1Correlation of transient elastography with hepatic venous pressure gradient in patients with cirrhotic portal hypertension: A study of 326 patients from India显示文摘AIM To study the diagnostic accuracy of transient elastography(TE) for detecting clinically significant portal hypertension(CSPH) in Indian patients with cirrhotic portal hypertension.METHODS This retrospective study was conducted at the Institute of Liver, Gastroenterology, and Panceatico-Biliary Sciences, Sir Ganga Ram Hospital, New Delhi, on consecutive patients with cirrhosis greater than 15 years of age who underwent hepatic venous pressure gradient(HVPG) and TE from July 2011 to May 2016. Correlation between HVPG and TE was analyzed using the Spearman's correlation test. Receiver operating characteristic ( ROC) curves were prepared for determining the utility of TE in predicting various stages of portal hypertension. The best cut-off value of TE forthe diagnosis of CSPH was obtained using the Youden index.RESULTS The study included 326 patients [median age 52(range 16-90) years; 81% males]. The most common etiology of cirrhosis was cryptogenic(45%) followed by alcohol(34%). The median HVPG was 16.0(range 1.5 to 30.5) mm Hg. Eighty-five percent of patients had CSPH. A significant positive correlation was noted between TE and HVPG(rho 0.361, P < 0.001). The area under ROC curve for TE in predicting CSPH was 0.740(95%CI: 0.662-0.818)(P < 0.01). A cut-off value of TE of 21.6 k Pa best predicted CSPH with a positive predictive value(PPV) of 93%.CONCLUSION TE has a fair positive correlation with HVPG; thus, TE can be used as a non-invasive modality to assess the degree of portal hypertension. 2在稍尖的巨大的肺的栓塞的机械故障和 thrombolysis : 未来的试用显示文摘 AIM: To assess role of combined modality of mechanical fragmentation and intralesional thrombolysis in patients with massive pulmonary embolism presenting subacutely. METHODS: Eight of 70 patients presenting in tertiary care centre of North India with massive pulmonary embolism within 4 years had subacute presentation (symptom onset more than 2 wk). These patients were subjected to pulmonary angiography with intention to treat basis via mechanical breakdown and intra lesional thrombolysis. Mechanical breakdown of embolus was accomplished with 5-F multipurpose catheter to reestablish flow, followed by intralesional infusion of urokinase (4400 IU/kg over 10 min followed by 4400 IU/kg per hour over 24 h). RESULTS: Eight patients, mean age 47.77±12.20 years presented with subacute pulmonary embolism (mean duration of symptoms 2.4 wk). At presentation, mean heart rate, shock index, miller score and mean pulmonary pressures were 101.5±15.2/min, 0.995±0.156, 23.87±3.76 and 37.62±6.67 mmHg which reduced to 91.5±12.2/min (P=0.0325), 0.789±0.139 (P=0.0019), 5.87±1.73 (P=0.0000004) and 27.75±8.66 mmHg (P=0.0003) post procedurally. Mean BP improved from 80.00±3.09 mmHg to 90.58±9.13 mmHg (P=0.0100) post procedurally. Minor complications in the form of local hematoma-minor hematoma in 1 (12.5%), and pseudoaneurysm (due to femoral artery puncture) in 1 (12.5 %) patient were seen. At 30 d and 6 mo follow up survival rate was 100% and all the patients were asymptomatic and in New York Heart Association class 1. 3All ileo-cecal ulcers are not Crohn's:Changing perspectives of symptomatic ileocecal ulcers显示文摘AIM To investigated clinical,endoscopic and histopathological parameters of the patients presenting with ileocecal ulcers on colonoscopy.METHODS Consecutive symptomatic patients undergoing colonoscopy,and diagnosed to have ulcerations in the ileocecal(I/C) region,were enrolled.Biopsy was obtained and theirclinical presentation and outcome were recorded.RESULTS Out of 1632 colonoscopies,104 patients had ulcerations in the I/C region and were included in the study.Their median age was 44.5 years and 59% were males.The predominant presentation was lower GI bleed(55,53%),pain abdomen ± diarrhea(36,35%),fever(32,31%),and diarrhea alone(9,9%).On colonoscopy,terminal ileum was entered in 96(92%) cases.The distribution of ulcers was as follows:Ileum alone 40%(38/96),cecum alone 33%(32/96),and both ileum plus cecum 27%(26/96).The ulcers were multiple in 98% and in 34% there were additional ulcers elsewhere in colon.Based on clinical presentation and investigations,the etiology of ulcers was classified into infective causes(43%) and noninfective causes(57%).Fourteen patients(13%) were diagnosed to have Crohn's disease(CD).CONCLUSION Non-specific ileocecal ulcers are most common ulcers seen in ileo-cecal region.And if all infections are clubbed together then infection is the most common(> 40%) cause of ulcerations of the I/C region.Cecal involvement and fever are important clues to infective cause.On the contrary CD account for only 13% cases as a cause of ileo-cecalulcers.So all symptomatic patients with I/C ulcers on colonoscopy are not Crohn's.Jay Toshniwal Romesh Chawlani Amit Thawrani Rajesh Sharma Anil Arora Hardik L Kotecha Mohan Goyal Vijendra Kirnake Pankaj Jain Pankaj Tyagi Naresh Bansal Munish Sachdeva Piyush Ranjan Mandhir Kumar Praveen Sharma Vikas Singla Rinkesh Bansal Vineet Shah Sunita Bhalla Ashish Kumar 2017World Journal of Gastrointestinal Endoscopy2017,9,7:5

