Hepatocellular Carcinoma-Cause,Treatment and Metastasis显示文摘In the recent decades, the incidence of hepatocellular carcinoma (HCC) has been found to be increasing in males in some countries. In China, HCC ranked second of cancer mortality since 1990s. Hepatitis B and C viruses (HBV and HCV) and dietary aflatoxin intake remain the major causative factors of HCC. Surgery plays a major role in the treatment of HCC, particularly for small HCC. Downstaging unresectable huge HCC to smaller HCC and followed by resection will probably be a new approach for further study. Liver transplantation is indicated for small HCC, however, some issues remain to be solved.Different modes of 'regional cancer therapy for HCC' have been tried. Systemic chemotherapy has been disappointing in the past but the future can be promising.Biotherapy, such as cytokines, differentiation inducers,anti-angiogenic agents, gene therapy and tumor vaccine will probably play a role, particularly in the prevention of tumor recurrence. HCC invasiveness is currently the major target of study. Tremendous works have been done at the molecular level, which will provide clues for biomarker of HCC progressionas well as targets for intervention.Zhao-You Tang Liver Cancer Institute & Zhongshan Hospital of Fudan University Professor of Surgery Chairman.Liver Cancer Institute of Fudan University(previous Liver Cancer Institute of Shanghai Medical University)136 Yixueyuan Road,Zhongshan Hospital,Shanghai 200032,China. 2001World Journal of Gastroenterology2001,7,4:213
Epidemiology, etiology, and prevention of esophageal squamous cell carcinoma in China显示文摘Esophageal cancer is one of the most fatal diseases worldwide mainly because of its rapid progression and poor prognosis.Although the incidence of esophageal adenocarcinoma has markedly risen in North America and Europe in the past several decades, esophageal squamous cell carcinoma is still the predominant subtype of esophageal cancer, especially in China. It accounts for more than 90% of all esophageal squamous cell carcinoma cases in China. Geographical differentiation is one of the most distinctive characteristics of esophageal cancer. The progression, risk factors, and prognosis of these two subtypes of esophageal cancer differ. This study reviews the epidemiology, etiology, and prevention of esophageal squamous cell carcinoma in China, thereby providing systematic references for policy-makers who will decide on issues of esophageal cancer prevention and control. He Liang Jin-Hu Fan You-Lin Qiao 2017Cancer Biology & Medicine2017,14,1:69
Can Chinese Medicine Be Used for Prevention of Corona Virus Disease 2019 (COVID-19)? A Review of Historical Classics, Research Evidence and Current Prevention Programs显示文摘Objective:Since December 2019,an outbreak of corona virus disease 2019(COVID-19)occurred in Wuhan, and rapidly spread to almost all parts of China. This was followed by prevention programs recommending Chinese medicine(CM)for the prevention. In order to provide evidenee for CM recommendations,we reviewed ancient classics and human studies. Methods:Historical records on prevention and treatment of infections in CM classics, clinical evidence of CM on the prevention of severe acute respiratory syndrome(SARS)and H1N1 in flue nza,and CM preve ntion programs issued by health authorities in China since the COVID-19 outbreak were retrieved from differe nt databases and websites till 12 February,2020.Research evide nee in eluded data from clinical trials, cohort or other population studies using CM for preventing contagious respiratory virus diseases. Results:The use of CM to prevent epidemics of infectious diseases was traced back to ancient Chinese practice cited in Huangdi's Internal Classic(Huang Di Nei Jing)where preventive effects were recorded. There were 3 studies using CM for prevention of SARS and 4 studies for H1N1 influenza.None of the participants who took CM contracted SARS in the 3 studies. The infection rate of H1N1 in flue nza in the CM group was significantly lower than the non-CM group(relative risk 0.36,95%confidence interval 0.24-0.52;n=4).For prevention of COVID-19,23 provinces in China issued CM programs. The main principles of CM use were to tonify qi to protect from external pathoge ns,disperse wind and discharge heat, and resolve damp ness. The most frequently used herbs in eluded Radix astragali(Huangqi),Radix glycyrrhizae(Gancao),Radix saposhnikoviae(Fangfeng),Rhizoma Atractylodis Macrocephalae(Baizhu),Lonicerae Japonicae Flos(Jinyinhua),and Fructus forsythia(Lianqiao). Conclusions:Based on historical records and human evidenee of SARS and H1N1 influenza prevention,Chinese herbal formula could be an alternative approach for prevention of COVID-19 in high-risk population.Prospective, rigorous population studies are warranted to confirm the potential preventive effect of CM.LUO Hui TANG Qiao-ling SHANG Ya-xi LIANG Shi-bing YANG Ming Nicola Robinson LIU Jian-ping 2020Chinese Journal of Integrative Medicine2020,26,4:61

