| 1 | Advances in Immunonutrient Application to Treat Cancer Cachexia Syndrome显示文摘Cachexia is a multifactorial syndrome characterized by the loss of body weight,and has been observed in more than 50%of cancer patients.It arises as a result of anorexia and increased energy expenditure,leads to a reduced tolerance of cancer therapy and a reduced quality of life,resulting in a poorer prognosis and decreased survival.In the past few years,tremendous achievements have been made in cancer cachexia research.Systemic inflammation has been proven to play important roles in the etiology of cancer cachexia,leading to functional impairment and rapid deterioration,which suggests that anti-inflammatory agents may represent a promising strategy for cancer cachexia treatment.Thus,a variety of agents have been postulated to treat cachexia,with modulation of inflammation,the immune response,and reactive oxygen species being the most promising.Some immune-enhancing nutrients,‘immunonutrients’,such asω-3 fatty acids,arginine,nucleotides,L-carnitine,probiotics,phytochemicals,and specific minerals have been tested for their anti-inflammatory and anti-oxidative properties.They have also been used to treat,prevent or attenuate cancer cachexia in both experimental models and clinical trials.A number of studies on the use of immunonutrients for the treatment of cancer cachexia have been published over the past decade,with some promising results supporting the routine use of immune-enhancing formulas in patients with cachexia.However,the effects and efficacy of these substances have not been conclusively proven.In this review,we discuss recent studies on the molecular mechanisms underlying cancer cachexia and the application of several immunonutrients. | Liang Zhang Wen Lei Zhuo | 2020 | Journal of Nutritional Oncology2020,5,1: | 10 |
| 2 | The Association of Diet-Modulated Gut Microbiota on Development and Treatment of Colorectal Cancer显示文摘Colorectal cancer is the third most common cancer worldwide.Etiology of colon cancer continues to be investigated with research findings ranging from environment to diet to inherited gene mutations.Risk factors associated with colon cancer include familial history of colon cancer and inflammatory bowel disease,as well as lifestyle-related modifiable influences like diet,weight,and exercise.Recent findings point to the connection between diet and gut microbiota,which is highly affected by changes in diet promoting either an environment of beneficial bacteria or dysbiosis.Dietary effects on the microbiome can impact health outcomes,including an increased risk for colorectal cancer.Although the mechanisms behind the effect of gut microbiota on the development of colorectal cancer continues to evolve,its effect on both inflammation and immune response may play a key role.Further,after development of colorectal cancer diagnosis,the changes to gut microbiota have a potential to continue to influence the effectiveness of therapy and treatment.This review presents the current knowledge around the diet-modulated gut microbiome and its association with,risk for,development of,and treatment of colorectal cancer.Overall,further research on dietary elements that affect the gut microbiome and its subsequent effect on development and treatment of colorectal cancer is needed for applicable recommendations that can be made for dietary modulation of the gut microbiota for improved health outcomes. | Elizabeth Clinger Lei Hao | 2021 | Journal of Nutritional Oncology2021,6,4: | 9 |
