1Pancreatic carcinoma coexisting with chronic pancreatitis versus tumor-forming pancreatitis:Diagnostic utility of the time-signal intensity curve from dynamic contrast-enhanced MR imaging显示文摘AIM: To evaluate the ability of the time-signal intensity curve (TIC) of the pancreas obtained from dynamic contrast-enhanced magnetic resonance imaging (MRI) for differentiation of focal pancreatic masses, especially pancreatic carcinoma coexisting with chronic pancreatitis and tumor-forming pancreatitis. METHODS: Forty-eight consecutive patients who underwent surgery for a focal pancreatic mass, including pancreatic ductal carcinoma (n = 33), tumor-forming pancreatitis (n = 8), and islet cell tumor (n = 7), were reviewed. Five pancreatic carcinomas coexisted with longstanding chronic pancreatitis. The pancreatic TICs were obtained from the pancreatic mass and the pancreatic parenchyma both proximal and distal to the mass lesion in each patient, prior to surgery, and were classified into 4 types according to the time to a peak: 25 s and 1, 2, and 3 min after the bolus injection of contrast material, namely, type-Ⅰ, Ⅱ, Ⅲ, and Ⅳ, respectively, and were then compared to the corresponding histological pancreatic conditions. RESULTS: Pancreatic carcinomas demonstrated type-Ⅲ (n = 13) or Ⅳ (n = 20) TIC. Tumor-forming pancreatitis showed type-Ⅱ (n = 5) or Ⅲ (n = 3) TIC. All islet cell tumors revealed type-Ⅰ. The type-Ⅳ TIC was only recognized in pancreatic carcinoma, and the TIC of carcinoma always depicted the slowest rise to a peak among the 3 pancreatic TICs measured in each patient, even in patients with chronic pancreatitis.CONCLUSION: Pancreatic TIC from dynamic MRI provides reliable information for distinguishing pancreatic carcinoma from other pancreatic masses, and may enable us to avoid unnecessary pancreatic surgery and delays in making a correct diagnosis of pancreatic carcinoma, especially, in patients with longstanding chronic pancreatitis.Yoshitsugu Tajima Tamotsu Kuroki Ryuji Tsutsumi Ichiro Isomoto Masataka Uetani Takashi Kanematsu 2007World Journal of Gastroenterology2007,13,6:28
2Hepatic schwannoma:Imaging findings on CT,MRI and contrast-enhanced ultrasonography显示文摘A primary benign schwannoma of the liver is extremely rare and is difficult to preoperatively discriminate from a malignant tumor.We compared the imaging and pathological findings,and examined the possibility of preoperatively diagnosing a benign liver schwannoma.A 72-year-old woman was admitted to our hospital because of a 4.6-cm mass in the liver.A malignant tumor was suspected,and a right hepatectomy was performed.After this,the diagnosis of a primary benign schwannoma of the liver was made through pathological examination.Contrast-enhanced ultrasonography(CEUS) with Sonazoid showed minute blood flows into the septum and solid areas of the tumor in the vascular phase;most likely due to increased arterial flow associated with infiltration of chronic inflammatory cells.In the postvascular phase,CEUS showed contrast defect of cystic areas and delayed enhancement of solid areas;most likely due to aggregation of siderophores.Because discriminating between a benign and malignant schwannoma of the liver is difficult,surgery is generally recommended.However,the two key findings from CEUS may be useful in discriminating ancient schwannoma by recognizing the hemorrhage involved in the secondary degeneration and aggregation of siderophores.Yu Ota Kazunobu Aso Kenji Watanabe Takahiro Einama Koji Imai Hidenori Karasaki Ryuji Sudo Yosui Tamaki Mituyoshi Okada Yosihiko Tokusashi Toru Kono Naoyuki Miyokawa Masakazu Haneda Masahiko Taniguchi Hiroyuki Furukawa 2012World Journal of Gastroenterology2012,18,35:11