Eosinophilic gastroenteritis:An unusual type of gastroenteritis显示文摘Eosinophilic gastroenteritis(EGE) is a rare disorder characterized by eosinophilic infiltration of the bowel wall with various gastrointestinal manifestations.Till date only 280 cases have been described in the literature.A high index of suspicion,by excluding other causes of peripheral eosinophilia,is a pre requisite for accurate diagnosis.EGE is an uncommon gastrointestinal disease affecting both children and adults. It was first described by Kaijser in 1937.Presentation may vary depending on location as well as depth and extent of bowel wall involvement and usually runs a chronic relapsing course. This condition can respond to low dose steroid therapy, thereby preventing grave complications like ascites and intestinal obstruction that might need surgical intervention. The natural history of EGE has not been well documented.Eosinophilic gastroenteritis is a chronic, waxing and waning condition.Mild and sporadic symptoms can be managed with reassurance and observation, whereas disabling gastrointestinal(GI) symptom flare-ups can often be controlled with oral corticosteroids. When the disease manifests in infancy and specific food sensitization can be identified, the likelihood of disease remission by late childhood is high.GI obstruction is the most common complication.Fatal outcomes are rare.Sachin B Ingle Chitra R Hinge(Ingle) 2013World Journal of Gastroenterology2013,19,31:47
Epidemiology and risk factors of colorectal cancer in China显示文摘In China, colorectal cancer(CRC) ranked fourth and fifth in the highest incidence and mortality rates of all malignancies in 2018, respectively. Although these rates are below the world average, China placed first worldwide in the number of new CRC cases and CRC-related deaths because of its comparatively large population. This disease represents a threat to the health of population and incurs a heavy economic burden on the society and individuals. CRC has various risk factors, including age, sex, lifestyle, genetic factors, obesity, diabetes, gut microbiota status, and precancerous lesions. Furthermore, incidence and mortality rates of CRC are closely related to socioeconomic development levels, varying according to regional and population characteristics. Prevention is the main strategy to reduce incidence and mortality rates of CRC. This can be achieved through strategies stimulating lifestyle changes, healthy diet habits, and early screening for high-risk individuals. To reduce the burden of CRC, public health officials should promote prevention and management of modifiable risk factors through national policies. The rising incidence and mortality rates of CRC in China may be timely curbed by clarifying specific epidemiological characteristics, optimizing early screening strategies, and strictly implementing diagnosis and treatment guidelines. Thus, this study aimed to collect and report the current research status on epidemiology and risk factors of CRC in China.Yong Yang Zihan Han Xin Li An Huang Jingyi Shi Jin Gu 2020Chinese Journal of Cancer Research2020,32,6:42

MicroRNAs as non-invasive diagnostic biomarkers for gastric cancer:Current insights and future perspectives显示文摘Non-invasive diagnostic biomarkers may contribute to an early identification of gastric cancer(GC) and improve the clinical management.Unfortunately,no sensitive and specific screening biomarkers are available yet and the currently available approaches are limited by the nature of the disease.GC is a heterogenic disease with various distinct genetic and epigenetic events that occur during the multifactorial cascade of carcinogenesis.Micro RNAs(mi RNAs) are commonly deregulated in gastric mucosa during the Helicobacter pylori infection and in stepwise manner from chronic gastritis, through preneoplastic conditions