| 1 | Adjuvant probiotics improve the eradication effect of triple therapy for Helicobacter pylori infection显示文摘AIM:To investigate whether the addition of probiotics can improve the eradication effect of triple therapy for Helicobacter pylori (H. pylori ) infection. METHODS:This open randomized trial recruited 234 H. pylori positive gastritis patients from seven local centers. The patients were randomized to one-week standard triple therapy (omeprazole 20 mg bid , clarithromycin 500 mg bid , and amoxicillin 1000 mg bid ; OCA group, n = 79); two weeks of pre-treatment with probiotics, containing 3 × 107 Lactobacillus acidophilus per day, prior to one week of triple therapy (POCA group, n = 78); or one week of triple therapy followed by two weeks of the same probiotics (OCAP group, n = 77). Successful eradication was defined as a negative C13 or C14 urease breath test four weeks after triple therapy. Patients were asked to report associated symptoms at baseline and during follow-up, and side effects related to therapy were recorded. Data were analyzed by both intention-to-treat (ITT) and per-protocol (PP) methods. RESULTS:PP analysis involved 228 patients, 78 in the OCA, 76 in the POCA and 74 in the OCAP group. Successful eradication was observed in 171 patients; by PP analysis, the eradication rates were significantly higher (P = 0.007 each) in the POCA (62/76; 81.6%, 95% CI 72.8%-90.4%) and OCAP (61/74; 82.4%, 95% CI 73.6%-91.2%) groups than in the OCA group (48/78; 61.5%, 95% CI 50.6%-72.4%). ITT analysis also showed that eradication rates were significantly higher in the POCA (62/78; 79.5%, 95% CI 70.4%-88.6%) and OCAP (61/77; 79.2%, 95% CI 70%-88.4%) groups than in the OCA group (48/79; 60.8%, 95% CI 49.9%-71.7%), (P = 0.014 and P = 0.015). The symptom relieving rates in the POCA, OCAP and OCA groups were 85.5%, 89.2% and 87.2%, respectively. Only one of the 228 patients experienced an adverse reaction. | 3 | Controlled attenuation parameter for non-invasive assessment of hepatic steatosis in Chinese patients显示文摘AIM:To evaluate the performance of a novel non-invasive controlled attenuation parameter(CAP)to assess liver steatosis.METHODS:This was a multi-center prospective cohort study.Consecutive patients(aged≥18 years)who had undergone percutaneous liver biopsy and CAP measurement were recruited from three Chinese liver centers.Steatosis was categorized as S0:<5%;S1:5%-33%;S2:34%-66%;or S3:≥67%,according to the nonalcoholic fatty liver disease(NAFLD)activity score.The FibroScan?502 equipped with the M probe(Echosens,Paris,France)was used to capture both CAP and liver stiffness measurement values simultaneously.Receiver operating characteristic curves were plotted,and the areas under the curves were calculated to determine the diagnostic efficacy.The accuracy of the CAP values at the optimal thresholds was defined by maximizing the sum of sensitivity and specificity(maximum Youden index).RESULTS:A total of 152 patients were recruited,including 52(34.2%)patients with NAFLD and 100(65.8%)with chronic hepatitis B(CHB)virus infection.After adjustment,the steatosis grade(OR=37.12;95%CI:21.63-52.60,P<0.001)and body mass index(BMI,OR=6.20;95%CI:2.92-9.48,P<0.001)were found independently associated with CAP by multivariate linear regression analysis.CAP was not influenced by inflammation,fibrosis or aetiology.The median CAP values and interquartile ranges among patients with S0,S1,S2 and S3 steatosis were 211(181-240)dB/m,270(253-305)dB/m,330(302-360)dB/m,and 346(313-363)dB/m,respectively.The cut-offs for the CAP values in all patients with steatosis≥5%,≥34%and≥67%were 253 dB/m,285 dB/m and 310 dB/m,respectively.The areas under the curves were 0.92,0.92and 0.88 for steatosis≥5%,≥34%and≥67%,respectively.No significant differences were found in the CAP values between the NAFLD group and the CHB group in each steatosis grade.CONCLUSION:CAP appears to be a promising tool for the non-invasive detection and quantification of hepatic steatosis,but is limited by BMI. | Feng Shen Rui-Dan Zheng Yu-Qiang Mi Xiao-Ying Wang Qin Pan Guang-Yu Chen Hai-Xia Cao Ming-Li Chen Liang Xu Jian-Neng Chen Yi Cao Rui-Nan Zhang Lei-Ming Xu Jian-Gao Fan | 2014 | World Journal of Gastroenterology2014，20，16： | 55 |

