3Expenditure of hospital care on cancer in China, from 2011 to 2015显示文摘Objective: A solid understanding of levels and trends of spending on cancer is important to evaluate whether our healthcare resources were wisely spent and to prioritize future resources for cancer treatment and prevention.However, studies on economic burden of cancers in China are scant and the results are inconsistent.Methods: The Chinese hospital information database and nearly 350 million inpatient medical record data were used. As the ratios of cancer inpatient payments to total inpatient payments were mainly influenced by the grades and sites of hospitals, the estimates of payments of cancer inpatients in this study were stratified by hospital grades and provinces. Only the payments of cancer inpatients happened in grade 2, grade 3 and specialized cancer hospitals were included in the analyses. The total medical payments of cancers in China were estimated based on the ratios of outpatient payments to inpatient payments in specialized cancer hospitals.Results: From 2011 to 2015, the payments of cancer inpatients in China have increased by 84.1% and the total inpatient payments reached 177.1 billion RMB in 2015, accounting for 4.3% of the total health expenditure in China. Based on the ratio of outpatient payments to inpatient payments, the total payments on cancer treatments in China were estimated to be 221.4 billion RMB in 2015. Among different cancer types, the highest payments were the treatment of trachea, bronchus and lung cancer. The major cancer inpatient payments(67.1% in 2015) spent in grade 3 general hospitals and this ratio increased continually from 2011 to 2015. The expenditure of cancer treatments also varies by region with the major expenditure in the eastern region of China.Conclusions: This study estimated the total payments of cancer treatments in China and analyzed how the money was spent on cancer treatments in the recent 5 years, which would provide information for decision makings on the allocation of resources to service provisioning, prevention strategies, research funding, and assessing whether the economic burden of cancer is affordable to the governments.Yue Cai Ming Xue Wanqing Chen Maogui Hu Zhiwen Miao Lan Lan Rongshou Zheng Qun Meng 2017Chinese Journal of Cancer Research2017,29,3:81
4Generalized thermal resistance for convective heat transfer and its relation to entransy dissipation显示文摘In order to further analyze and optimize convective heat transfer process further, the concepts of heat flux weighted average heat temperature and heat flux weighted average heat temperature difference in multi-dimensional heat transfer system were introduced in this paper. The ratio of temperature difference to heat flux is defined as the generalized thermal resistance of convective heat transfer processes, and then the minimum thermal resistance theory for convective heat transfer optimization was developed. By analyzing the relationship between generalized thermal resistance and entansy dissipation in convective heat transfer processes, it can be concluded that the minimum thermal resistance theory equals the entransy dissipation extremum theory. 5The extremum principle of mass entransy dissipation and its application to decontamination ventilation designs in space station cabins显示文摘In terms of the analogy between mass and heat transfer phenomena, a new physical quantity, i.e. mass entransy, is introduced to represent the ability of an object for transferring mass to outside. Meanwhile, the mass entransy dissipation occurs during mass transfer processes as an alternative to measure the mass transfer irreversibility. Then the concepts of mass entransy and its dissipation are used to develop the extremum principle of mass entransy dissipation and the corresponding method for convective mass transfer optimization, based on which an Euler's equation has been deduced as the optimization equation for the fluid flow to obtain the best convective mass transfer performance with some specific constraints. As an example, the ventilation process for removing gaseous pollutants in a space station cabin with a uniform air supply system has been optimized to reduce the energy consumption of the ventilation system and decrease the contaminant concentration in the cabin. By solving the op- timization equation, an optimal air velocity distribution with the best decontamination performance for a given viscous dissipation is firstly obtained. With the guide of this optimal velocity field, a suitable concentrated air supply system with appropriate air inlet position and width has been designed to replace the uniform air supply system, which leads to the averaged and the maximum contaminant concentrations in the cabin been decreased by 75% and 60%, respectively, and the contaminant concentration near the contaminant source surface been decreased by 50%, while the viscous dissipation been reduced by 30% simultaneously.CHEN Qun REN JianXun GUO ZengYuan 2009Chinese Science Bulletin2009,54,16:26