such as atrophic gastritis and intestinal metaplasia,to early dysplasia and invasive cancer.Identification of mi RNAs in blood in 2008 led to a great interest on mi RNA-based diagnostic, prognostic biomarkers in GC.In this review,we provide the most recent systematic review on the existing studies related to mi RNAs as diagnostic biomarkers for GC.Here,we systematically evaluate 75 studies related to differential expression of circulating mi RNAs in GC patients and provide novel view on various heterogenic aspects of the existing data and summarize the methodological differences. Finally,we highlight several important aspects crucial to improve the future translational and clinical research in the field.Alexander Link Juozas Kupcinskas 2018World Journal of Gastroenterology2018,24,30:32

Sorafenib in treatment of patients with advanced hepatocellular carcinoma:a systematic review显示文摘BACKGROUND: Sorafenib has become the standard first-line treatment for patients with advanced hepatocellular carcinoma (HCC). This study aimed to assess the efficacy and safety of sorafenib in advanced HCC patients and explore its true value for specific subgroups. DATA SOURCES: A computer-based systematic search from January 2005 to June 2011 with 'sorafenib' and 'advanced hepatocellular carcinoma' as search terms was performed for possible clinical trials. Hazard ratios (HR) and their 95% confidence intervals (CI) for overall survival (OS) and time to progression (TTP), rates of partial response (PR), rates of toxicity effects, and details of subgroup analysis were extracted. Meta-analyses were done using the software Review Manager (version 5.0). RESULTS: Six trials with 1164 patients were included. Based on three randomized controlled trials, the pooled HR (sorafenib/ placebo) was 0.66 for OS (95% CI: 0.56-0.78; P<0.00001) and 0.57 for TTP (95% CI: 0.47-0.68; P<0.00001). The pooled odds ratio (OR) for PR was 2.96 (95% CI: 0.96-9.15; P=0.06). For three single-arm trials, the pooled HR was 0.69 for OS (95% CI: 0.56-0.84; P=0.0002) and 0.64 for TTP (95% CI: 0.52-0.78; P<0.00001). The pooled OR for PR in three single-arm trials was 3.56 (95% CI: 1.22-10.39; P=0.02). Subgroup analysis indicated that sorafenib was less effective in patients with extrahepatic spread (with: P=0.13 vs without: P<0.0001), with normal alpha-fetoprotein level (AFP) (P=0.15 vs elevated: P=0.0006), and with elevated level of serum bilirubin (P=0.06 vs normal: P=0.0009). Sorafenib-based therapy significantly increased the risk of grade 3/4 hand-foot skin reaction, diarrhea, fatigue, and rash/desquamation.CONCLUSIONS: Sorafenib-based therapy benefits advanced HCC patients. Meanwhile, sorafenib is less effective for patients with extrahepatic spread, with normal AFP level and with elevated level of bilirubin.Xin Zhang, Xin-Rong Yang, Xiao-Wu Huang, Wei-Min Wang, Ruo-Yu Shi, Yang Xu, Zheng Wang, Shuang-Jian Qiu, Jia Fan ,Jian Zhou Liver Cancer Institute, Zhongshan Hospital, Fudan University, Shanghai Key Laboratory for Organ Transplantation, Shanghai 200032, China,Institute of Biomedical Sciences, Fudan University, Shanghai 200032, China 2012Hepatobiliary & Pancreatic Diseases International2012,11,5:29
Clinical research linking Traditional Chinese Medicine constitution types with diseases:a literature review of 1639 observational studies显示文摘OBJECTIVE:To analyze clinical studies on correlations between Traditional Chinese Medicine(TCM)body constitution types and diseases published in the past 10 years, and to provide an evidence base to support the use of such correlations for health maintenance and disease prevention.METHODS:We searched five databases for the period April 2009 to December 2019:China National Knowledge Infrastructure Database,Wanfang Database,China Science and Technology Journal Database,PubMed and Embase.Three types of observational studies on correlation between constitution types and diseases were included:cross-sectional,case-control and cohort studies.Descriptive statistical methods were employed for data analysis.RESULTS:A total of 1639 clinical studies were identified:1452(88.59%)cross-sectional studies,115(7.02%)case-control studies and 72(4.39%)cohort studies covering 30 regions of China and five other countries(Malaysia,South Korea,Singapore,Thailand and France). The collection of studies comprised 19 disease categories and 333 different diseases. The 10 most commonly studied diseases were