| 2 | Prognostic significance of tumor immune microenvironment and immunotherapy:Novel insights and future perspectives in gastric cancer显示文摘Despite a decrease in gastric cancer incidence, the development of novel biologic agents and combined therapeutic strategies, the prognosis of gastric cancer remains poor. Recently, the introduction of modern immunotherapy, especially using immune checkpoint inhibitors, led to an improved prognosis in many cancers. The use of immunotherapy was also associated with manageable adverse event profiles and promising results in the treatment of patients with gastric cancer, especially in heavily pretreated patients. These data have led to an accelerated approval of some checkpoint inhibitors in this setting. Understanding the complex relationship between the host immune microenvironment and tumor and the immune escape phenomenon leading to cancer occurrence and progression will subsequently lead to the identification of prognostic immune markers. Furthermore, this understanding will result in the discovery of both new mechanisms for blocking tumor immunosuppressive signals and pathways to stimulate the local immune response by targeting and modulating different subsets of immune cells. Due to the molecular heterogeneity of gastric cancers associated with differentclinico-biologic parameters, immune markers expression and prognosis, novel immunotherapy algorithms should be personalized and addressed to selected subsets of gastric tumors, which have been proven to elicit the best clinical responses. | 4 | New advances in targeted gastric cancer treatment显示文摘Despite a decrease in incidence over past decades,gastric cancer remains a major global health problem. In the more recent period, survival has shown only minor improvement, despite significant advances in diagnostic techniques, surgical and chemotherapeutic approaches, the development of novel therapeutic agents and treatment by multidisciplinary teams. Because multiple genetic mutations, epigenetic alterations, and aberrant molecular signalling pathways are involved in the development of gastric cancers, recent research has attempted to determine the molecular heterogeneity responsible for the processes of carcinogenesis, spread and metastasis. Currently, some novel agents targeting a part of these dysfunctional molecular signalling pathways have already been integrated into the standard treatment of gastric cancer, whereas others remain in phases of investigation within clinical trials. It is essential to identify the unique molecular patterns of tumours and specific biomarkers to develop treatments targeted to the individual tumour behaviour. This review analyses the global impact of gastric cancer, as well as the role of Helicobacter pylori infection and the efficacy of bacterial eradication in preventing gastric cancer development. | 8 | Malignant hepatic vascular tumors in adults: Characteristics,diagnostic difficulties and current management显示文摘Malignant vascular tumors of the liver include rare primary hepatic mesenchymal tumors developed in the background of a normal liver parenchyma. Most of them are detected incidentally by the increased use of performing imaging techniques. Their diagnosis is challenging, involving clinical and imaging criteria, with final confirmation by histology and immunohistochemistry. Surgery represents the mainstay of treatment. Liver transplantation(LT) has improved substantially the prognosis of hepatic epithelioid hemangioendothelioma(HEHE), with 5-year patient survival rates of up to 81%, based on the European Liver Intestine Transplantation AssociationEuropean Liver Transplant Registry study. Unfortunately, the results of surgery and LT are dismal in cases of hepatic angiosarcoma(HAS). Due to the disappointing results of very short survival periods of approximately 6-7 mo after LT, because of tumor recurrence and rapid progression of the disease, HAS is considered an absolute contraindication to LT. Recurrences after surgical resection are high in cases of HEHE and invariably present in cases of HAS. The discovery of reliable prognostic markers and the elaboration of prognostic scores following LT are needed to provide the best therapeutic choice for each patient.Studies on a few patients have demonstrated the stabilization of the disease in a proportion of patients with hepatic vascular tumors using novel targeted antiangiogenic agents, cytokines or immunotherapy. These new approaches,alone or in combination with other therapeutic modalities, such as surgery and classical