| 3 | Decreased Average Albumin-to-Globulin Ratio During Radiotherapy Worsens the Prognosis of Esophageal Squamous Cell Carcinoma:Implications for Nutritional Management显示文摘Objective Malnutrition and cachexia are common in esophageal squamous cell carcinoma(ESCC)patients undergoing radiotherapy.This study evaluated how malnutrition-and cachexia-related indicators,including the albumin-to-globulin ratio(AGR),and their changes during radiotherapy predict the treatment outcomes.Methods We reviewed a total of 172 ESCC patients receiving radiotherapy(as a primary cohort)and performed a Cox regression analysis on potential prognostic factors,including the AGR,as well as the TNM stage and concomitant chemotherapy.A subsequent receiver operating characteristics(ROC)curve and Kaplan Meier survival analysis was performed for ESCC patients stratified by the average AGR cut-off point.Results In addition to the well-documented factors(i.e.TNM stage and concomitant chemotherapy),the average AGR was a significant prognostic factor for ESCC patients receiving radiotherapy.By plotting the ROC curve of the average AGR with regard to the ESCC prognosis,we obtained cut-off points for the overall patients(cut-off point:1.5,AUC:0.636),for patients with stageⅡ/Ⅲdisease(cut-off point:1.5,AUC:0.611),and for patients with stageⅣdisease(cut-off point:1.84,AUC:0.900).The ESCC patients with higher average AGR had a significantly more favorable OS compared with those with a lower average AGR.Notably,ESCC patients who had an increasingΔAGR during radiotherapy had a considerably more favorable OS compared with those with a decreasingΔAGR.All of these findings were reproducible in the validation cohort.Conclusion A lower average AGR is indicative of a poorer prognosis for ESCC patients following radiotherapy.Improving the nutritional status and preventing or ameliorating cachexia might contribute to improving the prognosis of ESCC patients. | Fen Cai Xiu Lian Zhang Xiao Ling Wu Ting Ting Zhuang Lei Huang Yi Fei Tu Yuan Yuan Lu Min Xia Jiang Qing Ying Zhang Yan Lin Chang Chun Ma | 2021 | Journal of Nutritional Oncology2021,6,2: | 8 |

| 5 | A Review of the Associations between Dietary Fiber Intake and Cancer Prevention or Prognosis显示文摘The increase in tumor incidence is closely related to dietary factors.In recent years,many researchers have carried out in-depth research to investigate whether increasing the intake of dietary fiber can prevent tumors and improve the prognosis,but the results have been unclear.We herein searched the literature in common academic databases and incorporated and summarized the relevant articles.The literature clearly indicates that dietary fiber can protect against the following cancers:colorectal cancer,breast cancer,pancreatic cancer,ovarian cancer,and head and neck cancer.There is also evidence that dietary fiber intake can exert protective effects against cancers of the liver,lung,stomach,small bowel,kidney,and esophagus,as well as lymphoma,but the data have so far been relatively limited.The effects on endometrial cancer and prostate cancer still remain uncertain.In terms of the effects on the cancer prognosis,dietary fiber does not appear to have an impact on colorectal cancer or breast cancer.Although more research is needed for some kinds of cancer,it is clear that increasing the intake of dietary fiber is beneficial for preventing the occurrence and development of several major types of cancer,with no major adverse effects.We therefore recommend that most of individuals increase their intake of dietary fiber. | Ying Xie Yu Wei Ren Xiao Ling Li Zeng Ning Li | 2020 | Journal of Nutritional Oncology2020,5,3: | 7 |
| 6 | Postoperative Reduction in Serum Albumin Predicts Complications after Rectal Cancer Surgery显示文摘Background The serum albumin(Alb)level often decreases early in the postoperative period.However,it is unclear whether a decline in the serum albumin level(ΔAlb)is a predictor of postoperative complications.We aimed to determine the relationship between theΔAlb and complications following surgery for rectal cancer(RC).Methods We retrospectively collected the records of patients aged≥18 years who underwent RC surgery between January 2015 and August 2018.We determined the cut-off value of theΔAlb using subject working characteristic curve analysis and analyzed complications above grade II according to the Clavien-Dindo classification.Single and multifactor analyses of the risk factors for postoperative complications were performed.Results A total of 430 RC patients were included in this study.The area under theΔAlb curve was 0.786,with a critical value of 22%.We selected 20%as the critical point for grouping patients.The total postoperative complication rate was significantly