7Cardiotoxicity of 5-fluorouracil and capecitabine in Chinese patients: a prospective study显示文摘Background:5-Fluorouracil(5-FU)and capecitabine-associated cardiotoxicity ranging from asymptomatic electro-cardiography(ECG)abnormalities to severe myocardial infarction has been reported in a number of studies,but such cardiotoxicity in Chinese patients with malignant diseases has not been investigated to date.In the present study,we aimed to prospectively evaluate the incidence rates and clinical manifestations of 5-FU-and capecitabine-associated cardiotoxicity in cancer patients recruited from multiple centers in China.Methods:Among the 527 patients who completed the study,196 received 5-FU-based chemotherapy and 331 received capecitabine-based chemotherapy as either first-line or adjuvant therapy.Adverse events were reported during the treatment and up to 28 days of follow-up.Outcome measures included ECG,myocardial enzymes,cardiac troponin,brain natriuretic peptide and echocardiography.Univariate analysis and logistic regression were performed for subgroup analysis and identification of significant independent variables that are associated with cardiotoxicity of both agents.Results:In total,161 of 527 patients(30.6%)experienced cardiotoxicity.The incidence rate of cardiotoxicity was 33.8%(112/331)in the capecitabine group,which was significantly higher than the rate of 25%(49/196)in the 5-FU group(P=0.0042).110/527 patients(20.9%)suffered arrhythmia,105/527(19.9%)developed ischemic changes,while only 20/527 patients(3.8%)presented heart failure and 6/527 patients(1.1%)had myocardial infarction.Pre-existing cardiac disease,hypertension,capecitabine-based chemotherapy and duration of treatment were identified as sig-nificant risk factors associated with cardiotoxicity.The odds ratio were 15.7(prior history of cardiac disease versus no history),1.86(capecitabine versus 5-FU),1.06(5-8 versus 1-4 chemotherapy cycles)and 1.58(hypertension versus no hypertension),respectively.Conclusions:Cardiotoxicity induced by fluoropyrimidines in the Chinese population may be underestimated in clini-cal practice.Close monitoring of patients is recommended,especially for those patients at high risk for cardiotoxicity.Possible risk factors are duration of treatment,capecitabine-based chemotherapy,pre-existing cardiac diseases and hypertension.Trial registration This study was initiated on January 22,2014 and has been retrospectively registered with the registra-tion number ChiCTR1800015434 .Jianjun Peng Chao Dong Chang Wang Weihua Li Hao Yu Min Zhang Qun Zhao Bo Zhu Jun Zhang Wenliang Li Fenghua Wang Qiong Wu Wenhao Zhou Ying Yuan Meng Qiu Gong Chen 2018Cancer Communications2018,38,1:23
8Fluid flow field synergy principle and its application to drag reduction显示文摘The concept of field synergy for fluid flow is introduced, which refers to the synergy of the velocity field and the velocity gradient field in an entire flow domain. Analyses show that the flow drag depends not only on the velocity and the velocity gradient fields but also on their synergy. The principle of minimum dissipation of mechanical energy is developed, which may be stated as follows: the worse the synergy between the velocity and velocity gradient fields is, the smaller the resistance becomes. Furthermore, based on the principle of minimum dissipation of mechanical energy together with conservation equa-tions, a field synergy equation with a set of specified constraints has been established for optimizing flow processes. The optimal flow field can be obtained by solving the field synergy equation, which leads to the minimum resistance to fluid flow in the fixed flow domain. Finally, as an example, the field synergy analysis for duct flow with two parallel branches is presented. 