| 1 | Clinical characteristics of Helicobacter pylori-negative drugnegative peptic ulcer bleeding显示文摘AIM:To investigate the clinical characteristics and outcomes of idiopathic Helicobacter pylori(H.pylori)-negative and drug-negative]peptic ulcer bleeding(PUB).METHODS:A consecutive series of patients who experienced PUB between 2006 and 2012 was retrospectively analyzed.A total of 232 patients were enrolled in this study.The patients were divided into four groups according to the etiologies of PUB:idiopathic,H.pylori-associated,drug-induced and combined(H.pylori-associated and drug-induced)types.We compared the clinical characteristics and outcomes between the groups.When the silver stain or rapid urease tests were H.pylori-negative,we obtained an additional biopsy specimen by endoscopic re-examination and performed an H.pylori antibody test 6-8 wk after the initial endoscopic examination.For a diagnosis of idiopathic PUB,a negative result of an H.pylori antibody test was confirmed.In all cases,re-bleeding was confirmed by endoscopic examination.For the risk assessment,the Blatchford and the Rockall scores were calculated for all patients.RESULTS:For PUB,the frequency of H.pylori infection was 59.5%(138/232),whereas the frequency of idiopathic cases was 8.6%(20/232).When idiopathic PUB was compared to H.pylori-associated PUB,the idiopathic PUB group showed a higher rate of rebleeding after initial hemostasis during the hospital stay(30%vs 7.4%,P = 0.02).When idiopathic PUB was compared to drug-induced PUB,the patients in the idiopathic PUB group showed a higher rate of rebleeding after initial hemostasis upon admission(30%vs 2.7%,P < 0.01).When drug-induced PUB was compared to H.pylori-associated PUB,the patients in the drug-induced PUB were older(68.49 ± 14.76 years vs 47.83 ± 15.15 years,P< 0.01) and showed a higher proportion of gastric ulcer(77%vs 49%,P < 0.01).However,the Blatchford and the Rockall scores were not significantly different between the two groups.Among the patients who experienced drug-induced PUB,no significant differences were found with respect to clinical characteristics,irrespective of H.pylori infection.CONCLUSION:Idiopathic PUB has unique clinical characteristics such as re-bleeding after initial hemostasis upon admission.Therefore,these patients need to undergo close surveillance upon admission. | Woo Chul Chung Eun Jung Jeon Dae Bum Kim Hea Jung Sung Yeon-Ji Kim Eun Sun Lim Min-ah Kim Jung Hwan Oh | 2015 | World Journal of Gastroenterology2015,21,28: | 8 |
| 2 | Automatic Discontinuity Classification of Wind-turbine Blades Using A-scan-based Convolutional Neural Network显示文摘Recent development trends in wind power generation have increased the importance of the safe operation of wind-turbine blades(WTBs). To realize this objective, it is essential to inspect WTBs for any defects before they are placed into operation. However, conventional methods of fault inspection in WTBs can be rather difficult to implement, since complex curvatures that characterize the WTB structures must ensure accurate and reliable inspection. Moreover, it is considered useful if inspection results can be objectively and consistently classified and analyzed by an automated system and not by the subjective judgment of an inspector. To address this concern,the construction of a pressure-and shape-adaptive phased-array ultrasonic testing platform, which is controlled by a nanoengine operation system to inspect WTBs for internal defects, has been presented in this paper. An automatic classifier has been designed to detect discontinuities in WTBs by using an A-scanimaging-based convolutional neural network(CNN). The proposed CNN classifier design demonstrates a classification accuracy of nearly 99%. | 4 | Cancer-associated fibroblast-derived secreted phosphoprotein 1 contributes to resistance of hepatocellular carcinoma to sorafenib and lenvatinib显示文摘Background:Cancer-associated