Hepatic abscess induced by foreign body:Case report and literature review显示文摘Hepatic abscess due to perforation of the gastrointestinal tract caused by ingested foreign bodies is uncommon. Pre-operative diagnosis is difficult as patients are often unaware of the foreign body ingestion and symptoms and imagiology are usually non-specific. The authors report a case of 62-year-old woman who was admitted with fever and abdominal pain. Further investigation revealed hepatic abscess, without resolution despite antibiotic therapy. A liver abscess resulting from perforation and intra-hepatic migration of a bone coming from the pilorum was diagnosed by surgery. The literature concerning foreign body-induced perforation of the gastrointestinal tract complicated by liver abscess is reviewed.Sofia A Santos Sara CF Alberto Elsa Cruz Eduardo Pires Tomás Figueira lia Coimbra José Estevez Mário Oliveira Luís Novais Joo R Deus 2007World Journal of Gastroenterology2007,13,9:10
Understanding plant drought resistance in a Mediterranean coastal sand dune ecosystem:differences between native and exotic invasive species显示文摘Aims Mediterranean coastal dunes are habitats of great conservation interest, with a distinctive and rich flora. In the last century,Acacia spp.,native from Australia, have been introduced in Portugal, with the objective of stabilizing sand dunes, and since have become dominant in numerous sand dune habitats. This invasion process led to the reduction of native plant species richness, changed soil characteristics and modified habitat’s microclimatic char-acteristics.The aim of this research was to typify and compare,in Mediterranean sand dune ecosystems, the ecophysiological responses to drought of Helichrysum italicum and Corema album, two native species, and Acacia longifolia,an exotic invasive spe-cies.We addressed the following specific objectives:(i)to com-pare water relations and water use efficiencies,(ii)to evaluate water stress,(iii)to assess water use strategies and water sources used by plants and(iv)to evaluate the morphological adaptations at leaf and phyllode level. Methods In order to obtain an integrative view of ecophysiological patterns, water relations and performance measuring methods have been applied:predawn(ψPD)and midday(ψMD)water potential,chloro-phyll a fluorescence, oxygen isotopic composition of xylem, rain and groundwater(δ18O)and leaf carbon isotopic discrimination(Δ13C).The leaf characteristics of the three species,as well as the histochemistry of non-glandular trichome cell walls, were also studied to identify morpho-traits related to drought resistance.Important Findings The results support our initial hypothesis:although A.longifolia clearly possesses a degree of resistance to water stress, such ability is provided by a different water strategy, when compared to native species.Natives relied on morphological adaptations to restrict water loss, whereas the invasive species adjusted the water uptake as a way to balance their limited ability of restricting water loss.We corroborate that woody native species(i)have a conservative water-saving strategy and minor seasonal variations relative to invasive species,(ii)use enriched water sources during drought periods, indicating different water sources and root systems com-paring with invasive species and(iii)present drought leaf morpho-functional adaptations related with limiting water loss.Comparing the physiological performance of invasive and native species can offer causal explanations for the relative success of alien plant invasions on sand dunes ecosystems.Cristina Antunes Ana Júlia Pereira Patrícia Fernandes Margarida Ramos Lia Ascensão Otília Correia Cristina Máguas 2018Journal of Plant Ecology2018,11,1:4
How to manage inflammatory bowel disease during the COVID-19 pandemic:A guide for the practicing clinician显示文摘Managing inflammatory bowel disease(IBD)during the coronavirus disease 2019(COVID-19)pandemic has been a challenge faced by clinicians and their patients, especially concerning whether to proceed with biologics and immunosuppressive agents in the background of a global outbreak of a highly contagious new coronavirus(severe acute respiratory syndrome coronavirus 2,SARS-CoV-2).The knowledge about the impact of this virus on patients with IBD,although it is still scarce,is rapidly evolving. In particular, concerns surrounding medications’impact for IBD on the risk of acquiring SARS-CoV-2 infection or developing COVID-19,and potentially exacerbate viral replication and the COVID-19 course, are a current thinking of both practicing clinicians and providers caring for patients with IBD.Managing patients with IBD infected with SARS-CoV-2 depends on both the clinical activity of the IBD and the occasional development and severity of COVID-19.In this review,we summarize the current data regarding gastrointestinal involvement by SARS-CoV-2 and pharmacologic and surgical management for IBD concerning this infection, and the COVID-19 impact on both the patient's psychological functioning and endoscopy services, and we concisely summarize the telemedicine roles during the COVID-19 pandemic.Júlio Maria Fonseca Chebli Natália Sousa Freitas Queiroz Adérson Omar Mourão Cintra Damião Liliana Andrade Chebli Márcia Henriques de Magalhães Costa Rogério Serafim Parra 2021World Journal of Gastroenterology2021,27,11:2