3Boron neutron capture therapy for vulvar melanoma and genital extramammary Paget’s disease with curative responses显示文摘Background:Although the most commonly recommended treatment for melanoma and extramammary Paget’s disease(EMPD)of the genital region is wide surgical excision of the lesion,the procedure is highly invasive and can lead to functional and sexual problems.Alternative treatments have been used for local control when wide local exci-sion was not feasible.Here,we describe four patients with genital malignancies who were treated with boron neutron capture therapy(BNCT).Methods:The four patients included one patient with vulvar melanoma(VM)and three with genital EMPD.They underwent BNCT at the Kyoto University Research Reactor between 2005 and 2014 using para-boronophenylalanine as the boron delivery agent.They were irradiated with an epithermal neutron beam between the curative tumor dose and the tolerable skin/mucosal doses.Results:All patients showed similar tumor and normal tissue responses following BNCT and achieved complete responses within 6 months.The most severe normal tissue response was moderate skin erosion during the first 2 months,which diminished gradually thereafter.Dysuria or contact pain persisted for 2 months and resolved com-pletely by 4 months.Conclusions:Treating VM and EMPD with BNCT resulted in complete local tumor control.Based on our clinical expe-rience,we conclude that BNCT is a promising treatment for primary VM and EMPD of the genital region.Junichi Hiratsuka Nobuhiko Kamitani Ryo Tanaka Eisaku Yoden Ryuji Tokiya Minoru Suzuki Rolf F.Barth Koji Ono 2018Cancer Communications2018,38,1:10
4Extrahepatic collaterals and liver damage in embolotherapy for ruptured hepatic artery pseudoaneurysm following hepatobiliary pancreatic surgery显示文摘AIM: To evaluate the effects of extrahepatic collaterals to the liver on liver damage and patient outcome after embolotherapy for the ruptured hepatic artery pseudoa- neurysm following hepatobiliary pancreatic surgery. METHODS: We reviewed 9 patients who underwent transcatheter arterial embolization (TAE) for the ruptured hepatic artery pseudoaneurysm following major hepato- biliary pancreatic surgery between June 1992 and April 2006. We paid special attention to the extrahepatic arte- rial collaterals to the liver which may affect post-TAE liver damage and patient outcome. RESULTS: The underlying diseases were all malignan- cies, and the surgical procedures included hepatopancre- atoduodenectomy in 2 patients, hepatic resection with removal of the bile duct in 5, and pancreaticoduodenec- tomy in 2. A total of 11 pseudoaneurysm developed: 4 in the common hepatic artery, 4 in the proper hepatic artery, and 3 in the right hepatic artery. Successful he- mostasis was accomplished with the initial TAE in all patients, except for 1. Extrahepatic arterial pathways to the liver, including the right inferior phrenic artery, the jejunal branches, and the aberrant left hepatic artery, were identified in 8 of the 9 patients after the completion of TAE. The development of collaterals depended on the extent of liver mobilization during the hepatic resection, the postoperative period, the presence or absence of an aberrant left hepatic artery, and the concomitant arte- rial stenosis adjacent to the pseudoaneurysm. The liver tolerated TAE without significant consequences when at least one of the collaterals from the inferior phrenic ar-tery or the aberrant left hepatic artery was present. One patient, however, with no extrahepatic collaterals died of liver failure due to total liver necrosis 9 d after TAE. 5Narrow pelvic inlet plane area and obesity as risk factors for anastomotic leakage after intersphincteric resection显示文摘BACKGROUND Intersphincteric resection(ISR)has been increasingly used as the ultimate sphincter-preserving procedure in extremely low rectal cancer.The most critical complication of this technique is anastomotic leakage.The incidence rate of anastomotic leakage after ISR has been reported to range from 5.1%to 20%.AIM To investigate risk factors for anastomotic leakage after ISR based on clinicopathological variables and pelvimetry.METHODS This study was conducted at Department of Colorectal Surgery,Japanese Red Cross Medical Center,Tokyo,Japan,with a total of 117 patients.We enrolled 117 patients with extremely low rectal cancer who underwent laparotomic and laparoscopic ISRs at our hospital.We conducted retrospective univariate and multivariate regression analyses on 33 items to elucidate the risk factors for anastomotic leakage after ISR.Pelvic dimensions were measured using threedimensional reconstruction of computed tomography images.The optimal cutoff value of the pelvic inlet plane area that predicts anastomotic leakage was determined using a receiver operating characteristic(ROC)curve.RESULTS We observed