9Protective effects of MCP-1 inhibitor on a rat model of severe acute pancreatitis显示文摘BACKGROUND:Chemokines and their receptors play key roles in the pathogenesis of acute pancreatitis.This study aimed to establish a rat model of severe acute pancreatitis(SAP) for investigating monocyte chemotactic protein-1(MCP-1) expression in the pathogenesis of the disease.We assessed the effects of the inhibitor of MCP-1,Bindarit,on SAP and explored the mechanisms underlying SAP. METHODS:Seventy-two Sprague-Dawley rats were randomly divided into a saline control group(group S),an SAP group (group P),and a Bindarit group(group T).The SAP model was induced by retrograde infusion of 4%sodium taurocholate into the bilio-pancreatic duct.Based on the SAP model,Bindarit was injected intraperitoneally in group T,and 0.5% methyl cellulose was injected intraperitoneally in groups S and P.In group S,saline was retrogradely infused into the bili- pancreatic duct.Serum amylase levels and the histological changes in the pancreas were assessed at different time- points in each group.Expression of MCP-1 in serum was measured by enzyme-linked immunoadsorbent assay(ELISA). MCP-1 protein and mRNA expression levels were detected by immunohistochemistry,Western blotting,and semi- quantitative reverse transcriptase-polymerase chain reaction (RT-PCR). RESULTS:Serum amylase levels in groups P and T were higher than those in group S.Serum amylase levels were significantly lower in group T than in group P at 6 and 12 hours after operation.The levels of MCP-1 in serum at 6 and 12 hours after operation in group P were significantly higher than in group S,and significantly lower in group T than in group P at 6 and 12 hours after operation.The pathological damage in the pancreas was milder in group T than in group P.MCP-1 protein and mRNA expression levels in the pancreas were higher in groups P and T than in group S.These expression levels were positively correlated with the pathological damage of pancreatic tissues.The activity of MCP-1 in group T was significantly lower than in group P.CONCLUSION:MCP-1 may play important roles in the pathogenesis of SAP.The data suggest that Bindarit ameliorates SAP by inhibiting the activity of MCP-1 in vivo.Zhou, Guo-Xiong Zhu, Xue-Juan Ding, Xiao-Ling Zhang, Hong Chen, Jian-Ping Qiang, Hui Zhang, Hai-Feng Wei, Qun 2010Hepatobiliary & Pancreatic Diseases International2010,9,2:20
10Moisture transfer resistance method for liquid desiccant dehumidification analysis and optimization显示文摘The concepts of entransy, entransy dissipation and transfer resistance are introduced into liquid desiccant dehumidification analysis to reveal the irreversibility and moisture transfer resistance between moist air and liquid desiccant.By analyzing a typical water (vapor) transfer process coupled with heat transfer, we define the concepts of mass entransy of water and its dissipation, derive the expression of moisture transfer resistance (MTR) that reflects the irreversibility of water transfer during dehumidification processes, and also point out the relationship between MTR and dehumidification performance. With these concepts, both adiabatic and internal cooling liquid desiccant dehumidification systems with various operation conditions are analyzed and optimized. It is found that for the adiabatic dehumidification system, increasing the mass transfer coefficient leads to the reduction of MTR, and consequently, the improvement of dehumidification performance. 11A comparison of optimization theories for energy conservation in heat exchanger groups显示文摘In general,thermal processes can be classified into two categories: heat-work conversion processes and heat transfer processes. Correspondingly,the optimization of thermal processes has to have two different criteria:the well known entropy generation minimization method and the recently proposed entransy dissipation maximization method. This study analyzes the thermal issues in a heat exchanger group,and optimizes the unit arrangements under different constraints based on a suitable optimization crite-rion. The result indicates that the principle of minimum entropy generation rate is valid for optimizing heat exchangers in a ther-modynamic cycle with given boundary temperatures. In contrast,the entransy dissipation maximization is more suitable in heat exchanger optimizations involving only heat transfer processes. 