Appropriateness of outpatient gastrointestinal endoscopy in a non-academic hospital显示文摘AIM:To assess the appropriate use and the diagnostic yield of upper gastrointestinal endoscopy and colonoscopy in this subgroup of patients.METHODS:In total,789 consecutive outpatients referred for gastrointestinal(GI) endoscopy [381 for esophagogastroduodenoscopy(EGD) and 408 for colonoscopy] were prospectively enrolled in the study. The American Society for Gastrointestinal Endoscopy(ASGE) guidelines were used to assess the relationship between appropriateness and the presence of relevant endoscopic f indings.RESULTS:The overall inappropriate rate was 13.3%.The indications for EGD and colonoscopy were, respectively, appropriate in 82.7% and 82.6% of the exams, uncertain in 5.8% and 2.4% and inappropriate in 11.5% and 15%.The diagnostic yield was signif icant higher forEGDs and colonoscopies judged appropriate and uncertain when compared with those considered inappropriate(EGD:36.6% vs 36.4% vs 11.4%,P = 0.004;Colonoscopy:24.3% vs 20.0% vs 3.3%,P = 0.001).Of the 25 malignant lesions detected, all but one was detected in exams judged appropriate or uncertain.CONCLUSION:This study shows a good adherence to ASGE guidelines by the referring physicians and a significant increase of the diagnostic yield in appropriate examinations, namely in detecting neoplastic lesions. It underscores the importance that the appropriateness of the indication assumes in assuring high-quality GI endoscopic procedures.Joo Mangualde Marie I Cremers Ana M Vieira Ricardo Freire lia Gamito Cristina Lobato Ana L Alves Fátima Augusto Ana P Oliveira 2011World Journal of Gastrointestinal Endoscopy2011,3,10:2

Autoimmune gastritis presenting as iron deficiency anemia in childhood显示文摘AIM: To characterize clinical, laboratorial, and histological profile of pediatric autoimmune gastritis in the setting of unexplained iron deficiency anemia investigation.METHODS: A descriptive, observational study including pediatric patients with a diagnosis of autoimmune gastritis(positive parietal cell antibody and gastric corpus atrophy) established in a 6 year period(2006-2011) in the setting of refractory iron deficiency anemia(refractoriness to oral iron therapy for at least 6 mo and requirement for intravenous iron therapy) investigation, after exclusion of other potentially contributing causes of anemia. Helicobacter pylori(H. pylori) infection and anti-secretory therapy were also excluded. Data were retrospectively collected from clinical files, including: demographic data(age, gender, and ethnic background), past medical history, gastrointestinal symptoms, familial history, laboratorial evaluation(Hb, serum ferritin, serum gastrin, pepsinogen Ⅰ/ pepsinogen Ⅱ, B12 vitamin, intrinsic factor autoantibodies, thyroid autoantibodies, and anti-transglutaminase antibodies), and endoscopic and histological findings(HE, Periodic Acid-Schiff/Alcian blue, gastrin, chromogranin A and immunochemistry analysis for CD3, CD20 and CD68). Descriptive statistical analysis was performed(mean, median, and standard deviation).RESULTS: We report a case-series concerning 3 girls and 2 boys with a mean age of 13.6 ± 2.8 years(3 Caucasian and 2 African). One girl had type Ⅰ diabetes. Familial history was positive in 4/5 cases, respectively for autoimmune thyroiditis(2/5), sarcoidosis(1/5) and multiple myeloma(1/5). Laboratorial evaluation on admission included: Hb: 9.5 ± 0.7 g/d L; serum ferritin: 4.0 ± 0.9 ng/m L; serum gastrin: 393 ± 286 pg/m L; low pepsinogen Ⅰ/ pepsinogen Ⅱ ratio in 1/5 patients; normal vitamin B12 levels(analyzed in 3 patients). Endoscopy findings included: duodenal nodularity(2/5) and gastric fold softening(2/5), and histological evaluation showed corpus atrophic gastritis with lymphocytic infiltration(5/5), patchy oxyntic gland mononuclear cell infiltration(5/5), intestinal and/or pseudo-pyloric metaplasia in corpus mucosa(4/5), and enterochromaffin cell hyperplasia(4/5). Immunochemistry for gastrin on corpus biopsies was negative in all cases. Duodenal histology was normal. All biopsies were negative for H. pylori(Giemsa staining and cultural examination). CONCLUSION: We highlight autoimmune gastritis as a diagnosis to be considered when investigating refractory iron deficiency anemia in children, particularly in the setting of a personal/familial history of autoimmune disease, as well as the diagnostic contribution of a careful immunohistological evaluation.Cristina Gon?alves Maria Emília Oliveira Ana M Palha Anabela Ferr?o Anabela Morais Ana Isabel Lopes 2014World Journal of Gastroenterology2014,20,42:1
