1Diagnostic value of PIVKA-Ⅱ and alpha-fetoprotein in hepatitis B virus-associated hepatocellular carcinoma显示文摘AIM: To determine the cutoff values and to compare the diagnostic role of alpha-fetoprotein(AFP) and prothrombin induced by vitamin K absence-Ⅱ(PIVKA-Ⅱ) in chronic hepatitis B(CHB).METHODS: A total of 1255 patients with CHB, including 157 patients with hepatocellular carcinoma(HCC), 879 with non-cirrhotic CHB and 219 with cirrhosis without HCC, were retrospectively enrolled. The areas under the receiver operating characteristic(AUROC) curves of PIVKA-Ⅱ, AFP and their combination were calculated and compared.RESULTS: The optimal cutoff values for PIVKA-Ⅱ and AFP were 40 m AU/m L and 10 ng/m L, respectively, for the differentiation of HCC from nonmalignant CHB. The sensitivity and specificity were 73.9% and 89.7%, respectively, for PIVKA-Ⅱ and 67.5% and 90.3% for AFP, respectively. The AUROC curves of both PIVKA-Ⅱ and AFP were not significantly different(0.854 vs 0.853, P = 0.965) for the differentiation of HCC from nonmalignant CHB, whereas the AUROC of PIVKA-Ⅱ was significantly better than that of AFP in patients with cirrhosis(0.870 vs 0.812, P = 0.042). 2Recurrence after endoscopic piecemeal mucosal resection for large sessile colorectal polyps显示文摘AIM: To evaluate the safety and outcomes of endoscopic piecemeal mucosal resection (EPMR) for large sessile colorectal polyps. METHODS: The patients enrolled in this study were 47 patients with 50 large sessile polyps (diameter, 2 cm or greater) who underwent EPMR using a submucosal saline injection technique between December 2002 and October 2005. All medical records, including characteristics of the patients and polyps, complications, and recurrences, were retrospectively reviewed. The first follow-up endoscopic examination was performed at 3-6 mo after initial endoscopic resection, and the second at 12 mo postEPMR. Subsequent surveillance colonoscopic examinations were individualized, taking risk factors into account. RESULTS: The patients were 23 men and 24 women,with a mean age of 60 years. Mean polyp size was 30.1 mm. Of 50 polyps identified, 34 (68%) were benign and 16 (32%) were malignant. There were 6 (12%) cases with EPMR-related bleeding: 5 intra-procedural and 1 early post-procedural bleeding. All bleeding episodes were managed by endoscopic clipping or argon beam coagulation. There were no perforations. Recurrence was identified in 5 cases (12.2%): 4 local recurrences detected at 3 mo post-EPMR and 1 local recurrence detected at 14 mo post-EPMR. The recurrence rate after EPMR was 3.1% for benign polyps and 33.3% for malignant polyps (P < 0.05). Median follow-up time was 37 mo. CONCLUSION: EPMR is safe, but should be applied carefully in malignant polyps. 3Low-frequency pulsed electromagnetic field pretreated bone marrow-derived mesenchymal stem cells promote the regeneration of crush-injured rat mental nerve显示文摘Bone marrow-derived mesenchymal stem cells(BMSCs) have been shown to promote the regeneration of injured peripheral nerves. Pulsed electromagnetic field(PEMF) reportedly promotes the proliferation and neuronal differentiation of BMSCs. Low-frequency PEMF can induce the neuronal differentiation of BMSCs in the absence of nerve growth factors. This study was designed to investigate the effects of low-frequency PEMF pretreatment on the proliferation and function of BMSCs and the effects of low-frequency PEMF pre-treated BMSCs on the regeneration of injured peripheral nerve using in vitro and in vivo experiments. In in vitro experiments, quantitative DNA analysis was performed to determine the proliferation of