| 1 | Concordance of non-invasive mechanical and serum tests for liver fibrosis evaluation in chronic hepatitis C显示文摘AIM To determine the sensitivity and specificity of liver stiffness measurement(LSM) and serum markers(SM) for liver fibrosis evaluation in chronic hepatitis C.METHODS Between 2012 and 2014,81 consecutive hepatitis C virus(HCV) patients had METAVIR score from liver biopsy compared with concurrent results from LSM [transient elastography(TE) [FibroS can~/ARFI technology(Virtual Touch~)] and SM [FIB-4/aspartate aminotransferase-toplatelet ratio index(APRI)].The diagnostic performance of these tests was assessed using receiver operating characteristic curves.The optimal cut-off levels of each test were chosen to define fibrosis stages F ≥ 2,F ≥ 3 and F = 4.The Kappa index set the concordance analysis.RESULTS Fifty point six percent were female and the median age was 51 years(30-78).Fifty-six patients(70%) weretreatment-na?ve.The optimal cut-off values for predicting F ≥ 2 stage fibrosis assessed by TE were 6.6 kP a,for acoustic radiation force impulse(ARFI) 1.22 m/s,for APRI 0.75 and for FIB-4 1.47.For F ≥ 3 TE was 8.9 kP a,ARFI was 1.48 m/s,APRI was 0.75,and FIB-4 was 2.For F = 4,TE was 12.2 kP a,ARFI was 1.77 m/s,APRI was 1.46,and FIB-4 was 3.91.The APRI could not distinguish between F2 and F3,P = 0.92.The negative predictive value for F = 4 for TE and ARFI was 100%.Kappa index values for F ≥ 3 METAVIR score for TE,ARFI and FIB-4 were 0.687,0.606 and 0.654,respectively.This demonstrates strong concordance between all three screening methods,and moderate to strong concordance between them and APRI(Kappa index = 0.507).CONCLUSION Given the costs and accessibility of LSM methods,and the similarity with the outcomes of SM,we suggest that FIB-4 as well as TE and ARFI may be useful indicators of the degree of liver fibrosis.This is of particular importance to developing countries. | Denise C Paranaguá-Vezozzo Adriana Andrade Daniel F C Mazo Vinicius Nunes Ana L Guedes Taisa G Ragazzo Renata Moutinho Lucas S Nacif Suzane K Ono Venancio A F Alves Flair J Carrilho | 2017 | World Journal of Hepatology2017,9,8: | 7 |
| 2 | Effect of propranolol on the splanchnic and peripheral renin angiotensin system in cirrhotic patients显示文摘AIM:To evaluate the effect of β-blockade on angiotensins in the splanchnic and peripheral circulation of cirrhotic patients and also to compare hemodynamic parameters during liver transplantation according to propranolol pre-treatment or not. METHODS:Patients were allocated into two groups:outpatients with advanced liver disease(LD) and during liver transplantation(LT). Both groups were subdivided according to treatment with propranolol or not. Plasma was collected through peripheral venipuncture to determine plasma renin activity(PRA),Angiotensin(Ang) Ⅰ,Ang Ⅱ,and Ang-(1-7) levels by radioimmunoassay in LD group. During liver transplantation,hemodynamic parameters were determined and blood samples were obtained from the portal vein to measure renin angiotensin system(RAS) components.RESULTS:PRA,Ang Ⅰ,Ang Ⅱ and Ang-(1-7) were signifi cantly lower in the portal vein and periphery in all subgroups treated with propranolol as compared to non-treated. The relationships between Ang-(1-7) and Ang Ⅰ levels and between Ang Ⅱ and Ang Ⅰ were significantly increased in LD group receiving propranolol. The ratio between Ang-(1-7) and Ang Ⅱ remained unchanged in splanchnic and peripheral circulation in patients under β-blockade,whereas the relationship between Ang Ⅱ and Ang Ⅰ was significantly increased in splanchnic circulation of LT patients treated with propranolol. During liver transplantation,cardiac output and index as well systemic vascular resistance and index were reduced in propranolol-treated subgroup. CONCLUSION:In LD group,propranolol treatment reduced RAS mediators,but did not change the ratio between Ang-(1-7) and Ang Ⅱ in splanchnic and peripheral circulation. | 5 | Decomposition and stabilization of organic matter in an old-growth tropical riparian forest:effects of soil properties and vegetation structure显示文摘Background:Nutrient cycling in tropical forests has a large importance for primary productivity,and decomposition of litterfall is a major process influencing nutrient balance in forest soils.Although large-scale factors strongly influence decomposition patterns,small-scale factors can have major influences,especially in old-growth forests that have high structural complexity and strong plant-soil correlations.Here we evaluated the effects of forest structure and soil properties on decomposition rates and stabilization of soil organic matter using the Tea Bag Index(TBI)in an old-growth riparian forest in southeastern Brazil.These data sets were described separately