higher in patients who had a≥20%serum Alb decline than those had a<20%serum Alb decline(<20%ΔAlb group);with rates of 51.5%and 16%,respectively(P<0.001).A multifactorial logistic regression analysis revealed that theΔAlb(odds ratio[OR]=4.95,95%CI=3.126-7.838 P<0.001),surgical approach(OR=2.374,95%CI=1.335-4.224 P=0.003),and blood loss(>200 ml)(OR=1.684,95%CI=1.043-2.716 P=0.033)were independent risk factors for postoperative complications follow RC surgery.Conclusions TheΔAlb predicted the incidence of postoperative complications in patients after RC surgery.This could help identify patients at risk of complications early during the perioperative period,thereby helping to more effectively manage high-risk patients. | Yi Fan Cheng Yan Yu Chen Yong Lu Chang Bao Liu | 2021 | Journal of Nutritional Oncology2021,6,4: | 7 |
| 7 | Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Lung Cancer Patients显示文摘The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P < 0.001). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P < 0.001) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity. It seems to be a valid tool that can be used to access the nutritional status of Chinese lung cancer patients. | 8 | Effect of Weight Loss on Survival in Esophageal Cancer Patients undergoing Neoadjuvant Chemoradiotherapy and Surgery显示文摘Objective This study aimed to determine the impact of weight loss during neoadjuvant chemoradiotherapy on the survival of patients with esophageal carcinoma.Methods We retrospectively examined 102 consecutive patients with esophageal carcinoma who underwent neoadjuvant chemoradiotherapy followed by radical resection at Sichuan Cancer Hospital&Institute between 2003 and 2017.The patients were divided into three groups based on the amount of body weight lost during neoadjuvant chemoradiotherapy:severe weight loss(>10%),high weight loss(5%~10%),and low weight loss(<5%).The correlations of weight loss with toxicity,progressionfree survival,and overall survival were investigated.Results The median overall survival was 49.7 months in the low weight loss group compared with 35.4 and 25.1 months in the high and severe weight loss groups(P=0.041).The 1-year overall survival rates in the severe,high,and low weight loss groups were 62.5%,85.0%,and 90.7%,respectively;the corresponding 3-year overall survival rates were 21.9%,47.3%,and 68.8%,respectively,and the corresponding 5-year overall survival rates were 21.9%,31.0%,and 44.4%,respectively.The multivariate analysis indicated that a pathological complete response and severe weight loss were independent prognostic factors for overall survival.Any leukopenia(P=0.024),leukopenia of at least grade 3(P=0.021),and anemia(P=0.042)occurred more frequently in the severe weight loss group.Conclusions Weight loss during neoadjuvant CRT is an independent and adverse prognostic factor in esophageal carcinoma patients,whereas a stable weight confers a better prognosis. | Jia Hua Lyu Tao Li Yong Tao Han Lei Wu Lin Peng Qi Feng Wang Long Liang | 2020 | Journal of Nutritional Oncology2020,5,3: | 6 |

The aim of this research is to validate the Chinese version of the PGSGA in Chinese gastric cancer patients by conducting analyses of nutrition screening and assessment data. Methods Two thousand consecutive adult patients with gastric cancer were included in this observational, cross-sectional study in several tertiary hospitals in China. A survey was made among those patients, which included several questionnaires and some anthropometric parameters. We calculated the parameters to validate the use of PG-SGA categorizing the nutritional status of gastric cancer patients. Then we performed statistical analysis to evaluate the internal consistency, external consistency, and construct validity. Results This study verified the ability of the PG-SGA to differentiate gastric cancer patients into different nutritional groups. The confirmatory factor analysis verified the original four-factor structure of the PG-SGA. The internal and external consistency was measured by a concordance analysis which showed an intraclass correlation coefficients (ICC) of 