BMSCs, and reverse transcription-polymerase chain reaction was performed to detect S100(Schwann cell marker), glial fibrillary acidic protein(astrocyte marker), and brain-derived neurotrophic factor and nerve growth factor(neurotrophic factors) m RNA expression. In the in vivo experiments, rat models of crush-injured mental nerve established using clamp method were randomly injected with low-frequency PEMF pretreated BMSCs, unpretreated BMSCs or PBS at the injury site(1 × 106 cells). Di I-labeled BMSCs injected at the injury site were counted under the fluorescence microscope to determine cell survival. One or two weeks after cell injection, functional recovery of the injured nerve was assessed using the sensory test with von Frey filaments. Two weeks after cell injection, axonal regeneration was evaluated using histomorphometric analysis and retrograde labeling of trigeminal ganglion neurons. In vitro experiment results revealed that low-frequency PEMF pretreated BMSCs proliferated faster and had greater m RNA expression of growth factors than unpretreated BMSCs. In vivo experiment results revealed that compared with injection of unpretreated BMSCs, injection of low-frequency PEMF pretreated BMSCs led to higher myelinated axon count and axon density and more Di I-labeled neurons in the trigeminal ganglia, contributing to rapider functional recovery of injured mental nerve. 4膝关节Rosenberg位片上影像学分级与内侧开放楔形胫骨高位截骨术术后临床疗效的关系探讨显示文摘目的探讨与内侧开放楔形胫骨高位截骨术(open-wedge high-tibialosteotomy,OWHTO)术后临床疗效最密切相关的影像学分级方法,并找出与OWHTO预后相关的有利和不利因素。方法分析79例膝骨关节炎伴固有胫骨内翻畸形的患者,均接受OWHTO治疗。根据改良关节镜下Outerbridge分级以及影像学Kellgren-Lawrence(K-L)分级进行骨关节炎严重程度评估,影像学检查包括站立前后位(anteroposterior,AP)及屈曲45°后前(posteroanterior,PA)负重位(Rosenberg位)。临床疗效采用牛津膝关节评分(Oxford knee score,OKS)进行评估,包括术前和术后2年的随访。采用多元回归分析相关数据。结果随访2年,OKS评分均值由(20±4)分提高到(39±5)分(P<0.001)。平均机械胫股角由内翻的(6.9±3.4)°变为外翻的(2.7±2.8)°,胫骨近端内侧角(medial proximal tibial angle,MPTA)由(85.6±2.4)°变为(92.9±3.7)°(P<0.001)。术前膝关节站立前后位上K-L分级为(2.4±0.9)分,Rosenberg位上K-L分级为(2.8±0.9)分(P=0.003),关节镜下改良Outerbridge骨关节炎分级为(3.4±0.7)分。在多元回归分析中,Rosenberg位下K-L分级(P=0.01)、术后MPTA值(P=0.01)与术后2年随访的OKS评分呈显著负相关(P=0.01)。结论采用三种不同的骨关节炎分级方案(分别是改良关节镜下Outerbridge分级、站立前后位的K-L分级、Rosenberg位的K-L分级)来确定HTO手术指证,其中基于Rosenberg位的K-L分级与OWHTO术后OKS评分呈显著负相关,这与Rosenberg位下股骨髁与胫骨髁距离最近有关。说明基于Rosenberg位的K-L分级对HTO手术指证评估以及对晚期膝骨关节炎患者临床疗效的评估具有重要意义。Kyung Wook Nha Seung Min Oh Yoon Won Ha Manoj Kumar Patel Ji Hyun Seo Byung Hoon Lee 黄景星(整理) 王浩浩(整理) 张民(整理) 2019实用骨科杂志2019,25,10:4

6Effect of composite yogurt enriched with acacia fiber and Bifidobacterium lactis显示文摘AIM: To investigate whether composite yogurt with acacia dietary fiber and Bifidobacterium lactis (B. lactis) has additive effects in irritable bowel syndrome (IBS). METHODS: A total of 130 patients were randomly allocated to consume, twice daily for 8 wk, either the composite yogurt or the control product. The composite yogurt contained acacia dietary fiber and high-dose B. lactis together with two classic yogurt starter cultures. Patients were evaluated using the visual analogscale via a structured questionnaire administered at baseline and after treatment. RESULTS: Improvements in bowel habit satisfaction and overall IBS symptoms from baseline were significantly higher in the test group than in the control group (27.16 vs 15.51, P = 0.010, 64.2 ± 17.0 vs 50.4 ± 20.5, P < 0.001; respectively). In constipation-predominant IBS, improvement in overall IBS symptoms was significantly higher in the test group than in the control group (72.4 ± 18.4 vs 50.0 ± 21.8, P < 0.001). In patients with diarrhea-predominant IBS, improvement in bowel habit satisfaction from baseline was significantly higher in the test group than in the control group (32.90 vs 7.81, P = 0.006). 