hypertension, diabetes, stroke, coronary atherosclerotic heart disease(CAHD),sleep disorders, neoplasm of the breast, dysmenorrhea, fatty liver disease, chronic viral hepatitis B and dyslipidemia. We found high distributions for each biased constitution type in different patient populations as follows:Qi-deficiency constitution in stroke, diabetes, chronic obstructive pulmonary disease, acquired immunodeficiency syndrome and hypertension;Yang-deficiency constitution in female infertility, osteoporosis, irritable bowel syndrome, gonarthrosis and dysmenorrhea;Yin-deficiency constitution in hypertension, diabetes, constipation, female climacteric states and osteoporosis;phlegm-dampness constitution in hypertension, stroke, fatty liver disease, diabetes and metabolic syndrome;damp-heat constitution in acne, chronic gastritis, chronic viral hepatitis B,human papillomavirus infection and hyperuricemia;blood-stasis constitution in CAHD,endometriosis and stroke;Qi-stagnation constitution in hyperplasia and neoplasms of the breast, insomnia, depression and thyroid nodules;and inherited-special constitution in asthma and allergic rhinitis.CONCLUSION:Eight biased TCM constitutions were closely related to specific diseases, and could be used to guide individualized prevention and treatment.More rigorously designed studies are recommended to further verify the constitution-disease relationship.Liang Xue Wang Qi Jiang Zeqiang Li Zhuqing Zhang Mengxing Yang Peiying Wang Xin Wang Yanqiu Qin Yuehua Li Tianxing Zhang Tianyi Wang Yi Sun Jianxiang Li Yusi Luo Hui Li Lingru 2020Journal of Traditional Chinese Medicine2020,40,4:29
Diagnosis of Helicobacter pylori : What should be the gold standard?显示文摘Since the discovery of Helicobacter pylori(H. pylori) in 1983, numerous detection methods for the presence of the bacterium have been developed. Each one of them has been associated with advantages and disadvantages. Noninvasive tests such as serology, 13 C urea breath test(UBT) and stool antigen tests are usually preferred by the clinicians. Serology has its own limitation especially in endemic areas while 13 C UBT is technically very demanding. The stool antigen detection method, although specific, is usually associated with poor sensitivity. The 13 C UBT is believed to be specific, but with present revelation of the fact that stomach is colonized by many other urease producing bacteria makes it questionable. Histology, culture, rapid urease test and polymerase chain reaction(PCR) are the tests which are carried out on antral biopsies collected by invasive means. Histology has been proposed to be very sensitive and specific but the question is how by simply looking the morphology of the bacteria in the microscope, one can claim that the curved bacterium is exclusively H. pylori. Rapid urease test(RUT), the doctor's test, is also challenged because the presence of other urease producing bacteria in the stomach cannot be denied. Moreover, RUT has been reported with poor sensitivity specially, when density of the bacterium is low. Isolation of H. pylori is essential to investigate its growth requirements, antibiotic susceptibility testing, studying virulence factor to develop vaccine and many more explorations. It has also got several disadvantages i.e., special condition for transporting, media, incubation and few days waiting for the colonies to appear, apart from the speed essentially needed to process the specimens. Till date, majority of the microbiological laboratories in the world are not equipped and trained to isolate such fastidious bacterium. The option left is PCR methods to detect H. pylori 's DNA in gastric mucosa, gastric juice, saliva, dental plaques and environmental specimens. There are speculations for false positivity due to detection of non-pylori Helicobacters due to genetic sharing; and false negativity due to low bacterial counts and presence of PCR inhibitors. However, specimen collection, transportation and processing do not require speed and special conditions. PCR based diagnosis may be considered as gold standard by designing primers extremely specific to H. pylori and targeting at least more than one conserved genes. Similarly specificity of PCR may be improved by use of internal Primers. Further, nested PCR will take care of false negatives by countering the effect of PCR inhibitors and low bacterial counts. Therefore, nested PCR based methods if performed properly, may be proposed as gold standard test.Saurabh Kumar Patel Chandra Bhan Pratap Ashok Kumar Jain Anil Kumar Gulati Gopal Nath 2014World Journal of Gastroenterology2014,20,36:30
