| 2 | Adalimumab in prevention of postoperative recurrence of Crohn's disease in high-risk patients显示文摘AIM:To evaluate the effectiveness of adalimumab in preventing recurrence after intestinal resection for Crohn's disease in high-risk patients.METHODS:A multicenter,prospective,observational study was conducted from June 2009 until June 2010.We consecutively included high-risk Crohn's disease patients who had undergone an ileal/ileocolonic resection.High-risk patients were defined as two or more criteria:smokers,penetrating pattern,one or more previous surgical resections or prior extensive resection.Subcutaneous adalimumab was administered 2 wk(± 5 d) after surgery at a dose of 40 mg eow,with an initial induction dose of 160/80 mg at weeks 0 and 2.Demographic data,previous and concomitant treatments(antibiotics,5-aminosalicylates,corticosteroids,immunomodulators or biologic therapies),smoking status at the time of diagnosis and after the index operation and number of previous resections(type and reason for surgery) were all recorded.Biological status was assessed with C-reactive protein,erythrocyte sedimentation rate and fecal calprotectin.One year(± 3 mo) after surgery,an ileocolonoscopy and/or magnetic resonance enterography was performed.Endoscopic recurrence was defined as Rutgeerts score ≥ i2.Morphological recurrence was based on magnetic resonance(MR) score ≥ MR1.RESULTS:Twenty-nine patients(55.2% males,48.3% smokers at diagnosis and 13.8% after the index operation),mean age 42.3 years and mean duration of the disease 13.8 years were included in the study.A mean of 1.76(range:1-4) resections previous to adalimumab administration and in 37.9% was considered extensive resection.51.7% had previously received infliximab.Immunomodulators were given concomitantly to 17.2% of patients.Four of the 29(13.7%) developed clinical recurrence,6/29(20.7%) endoscopic recurrence and 7/19(36.8%) morphological recurrence after 1-year.All patients with clinical recurrence showed endoscopic and morphological recurrence.A high degree of concordance was found between clinical-endoscopic recurrence(k = 0.76,P < 0.001) and clinical-morphological recurrence(k = 0.63,P = 0.003).Correlation between endoscopic and radiological findings was good(comparing the 5-point Rutgeerts score with the 4-point MR score,a score of i4 was classified as MR3,i3 as MR2,and i2-i1 as MR1)(P < 0.001,r s = 0.825).During follow-up,five(17.2%) patients needed adalimumab dose intensification(40 mg/wk);Mean time to intensification after the introduction of adalimumab treatment was 8 mo(range:5 to 11 mo).In three cases(10.3%),a biological change was needed due to a worsening of the disease after the dose intensification to 40 mg/wk.One patient suffered an adverse event.CONCLUSION:Adalimumab seems to be effective and safe in preventing postoperative recurrence in a selected group of patients who had undergone an intestinal resection for their CD. | Mariam Aguas Guillermo Bastida Elena Cerrillo Belén Beltrán Marisa Iborra Cristina Sánchez-Montes Fernando Muoz Jesús Barrio Sabino Riestra Pilar Nos | 2012 | World Journal of Gastroenterology2012,18,32: | 3 |
| 3 | Occult hepatitis B virus infection among Mexican human immunodeficiency virus-1-infected patients显示文摘AIM: To determine the frequency of occult hepatitis B infection(OHBI) in a group of human immunodeficiency virus(HIV)-1+/ hepatitis B surface antigen negative(HBsAg)- patients from Mexico. METHODS: We investigated the presence of OHBI in 49 HIV-1+/HBsAg- patients. Hepatitis B virus(HBV) DNA was analyzed using nested PCR to amplify the Core(C) region and by real-time PCR to amplify a region of the S and X genes. The possible associations between the variables and OHBI were investigated using Pearson's χ2 and/or Fisher's exact test.RESULTS: We found that the frequency of OHBI was 49% among the group of 49 HIV-1+/HBsAg- patients studied. The presence of OHBI was significantly associated with the HIV-1 RNA viral load [odds ratio(OR) = 8.75; P = 0.001; 95%CI: 2.26-33.79] and with HIVantiretroviral treatment with drugs that interfere with HBV replication(lamivudine, tenofovir or emtricitabine)(OR = 0.25; P = 0.05; 95%CI: 0.08-1.05). | 11 | Gallbladder perforation with fistulous communication显示文摘BACKGROUND The management of gallbladder perforation(GBP)with fistulous communication(Neimeier type I)is controversial.AIM To recommend management options for