anastomotic leakage in 10(8.5%)of the 117 patients.In the multivariate analysis,we identified high body mass index(odds ratio 1.674;95%confidence interval:1.087-2.58;P=0.019)and smaller pelvic inlet plane area(odds ratio 0.998;95%confidence interval:0.997-0.999;P=0.012)as statistically significant risk factors for anastomotic leakage.According to the receiver operating characteristic curves,the optimal cutoff value of the pelvic inlet plane area was 10074 mm2.Narrow pelvic inlet plane area(≤10074 mm2)predicted anastomotic leakage with a sensitivity of 90%,a specificity of 85.9%,and an accuracy of 86.3%.CONCLUSION Narrow pelvic inlet and obesity were independent risk factors for anastomotic leakage after ISR.Anastomotic leakage after ISR may be predicted from a narrow pelvic inlet plane area(≤10074 mm2).Akira Toyoshima Toshihiro Nishizawa Eiji Sunami Ryuji Akai Takahiro Amano Akiyoshi Yamashita Shin Sasaki Takeshi Endo Yoshihiro Moriya Osamu Toyoshima 2020World Journal of Gastrointestinal Surgery2020,12,10:4

7Modulation of Tropical Cyclogenesis in the Western North Pacific by the Quasi-Biweekly Oscillation显示文摘The quasi-biweekly oscillation(QBWO) is the second most dominant intraseasonal mode over the western North Pacific(WNP) during boreal summer. In this study, the modulation of WNP tropical cyclogenesis(TCG) by the QBWO and its association with large-scale patterns are investigated. A strong modulation of WNP TCG events by the QBWO is found.More TCG events occur during the QBWO's convectively active phase. Based on the genesis potential index(GPI), we further evaluate the role of environmental factors in affecting WNP TCG. The positive GPI anomalies associated with the QBWO correspond well with TCG counts and locations. A large positive GPI anomaly is spatially correlated with WNP TCG events during a life cycle of the QBWO. The low-level relative vorticity and mid-level relative humidity appear to be two dominant contributors to the QBWO-composited GPI anomalies during the QBWO's active phase, followed by the nonlinear and potential intensity terms. These positive contributions to the GPI anomalies are partly offset by the negative contribution from the vertical wind shear. During the QBWO's inactive phase, the mid-level relative humidity appears to be the largest contributor, while weak contributions are also made by the nonlinear and low-level relative vorticity terms.Meanwhile, these positive contributions are partly cancelled out by the negative contribution from the potential intensity.The contributions of these environmental factors to the GPI anomalies associated with the QBWO are similar in all five flow patterns—the monsoon shear line, monsoon confluence region, monsoon gyre, easterly wave, and Rossby wave energy dispersion associated with a preexisting TC. Further analyses show that the QBWO strongly modulates the synoptic-scale wave trains(SSWs) over the WNP, with larger amplitude SSWs during the QBWO's active phase. This implies a possible enhanced(weakened) relationship between TCG and SSWs during the active(inactive) phase. 13Atomically phase-matched second-harmonic generation in a 2D crystal显示文摘Second-harmonic generation(SHG)has found extensive applications from hand-held laser pointers to spectroscopic and microscopic techniques.Recently,some cleavable van der Waals(vdW)crystals have shown SHG arising from a single atomic layer,where the SH light elucidated important information such as the grain boundaries and electronic structure in these ultra-thin materials.However,despite the inversion asymmetry of the single layer,the typical crystal stacking restores inversion symmetry for even numbers of layers leading to an oscillatory SH response,drastically reducing the applicability of vdW crystals such as molybdenum disulfide(MoS_(2)).Here,we probe the SHG generated from the noncentrosymmetric 3R crystal phase of MoS_(2).We experimentally observed quadratic dependence of second-harmonic intensity on layer number as a result of atomically phase-matched nonlinear dipoles in layers of the 3R crystal that constructively interfere.By studying the layer evolution of the A and B excitonic transitions in 3R-MoS_(2) using SHG spectroscopy,we also found distinct electronic structure differences arising from the crystal structure and the dramatic effect of symmetry and layer stacking on the nonlinear properties of these atomic crystals.The constructive nature of the SHG in this 2D crystal provides a platform to reliably develop atomically flat and controllably thin nonlinear media.Mervin Zhao Ziliang Ye Ryuji Suzuki Yu Ye Hanyu Zhu Jun Xiao Yuan Wang Yoshihiro Iwasa Xiang Zhang 2016Light(Science & Applications)2016,5,1:2