0.987. The correlations of the Box 4 score with the Karnofsky performance status score demonstrated alternative form validity with good external consistency (Pearson, r = 0.643, P < 0.001). All three anthropometric dimensions of the PG-SGA were supported by principal component analysis. Spearman correlations (P < 0.001) were significant in the majority of tests. | 12 | The Values of the Prognostic Nutrition Index and the Patient Generated-Subjective Global Assessment during the Nutritional Assessment of Patients with Gastrointestinal Tumors显示文摘Objective To compare the value and consistency among the Patient Generated-Subjective Global Assessment(PG-SGA)and the Prognostic Nutrition Index(PNI)for assessing nutritional status in gastrointestinal tumor patients.Methods 251 patients from gastric cancer surgical ward from January 2019 to January 2020 were recruited through convenience sampling in this respective study.Nutritional screening and assessment were conducted for 251 gastrointestinal tumor patients using the nutritional risk screening 2002(NRS 2002)PG-SGA,and the PNI.PNI was calculated using the serum albumin level and the total lymphocyte count obtained from the patients’routine laboratory examination when they were admitted to the hospital.The receiver operating characteristic(ROC)of the PG SGA and the PNI were plotted with the NRS 2002 used as the gold standard,and the diagnostic value of the PG-SGA and PNI was reflected by the area under the curve(AUC),sensitivity,specificity and Youden index.We then determined the optimal cut-off for the PNI and tested the consistency of the PG-SGA and PNI.Results The optimal cut-off point for the PNI was calculated to be 50.78.The AUC of the PG-SGA was 0.908(95%CI 0.871-0.944).The sensitivity was 89.9%,specificity was 76.2%and the Youden index was 0.661.The AUC of the PNI was 0.594(95%CI 0.516-0.572).The sensitivity was 73.8%,specificity was 44.3%and the Youden index was 0.181.In the consistency test,the kappa value was 0.838(P<0.001).Conclusion The PNI is of limited value for assessing malnutrition,although it did have good consistency with the PG-SGA.The combination of the PNI and PG-SGA can be used for diagnosing assessing malnutrition in clinical practice. | Jian Jian Wei Hong Xia Yan Qi Li | 2021 | Journal of Nutritional Oncology2021,6,3: | 5 |
| 13 | β-hydroxy-β-methylbutyrate Improves Chronic Renal Failure-Induced Sarcopenia in SD Rats by Regulating Autophagy显示文摘Background Chronic renal failure-induced sarcopenia(CRF-S)seriously affects public health by increasing morbidity and mortality.This study evaluated the therapeutic effects ofβ-hydroxy-β-methylbutyrate acid(HMB)and the underlying mechanism in an adenine-induced CRF-S rat model.Methods Sprague Dawley rats were divided into control,model(CRF-S),and HMB treatment(CRF-S+HMB)groups.The rats in the CRF-S group received 250 mg/kg/d adenine by oral gavage every day for 14 days,followed by administration every other day until the 28th day.In the CRF-S+HMB group,each rat was given 320 mg/kg/d HMB by oral gavage starting from 7 days prior to adenine treatment,and this treatment was maintained for a total of 35 days.The body weight,right anterior tibial muscle weight,blood serum parameters,gastrocnemius muscle and renal tissue histology,and expression of autophagy-related transcripts and proteins were evaluated.Results A rat model of CRF-S was successfully established via adenine gavage,as indicated by reduced body weight and muscle weights,and markers of sarcopenia.There were increased mRNA levels of LC3B,Beclin1,Ulk1,Atg5,Atg7,Atg13 and vps-34,and increased protein expression levels of LC3B and P62.In the rats with HMB intervention,the body weight,right anterior tibial weight and gastrocnemius muscle histology were not significantly different from those in the control group,and the expression levels of autophagy-related proteins(LC3B and P62)were consistent with those in the control group.Conclusion We successfully established a rat model of CRF-S using adenine administered via oral gavage,and observed that HMB treatment prevented CRF-S. | Zun Yue Zhang Liu Qing Yang Ya Nan Yang Yun Lin Wang Long Mei Zhang Kun Hua Wang Min Weng | 2021 | Journal of Nutritional Oncology2021,6,4: | 4 |