GBP with fistulous communication.METHODS A systematic review of studies describing the management of Neimeier type I GBP was performed according to the PRISMA guidelines.The search strategy was conducted in Scopus,Web of Science,MEDLINE,and EMBASE(May 2022).Data extraction was obtained for patient characteristics,type of intervention,days of hospitalization(DoH),complications,and site of fistulous communication.RESULTS A total of 54 patients(61%female)from case reports,series,and cohorts were included.The most frequent fistulous communication occurred in the abdominal wall.Patients from case reports/series had a similar proportion of complications between open cholecystectomy(OC)and laparoscopic cholecystectomy(LC)(28.6 vs 12.5;P=0.569).Mortality was higher in OC(14.3 vs 0.0;P=0.467)but this proportion was given by only one patient.DoH were higher in OC(mean 26.3 d vs 6.6 d).There was no clear association between higher rates of complications of a given intervention in cohorts,and no mortality was observed.CONCLUSION Surgeons must evaluate the advantages and disadvantages of the therapeutic options.OC and LC are adequate options for the surgical management of GBP,with no significant differences. | Alejandro Quiroga-Garza Neri Alejandro Alvarez-Villalobos Milton Alberto Muñoz-Leija Mariano Garcia-Campa Hermilo Jeptef Angeles-Mar Guillermo Jacobo-Baca Rodrigo Enrique Elizondo-Omana Santos Guzman-Lopez | 2023 | World Journal of Gastrointestinal Surgery2023,15,6: | 1 |

| 13 | Molecular diagnosis of Chagas disease:a systematic review and meta-analysis显示文摘Background The complexity of the Chagas disease and its phases is impossible to have a unique test for both phases and a lot of different epidemiological scenarios.Currently,serology is the reference standard technique;occasionally,results are inconclusive,and a different diagnostic technique is needed.Some guidelines recommend molecular testing.A systematic review and meta-analysis of available molecular tools/techniques for the diagnosis of Chagas disease was performed to measure their heterogeneity and efficacy in detecting Trypanosoma cruzi infection in blood samples.Methods A systematic review was conducted up to July 27,2022,including studies published in international databases.Inclusion and exclusion criteria were defined to select eligible studies.Data were extracted and presented according to PRISMA 2020 guidelines.Study quality was assessed using Quality Assessment of Diagnostic Accuracy Studies-2(QUADAS-2).A random-effects model was used to calculate pooled sensitivity,specificity,and diagnostic odds ratio(DOR).Forest plots and a summary of the receiving operating characteristics(SROC)curves displayed the outcomes.Heterogeneity was determined by I^(2)and Tau^(2)statistics and P values.Funnel plots and Deek's test were used to assess publication bias.A quantitative meta-analysis of the different outcomes in the two different clinical phases was performed.Results We identified 858 records and selected 32 papers.Studies pertained to endemic countries and nonendemic areas with adult and paediatric populations.The sample sizes ranged from 17 to 708 patients.There were no concerns regarding the risk of bias and applicability of all included studies.A positive and nonsignificant correlation coefficient(S=0.020;P=0.992)was obtained in the set of studies that evaluated diagnostic tests in the acute phase population(ACD).A positive and significant correlation coefficient(S=0.597;P<0.000)was obtained in the case of studies performed in the chronic phase population(CCD).This resulted in high heterogeneity between studies,with the master mix origin and guanidine addition representing significant sources.Interpretation/Conclusions and relevance The results described in this meta-analysis(qualitative and quantitative analyses)do not allow the selection of the optimal protocol of molecular method for the study of Trypanosoma cruzi infection in any of its phases,among other reasons due to the complexity of this infection.Continuous analysis and optimization of the different molecular techniques is crucial to implement this efficient diagnosis in endemic areas. | Guillermo Pascual-Vázquez Montserrat Alonso-Sardón Beatriz Rodríguez-Alonso Javier Pardo-Lledías Angela Romero Alegría Pedro Fernández-Soto Juan Luis Muñoz Bellido Antonio Muro Moncef Belhassen-García | 2023 | Infectious Diseases of Poverty2023,12,5: | 0 |
