| 4 | Interstitial lung disease in rheumatoid arthritis:Current concepts in pathogenesis,diagnosis and therapeutics显示文摘Rheumatoid arthritis(RA) is the most common chronic autoimmune inflammatory joint disease. RA-associated interstitial lung disease(RA-ILD) is a major extraarticular complication and causes symptoms that lead to a deterioration in the quality of life, high utilization of health resources, and an increased risk of earlier mortality. Early in the course of RA-ILD, symptoms are highly variable, making the diagnosis difficult. Therefore, a rational diagnostic strategy that combines an adequate clinical assessment with the appropriate use of clinical tests, including pulmonary function tests and high-resolution computed tomography, should be used. In special cases, lung biopsy or bronchioalveolar lavage should be performed to achieve an early diagnosis. Several distinct histopathological subtypes of RA-ILD are currently recognized. These subtypes also have different clinical presentations, which vary in therapeutic response and prognosis. This article reviews current evidence about the epidemiology of RA-ILD and discusses the varying prevalence rates observed in different studies. Additionally, aspects of RA-ILD pathogenesis, including the role of cytokines and other molecules such as autoantibodies, as well as the evidence linking several drugs used to treat RA with lung damage will be discussed. Some aspects of the clinical characteristics of RA-ILD are noted, and diagnostic strategies are reviewed. Finally, this article analyzes current treatments for RA-ILD, including immunosuppressive therapies and biologic agents, as well as other therapeutic modalities. The prognosis of this severe complication of RA is discussed. These rearrangements lead to the scrambling of the elements of one genome with respect to another descended from a common ancestor. Multidisciplinary work undertakes to mathematically model these processes and to develop statistical analyses and mathematical algorithms to understand the scrambling in the chromosomes of two or more related genomes. | 11 | Exploring technologies to better link physical evidence and digital information for disaster victim identification显示文摘Disaster victim identification(DVI)entails a protracted process of evidence collection and data matching to reconcile physical remains with victim identity.Technology is critical to DVI by enabling the linkage of physical evidence to information.However,labelling physical remains and collecting data at the scene are dominated by low-technology paper-based practices.We ask,how can technology help us tag and track the victims of disaster?Our response to this question has two parts.First,we conducted a human–computer interaction led investigation into the systematic factors impacting DVI tagging and tracking processes.Through interviews with Australian DVI practitioners,we explored how technologies to improve linkage might fit with prevailing work practices and preferences;practical and social considerations;and existing systems and processes.We focused on tagging and tracking activities throughout the DVI process.Using insights from these interviews and relevant literature,we identified four critical themes:protocols and training;stress and stressors;the plurality of information capture and management systems;and practicalities and constraints.Second,these findings were iteratively discussed by the authors,who have combined expertise across electronics,data science,cybersecurity,human–computer interaction and forensic pathology.We applied the themes identified in the first part of the investigation to critically review technologies that could support DVI practitioners by enhancing DVI processes that link physical evidence to information.This resulted in an overview of candidate technologies matched with consideration of their key attributes.This study recognises the importance of considering human factors that can affect technology adoption into existing practices.Consequently,we provide a searchable table(as Supplementary information)that relates technologies to the key considerations and attributes relevant to DVI practice,for readers to apply to their own context.While this research directly contributes to DVI,it also has applications to other domains in which a physical/digital linkage is required,and particularly within high stress environments with little room for error. | David Lovell Kellie Vella Diego Muñoz Matt McKague Margot Brereton Peter Ellis | 2022 | Forensic Sciences Research2022,7,3: | 0 |