17Comparison of the chloride channel activator lubiprostone and the oral laxative Polyethylene Glycol 3350 on mucosal barrier repair in ischemic-injured porcine intestine显示文摘AIM: To investigate the effects of lubiprostone and Polyethylene Glycol 3350 (PEG) on mucosal barrier repair in ischemic-injured porcine intestine. METHODS: Ileum from 6 piglets (approximately 15 kg body weight) was subjected to ischemic conditions by occluding the local mesenteric circulation for 45 min in vivo. Ileal tissues from each pig were then harvested and mounted in Ussing chambers and bathed in oxygenated Ringer's solution in vitro. Intestinal barrier function was assessed by measuring transepithelial electrical resistance (TER) and mucosal-to-serosal fluxes of 3H-mannitol and 14C-inulin. Statistical analyses of data collected over a 120-min time course included 2-way ANOVA for the effects of time and treatment on indices of barrier function. RESULTS: Application of 1 μmol/L lubiprostone to the mucosal surface of ischemic-injured ileum in vitro induced significant elevations in TER compared to non-treated tissue. Lubiprostone also reduced mucosal-to-serosal fluxes of 3H-mannitol and 14C-inulin. Alternatively,application of a polyethylene laxative (PEG,20 mmol/L) to the mucosal surface of ischemic tissues significantly increased flux of 3H-mannitol and 14C-inulin. CONCLUSION: This experiment demonstrates that lubiprostone stimulates recovery of barrier function in ischemic intestinal tissues whereas the PEG laxative had deleterious effects on mucosal repair. 19Daikenchuto (Da-Jian-Zhong-Tang) ameliorates intestinal fibrosis by activating myofibroblast transient receptor potential ankyrin 1 channel显示文摘AIM To investigate the anti-fibrotic effects of the traditional oriental herbal medicine Daikenchuto(DKT) associated with transient receptor potential ankyrin 1(TRPA1) channels in intestinal myofibroblasts. METHODS Inflammatory and fibrotic changes were detected in a2,4,6-trinitrobenzenesulfonic acid(TNBS) chronic colitis model of wild-type and TRPA1-knockout(TRPA1-KO) mice via pathological staining and immunoblotting analysis.Ca^(2+) imaging experiments examined the effects of DKT and its components/ingredients on intestinal myofibroblast(In Myo Fib) cell TRPA1 channel function.Profibrotic factors and transforming growth factor (TGF) -β1-associated signaling were tested in an In Myo Fib cell line by q PCR and immunoblotting experiments.Samples from non-stenotic and stenotic regions of the intestines of patients with Crohn’s disease (CD) were used for pathological analysis. 20Synthetic seismograms of ground motion near earthquake fault using simulated Green's function method显示文摘Seismograms near source fault were synthesized using the hybrid empirical Green’s func-tion method where the discretely simulated seismic waveforms are used for Green’s functions instead of the observed waveforms of small earthquakes. The Green’s function seismic waveforms for small earth-quake were calculated by solving wave equation us-ing the pseudo-spectral method with the staggered grid real FFT strategy under a detailed 2-D velocity structure in Kobe region. Magnitude and seismic moment of simulated Green’s function waveforms were firstly determined by using the relationship be-tween fault length and corner frequency of source spectrum. The simulated Green’s function waveforms were employed to synthesize seismograms of strong ground motion near the earthquake fault. The syn-thetic seismograms of the target earthquake were performed based on the model with multiple source rupture processes. The results suggest that synthe-sized seismograms coincide well with observed seismic waveforms of the 1995 Hyogo-ken Nanbu earthquake. The simulated Green’s function method is very useful for prediction of the strong ground mo-tion in region without observed seismic waveforms.