A gist of gastrointestinal stromal tumors: A review显示文摘Gastrointestinal stromal tumors (GISTs) have been recognized as a biologically distinctive tumor type, different from smooth muscle and neural tumors of the gastrointestinal tract (GIT). They constitute the majority of gastrointestinal mesenchymal tumors of the GIT and are known to be refractory to conventional chemotherapy or radiation. They are defined and diagnosed by the expression of a proto-oncogene protein detected by immunohistochemistry which serves as a crucial diagnostic and therapeutic target. The identification of these mutations has resulted in a better understanding of their oncogenic mechanisms. The remarkable antitumor effects of the molecular inhibitor imatinib have necessitated accurate diagnosis of GIST and their distinction from other gastrointestinal mes-enchymal tumors. Both traditional and minimally invasive surgery are used to remove these tumors with minimal morbidity and excellent perioperative outcomes. The revolutionary use of specific, molecularlytargeted therapies, such as imatinib mesylate, reduces the frequency of disease recurrence when used as an adjuvant following complete resection. Neoadjuvant treatment with these agents appears to stabilize disease in the majority of patients and may reduce the extent of surgical resection required for subsequent complete tumor removal. The important interplay between the molecular genetics of GIST and responses to targeted therapeutics serves as a model for the study of targeted therapies in other solid tumors. This review summarizes our current knowledge and recent advances regarding the histogenesis, pathology, molecular biology, the basis for the novel targeted cancer therapy and current evidence based management of these unique tumors.Ashwin Rammohan Jeswanth Sathyanesan Kamalakannan Rajendran Anbalagan Pitchaimuthu Senthil-Kumar Perumal UP Srinivasan Ravi Ramasamy Ravichandran Palaniappan Manoharan Govindan 2013World Journal of Gastrointestinal Oncology2013,5,6:27
Epidemiology, causes, clinical manifestation and diagnosis, prevention and control of coronavirus disease (COVID-19) during the early outbreak period: a scoping review显示文摘Background:The coronavirus disease(COVID-19)has been identified as the cause of an outbreak of respiratory illness in Wuhan,Hubei Province,China beginning in December 2019.As of 31 January 2020,this epidemic had spread to 19 countries with 11791 confirmed cases, including 213 deaths. The World Health Organization has declared it a Public Health Emergency of International Concern. Methods:A scoping review was conducted following the methodological framework suggested by Arksey and O'Malley. In this scoping review,65 research articles published before 31 January 2020 were analyzed and discussed to better understand the epidemiology, causes, clinical diagnosis, prevention and control of this virus. The research domains, dates of publication, journal language, authors'affiliations, and methodological characteristics were included in the analysis. All the findings and statements in this review regarding the outbreak are based on published information as listed in the references. Results:Most of the publications were written using the English language(89.2%).The largest proportion of published articles were related to causes(38.5%)and a majority(67.7%)were published by Chinese scholars.Research articles initially focused on causes, but over time there was an increase of the articles related to prevention and control.Studies thus far have shown that the virus7 origination is in connection to a seafood market in Wuhan, but specific animal associations have not been confirmed.Reported symptoms include fever, cough, fatigue, pneumonia, headache, diarrhea, hemoptysis, and dyspnea.Preventive measures such as masks, hand hygiene practices, avoidance of public contact, case detection, contact tracing, and quarantines have been discussed as ways to reduce transmission. To date,no specific antiviral treatment has proven effective;hence, infected people primarily rely on symptomatic treatment and supportive care. Conclusions:There has been a rapid surge in research in response to the outbreak of COVID-19.During this early period, published research primarily explored the epidemiology, causes, clinical manifestation and diagnosis,as well as prevention and control of the novel coronavirus.Although these studies are relevant to control the current public emergency, more high-quality research is needed to provide valid and reliable ways to manage this kind of public health emergency in both the short-and long-term.Sasmita Poudel Adhikari Sha Meng Yu-Ju Wu Yu-Ping Mao Rui-Xue Ye Qing-Zhi Wang Chang Sun Sean Sylvia Scott Rozelle Hein Raat Huan Zhou 2020Infectious Diseases of Poverty2020,9,2:28