| 5 | Correlation of E-selectin gene polymorphisms with risk of ischemic stroke A meta-analysis显示文摘OBJECTIVE: To investigate the association of E-selectin S128R polymorphisms with ischemic stroke. DATA SOURCES: A computer-based online search was conducted in PubMed, Elsevier, Ovid Database, the China National Knowledge Infrastructure, and Wanfang Database between January 1998 and December 2010. STUDY SELECTION: Case-controlled studies addressing the association of the E-selectin polymorphism and ischemic stroke were included in this review. The genotype distribution complied with the Hardy-Weinberg genetic equilibrium. The included reports were evaluated by two authors for strict quality screening. Meta-analysis software, REVMAN 5.1, was used to investigate heterogeneity, pooled odds ratio (OR) and 95% confidence interval (CI) in individual studies. MAIN OUTCOME MEASURES: Genotype and allele distributions at the E-selectin S128R site. RESULTS: Six case-controlled studies were included after screening and application of inclusion and exclusion criteria. There was no heterogeneity in the genotype and allele frequencies, and no publication bias was found. Meta-analysis of the pooled data showed that the OR value of the (AC+CC)/AA genotype was 1.93 (95% CI: 1.55-2.41, Z = 5.80, P < 0.000 01), and the OR for the C/A allele was 1.80 (95% CI: 1.47-2.22, Z = 5.59, P < 0.000 01) in the ischemic stroke group, compared with control group. Results of pooled data in Chinese subjects showed that the OR value of (AC+CC)/AA was 2.36 (95% CI: 1.68-3.31, Z = 4.99, P < 0.000 01), and the OR value of the C/A allele was 2.25 (95% CI: 1.63-3.12, Z = 4.89, P < 0.000 01). This article presents an overview of potential network architecture and highlights several promising techniques which could be employed in the future 5G systems. | 8 | Gut microbiota dysbiosis in patients with nonalcoholic fatty liver disease显示文摘BACKGROUND:Gut microbiota plays a significant role in the pathogenesis of non-alcoholic fatty liver disease(NAFLD)This study aimed to assess the contribution of gut microbiota dysbiosis to the pathogenesis of NAFLD.METHODS:Forty-seven human feces samples(25 NAFLD patients and 22 healthy subjects) were collected and 16 S r DNA amplicon sequencing was conducted on Hiseq 2000 platform Discrepancy of species composition between controls and NAFLD group was defined by Metastats analysis under P value<0.01.RESULTS:NAFLD patients harbored lower gut microbiota diversity than healthy subjects did.In comparison to the control group,the Proteobacteria(13.50%) and Fusobacteria(2.76%) phyla were more abundant in NAFLD patients.Additionally,the Lachnospiraceae(21.90%),Enterobacteriaceae(12.02%),Erysipelotrichaceae(3.83%),and Streptococcaceae(1.39%) families,as well as the Escherichia_Shigella(10.84%)Lachnospiraceae_Incertae_Sedis(7.79%),and Blautia(4.95%)genera were enriched in the NAFLD group.However,there was a lower abundance of Prevotella in the NAFLD group than that in the control group(5.83% vs 27.56%,P<0.01)The phylum Bacteroidetes(44.63%) also tended to be more abundant in healthy subjects,and the families Prevotellaceae(28.66%) and Ruminococcaceae(26.44%) followed the same trend.Compared to those without non-alcoholic steatohepatitis(NASH),patients with NASH had higher abundance of genus Blautia(5.82% vs 2.25%;P=0.01) and the corresponding Lachnospiraceae family(24.33% vs 14.21%;P<0.01).Patients with significant fibrosis had a higher abundance of genus Escherichia_Shigella(12.53% vs 1.97%;P<0.01) and the corresponding Enterobacteriaceae family(13.92% vs 2.07%;P<0.01) compared to those with F0/F1 fibrosis.CONCLUSIONS:NAFLD patients and healthy subjects harbor varying gut microbiota.In contrast to the results of previous research on children,decreased levels of Prevotella might be detrimental for adults with NAFLD.The increased level of the genus Blautia,the family Lachnospiraceae,the genus Escherichia_Shigella,and the family Enterobacteriaceae may be a primary contributor to NAFLD progression. | Feng Shen Rui-Dan Zheng Xing-Qiang Sun Wen-Jin Ding Xiao-Ying Wang Jian-Gao Fan | 2017 | Hepatobiliary & Pancreatic Diseases International2017，16，4： | 46 |

| 10 | Clinical efficacy of tolvaptan for treatment of refractory ascites in liver cirrhosis patients显示文摘AIM: To evaluate the efficacy and safety of tolvaptan to treat refractory ascites in decompensated liver cirrhosis patients with or without further complications, such as hepatorenal syndrome and/or hepatocellular carcinoma.METHODS: Thirty-nine patients(mean age 55 years, males: 32) with decompensated liver cirrhosis and refractory ascites were enrolled. All patients received a combination of tolvaptan(15 mg/d for 5-14 d) and diuretics(40-80 mg/d of furosemide and 80-160 mg/d of spironolactone). The etiology of cirrhosis included hepatitis B(69.2%), hepatitis C(7.7%) and alcohol-in-duced(23.1%). Changes