using Principal Components Analysis(PCA).The main axes for each analysis,together with soil physical properties(clay content and soil moisture),were used to construct structural equations models that evaluated the different parameters of the TBI,decomposition rates and stabilization factor.The best model was selected using Akaike’s criterion.Results:Forest structure and soil physical and chemical properties presented large variation among plots within the studied forest.Clay content was strongly correlated with soil moisture and the first PCA axis of soil chemical properties,and model selection indicated that clay content was a better predictor than this axis.Decomposition rates presented a large variation among tea bags(0.009 and 0.098 g·g^(−1)·d−1)and were positively related with forest structure,as characterized by higher basal area,tree density and larger trees.The stabilization factor varied between 0.211–0.426 and was related to forest stratification and soil clay content.Conclusions:The old-growth forest studied presented high heterogeneity in both forest structure and soil properties at small spatial scales,that influenced decomposition processes and probably contributed to small-scale variation in nutrient cycling.Decomposition rates were only influenced by forest structure,whereas the stabilization factor was influenced by both forest structure and soil properties.Heterogeneity in ecological processes can contribute to the resilience of old-growth forests,highlighting the importance of restoration strategies that consider the spatial variation of ecosystem processes. | Pedro Henrique de Godoy Fernandes Andréa Lúcia Teixeira de Souza Marcel Okamoto Tanaka Renata Sebastiani | 2021 | Forest Ecosystems2021,8,1: | 1 |

| 18 | Fibrosis assessment using Fibro Meter combined to first generation tests in hepatitis C显示文摘AIM To evaluate the performance of FibroMeter^(Virus3G) combined to the first generation tests aspartate aminotransferase-to-platelet ratio index(APRI) or Forns index to assess significant fibrosis in chronic hepatitis C(CHC).METHODS First generation tests APRI or Forns were initially applied in a derivation population from Rio de Janeiro in Brazil considering cut-offs previously reported in the literature to evaluate significant fibrosis.FibroMeter^(Virus3G) was sequentially applied to unclassified cases from APRI or Forns.Accuracy of non-invasive combination of tests,APRI plus FibroMeter^(Virus3G) and Forns plus FibroMeter^(Virus3G) was evaluated in the Brazilian derivation population.APRI plus FibroMeter^(Virus3G) combination was validated in a population of CHC patients from Angers in France.All patients were submitted to liver biopsy staged according to METAVIR score by experienced hepatopathologists.Significant fibrosis was considered as METAVIR F≥2.The fibrosis stage classification was used as the reference for accuracy evaluation of non-invasive combination of tests.Blood samples for the calculation of serum tests were collected on the same day of biopsy procedure or within a maximum 3 mo interval and stored at-70 ℃.RESULTS Seven hundred and sixty CHC patients were included(222 in the derivation population and 538 in the validation group).In the derivation population,the FibroMeter^(Virus3G) AUROC was similar to APRI AUROC(0.855 vs 0.815,P=0.06) but higher than Forns AUROC(0.769,P<0.001).The best FibroMeter^(Virus3G) cut-off to discriminate significant fibrosis was 0.61(80% diagnostic accuracy;75% in the validation population,P=0.134).The sequential combination of APRI or Forns with FibroMeter^(Virus3G) in derivation population presented similar performance compared to FibroMeter^(Virus3G) used alone(79% vs 78% vs 80%,respectively,P=0.791).Unclassified cases of significant fibrosis after applying APRI and Forns corresponded to 49% and 54%,respectively,of the total sample.However,the combination of APRI or Forns with FibroMeter^(Virus3G) allowed 73% and 77%,respectively,of these unclassified cases to be correctly evaluated.Moreover,this combination resulted in a reduction of FibroMeter^(Virus3G) requirement in approximately 50% of the entire sample.The stepwise combination of APRI and FibroMeter^(Virus3G) applied to the validation population correctly identified 74% of patients with severe fibrosis(F≥3).CONCLUSION The stepwise combination of APRI or Forns with FibroMeter^(Virus3G) may represent an accurate lower cost alternative when evaluating significant fibrosis,with no need for liver biopsy. | Maria Chiara Chindamo Jerome Boursier Ronir Raggio Luiz Isabelle Fouchard-Hubert Vera Lúcia Nunes Pannain Joao Marcello de Araújo Neto Henrique Sérgio Moraes Coelho Renata de Mello Perez Paul Calès Cristiane Alves Villela-Nogueira | 2017 | World Journal of Hepatology2017,9,6: | 1 |