6Influence of peach(Prunus persica Batsch)phenological stage on the short-term changes in oxidizable and labile pools of soil organic carbon and activities of carbon-cycle enzymes in the North-Western Himalayas显示文摘The labile organic carbon(C)and C-related enzymes are sensitive indicators capturing alterations of soil organic matter(SOM),even in a short-time scale.Although the effects of crop husbandry and land use change on these attributes have been well studied,there is no consensus about how plant phenology may impact them.This study aimed to determine the short-term effect o f six distinct phenological stages(PS-1:full bloom;PS-2:fruit set;PS-3:pit hardening;PS-4:physiological maturity;PS-5:60 d after physiological maturity;and PS-6:fall)o f peach on the changes in soil organic carbon(SOC)fractions of different oxidizability,labile C pools,and C-cycle enzyme activities in soils,for two consecutive years(2015 and 2016)in the North-Western Himalayas(NWH).Peach rhizosphere soils were sampled at the topsoil(0-15 cm)and subsoil(16-30 cm)layers,along with rhizosphere soils from adjacent perennial grasses,which served as a control.Values for most of the assessed parameters,including very labile C,labile C,microbial biomass C,permanganate oxidizable C,dissolved organic C,mineralizable C,amylase activity,and carboxymethyl-cellulase activity,were significantly(P<0.05)higher at PS-3 than at other phenological stages of peach.Conversely,a sudden decline in these soil variables was recorded at PS-5,followed by a slight buildup at PS-6,particularly in the topsoil of the peach orchard.Short-term changes in organic C fractions of different oxidizability,influenced by peach phenological stage,significantly(P<0.05)affected C management index,C pool index,and lability index.Both the C management index and lability index showed their highest values at PS-3 and their lowest values at PS-5,clearly indicating short-term accretion and depletion of SOC,in tandem with the peach phenological events.Principal component analysis suggested that a composite of soil indicators,including microbial biomass C,dissolved organic C,amylase,and invertase,could help detect short-term changes in SOC content.It is concluded that peach phenological events had a major impact on the short-term variations of the studied soil variables,which could be attributed to changes in the above-and belowground plant residues,as well as the extent of nutrients and water acquisition.Sovan DEBNATH Brij Lai ATTRI Anil KUMAR Arun KISHOR Raj NARAYAN Kanchan SINHA Arpan BHOWMIK Anil SHARMA Desh Beer SINGH 2020Pedosphere2020,30,5:2

10On-road emissions of CO,CO2 and NOx from four wheeler and emission estimates for Delhi显示文摘This study presents the emission factor of gaseous pollutants(CO, CO_2, and NO X) from on-road tailpipe measurement of 14 passenger cars of different types of fuel and vintage. The trolley equipped with stainless steel duct, vane probe velocity meter, flue gas analyzer, Nondispersive infra red(NDIR) CO_2 analyzer, temperature, and relative humidity(RH) sensors was connected to the vehicle using a towing system. Lower CO and higher NO X emissions were observed from new diesel cars(post 2010) compared to old cars(post 2005), which implied that new technological advancement in diesel fueled passenger cars to reduce CO emission is a successful venture,however, the use of turbo charger in diesel cars to achieve high temperature combustion might have resulted in increased NO X emissions. Based on the measured emission factors(g/kg), and fuel consumption(kg), the average and 95% confidence interval(CI) bound estimates of CO, CO_2,and NO X from four wheeler(4W) in Delhi for the year 2012 were 15.7(1.4–37.1), 6234(386–12,252),and 30.4(0.0–103) Gg/year, respectively. The contribution of diesel, gasoline and compressed natural gas(CNG) to total CO, CO_2 and NO X emissions were 7:84:9, 50:48:2 and 58:41:1respectively. 