in the urine excretion volume, abdominal circumference and edema were assessed. The serum sodium levels were also measured, and adverse events were recorded. A follow-up assessment was conducted 1 mo after treatment with tolvaptan.RESULTS: Tolvaptan increased the mean urine excretion volume(1969.2 ± 355.55 mL vs 3410.3 ± 974.1 mL, P < 0.001), and 89.7% of patients showed improvements in their ascites, 46.2% of whom showed significant improvements. The overall efficacy of tolvaptan in all patients was 89.7%; the efficacies in patients with hepatocellular carcinoma and hepatorenal syndrome were 84.2% and 77.8%, respectively. The incidence of hyponatremia was 53.8%. In patients with hyponatremia, the serum sodium levels increased after tolvaptan treatment(from 128.1 ± 4.22 mEq/L vs 133.1 ± 3.8 mEq/L, P < 0.001). Only mild drug-related adverse events, including thirst and dry mouth, were observed. | 11 | Caspase-11/4 and gasdermin D-mediated pyroptosis contributes to podocyte injury in mouse diabetic nephropathy显示文摘Diabetic nephropathy(DN)is characterized by sterile inflammation with continuous injury and loss of renal inherent parenchyma cells.Podocyte is an essential early injury target in DN.The injury and loss of podocytes are closely associated with proteinuria,the early symptom of renal injury in DN.However,the exact mechanism for podocyte injury and death in DN remains ambiguous.In this study we investigated whether pyroptosis,a newly discovered cell death pathway was involved in DN.Diabetic mice were generated by high-fat diet/STZ injections.We showed that the expression levels of caspase-11 and cleavage of gasdermin D(GSDMD-N)in podocytes were significantly elevated,accompanied by reduced expression of podocyte makers nephrin and podocin,loss and fusion in podocyte foot processes,increased inflammatory cytokines NF-κB,IL-1β,and IL-18,macrophage infiltration,glomerular matrix expansion and increased urinary albumin to creatinine ratio(UACR).All these changes in diabetic mice were blunted by knockout of caspase-11 or GSDMD.Cultured human and mouse podocytes were treated with high glucose(30 mM),which significantly increased the expression levels of caspase-11 or caspase-4(the homolog of caspase-11 in human),GSDMD-N,NF-κB,IL-1β,and IL-18,and decreased the expression of nephrin and podocin.Either caspase-4 or GSDMD knockdown by siRNA significantly blunted these changes.In summary,our results demonstrate that caspase-11/4 and GSDMD-mediated pyroptosis is activated and involved in podocyte loss under hyperglycemia condition and the development of DN. | Qian Cheng Jing Pan Zhuan-li Zhou Fan Yin Hong-yan Xie Pan-pan Chen Jing-yao Li Pei-qing Zheng Li Zhou Wei Zhang Jun Liu Li-min Lu | 2021 | Acta Pharmacologica Sinica2021，42，6： | 33 |
| 12 | Sorafenib in treatment of patients with advanced hepatocellular carcinoma:a systematic review显示文摘BACKGROUND: Sorafenib has become the standard first-line treatment for patients with advanced hepatocellular carcinoma (HCC). This study aimed to assess the efficacy and safety of sorafenib in advanced HCC patients and explore its true value for specific subgroups. DATA SOURCES: A computer-based systematic search from January 2005 to June 2011 with 'sorafenib' and 'advanced hepatocellular carcinoma' as search terms was performed for possible clinical trials. Hazard ratios (HR) and their 95% confidence intervals (CI) for overall survival (OS) and time to progression (TTP), rates of partial response (PR), rates of toxicity effects, and details of subgroup analysis were extracted. Meta-analyses were done using the software Review Manager (version 5.0). RESULTS: Six trials with 1164 patients were included. Based on three randomized controlled trials, the pooled HR (sorafenib/ placebo) was 0.66 for OS (95% CI: 0.56-0.78; P<0.00001) and 0.57 for TTP (95% CI: 0.47-0.68; P<0.00001). The pooled odds ratio (OR) for PR was 2.96 (95% CI: 0.96-9.15; P=0.06). For three single-arm trials, the pooled HR was 0.69 for OS (95% CI: 0.56-0.84; P=0.0002) and 0.64 for TTP (95% CI: 0.52-0.78; P<0.00001). The pooled OR for PR in three single-arm trials was 3.56 (95% CI: 1.22-10.39; P=0.02). Subgroup analysis indicated that sorafenib was less effective in patients with extrahepatic spread (with: P=0.13 vs without: P<0.0001), with normal alpha-fetoprotein level (AFP) (P=0.15 vs elevated: P=0.0006), and with elevated level of serum bilirubin (P=0.06 vs normal: P=0.0009). Sorafenib-based therapy significantly increased the risk of grade 3/4 hand-foot skin reaction, diarrhea, fatigue, and