Rapeseed research and production in China显示文摘Rapeseed(Brassica napus L.) is the largest oilseed crop in China and accounts for about 20% of world production. For the last 10 years, the production, planting area, and yield of rapeseed have been stable, with improvement of seed quality and especially seed oil content.China is among the leading countries in rapeseed genomic research internationally, having jointly with other countries accomplished the whole genome sequencing of rapeseed and its two parental species,Brassica oleracea and Brassica rapa.Progress on functional genomics including the identification of QTL governing important agronomic traits such as yield, seed oil content, fertility regulation, disease and insect resistance, abiotic stress, nutrition use efficiency, and pod shattering resistance has been achieved. As a consequence, molecular markers have been developed and used in breeding programs.During 2005–2014,215 rapeseed varieties were registered nationally, including 210 winter-and 5 spring-type varieties.Mechanization across the whole process of rapeseed production was investigated and operating instructions for all relevant techniques were published.Modern techniques for rapeseed field management such as high-density planting,controlled-release fertilizer, and biocontrol of disease and pests combined with precision tools such as drones have been developed and are being adopted in China. With the application of advanced breeding and production technologies,in the near future, the oil yield and quality of rapeseed varieties will be greatly increased, and more varieties with desirable traits, especially early maturation, high yield, high resistance to biotic and abiotic stress, and suitability for mechanized harvesting will be developed.Application of modern technologies on the mechanized management of rapeseed will greatly increase grower profit.Qiong Hu Wei Hua Yan Yin Xuekun Zhang Lijiang Liu Jiaqin Shi Yongguo Zhao Lu Qin Chang Chen Hanzhong Wang 2017The Crop Journal2017,5,2:25
Update on hepatocellular carcinoma: Pathologists' review显示文摘Histopathologic diversity and several distinct histologic subtypes of hepatocellular carcinoma(HCC) are well-recognized. Recent advances in molecular pathology and growing knowledge about the biology associated with distinct histologic features and immuno-profile in HCC allowed pathologists to update classifications. Improving sub-classification will allow for more clinically relevant diagnoses and may allow for stratification into biologically meaningful subgroups. Therefore, immuno-histochemical and molecular testing are not only diagnostically useful, but also are being incorporated as crucial components in predicting prognosis of the patients with HCC. Possibilities of targeted therapy are being explored in HCC, and it will be important for pathologists to provide any data that may be valuable from a theranostic perspective. Herein, we review and provide updates regarding the pathologic sub-classification of HCC.Pathologic diagnostic approach and the role of biomarkers as prognosticators are reviewed. Further, the histopathology of four particular subtypes of HCC:Steatohepatitic, clear cell, fibrolamellar and scirrhous-and their clinical relevance, and the recent consensus on combined HCC-cholangiocarcinoma is summarized. Finally, emerging novel biomarkers and new approaches to HCC stratification are reviewed.Tony El Jabbour Stephen M Lagana Hwajeong Lee 2019World Journal of Gastroenterology2019,25,14:25