13Reducing shoulder dislocation without anaesthesia or assistant:Validation of a new reduction manoeuvre显示文摘Purpose:Shoulder dislocation comprises 60%of all major joint dislocations worldwide and a number of reduction techniques are described in the literature w让h varying degrees of success.The description of a large number of techniques speaks for itself that no method is effective all the times and one should be acquainted with more than one technique.An ideal method of reduction should be simple,easily reproducible,relatively painless that can be performed unassisted without sedation or anaesthesia w让h minimal or no further complications.We report our results of using a novel method of anterior shoulder reduction described recently in the literature that claims to fulfil most of the characteristics of an ideal method if not all.Methods:This prospective study was conducted in a tertiary care centre.All the cases of primary anterior shoulder dislocation presenting within three days of injury without any associated fracture or spine trauma with or without greater tuberosity fracture were included.The reduction was done using a novel method by orthopaedic residents in all cases.The need for a second reduction attempt or anaesthesia was considered a treatment failure.Time taken for reduction,pain felt during reduction and complications if any were noted.Results:There were 47(77.04%)males and 14(22.95%)females with a mean age of(37.04±12.63)years.The new technique was effective in locating a shoulder dislocation on the first attempt in 58 of the 61 dislocated shoulders(95.08%).The remaining three shoulders were reduced on second attempt by the same technique.Sedative,pre-medication or anaesthesia was not used in any case.The average time taken for the shoulder reductio n was(130.5±25.8)sec onds and con fide nee interval(95%)124-137 s.Iatrogenic complications were not seen in any of the patients.Conclusion:This relatively painless technique of shoulder reduction is easy to acquire and practice in emergency department.The advantages of this manoeuvre and its associated safety may justly lead surgeons to select it as their primary method for reduction of anterior shoulder dislocations.Rashid Anjum Subodh Pathak Atul Rai Sharma Jatin Aggarwal Aryan Sharma Vineet Pruthi Anil Kumar Chaudhary 2019Chinese Journal of Traumatology2019,22,5:1

15Phosphate uptake and translocation in a tropical Canna-based constructed wetland显示文摘Introduction:Considering the problem of eutrophication of the water bodies,phosphate removal from water has emerged as a research of topical interest.The present study aims to investigate the efficacy of Canna lily-based constructed wetland to remove phosphate from wastewater.The translocation of phosphate in plant tissue and its biochemical transformation in sediments is also studied to understand its accumulation and recirculation within the system.Results:The removal of phosphate stabilized at around 50%in the present study and plant uptake was found to be the major removal mechanism.Average removal was 167 mg/m2 day for total phosphate and 84 mg/m2 day for available phosphate for an initial loading rate of 200 mg/m2 day and 85 mg/m2 day,respectively,at a HRT of 24 h.Most of the phosphate concentrated in above ground tissue of plant and its relative accumulation was maximum in flowers.Fractionation of phosphate in sediments confirmed removal by sediments with an accumulation of apatite phosphate(Ca and Mg bound),but release of non-apatite form(Fe and Al bound).Conclusion:The study concludes that Canna-based constructed wetland can be an effective tool for phosphate removal from wastewater and sediments particularly under tropical conditions.Regular harvesting of above ground tissue of Canna can result in nutrient export from the system,whereas autochthonous addition may result in recirculation.Anil Kumar Haritash Sarbari Dutta Ashish Sharma 2017Ecological Processes2017,6,1:1

19Resolving the Colletotrichum siamense species complex using ApMat marker显示文摘Colletotrichum gloeosporioides sensu lato has been associated with anthracnose in diverse commercial crops.It is now established that C.gloeosporioides sensu lato comprises 33 phylogenetic species and C.gloeosporioides sensu stricto is not a common pathogen of tropical fruits.In this study,we investigated the phylogenetic relationships of 85 Colletotrichum isolates associated with select tropical fruits and flowering plants from India.In the ApMat marker analysis,the 85 isolates clustered with 7 known Colletotrichum species(C.aotearoa,C.dianesei,C.endomangiferae,C.musae,C.siamense,C.theobromicola,Glomerella cingulata f.sp.camelliae)and six novel lineages.One of the novel lineages is described and illustrated in this paper as Colletotrichum communis sp.nov.,while new-host pathogen associations for C.aotearoa,C.endomangiferae,C.dianesei and C.theobromicola are reported from India.Out of the 85 isolates analysed in this paper,73 isolates clustered within the C.siamense species complex,indicating that C.siamense species complex,not C.gloeosporioides sensu stricto,is common on tropical fruits.In comparison with act,cal,gapdh,ITS and tub2 gene markers,we recommend the use of the ApMat marker for accurate identification of cryptic species within the C.siamense species complex.We believe that the ApMat marker,in combination with one or two similar‘phylogenetically superior’gene markers,is a better candidate for specieslevel classification of fungi that were traditionally identified as‘Colletotrichum gloeosporioides’.Gunjan Sharma Anil Kumar Pinnaka Belle Damodara Shenoy 2015Fungal Diversity2015,,2:1