| 12 | Pathological response to neoadjuvant therapy with chemotherapy vs chemoradiotherapy in stage III NSCLC-contribution of IASLC recommendations显示文摘BACKGROUND Neoadjuvant treatment(NT)with chemotherapy(Ch)is a standard option for resectable stage III(N2)NSCLC.Several studies have suggested benefits with the addition of radiotherapy(RT)to NT Ch.The International Association for the Study of Lung Cancer(IASLC)published recommendations for the pathological response(PHR)of NSCLC resection specimens after NT.AIM To contribute to the IASLC recommendations showing our results of PHR to NT Ch vs NT chemoradiotherapy(ChRT).METHODS We analyzed 67 consecutive patients with resectable stage III NSCLC with positive mediastinal nodes treated with surgery after NT Ch or NT ChRT between 2013 and 2020.After NT,all patients were evaluated for radiological response(RR)according to Response Evaluation Criteria in Solid Tumours criteria and evaluated for surgery by a specialized group of thoracic surgeons.All histological samples were examined by the same two pathologists.PHR was evaluated by the percentage of viable cells in the tumor and the resected lymph nodes.RESULTS Forty patients underwent NT ChRT and 27 NT Ch.Fifty-six(83.6%)patients underwent surgery(35 ChRT and 21 Ch).The median time from ChRT to surgery was 6 wk(3-19)and 8 wk(3-21)for Ch patients.We observed significant differences in RR,with disease progression in 2.5%and 14.8%of patients with ChRT and Ch,respectively,and partial response in 62.5%ChRT vs 29.6%Ch(P=0.025).In PHR we observed≤10%viable cells in the tumor in 19(54.4%)and 2 cases(9.5%),and in the resected lymph nodes(RLN)30(85.7%)and 7(33.3%)in ChRT and Ch,respectively(P=0.001).Downstaging was greater in the ChRT compared to the Ch group(80%vs 33.3%;P=0.002).In the univariate analysis,NT ChRT had a significant impact on partial RR[odds ratio(OR)12.5;95%confidence interval(CI):1.21-128.61;P=0.034],a decreased risk of persistence of cancer cells in the tumor and RLN and an 87.5%increased probability for achieving downstaging(OR 8;95%CI:2.34-27.32;P=0.001).CONCLUSION We found significant benefits in RR and PHR by adding RT to Ch as NT.A longer follow-up is necessary to assess the impact on clinical outcomes. | Diego Muñoz-Guglielmetti David Sanchez-Lorente Roxana Reyes Daniel Martinez Carmen Lucena Marc Boada Pilar Paredes Marta Parera-Roig Ivan Vollmer Joel Mases Roberto Martin-Deleon Sergi Castillo Mariana Benegas Silvia Muñoz Maria Mayoral Carla Cases Meritxell Mollà Francesc Casas | 2021 | World Journal of Clinical Oncology2021,12,11: | 0 |

| 15 | Effects of fire history on animal communities:a systematic review显示文摘Fire is a natural agent with a paramount role in ecosystem functioning and biodiversity maintenance.Still,it can also act as a negative force against many ecosystems.Despite some knowledge of the interactions of fire and vegetation,there is no clear understanding of how different components of fire regimes(i.e.,severity,history,or frequency)influence known patterns of animal communities.Therefore,we performed a systematic review on the global responses of arthropods,birds,mammals,reptiles,and amphibians to different fire regimes.Specifically,we focused on assessing how fire severity,history,and frequency modulate the effect of fire on the richness and abundance of faunal communities.We conducted a systematic review of 566 papers retrieved from the Scopus database.We also scrutinized all the documents included in the meta‑analysis of Pastro et al.(Pastro et al.Glob Ecol Biogeogr 23:1146–1156,2014).Our selection criteria excluded studies without data on species richness or abundance.We also excluded studies without adequate controls and those without information about the fire regime of the study zone.After careful examination,we used data from 162 studies to perform a quantitative meta‑analysis.From the 162 studies meeting our selection criteria,nearly 60%of the studies are from North America,25%from Australia,11%from Europe,and 4%from the tropics.According to the ecological role of fire,90%of the studies were carried out in fire‑dependent ecosystems(i.e.,conifer forests,natural savannas,pastures).Finally,40%of the studies analyzed birds,22%mammals,and 20%arthropods.The meta‑analysis of the available evidence indicates that fire history is an important modulator of animal richness and abundance.Whether negative or positive,animal responses depended on the time since the last fire event.Considering that short‑term studies may not capture such a long‑term effect on fauna,this translates to more challenges at implementing fire management strategies.Whether or not we can anticipate the impact of the fire will then depend on future efforts to implement long‑term research. | Tania Marisol González Juan David González‑Trujillo Alberto Muñoz Dolors Armenteras | 2022 | Ecological Processes2022,11,1: | 0 |