chemotherapy, need further investigation to assess their role in prolonging patient survival. | 10 | Sodium alendronate loaded poly(L-lactideco-glycolide)microparticles immobilized on ceramic scaffolds for local treatment of bone defects显示文摘Bone tissue regeneration in critical-size defects is possible after implantation of a 3D scaffold and can be additionally enhanced once the scaffold is enriched with drugs or other factors supporting bone remodelling and healing.Sodium alendronate(Aln),a widely used anti-osteoporosis drug,exhibits strong inhibitory effect on bone resorption performed by osteoclasts.Thus,we propose a new approach for the treatment of bone defects in craniofacial region combining biocompatible titanium dioxide scaffolds and poly(L-lactide-co-glycolide)microparticles(MPs)loaded with Aln.The MPs were effectively attached to the surface of the scaffolds’pore walls by human recombinant collagen.Drug release from the scaffolds was characterized by initial burst(2466% of the drug released within first 24 h)followed by a sustained release phase(on average 5 mg of Aln released per day from Day 3 to Day 18).In vitro tests evidenced that Aln at concentrations of 5 and 2.5 mg/ml was not cytotoxic for MG-63 osteoblast-like cells(viability between 8166% and 9863% of control),but it prevented RANKL-induced formation of osteoclast-like cells from macrophages derived from peripheral blood mononuclear cells,as shown by reduced fusion capability and decreased tartrateresistant acid phosphatase 5b activity(5665% reduction in comparison to control after 8 days of culture).Results show that it is feasible to design the scaffolds providing required doses of Aln inhibiting osteoclastogenesis,reducing osteoclast activity,but not affecting osteoblast functions,which may be beneficial in the treatment of critical-size bone tissue defects. | Lucja Rumian Cornelia Wolf-Brandstetter Sina Roβler Katarzyna Reczynska Hanna Tiainen Havard J.Haugen Dieter Scharnweber Elzbieta Pamula | 2020 | Regenerative Biomaterials2020,7,3: | 5 |

| 15 | Multiple gastrointestinal stromal tumors and bilateral pheochromocytoma in neurofibromatosis显示文摘The coincidence of a gastrointestinal stromal tumor (GIST) and a neuroendocrine tumor (NET) in neurofibromatosis type 1 (NF1) is described only five times within the literature. We report on a 63 year old Caucasian female with the rare condition of neurofibromatosis type 1 coinciding with recurrent gastrointestinal stromal tumor plus bilateral pheochromocytoma (PCC). After a history of palpitations and dizziness that lasted for years, a left adrenal mass was detected by CT. Laparotomy revealed a pheochromocytoma of the left adrenal gland while an ileoterminal GIST was found incidentally intraoperatively. After six months contralateral PCC and multiple recurrent GIST were resected again. After four years the patient is doing well without any signs of further recurrent tumors. | 16 | Can periprosthetic hip joint infections be successfully managed by debridement and prosthesis retention?显示文摘To evaluate the current literature about how successfully periprosthetic hip joint infections can be managed by debridement and prosthesis retention.A literature search was performed through Pub Med until September 2013.Search terms were'DAIR(debridement,antibiotics,irrigation,and retention)'alone and in combination with'hip'as well as'hip infection+prosthesis retention'.A total of 11 studies reporting on 292 cases could be identified.Five different treatment modalities have been described with varying success rates(debridement-21%infection eradication rate;debridement+lavage-75%infection eradication rate;debridement,lavage,with change of modular prosthesis components-70.4%infection eradication rate;debridement,lavage,change of modular prosthesis components+vacuum-assisted closure-92.8%infection eradication rate;acetabular cup removal+spacer head onto retained stem-89.6%infection eradication rate).With regard to the postoperative antibiotic therapy,no general consensus could be drawn from the available data.Debridement,antibiotic therapy,irrigation,and prosthesis retention is an acceptable solution in the management of early and acute hematogenous periprosthetic hip joint infections.The current literature does not allow for generalization of conclusions with regard to thebest treatment modality.A large,multi-center study is required for identification of the optimal treatment of these infections. | Konstantinos Anagnostakos Cornelia Schmitt | 2014 | World Journal of Orthopedics2014,5,3: | 3 |