12Concurrent chemoradiotherapy combined with enteral nutrition support:a radical treatment strategy for esophageal squamous cell carcinoma patients with malignant istulae显示文摘Background: Concurrent chemoradiotherapy(CCRT) significantly increases the survival rate of esophageal squa?mous cell carcinoma(ESCC) patients with malignant fistulae. Recent clinical evidence has shown the benefits of enteral nutrition for malnourished cancer patients. In this study, we aimed to validate that, with the support of enteral nutrition, ESCC patients who develop malignant fistulae might be able to complete CCRT and achieve long?term survival.Methods: We reviewed the medical records of 652 patients with ESCC who received definitive CCRT at Sun Yat?sen University Cancer Center between January 2010 and December 2012. Treatment outcome and toxicity were ret?rospectively evaluated in 40 ESCC patients with malignant fistulae. All the 40 patients were treated with CCRT and evaluated by clinical nutritionists using nutrition risk screening(NRS) before, during, and after treatment. Twenty?two patients received a nasogastric tube, and 18 underwent percutaneous endoscopic gastrostomy feeding. The median energy intake was 2166 kcal/day. Treatment response was evaluated at 3 months after the completion of CCRT.Results: With a median follow?up of 18 months(range, 3–39 months), patients' 1?year overall survival(OS) rate was 62.5%, and the estimated OS time was 25.5 months. Univariate analysis showed that the NRS score(P n NRS score(P se to treatment(P < 0.001) were sig= 0.003), increase i= 0.024), fistula closure(P = 0.011), and responnifi?cantly associated with OS. Multivariate analysis showed that tumor response(P = 0.044) and increase in NRS score(P = 0.044) were independent predictors of OS. Grade 3 vomiting was observed in 8 patients(20.0%), grade 3 neutro?penia was observed in 11 patients(27.5%), and grade 3 cough was observed in 13 patients(32.5%); 2 patients(5.0%) died of massive bleeding during treatment.Conclusions: CCRT combined with enteral nutrition support is effective for ESCC patients with malignant fistulae. Patients have an increased potential to be cured, especially those who experience complete response and have an increase in NRS score. 13The property diagram in heat transfer and its applications显示文摘Inspired by the property diagrams in thermodynamics,which distinctly reflect the performance and characteristics of thermodynamic cycles,we establish a state equation for heat motion and introduce a two-dimension property diagram,T-q diagram,in heat transfer to analyze and optimize the performance of heat exchangers,where heat flow is a state parameter for heat motion.According to the property diagram,it is convenient to obtain the influences of heat exchanger area,heat capacity rate and flow arrangement on the heat transfer performance during the analysis of heat exchangers and their networks.For instance,when analyzing the heat exchanger network in a district heating system,it is obvious to find that:if both the heat demand and the indoor air temperature in each branch of the network are the same,the total area of heat exchangers,the flow rate of water and the return water temperature in each branch are all the same;if the indoor air temperatures in different branches are different,the temperatures of the waters after flowing through different branches are different,which means that the mixing process of return waters with the same temperature is not an essential requirement to realize the best performance of district heating systems.CHEN Qun XU YunChao GUO ZengYuan 2012Chinese Science Bulletin2012,57,35:17
14Value of circulating cell-free DNA in diagnosis of hepatocelluar carcinoma显示文摘AIM:To investigate the value of combined detection of circulating cell-free DNA(cfDNA),a-fetal protein(AFP) and a L-fucosidase(AFU) for diagnosis of hepatocellular carcinoma(HCC).METHODS:Serum samples from 39 HCC patients and 45 normal controls were collected.Branched DNA(bDNA) was used to detect the level of cfDNA,and a receiver operating characteristic curve was employed to evaluate the diagnostic sensitivity,specificity,accuracy,positive predictive value,negative predictive value,positive likelihood ratio,negative likelihood ratio and Youden index,and to assess the diagnostic efficiency and their correlations with the clinicopathological features.AFP and AFU were detected by chemiluminescence and colorimetry,respectively.The significance of combined detection of the three biomarkers was discussed.RESULTS:cfDNA level was increased in 22 of the 39 HCC samples and in 2 of the 45 normal controls.cfDNA level in HCC samples was significantly higher than that in normal controls(P < 0.05).There were significant