fibroblasts(CAFs)play an important role in the induction of chemo-resistance.This study aimed to clarify the mechanism underlying CAF-mediated resistance to two tyrosine kinase inhibitors(TKIs),sorafenib and lenvatinib,and to identify a novel therapeutic target for overcoming TKI resistance in hepatocellular carcinoma(HCC).Methods:We performed a systematic integrative analysis of publicly available gene expression datasets and whole-transcriptome sequencing data from 9 pairs of CAFs and para-cancer fibroblasts isolated from human HCC and para-tumor tissues,respectively,to identify key molecules that might induce resistance to TKIs.We then performed in vitro and in vivo experiments to validate selected targets and related mechanisms.The associations of plasma secreted phosphoprotein 1(SPP1)expression levels before sorafenib/lenvatinib treatment with progression-free survival(PFS)and overall survival(OS)of 54 patients with advanced HCC were evaluated using Kaplan-Meier and Cox regression analysis.Results:Bioinformatic analysis identified CAF-derived SPP1 as a candidate molecule driving TKI resistance.SPP1 inhibitors reversed CAF-induced TKI resistance in vitro and in vivo.CAF-derived SPP1 activated rapidly accelerated fibrosarcoma(RAF)/mitogen-activated protein kinase(MAPK)and phosphatidylinositol 3-kinase(PI3K)/protein kinase B(AKT)/mammalian target of rapamycin(mTOR)through the integrin-protein kinase C-alpha(PKCα)signaling pathway and promoted epithelial-to-mesenchymal transition(EMT).A high plasma SPP1 level before TKI treatment was identified as an independent predictor of poor PFS(P=0.026)and OS(P=0.047)in patients with advanced HCC after TKI treatment.Conclusions:CAF-derived SPP1 enhances TKI resistance in HCC via bypass activation of oncogenic signals and EMT promotion.Its inhibition represents a promising therapeutic strategy against TKI resistance inHCC.Moreover,plasma SPP1 level before TKI treatment represents a potential biomarker for treatment response prediction. | JungWoo Eun Jung Hwan Yoon Hye Ri Ahn Seokhwi Kim Young Bae Kim Su Bin Lim Won Park TaeWook Kang Geum Ok Baek Moon Gyeong Yoon Ju A Son Ji HyangWeon Soon Sun Kim Hyo Jung Cho Jae Youn Cheong | 2023 | Cancer Communications2023,43,4: | 2 |

| 14 | Blockade of PD-L1/PD-1 signaling promotes osteo-/odontogenic differentiation through Ras activation显示文摘The programmed cell death ligand 1(PD-L1)and its receptor programmed cell death 1(PD-1)deliver inhibitory signals to regulate immunological tolerance during immune-mediated diseases.However,the role of PD-1 signaling and its blockade effect on human dental pulp stem cells(h DPSCs)differentiation into the osteo-/odontogenic lineage remain unknown.We show here that PD-L1 expression,but not PD-1,is downregulated during osteo-/odontogenic differentiation of h DPSCs.Importantly,PD-L1/PD-1 signaling has been shown to negatively regulate the osteo-/odontogenic differentiation of h DPSCs.Mechanistically,depletion of either PD-L1 or PD-1 expression increased ERK and AKT phosphorylation levels through the upregulation of Ras enzyme activity,which plays a pivotal role during h DPSCs osteo-/odontogenic differentiation.Treatment with nivolumab(a human anti-PD-1 monoclonal antibody),which targets PD-1 to prevent PD-L1 binding,successfully enhanced osteo-/odontogenic differentiation of h DPSCs through enhanced Ras activity-mediated phosphorylation of ERK and AKT.Our findings underscore that downregulation of PD-L1 expression accompanies during osteo-/odontogenic differentiation,and h DPSCs-intrinsic PD-1 signaling inhibits osteo-/odontogenic differentiation.These findings provide a significant basis that PD-1 blockade could be effective immunotherapeutic strategies in h DPSCs-mediated dental pulp regeneration. | So Mi Jeon Je Sun Lim Su Hwan Park Hyung Joon Kim Hyung-Ryong Kim Jong-Ho Lee | 2022 | International Journal of Oral Science2022,14,2: | 0 |
