1心肺脑复苏研究的现状和对策显示文摘自从40多年前现代心肺复苏术确立以来,已经成功地拯救了无数的生命。尽管如此,当前心跳骤停和心肺脑复苏仍然是医学上的难题之一。国内外的专家们长期以来致力于本领域的研究,以期寻找最佳的方法从而最大限度地提高生存率和减少致残率。在这一探索过程中,以Peter Safar和Max Harry Weil等为首的国际先驱们,分别在各自的研究领域中,对心肺脑复苏做出了巨大的贡献。从1992年的国际心肺复苏指南首次发行到2005年复苏指南的出版,从1975年举办的首届国际Wolf Creek心肺复苏高峰论坛到2005年的第八届Wolf Creek心肺复苏高峰论坛,在这一系列现代心肺脑复苏研究过程中,即便作为有多年深厚感情的好朋友,为了共同目标,他们往往为学术上的分歧激烈争辩。正是他们锲而不舍、严谨治学、无私的科学态度,极大地推动了心肺脑复苏研究的向前发展。在此我们向这些先驱们致以崇高敬意!Wanchun Tang 方向韶 2007中华急诊医学杂志2007,16,1:26

3肾上腺素作为血管收缩剂在心肺复苏中的使用显示文摘心肺复苏(CPR)中,尽快恢复心脏和脑的血供是成功恢复自主循环的重要决定因素。这两个重要脏器的血流量主要依赖于两个因素:人工循环所产生的心输出量和周围血管阻力。目前认为心跳骤停时所产生的代谢性血管舒张物质(包括腺苷、二氧化碳、乳酸和氢离子等)和神经性的血管张力降低会导致全身血管阻力降低和心、脑血循环量降低。由于CPR期间全身血管阻力显著降低,因此使用肾上腺素能血管收缩剂,尤其是肾上腺素,受到了业界的广泛认同。这类药物能增加动脉阻力,从而增加CPR期间的主动脉舒张压、冠脉灌注压和心脏血流量。而电除颤成功与否有赖于心脏血流量的恢复,尤其是心跳骤停持续时间较长时更是如此。Wanchun Tang 万智 2008中华急诊医学杂志2008,17,1:4
