Brain and lung strongly interact via complex pathways from the brain to the lung but also from the lung to the brain. Several hypotheses have been proposed with a particular interest for the recently described 'double hit' model. Ventilator setting in brain-injured patients with lung injuries has been poorly studied and intensivists are often fearful to use some parts of protective ventilation in patients with brain injury. | 6 | Regional gas drainage techniques in Chinese coal mines显示文摘China's rapid economic development has increased the demand for coal.These results in Chinese coal mines being extended to deeper levels.The eastern Chinese,more economical developed,regions have a long history of coal mining and many coal mines have now started deep mining at a depth from 800 to 1500 m.This increase in mining depth,geostresses,pressures,and gas content of the coal seam complicates geologic construction conditions.Lower permeability and softer coal contribute to increasing numbers of coal and gas outburst,and gas explosion,disasters.A search on effective methods of preventing gas disasters has been provided funds from the Chinese government since 1998.The National Engineering Research Center of Coal Gas Control and the Huainan and Huaibei Mining Group have conducted theoretical and experimental research on a regional gas extraction technology.The results included two important findings.First,grouped coal seams allow adoption of a method where a first,key protective layer is mined to protect upper and lower coal seams by increasing permeability from 400 to 3000 times.Desorption of gas and gas extraction in the protected coal seam of up to 60%,or more,may be achieved in this way.Second,a single seam may be protected by using a dense network of extraction boreholes consisting of cross and along-bed holes.Combined with this is increased use of water that increases extraction of coal seam gas by up to 50%.Engineering practice showed that regional gas drainage technology eliminates regional coal and gas outburst and also enables mining under low gas conditions.These research results have been adopted into the national safety codes of production technology.This paper systematically introduces the principles of the technology,the engineering methods and techniques,and the parameters of regional gas drainage.Engineering applications are discussed. | Wang Haifeng Cheng Yuanping Wang Lei | 2012 | International Journal of Mining Science and Technology2012,22,6: | 13 |

| 9 | Role of protective stoma in low anterior resection for rectal cancer:A meta-analysis显示文摘AIM:To provide a comprehensive evaluation of the role of a protective stoma in low anterior resection(LAR)for rectal cancer.METHODS:The Pub Med,EMBASE,and MEDLINE databases were searched for studies and relevant literature published between 2007 and 2014 regarding the construction of a protective stoma during LAR.A pooled risk ratio(RR)with 95%confidence intervals(CIs)was used to assess the outcomes of the studies,including the rate of postoperative anastomotic leakage and reoperations related to leakage.Funnel plots and Egger’s tests were used to evaluate the publication biases of the studies.P values<0.05 were considered statistically significant.RESULTS:A total of 11 studies were included in the meta-analysis.In total,5612 patients were examined,2868 of whom had a protective stoma and 2744 of whom did not.The sample size of the studies varied from 34 to 1912 patients.All studies reported the number of patients who developed an anastomotic leakage and required a reoperation related to leakage.A random effects model was used to calculate the pooled RR with the corresponding 95%CI because obvious heterogeneity was observed among the 11 studies(I2=77%).The results indicated that the creation of a protective stoma during LAR significantly reduces the rate of anastomotic leakage and the number of reoperations related to leakage,with pooled RRs of 0.38(95%CI:0.30-0.48,P<0.00001)and 0.37(95%CI:0.29-0.48,P<0.00001),respectively.The shape of the funnel plot did not reveal any evidence of obvious asymmetry.CONCLUSION:The presence of a protective stoma effectively decreased the incidences of anastomotic leakage and reoperation and is recommended in patients undergoing low rectal anterior resections for rectal cancer. | Sheng-Wen Wu Cong-Chao Ma Yu Yang | 2014 | World Journal of Gastroenterology2014,20,47: | 12 |
| 10 | Increased postoperative complications after protective ileostomy closure delay: An institutional study显示文摘AIM: To study the morbidity and complications as-sociated to ileostomy reversal in colorectal surgery pa-tients, and if these are related to the time of closure. METHODS: A retrospective analysis of 93 patients, who had undergone elective ileostomy closure between 2009 and 2013 was performed. Demographic, clinical and surgical variables were reviewed for analysis. All complications were recorded, and classified according to the Clavien-Dindo Classification. Statistical univariate and multivariate analysis was performed, setting a P value of 0.05 for significance.RESULTS: The patients had a mean age of 60.3 years, 58% male. The main procedure for ileostomy cre-ation was rectal cancer(56%), and 37% had received preoperative chemo-radiotherapy. The average delay from creation to closure of the ileostomy was 10.3 mo. Postoperative complications occurred in 40% of the pa-tients, with 1% mortality. The most frequent were ileus(13%) and wound infection(13%). Pseudomembra-nous colitis appeared in 4%. Increased postoperative complications were associated with delay in ileostomyclosure(P = 0.041). Male patients had more complica-tions(P = 0.042), mainly wound infections(P = 0.007). Pseudomembranous colitis was also associated with the delay in ileostomy closure(P = 0.003). End-to-end in-testinal anastomosis without resection was significantly associated with postoperative ileus(P = 0.037). CONCLUSION: Although closure of a protective il-eostomy is a fairly common surgical procedure, it has a high rate of complications, and this must be taken into account when the indication is made. | 15 | Heavy rockbursts due to longwall mining near protective pillars:A case study显示文摘Rockburst represents a very dangerous phenomenon in deep underground mining in unfavourable conditions such as great depth, high horizontal stress, proximity of important tectonic structures, and unmined pillars. The case study describes a recorded heavy rockburst in the Czech part of the Upper Silesian Coal Basin, which occurred during longwall mining near the protective pillar. The artificial dividing of geological blocks and creation of mining protective pillars(shaft pillars, crosscut pillars etc.) is a dangerous task in light of rockbursts occurring mainly due to overstressing of remaining pillars. A simple model of this situation is presented. Natural and mining conditions are analysed and presented in detail as well as registered seismicity during longwall mining in the area. Recorded rockbursts in the area of interest are described and their causes discussed. Many rockbursts near protective pillars were recorded in this mining region. Methodical instructions for rockburst prevention in proximity of protective pillars as well as for gates driving were devised based on the evaluation of rockburst causes. Normally, bone has an innate capacity to heal from damage.However, massive bone defects due to traumatic injury, tumor resection, or congenital diseases pose a great challenge to reconstructive surgery. Scaffold-based tissue engineering(TE) is a promising strategy for bone regenerative medicine, because biomaterial scaffolds show advanced mechanical properties and a good degradation profile, as well as the feasibility of controlled release of growth and differentiation factors or immobilizing them on the material surface. Additionally, the defined structure of biomaterial scaffolds, as a kind of mechanical cue, can influence cell behaviors, modulate local microenvironment and control key features at the molecular and cellular levels. Recently, nano/micro-assisted regenerative medicine becomes a promising application of TE for the reconstruction of bone defects. For this reason, it is necessary for us to have in-depth knowledge of the development of novel nano/micro-based biomaterial scaffolds.