16Co-administration of ursodeoxycholic acid with rosuvastatin/ezetimibe in a non-alcoholic fatty liver disease model显示文摘Background Ursodeoxycholic acid(UDCA),statins,and ezetimibe(EZE)have demonstrated beneficial effects against nonalcoholic fatty liver disease(NAFLD).We investigated the efficacy of the combination of UDCA and the mix of rosuvastatin(RSV)/EZE in the treatment of NAFLD.Methods NAFLD mouse models were developed by injecting thioacetamide,fasting,and high-carbohydrate refeeding,highfat diet,and choline-deficient L-amino acid-defined high-fat diet(CDAHFD).Low-dose UDCA(L-UDCA;15 mg/kg)or highdose UDCA(H-UDCA;30 mg/kg)was administered with RSV/EZE.We also employed an in vitro model of NAFLD developed using palmitic acid-treated Hepa1c1c7 cells.Results Co-administration of RSV/EZE with UDCA significantly decreased the collagen accumulation,serum alanine aminotransferase(ALT)levels,and mRNA levels of fibrosis-related markers than those observed in the vehicle group in thioacetamide-treated mice(all P<0.01).In addition,in the group fasted and refed with a high-carbohydrate diet,UDCA/RSV/EZE treatment decreased the number of apoptotic cells and serum ALT levels compared with those observed in the vehicle group(all P<0.05).Subsequently,H-UDCA/RSV/EZE treatment decreased the number of ballooned hepatocytes and stearoyl-CoA desaturase 1(SCD-1)mRNA levels(P=0.027)in the liver of high-fat diet-fed mice compared with those observed in the vehicle group.In the CDAHFD-fed mouse model,UDCA/RSV/EZE significantly attenuated collagen accumulation and fibrosis-related markers compared to those observed in the vehicle group(all P<0.05).In addition,UDCA/RSV/EZE treatment significantly restored cell survival and decreased the protein levels of apoptosis-related markers compared to RSV/EZE treatment in palmitic acid-treated Hepa1c1c7 cells(all P<0.05).Conclusion Combination therapy involving UDCA and RSV/EZE may be a novel strategy for potent inhibition of NAFLD progression.Sang Hyun Seo Da Hyun Lee Yu Seol Lee Kyung Joo Cho Hye Jung Park Hye Won Lee Beom Kyung Kim Jun Yong Park Do Young Kim Sang Hoon Ahn Soo Han Bae Seung Up Kim 2022Gastroenterology Report2022,10,1:0
17Prediction of nephrotoxicity induced by cisplatin combination chemotherapy in gastric cancer patients显示文摘AIM:To evaluate the treatment options for nephrotoxicity due to cisplatin combination chemotherapy.METHODS:We retrospectively reviewed patients who had received cisplatin combination chemotherapy for gastric cancer between January 2002 and December 2008.We investigated patients who had shown acute renal failure(ARF),and examined their clinical characteristics,laboratory data,use of preventive measures,treatment cycles,the amount of cisplatin administered,recovery period,subsequent treatments,and renal status between the recovered and unrecovered groups.RESULTS:Forty-one of the 552 patients had serum creatinine(SCR)levels greater than 1.5 mg/dL.We found that pre-ARF SCR,ARF SCR,and ARF glomerular filtration rates were significantly associated with renal status postARF between the two groups(P=0.008,0.026,0.026,respectively).On the receiver operating characteristic curve of these values,a 1.75 mg/dL ARF SCR value had 87.5%sensitivity and 84.8%specificity(P=0.011).CONCLUSION:Cessation or reduction of chemotherapy should be considered for patients who have an elevation of SCR levels during cisplatin combination chemotherapy.Hyung Hwan Moon Kyung Won Seo Ki Young Yoon Yeon Myung Shin Kyung Hyun Choi Sang Ho Lee 2011World Journal of Gastroenterology2011,17,30:0