| 18 | 儿童癫痫中的神经元抗体:临床特征和未经免疫治疗的历史队列远期预后显示文摘神经元细胞表面抗体在自身免疫性脑炎的发病中起着明确的作用;早期诊断和治疗的患者预后更好。在不伴脑炎的儿童癫痫中也见神经元抗体阳性的报道。文章旨在评估神经元抗体对儿童癫痫患者远期预后是否有影响。该前瞻性研究患者来自荷兰儿童癫痫研究(Dutch Study of Epilepsy,DSEC)的4个中心,于1988年-1992年期间招募患者(n=178),均未接受免疫治疗。以健康且年龄匹配的骨髓捐赠者作为正常对照(n=112)。所有受试者均使用标准方法,检测血清N-甲基-D-天冬氨酸受体(N-methyl-D-aspartate,NMDAR)、α氨基-3-羟基-5-甲基-4-异恶唑丙酸受体、富含亮氨酸胶质瘤失活1蛋白,接触蛋白相关蛋白2(Contactin associated protein like 2,CASPR2)、contactin-2、谷氨酸脱羧酶和电压门控钾通道(Voltage gated potassium channel,VGKC)-复合物抗体。均未使用脑脊液(Cerebrospinal fluid,CSF)样本检测。并将抗体检测结果与随访15年以上的临床资料进行相关性分析。17例患者(9.5%)神经元抗体检测为阳性,分别为VGKC复合物(n=3),NMDAR(n=7),CASPR2(n=4)和contactin-2(n=3),同时有3名(3/112,2.6%)健康对照者神经元抗体检测为阳性,VGKC复合物(n=1),NMDAR(n=2)(P=0.03)。虽然抗体滴度相对较低(细胞表面抗体≤1:100),但17例阳性样本中有8例(47%)可与活性海马神经元表面结合,提示具有潜在的致病性。在抗体阳性患者中更多见预先存在的认知障碍(9/17vs.33/161,P=0.01)。14例抗体阳性患者接受了规范的抗癫痫药物(AEDs)治疗。其中3例(17%)为耐药性癫痫,但与161例抗体阴性的患者中16例为耐药性癫痫(16人,10%)相比,不存在统计学差异。在6和/或12个月有随访样本的96例患者中,7例之前抗体阳性患者中6例抗体转阴,相反,另有7例患者在随访时第一次出现了抗体阳性。在9.5%的儿童新发癫痫患者中发现低水平的神经元抗体阳性,虽然抗体不一定会持续存在,但在随访中可见神经元抗体由阴性转为阳性,这表明抗体可能是由于神经元的损伤或炎症的继发反应所产生的。此外,由于抗体阳性的儿童癫痫患者对规范AEDs的反应和远期预后与抗体阴性患者没有差异,提示在儿童癫痫中常规进行神经元抗体检测意义不大。抗体阳性组中预先存在的认知障碍的发生率较高,17例患者中7例患者CASPR2和contactin-2抗体阳性,以及17例血清样品中8例与活性海马神经元的结合表明,即使是继发反应,神经元抗体也可能参与到儿童癫痫的共病发生中。 | Sukhvir Wright Ada T Geerts Cornelia Maria Jol-van der Zijde 熊维希 慕洁 | 2017 | 癫痫杂志2017,0,4: | 3 |
| 19 | Dental stem cells:The role of biomaterials and scaffolds in developing novel therapeutic strategies显示文摘Dental stem cells(DSCs)are self-renewable cells that can be obtained easily from dental tissues,and are a desirable source of autologous stem cells.The use of DSCs for stem cell transplantation therapeutic approaches is attractive due to their simple isolation,high plasticity,immunomodulatory properties,and multipotential abilities.Using appropriate scaffolds loaded with favorable biomolecules,such as growth factors,and cytokines,can improve the proliferation,differentiation,migration,and functional capacity of DSCs and can optimize the cellular morphology to build tissue constructs for specific purposes.An enormous variety of scaffolds have been used for tissue engineering with DSCs.Of these,the scaffolds that particularly mimic tissue-specific micromilieu and loaded with biomolecules favorably regulate angiogenesis,cell-matrix interactions,degradation of extracellular matrix,organized matrix formation,and the mineralization abilities of DSCs in both in vitro and in vivo conditions.DSCs represent a promising cell source for tissue engineering,especially for tooth,bone,and neural tissue restoration.The purpose of the present review is to summarize the current developments in the major scaffolding approaches as crucial guidelines for tissue engineering using DSCs and compare their effects in tissue and organ regeneration. | Cornelia Larissa Granz Ali Gorji | 2020 | World Journal of Stem Cells2020,12,9: | 3 |
| 20 | Value of a comprehensive geriatric assessment for predicting one-year outcomes in patients undergoing transcatheter aortic valve implantation: results from the CGA-TAVI multicentre registry显示文摘Background In a three-month report from the CGA-TAVI registry, we found the Multidimensional Prognostic Index (MPI) and Short Physical Performance Battery (SPPB) to be of value for predicting short-term outcomes in elderly patients undergoing transcatheter aortic valve implantation (TAVI). In the present analysis, we examined the association of these tools with outcomes up to one year post-TAVI. Methods CGA-TAVI is an international, observational registry of geriatric patients undergoing TAVI. Patients were assessed using the MPI and SPPB. Efficacy of baseline values and any postoperative change for predicting outcome were established using logistic regression. Kaplan- Meier analysis was carried out for each comprehensive geriatric assessment tool, with survival stratified by risk category. Results One year after TAVI, 14.1% of patients deceased, while 17.4% met the combined endpoint of death and/or non-fatal stroke, and 37.7% the combined endpoint of death and/or hospitalisation and/or non-fatal stroke. A high-risk MPI score was associated with an increased risk of all-cause mortality (aOR = 36.13, 95% CI: 2.77–470.78, P = 0.006) and death and/or non-fatal stroke (aOR = 10.10, 95% CI: 1.48–68.75, P = 0.018). No significant associations were found between a high-risk SPPB score and mortality or two main combined endpoints. In contrast to a worsening SPPB, an aggravating MPI score at three months post-TAVI was associated with an increased risk of death and/or non-fatal stoke at one year (aOR = 95.16, 95% CI: 3.41–2657.01).