Prevalence of celiac disease in an urban area of Brazil with predominantly European ancestry显示文摘AIM: To determine the prevalence of celiac disease in a group of volunteer blood donors at a blood bank in the city of Curitiba, Brazil through detection of the serum marker immunoglobulin A (IgA) antitransglutaminase an-tibody.METHODS: Blood samples collected from 2086 healthy subjects at the Paraná State Center for Hematology and Hemotherapy in Curitiba were submitted to ELISA testing for the IgA antitransglutaminase antibody. Positive samples received IgA antiendomysium antibody test through indirect immunofluorescence using human umbilical cord as substrate. Subsequently, patients who were positive on both tests underwent small bowel (distal duodenum) biopsy.RESULTS: Six subjects, four males and two females, tested positive for the two serum markers. Five of the six were submitted to intestinal biopsy (one declined the procedure). Biopsy results revealed changes in the distal duodenum mucosa (three classified as Marsh Ⅲb lesions and two as Marsh Ⅱ lesions). Most donors diagnosed having celiac disease presented multiple symptoms (gastrointestinal tract complaints). One donor reported having a family history of celiac disease (in a niece).CONCLUSION: Among apparently healthy blood donors, the prevalence of biopsy-confirmed celiac disease was approximately 1:417, similar to that seen in European countries.Maria Angélica G Pereira Carmen L Ortiz-Agostinho Iêda Nishitokukado Maria N Sato Adérson OMC Damio Marília L Alencar Clarice P Abrantes-Lemos Eduardo LR Canado Thales de Brito Sérgio O Ioshii Sandra BM Valarini Aytan M Sipahi 2006World Journal of Gastroenterology2006,12,40:1
The ecological performance of metallophyte plants thriving in geochemical islands is explained by the Inclusive Niche Hypothesis显示文摘Aims The Inclusive Niche Hypothesis has not been validated for plants using ecophysiological performance. The few experiments have measured growth and competition but not the physiological response of plants.A metallophyte plant that hyperaccumulates aluminium(Al),Plantago almogravensis, showed a defined spa-tial distribution by occurring mostly on vegetation gaps associ-ated with metalliferous areas(geochemical islands). This case was used to determine,in situ, whether the Inclusive Niche Hypothesis was suitable to explain the extent of the species real-ized niche. Methods The vegetation associated with P.almogravensis geochemical islands in the SW coast of Portugal was mapped. The biotic(neighbouring plants)and abiotic(edaphic)components of the niche were cor-related with parameters of the plant’s ecological and physiological performances(plant density and cover;leaf C and N concentration and isotopic composition;growth). The results were obtained using image analysis, abundance and morphological measures, isotopic signatures and chemical composition.Important Findings The species showed better physiological performance where its ecological performance was lower due to trade-offs with environ-mental constraints. The species’realized niche was mostly limited by shrub competition and soil Al-toxicity.These limits contribute to explain the rarity status of the species:the species has a poor capacity to compete but,due to an enhanced Al-tolerance and Al-hyperaccumulator trait, has the ability to find refuge in geochem-ical islands that are too harsh for most other species. This work, based on ecophysiological field studies, provides support for the Inclusive Niche Hypothesis relating to plant species.Helena C.Serrano Cristina Antunes Manuel J.Pinto Cristina Máguas Maria Amélia Martins-Loução Cristina Branquinho 2015Journal of Plant Ecology2015,8,1:0
Reference genes for quantitative RT-PCR data in gastric tissues and cell lines显示文摘AIM:To evaluate the suitability of reference genes in gastric tissue samples and cell lines.METHODS:The suitability of genes ACTB,B2M,GAPDH,RPL29,and 18S rRNA was assessed in21 matched pairs of neoplastic and adjacent nonneoplastic gastric tissues from patients with gastric adenocarcinoma,27 normal gastric tissues from patients without cancer, and 4 cell lines using reverse transcription quantitative real-time polymerase chain reaction(RT-qPCR).The ranking of the best single and combination of reference genes was determined by NormFinder,geNorm,BestKeeper, and DataAssist. In addition,GenEx software was used to determine the optimal number of reference genes. To validate the results, the mRNA expression of a target gene,DNMT1,was quantified using the different reference gene combinations suggested by the various software packages for normalization.RESULTS:ACTB was the best reference gene for all gastric tissues, cell lines and all gastric tissues plus cell lines.GAPDH+B2M or ACTB+B2M was the best combination of reference genes for all the gastric tissues. On the other hand,ACTB+B2M was the best combination for all the cell lines tested and was also the best combination for analyses involving all the gastric tissues plus cell lines.According to the GenEx software,2 or 3 genes were the optimal number of references genes for all the gastric tissues. The relative quantification of DNMT1 showed similar patterns when normalized by each combination of reference genes. The level of expression of DNMT1 in neoplastic, adjacent non-neoplastic and normal gastric tissues did not differ when these samples were normalized using GAPDH+B2M(P=0.32),ACTB+B2M(P=0.61),or GAPDH+B2M+ACTB(P=0.44).CONCLUSION:GAPDH+B2M or ACTB+B2M is the best combination of reference gene for all the gastric tissues, and ACTB+B2M is the best combination for the cell lines tested.Fernanda Wisnieski Danielle Queiroz Calcagno Mariana Ferreira Leal Leonardo Caires dos Santos Carolina de Oliveira Gigek Elizabeth Suchi Chen Thaís Brilhante Pontes Paulo Pimentel Assumpo Mnica Barauna de Assumpo Smia Demachki Rommel Rodríguez Burbano Marília de Arruda Cardoso Smith 2013World Journal of Gastroenterology2013,19,41:0