differences in sex and extra-and intrahepatic metastasis(P < 0.05).There was no significant correlation between cfDNA,AFP and AFU in the detection of HCC.The sensitivity of combined detection of cfDNA with one marker(AFP or AFU) and cfDNA with two markers(AFP and AFU) was 71.8%,87.2% and 89.7% vs 56.4%,53.8% and 66.7% for cfDNA,AFP and AFU used alone,respectively,the difference being statistically significant(P < 0.05).CONCLUSION:Quantitative analysis of cfDNA is sensitive and feasible,and the combined detection of cfDNA with AFP or AFU or both could improve the diagnostic sensitivity for HCC.Ken Chen Hong Zhang Li-Na Zhang Shao-Qing Ju Jing Qi Dong-Feng Huang Feng Li Qun Wei Jing Zhang 2013World Journal of Gastroenterology2013,19,20:16
15Role of Tenascin-X in regulating TGF-β/Smad signaling pathway in pathogenesis of slow transit constipation显示文摘BACKGROUND Chronic constipation is a gastrointestinal functional disease that seriously harms physical and mental health and impacts the quality of life of patients.Its incidence rate is 2%-27%.Slow transit constipation(STC)is a common type of chronic functional constipation,accounting for 10.3%-45.5%of such cases.Scholars have performed many studies on the pathogenesis of STC.These studies have indicated that the occurrence of STC may be related to multiple factors,such as dysfunction of the enteric nervous system,interstitial cells of Cajal(ICC)damage,and changes in neurotransmitters regulating intestinal peristalsis.AIM To investigate the role of Tenascin-X(TNX)in regulating the TGF-β/Smad signaling pathway in the pathogenesis of STC.METHODS This study included an experimental group and a control group.The experimental group included 28 patients with severe colonic STC,and the control group included 18 patients with normal colon tissues.Immunohistochemistry(IHC)was used to detect c-Kit,a specific marker of the ICC.Western blot,immunofluorescence,and IHC were used to detect the localization and expression of TNX and TGF-β/Smad.RESULTS IHC showed that the number of ICC with positive c-Kit expression was significantly reduced in the colon of STC patients(22.17±3.28 vs 28.69±3.53,P<0.05)and that the distribution was abnormal.Western blot results showed that c-Kit and Smad7 levels were significantly decreased in the colon of STC patients(c-kit:0.462±0.099 vs 0.783±0.178,P<0.01;Smad7:0.626±0.058 vs 0.799±0.03,P<0.01)and that TNX and Smad2/3 levels were higher in the STC group(TNX:0.868±0.028 vs 0.482±0.032,P<0.01).There was no significant difference in TGF-βbetween the two groups(0.476±0.028 vs 0.511±0.044,P=0.272).Pearson correlation analysis showed that the TNX protein exhibited a strong correlation with Smad2/3 and Smad7(P<0.05,|R|>0.8)and TGF-β(P<0.05,|R|=0.7).CONCLUSION The extracellular matrix protein TNX may activate the TGF-β/Smad signaling pathway by upregulating the Smad 2/3 signaling protein and thereby induce slight or complete epithelial stromal cell transformation,leading to an abnormal distribution and dysfunction of ICC in the diseased colon,which promotes the occurrence and development of STC.Yi-Chao Zhang Bao-Xiang Chen Xiao-Yu Xie Yan Zhou Qun Qian Cong-Qing Jiang 2020World Journal of Gastroenterology2020,26,7:16

17Apoptosis of hepatoma cells SMMC-7721 induced by Ginkgo biloba seed polysaccharide显示文摘AIM: To study the apoptosis of hepatoma cells SMMC-7721induced by polysaccharide isolated from Ginkgo biloba seed.METHODS: Ginkgo biloba seed polysaccharide (GBSP) wasisolated by ethanol fractionation of Ginkgo biloba seed andpurified by Sephadex G-200 chromatography. The purity ofGBSP was verified by reaction with iodine-potassium iodideand ninhydrin and confirmed by UV spectrophotometer,cellulose acetate membrane electrophoresis and Sepharose4B gel filtration chromatography. The Scanning ElectronMicroscope (SEM) and Flow Cytometrv (FCM) were used toexamine the SMMC-7721 cells with and without GBSPtreatment at 500 mg/ml for 36 h.RESULTS: GBSP product obtained was of high purity withthe average molecular weight of 1.86 × 105. Quantitativeanalysis of SMMC-7721 cells in vitro with FCM showed thatthe percentages of G2-M cells without and with GBSPtreatment were 17.01±1.28 % and 11.77±1.50% (P<0.05),the debds ratio of the cells were 0.46±0.12 % and 0.06±0 .06 %(P<0.01), and the apoptosis ratio of cells was 3.84±0 .55 %and 9.13±1.48 %(P<0.01) respectively. Following GBSPtreatment, microvilli of SMMC-7721 cells appeared thinnerand the number of spherical cells increased markedly.