Treatment technologies for aqueous perfluorooctanesulfonate (PFOS) and perfluorooctanoate (PFOA)显示文摘Fluorochemicals(FCs)are oxidatively recalcitrant, environmentally persistent, and resistant to most conventional treatment technologies.FCs have unique physiochemical properties derived from fluorine which is the most electronegative element.Perfluorooctanesulfonate(PFOS),and perfluorooctanoate(PFOA)have been detected globally in the hydrosphere, atmosphere and biosphere.Reducing treatment technologies such as reverses osmosis,nano-filtration and activated carbon can remove FCs from water. However, incineration of the concentrated waste is required for complete FC destruction.Recently,a number of alternative technologies for FC decomposition have been reported. The FC degradation technologies span a wide range of chemical processes including direct photolysis, photocatalytic oxidation, photochemical oxidation, photochemical reduction,thermally-induced reduction, and sonochemical pyrolysis. This paper reviews these FC degradation technologies in terms of kinetics, mechanism, energetic cost, and applicability. The optimal PFOS/PFOA treatment method is strongly dependent upon the FC concentration, background organic and metal concentration, and available degradation time.Chad D.VECITIS Hyunwoong PARK Jie CHENG Brian T.MADER Michael R.HOFFMANN 2009Frontiers of Environmental Science & Engineering2009,3,2:26
MicroRNAs as possible biomarkers for diagnosis and prognosis of hepatitis b- and c-related-hepatocellularcarcinoma显示文摘Aim of the present review is to summarize the current knowledge about the potential relationship between mi RNAs and hepatitis B virus(HBV)-hepatitis C virus(HCV) related liver diseases. A systematic computerbased search of published articles, according to the Preferred Reporting Items for Systematic reviews and Meta-Analysis Statement, was performed to identify relevant studies on usefulness of serum/plasma/urine mi RNAs, as noninvasive biomarkers for early detection of HBV and HCV-induced hepatocellular carcinoma(HCC) development, as well as for its prognostic evaluation. The used Medical Subject Headings terms and keywords were: 'HBV', 'HCV', 'hepatocellular carcinoma', 'micro RNAs', 'mi RNAs', 'diagnosis', 'prognosis', 'therapy', 'treatment'. Some serum/plasma mi RNAs, including mi R-21, mi R-122, mi-125a/b, mi R-199a/b, mi R-221, mi R-222, mi R-223, mi R-224 might serve as biomarkers for early diagnosis/prognosis of HCC, but, to date, not definitive results or well-defined panels of mi RNAs have been obtained. More well-designed studies, focusing on populations of different geographical areas and involving larger series of patients, should be carried out to improve our knowledge on the potential role of mi RNAs for HCC early detection and prognosis.Sirio Fiorino Maria Letizia Bacchi-Reggiani Michela Visani Giorgia Acquaviva Adele Fornelli Michele Masetti Andrea Tura Fabio Grizzi Matteo Zanello Laura Mastrangelo Raffaele Lombardi Luca Di Tommaso Arrigo Bondi Sergio Sabbatani Andrea Domanico Carlo Fabbri Paolo Leandri Annalisa Pession Elio Jovine Dario de Biase 2016World Journal of Gastroenterology2016,22,15:25
Outcomes of robotic vs laparoscopic hepatectomy:A systematic review and meta-analysis显示文摘AIM: To perform a systematic review and metaanalysis on robotic-assisted vs laparoscopic liver resections.METHODS: A systematic literature search was performed using Pub Med, Scopus and the Cochrane Library Central. Participants of any age and sex, who underwent robotic or laparoscopic liver resection were considered following these criteria:(1) studies comparing robotic and laparoscopic liver resection;(2) studies reporting at least one perioperative outcome; and(3) if more than one study was reported by the same institute, only the most recent was included. The primary outcome measures were set for estimated blood loss, operative time, conversion rate, R1 resection rate, morbidity and mortality rates, hospital stay and major hepatectomy rates.RESULTS: A total of 7 articles, published between 2010 and 2014, fulfilled the selection criteria. The laparoscopic approach was associated with a significant reduction in blood loss and lower operative time(MD = 83.96, 95%CI: 10.51-157.41, P = 0.03; MD = 68.43, 95%CI: 39.22-97.65, P < 0.00001, respectively). No differences were found with respect to conversion rate, R1 resection rate, morbidity and hospital stay.CONCLUSION: Laparoscopic liver resection resulted in reduced blood loss and shorter surgical times compared to robotic liver resections. There was no difference in conversion rate, R1 resection rate, morbidity and length of postoperative stay.Roberto Montalti Giammauro Berardi Alberto Patriti Marco Vivarelli Roberto Ivan Troisi 2015World Journal of Gastroenterology2015,21,27:22
