| 6 | Extracorporeal membrane oxygenation in adult patients with sepsis and septic shock: Why, how, when, and for whom显示文摘Sepsis and septic shock remain the leading causes of death in intensive care units.Some patients with sepsis fail to respond to routine treatment and rapidly progress to refractory respiratory and circulatory failure,necessitating extracorporeal membrane oxygenation(ECMO).However,the role of ECMO in adult patients with sepsis has not been fully established.According to existing studies,ECMO may be a viable salvage therapy in carefully selected adult patients with sepsis.The choice of venovenous,venoarterial,or hybrid ECMO modes is primarily determined by the patient’s oxygenation and hemodynamics(distributive shock with preserved cardiac output,septic cardiomyopathy(left,right,or biventricular heart failure),or right ventricular failure caused by acute respiratory distress syndrome).Veno-venous ECMO can be used in patients with sepsis and severe acute respiratory distress syndrome when conventional mechanical ventilation fails,and early application of veno-arterial ECMO in patients with sepsis-induced refractory cardiogenic shock may be critical in improving their chances of survival.When ECMO is indicated,the choice of an appropriate mode and determination of the optimal timing of initiation and weaning are critical,particularly in an experienced ECMO center.Furthermore,some special issues,such as ECMO flow,anticoagulation,and antibiotic therapy,should be noted during the management of ECMO support. | Hongling Zhang Youdong Xu Xin Huang Shunyin Yang Ruiting Li Yongran Wu Xiaojing Zou Yuan Yu You Shang | 2024 | Journal of Intensive Medicine2024,4,1: | 0 |

| 9 | Evolution of chemical composition of fogwater in winter in Chengdu, China显示文摘Two sampling sites representing the urban and suburban area of Chengdu, China were sampled and analyzed for selected chemicals to characterize the evolution of the chemical composition of fogwater. A trend of total organic carbon (TOC) > total nitrogen (TN) > total inorganic carbon (TIC) was observed for both sites. Variation of inorganic ions indicated that inorganic pollutants were not accumulated in the fog. Concentrations of n-alkanes (C11-C37) at the urban site ranged from 7.58 to 27.75 ng/mL while at the suburban site concentrations were 2.57-21.55 ng/mL. The highest concentration of n-alkanes was observed in the mature period of fog (393.12 ng/mL) which was more than ten times that in the fog formation period (27.83 ng/mL) and the fog dissipation period (14.87 ng/mL). Concentrations of Σ15PAHs were in the range of 7.27-38.52 ng/mL at the urban site and 2.59-22.69 ng/mL at the suburban site. Contents of PAHs in the mature period of fog (27.15 ng/mL) > fog dissipation period (11.59 ng/mL) > fog formation period (6.42 ng/mL). Concentrations of dicarboxylic acids (C5-C9) ranged from 10.92 to 40.78 ng/mL, with glutaric acid (C5) as the dominant dicarboxylic acid. | 10 | A photo-elutable and template-free isothermal amplification strategy for sensitive fluorescence detection of 5-formylcytosine in genomic DNA显示文摘5-Formylcytosine(5fC), as an important epigenetic modification, plays a vital role in diverse biological processes and multiple diseases by regulating gene expression. Owing to the extremely low abundance of 5fC in all mammalian tissues and high structural similarity with other cytosine derivatives, the precise and sensitive detection of 5fC is challenging. Herein, a photo-elutable and template-free isothermal amplification strategy has been proposed for the sensitive detection of 5fC in genomic DNA based on5fC-specific biotinylation, enrichment, photocleavage, and terminal deoxynucleotidyl transferase(Td T)-assisted fluorescence signal amplification, which is termed 5fC-PTIAS. By introducing the highly specific chemolabeling and the one-step photoelution processes, this strategy possesses a minimal nonspecific background as well as a much higher amplification efficiency. With the high signal-to-noise ratio, this strategy can achieve the accurate quantification of 5fC in various biological samples including mouse brain, kidney, and liver, with a limit of detection(LOD) of 0.025‰ in DNA(S/N=3). These results not only confirm the widespread distribution of 5fC but also indicate its significant variation in different tissues and ages.