| 15 | Malnutrition Increases the Risk of Mortality in Hospitalized Lung Cancer Patients显示文摘Background The Global Leadership Initiative on Malnutrition(GLIM)recently developed a new set of diagnostic criteria for identifying patients with malnutrition.Because the GLIM criteria were only introduced a little over 3 years ago,additional validation and reliability testing are needed in a variety of populations.Methods We performed an observational,multicenter cohort study.From July 2013 to October 2018,lung cancer patients were recruited from the Daping Hospital of Army Medical University and the First Hospital of Jilin University as part of the INSCOC project.Previously-established cut-off values for the calf circumference(CC,male<30 cm,female<29.5 cm)were applied as the reduced muscal mass of phenotypic criteria to establish the GLIM diagnosis.Multivariate Cox regression analyses were performed to analyze the association between the GLIM criteria and survival.Results A total of 1219 patients with lung cancer were studied as subjects.Their age was 58.81±9.92 years old,and 820 were male and 399 were female.According to the GLIM diagnostic criteria using the CC as a muscle mass measurement,303 patients(24.9%)were categorized as malnourished,142 patients(23.1%)in the adult group(18≤age<60)and 161 patients(26.7%)in the older group(age≥60 years).The patients with malnutrition had a higher incidence of anemia than the nourished patients(P=0.012).The QLQ-C30 score and KPS score indicating that the malnourished patients had a consistently worse quality of life compared to the nourished group(all P<0.001).The median survival of the malnutrition group was 42(95%CI:34-50)months,which was much shorter than the 62(95%CI:57-66)months in the nourished group(P<0.001).In the adult group,the median survival decreased from 65(95%CI:55-72)months in nourished group to 34(95%CI:25-48)months in the patients with malnutrition(P<0.001).In the older group,it decreased from 61(95%CI:55-67)months to 48(95%CI:39-59)months(P=0.001).A Cox regression analysis showed that GLIM-diagnosed malnutrition was associated with an increased risk of death among adult group(HR=1.670,95%CI:1.29-2.16),older group(HR=1.332,95%CI:1.05-1.69)and overall(HR=1.453,95%CI:1.22-1.72).Conclusion All of these results demonstrate that GLIM-diagnosed malnutrition is associated with a poorer survival for all lung cancer patients,independent of age. | Fei Fei Chong Liang Yu Yin Jie Liu Na Li Jing Guo Yang Fan Meng Yuan Zhang Ling Zhang Xiu Mei He Hong Mei Zhang Mu Li Shi Wei Li Hong Xia Xu | 2022 | Journal of Nutritional Oncology2022,7,1: | 4 |

| 17 | Combining the Patient-Generated Subjective Global Assessment (PGSGA) and Objective Nutrition Assessment Parameters Better Predicts Malnutrition in Elderly Patients with Colorectal Cancer显示文摘Background Malnutrition is common in colorectal cancer(CRC)patients,especially in elderly patients.The Patient-Generated Subjective Global Assessment(PG-SGA)is a widely used tool developed to detect malnutrition.The aim of this study was to compare the value of the PG-SGA and objective nutrition assessment parameters,in order to identify a better predictive index for malnutrition in elderly patients with CRC.Methods A total of 131 elderly patients(age≥60 years)with CRC were included and were evaluated for their individual nutritional status using the PG-SGA.Anthropometric and serological indicators were also assayed within 48 h of admission to the hospital.Body composition analysis was implemented by bioelectrical impedance analysis(BIA)instrument.The Chi-squared test,univariate and multivariate logistic regression analysis,or Spearman’s rank correlation analysis were used to determine the differences among the above indices and parameters with regard to predicting malnutrition.Results According to the PG-SGA score,the incidence of total malnutrition in elderly patients with CRC was 80.92%(PG-SGA score≥2),which increased with age.It was found that 28%of the patients with PG-SGA classification A(PG-SGA score 0-1)had a low fat-free mass index(FFMI).Compared with those with PG-SGA A,patients with PG-SGA C PG-SGA score≥9)showed an increased neutrophil to lymphocyte ratio(NLR)and an increased platelet and lymphocyte ratio(PLR)(median=1.78 VS.2.35,P=0.015 and median=108.8 VS.141.6,P=0.001,respectively).In terms of objective nutrition assessment parameters,severely malnourished CRC patients had significantly lower values of serum albumin(ALB),and retinol conjugated protein(RBP)than those who were well-nourished[(38.35±4.84)g/L VS.