rash/desquamation.CONCLUSIONS: Sorafenib-based therapy benefits advanced HCC patients. The expression of classical PKC (cPKC)isoenzymes in SGC7901 cells and SGC7901/VCR cells weredetected using immunofluorescent cytochemistry, laserconfocal scanning microscope and Wsstern blot. The effectsof anti-PKC isoenzymes antibody of adriamycinaccumulation in SGC7901/VCR cells were determined usingflow cytometric analysis.RESULTS: (1) SGC7901 cells exhibited positive staining ofPKC-α. SGC7901/VCR cells exhibited stronger staining ofPKC-α than SGC7901 cells. The higher dosage vincristineselected, the much stronger staining of PKC-α was observedon SGC7901/VCR cells. (2) Both SGC7901 and SGC7901/VCRcells exhibited positive staining of PKC-βⅠ and PKC-βⅡ withno significant difference. ( 3 ) Compared with SGC7901,SGC7901/VCR cells had decreased adriamycin accumulationand retention. | 16 | International consensus statement on robotic hepatectomy surgery in 2018显示文摘The robotic surgical system has been applied in liver surgery. However,controversies concerns exist regarding a variety of factors including the safety,feasibility, efficacy, and cost-effectiveness of robotic surgery. To promote the development of robotic hepatectomy, this study aimed to evaluate the current status of robotic hepatectomy and provide sixty experts' consensus and recommendations to promote its development. Based on the World Health Organization Handbook for Guideline Development, a Consensus Steering Group and a Consensus Development Group were established to determine the topics, prepare evidence-based documents, and generate recommendations. The GRADE Grid method and Delphi vote were used to formulate the recommendations. A total of 22 topics were prepared analyzed and widely discussed during the 4 meetings. Based on the published articles and expert panel opinion, 7 recommendations were generated by the GRADE method using an evidence-based method, which focused on the safety, feasibility, indication,techniques and cost-effectiveness of hepatectomy. | 17 | Establishing dynamic thresholds for potato nitrogen status diagnosis with the SPAD chlorophyll meter显示文摘The hand-held soil plant analysis development(SPAD) chlorophyll meter has proved to be a promising tool in evaluating the nitrogen status of the potato and guiding fertilization recommendations. In the process of N evaluation of potato plants and N recommendation, it is critical to establish the threshold SPAD value(SPAD reading), below which nitrogen supplement is required. And taking convenient using into account, the threshold needs to be dynamic throughout the potato growing season so that the users can test their potato plants and make fertilization decision at any growing time of potato. To complete this goal, field experiments with different nitrogen supply levels were conducted in different sites in northern China from 2009 to 2011. The results showed that threshold SPAD values decrease as the growing season progresses for all cultivars and planting sites. | 19 | Sutureless Intestinal Anastomosis with a Novel Device of Magnetic Compression Anastomosis显示文摘Objective To explore the feasibility and efficiency of a novel magnetic compression anastomats(MCAs) in intestinal anastomosis.Methods A total of 36 male mongrel canines underwent intestinal anastomosis using traditional hand-sewn(n=18) or a novel MCAs(n=18).We compared the anastomosis time,postoperative complications,bursting strength of anastomoses,gross appearance,and pathology between two groups at each time-point of follow-up.Results The mean anastomosis time with MCAs was significantly less than that with hand-sewn(8.50±1.95 vs.31.1±4.32 minutes,P<0.001).The blood stools and intussusceptions occurred in both groups during follow-up period.Only 1 mongrel canine receiving intestinal anastomosis by MCAs experienced anastomotic leakage.The average bursting pressure of anastomoses obtained from mongrel canines undergoing intestinal anastomosis by MCAs was significantly higher than that by traditional hand-sewn at 1 week's follow-up time(P<0.05).Gross appearance of the anastomoses constructed by MCAs was relatively smoother and flatter.Pathological evalution of anastomoses revealed that general inflammation was greater in hand-sewn anastomoses than magnetic anastomosis.Conclusion The magnetic compression anastomat is a safe and effective device of sutureless intestinal anastomosis in canine models. | Chao Fan Jia Ma Hong-ke Zhang Rui Gao Jian-hui Li Liang Yu Zheng Wu Yi Lv | 2011 | Chinese Medical Sciences Journal2011，26，3： | 20 |
