.Therefore,the Chinese authorities should adjust the national cancer control program with reference to the practices of cancer control which have been well-established in the developed countries,and taking consideration of the diversity of cancer types by of different regions in China at the same time. | Haibo Qiu Sumei Cao Ruihua Xu | 2021 | Cancer Communications2021,41,10: | 194 |
| 2 | Prognostic nutritional index is an independent prognostic factor for gastric cancer patients with peritoneal dissemination显示文摘Objective: The predictive and prognostic role of prognostic nutritional index(PNI) in gastric cancer patients with peritoneal dissemination remains unclear. This study aims to explore the role of the PNI in predicting outcomes of gastric cancer patients with peritoneal dissemination.Methods: A total of 660 patients diagnosed with gastric adenocarcinoma with peritoneal metastasis between January 2000 and April 2014 at Sun Yat-sen University Cancer Center and the Sixth Affiliated Hospital of Sun Yatsen University were retrospectively analyzed. The clinicopathologic characteristics and clinical outcomes of patients with peritoneal dissemination were analyzed.Results: Compared with PNI-high group, PNI-low group was correlated with advanced age(P=0.036), worse performance status(P<0.001), higher frequency of ascites(P<0.001) and higher frequency of multisite distant metastasis(P<0.001). Kaplan-Meier survival curves showed that PNI-high group had a significantly longer median overall survival than PNI-low group(13.13 vs. 9.03 months, P<0.001). Multivariate survival analysis revealed that Borrmann type IV(P=0.014), presence of ascites(P=0.017) and lower PNI(P=0.041) were independent poor prognostic factors, and palliative surgery(P<0.001) and first-line chemotherapy(P<0.001) were good prognostic factors. For patients receiving palliative surgery, the postoperative morbidity rates in the PNI-low group and PNIhigh group were 9.1% and 9.9%, respectively(P=0.797). | 3 | Prognostic factors of primary gastrointestinal stromal tumors: a cohort study based on high-volume centers显示文摘Objective: We aimed to evaluate the clinicopathologic characteristics, immunohistochemical expression and prognostic factors of patients with primary gastrointestinal stromal tumors(GISTs).Methods: Data from 2,570 consecutive GIST patients from four medical centers in China(January2001–December 2015) were reviewed. Survival curves were constructed by the Kaplan-Meier method, and Cox regression models were used to identify independent prognostic factors.Results: Of the included patients, 1,375(53.5%) were male, and the patient age range was 18 to 95(median, 58)years. The tumors were mostly found in the stomach(64.5%), small intestine(25.1%) and colorectal region(5.1%).At the time of diagnosis, the median tumor size was 4.0(range: 0.1–55.0) cm, and the median mitotic index per 50 high power fields(HPFs) was 3(range: 0–254). Of the 2,168 resected patients, 2,009(92.7%) received curative resection. According to the modified National Institutes of Health(NIH) classification, 21.9%, 28.9%, 14.1% and35.1% were very low-, low-, intermediate-and high-risk tumors, respectively. The rate of positivity was 96.4% for c-Kit, 87.1% for CD34, 96.9% for delay of germination 1(DOG-1), 8.0% for S-100, 31.0% for smooth muscle actin(SMA) and 5.1% for desmin. However, the prognostic value of each was limited. Multivariate analysis showed that age, tumor size, mitotic index, tumor site, occurrence of curative resection and postoperative imatinib were independent prognostic factors. Furthermore, we found that high-risk patients benefited significantly from postoperative imatinib(P<0.001), whereas intermediate-risk patients did not(P=0.954).Conclusions: Age, tumor size, mitotic index, tumor site, occurrence of curative resection and postoperative imatinib were independent prognostic factors in patients with GISTs. | 4 | The Cenozoic activities of Yangjiang-Yitongdong Fault: insights from analysis of the tectonic characteristics and evolution processes in western Zhujiang(Pearl) River Mouth Basin显示文摘The Yangjiang-Yitongdong Fault (YJF) is an important NW-trending regional fault, which divides the Zhujiang (Pearl) River Mouth Basin (ZRMB) into western and eastern segments. In Cenozoic, the northern continental margin of the South China Sea (SCS) underwent continental rifting, breakup, seafloor spreading and thermal subsidence processes, and the Cenozoic activities of YJF is one part of this series of complex processes. Two long NW-trending multichannel seismic profiles located on both sides of the YJF extending from the continental shelf to Continent-Ocean Boundary (COB) were used to study the tectonic and sedimentary characteristics of western ZRMB. Using the 2D-Move software and back-stripping method, we constructed the balance cross-section model and calculated the fault activity rate. Through the comprehensive consideration of tectonic position, tectonic evolution history, featured structure, and stress analysis, we deduced the activity history of the YJF in Cenozoic. The results showed that the YJF can be divided into two segments by the central uplift belt. From 65 Ma to 32 Ma, the YJF was in sinistral motion as a whole, inherited the preexisting sinistral motion of Mesozoic YJF, in which, the southern part of YJF was mainly in extension activity, controlling the formation and evolution of Yunkai Low Uplift, coupled with slight sinistral motion. From 32 Ma to 23.8 Ma, the sinistral motion in northern part of YJF continued, while the sinistral motion in southern part began to stop or shifted to a slightly dextral motion. After 23.8 Ma, the dextral motion in southern part of YJF continued, while the sinistral motion in northern part of YJF gradually stopped, or shifted to the slightly dextral motion. The shift of the YJF strike-slip direction may be related to the magmatic underplating in continent-ocean transition, southeastern ZRMB.