(40.56±3.44)g/L,P=0.039 and(30.31±15.83)mg/L VS.(39.01±11.95)mg/L,P=0.033,respectively].The Spearman’s rank correlation analysis showed that the PG-SGA findings had positive correlations with the NLR and PLR;while it had negative correlations with the FFMI,body mass index(BMI),ALB,prealbumin,and RBP.Conclusions In clinical practice,a comprehensive nutritional diagnosis,including the PG-SGA score and these objective indicators,can avoid the underdiagnosis of malnutrition,and is more suitable to detect malnutrition(as well as its causes)in elderly patients with CRC. | Wen Jun Wang Ting Ting Li Xu Wang Wei Li Jiu Wei Cui | 2020 | Journal of Nutritional Oncology2020,5,1: | 3 |

| 20 | Validity of the Patient-Generated Subjective Global Assessment(PG SGA)in Colorectal Cancer Patients in China显示文摘Objective The patient-generated subjective global assessment(PG-SGA)is a nutritional assessment tool specially designed for cancer patients.This study tested the validity of the PG-SGA for the nutritional assessment of colorectal cancer(CRC)patients in China.Methods A total of 8,093 Chinese patients with any stage CRC were enrolled in this cross-sectional,observational study.Within the first 48 hours of admission,patients were evaluated using the PG-SGA,nutritional risk screening 2002(NRS 2002),Karnofsky Performance Status(KPS),and some anthropometric parameters,including the triceps skinfold thickness(TSF),mid-arm circumference(MAC),mid-arm muscle circumference(MAMC),hand grip strength(HGS),maximum left calf circumference(MLCC),and maximum right calf circumference(MRCC),among others.Spearman’s correlation test was performed to analyze item-total score correlations and correlations between the total PG-SGA score and other parameters or performance scores,further testing convergent validity.The discriminative ability of the PG-SGA was measured by comparing different characteristics between several nutritional groups.A principal component analysis was performed with selected parameters to evaluate the construct validity.Results The average total PG-SGA score of all patients was 5.69±4.46,and they had a median age of 59 years(interquartile range(IQR),51-67 years)and an average body mass index(BMI)of 22.57±3.29 kg/m^(2).The item-total correlations of the total PG-SGA score between the global PG-SGA rating(Spearman,r=0.94)and PG-SGA score(patient-generated)(Spearman,r=0.97)were strong.Significant correlations were also found between the total PG-SGA score and other nutritional screening tools,including the BMI(Spearman,r=-0.26),KPS(Spearman,r=-0.36),and NRS 2002(Spearman,r=-0.47).Well-nourished(41.2%),mildly or moderately malnourished(35.3%)and severely malnourished(23.5%)groups defined according to the PG-SGA had significantly different characteristics.Patients with a worse nutritional status tended to have a decreased BMI(well-nourished,23.32 kg/m^(2)vs.mildly or moderately malnourished,22.52 kg/m^(2)vs.severely malnourished,21.35 kg/m^(2);P<0.001),hemoglobin,body fat mass,muscle mass,protein mass,TSF,MAC,HGS,MAMC,and MLCC,and a dramatically increased C-reactive protein level.The Kaiser-Meyer-Olkin measure was>0.7,and the P value of Bartlett’s test of sphericity was<0.001.One component was extracted by the principal component analysis,and the analysis showed that the total PG-SGA score explained the total variances of 97.3%.Conclusion The PG-SGA is a valid tool for evaluating the nutritional status for Chinese CRC patients. | Rui Kang Yin Ling Li Fu Ping Chen Zhen Ming Fu Hong Xia Xu Chun Hua Song Ming Hua Cong Han Ping Shi | 2021 | Journal of Nutritional Oncology2021,6,3: | 3 |
