| 1 | Contrast enhanced ultrasound of renal masses显示文摘Contrast enhanced ultrasound(CEUS) has gained clinical importance over the last years for the characterization of hepatic masses.Its role in extrahepatic indications has been investigated repeatedly but has been less comprehensively studied.Currently more than 50% of renal masses are incidentally diagnosed,mostly by B-mode ultrasound.The method of choice for characterization of renal lesions is contrast enhanced computed tomography(CECT).In the case of cystic lesions CECT refers to the Bosniak classification for cystic lesions to assess the risk of malignant behavior.The majority of masses are renal cell carcinoma,but the exact proportion is controversial.Disadvantages of CECT are a significant risk for patients with impaired renal function,allergic reactions and hyperthyroidism due to iodinated contrast agents.Several studies concerning CEUS for the characterization of both solid and cystic renal lesions have been published,but prospective multicenter studies are missing,the presented data being mainly descriptive.The aim of the this manuscript is to review the current literature for CEUS in renal masses,to summarize the available data and focus on possible concepts for studies in the future. | Andre Ignee Bad Mergentheim Gudrun Schuessler Christoph Frank Dietrich | 2010 | World Journal of Radiology2010，2，1： | 11 |

| 3 | Prospective observational multicenter study to define a diagnostic algorithm for biliary candidiasis显示文摘AIM:To develop an algorithm to improve the diagnosis and treatment of patients with biliary candidiasis.METHODS:We performed a prospective study of 127patients who underwent endoscopic retrograde cholangiopancreatography,for various biliary disorders,at 3 tertiary referral centers in Germany from July 2011through July 2012(ClinicalTrials.gov:NCT01109550).Bile,buccal,and stool samples were collected.When indicated,endoscopic transpapillary bile duct biopsies were performed to clarify the etiology of bile duct strictures and to prove invasive fungal infections.RESULTS:Candida species were detected in 38 of the 127 bile samples(29.9%).By multivariate analysis patients’age and previous endoscopic sphincterotomy were independent risk factors for biliary candidiasis(P<0.05).Patients with immunosuppression(P=0.058)and recent long-term antibiotic therapy(>7 d)(P=0.089)tend to be at risk for biliary candidiasis.One patient was negative in mycological culture of bile fluid but invasive biliary candidiasis was diagnosed histologically.Of Candida subspecies detected,36.7%were azole-resistant,such as C glabrata.Eight patients received anti-mycotic therapy,based on our algorithm.Of these,3 had cancer with biliary tract involvement,2had secondary sclerosing cholangitis,1 had retroperitoneal fibrosis,and 5 had septicemia.In all patients contamination was ruled out by smears of the endoscope channel.CONCLUSION:Gastroenterologists should be aware of frequent candida colonization in patients with cholangitis and biliary disorders.Our suggested algorithm facilitates the further clinical management. | Philipp Lenz Franziska Eckelskemper Thomas Erichsen Tim Lankisch Alexander Dechêne Gabriele Lubritz Frank Lenze Torsten Beyna Hansjorg Ullerich Andre Schmedt Dirk Domagk | 2014 | World Journal of Gastroenterology2014，20，34： | 2 |
| 4 | NOD2,IL23R and ATG16L1 polymorphisms in Lithuanian patients with inflammatory bowel disease显示文摘AIM:To investigate the frequency of NOD2, IL23R and ATG16L1 genetic variants in a case-control panel for inflammatory bowel disease (IBD) from Lithuania.METHODS: One hundred and eighty unrelated IBD pa- tients [57 Crohn's disease (CD) and 123 ulcerative colitis (UC)] and 186 healthy controls were genotyped for the following known genetic susceptibility variants:NOD2-Arg702Trp (rs2066844), Gly908Arg (rs2066845) and Leu1007insC (rs2066847), as well as IL23R-Arg381Gln (rs11209026) and ATG16L1-Thr300Ala (rs2241880).RESULTS:The effect that carriership of at least one NOD2 risk allele predisposes to CD was replicated in the Lithuanian population (41.1% CD vs 16.9% controls, P=2×10-4, OR=3.48,95% CI:1.81-6.72). In the allelic single marker analysis, Leu1007insC was strongly associated with CD (21.4% CD vs 4.7% controls, P=3.687×10-8, OR=5.54, 95% CI:2.85-10.75). Neither the other two NOD2 variants, nor the known variants in IL23R and ATG16L1 were found to be risk factors for CD, UC or IBD.
