| 1 | A life support-based comprehensive treatment regimen dramatically lowers the in-hospital mortality of patients with fulminant myocarditis: a multiple center study显示文摘Fulminant myocarditis(FM) has unacceptable high mortality. This study aimed to evaluate the therapeutic efficacy of a life support-based comprehensive treatment regimen(LSBCTR), a completely novel treatment regimen, for FM. A total of 169 FM patients recruited from January 2008 to December 2018 were divided into two groups: patients receiving LSBCTR(81 cases),which includes(i) mechanical life support(positive pressure respiration, intra-aortic balloon pump with or without extracorporeal membrane oxygenation),(ii) immunomodulation therapy using sufficient doses of glucocorticoids and immunoglobulins, and(iii) application of neuraminidase inhibitors, and those receiving conventional treatment(88 cases). The endpoints were in-hospital death and heart-transplantation. Of all the population, 44 patients(26.0%) died in hospitals. Inhospital mortality was 3.7%(3/81) for LSBCTR group and 46.6%(41/88) for traditional treatment(P<0.001). Early application of LSBCTR, mechanical life support, neuraminidase inhibitors, and immunomodulation therapy significantly contributed to reduction in in-hospital mortality. This study describes a novel treatment regimen for FM patients that dramatically reduces inhospital mortality. | 2 | Chinese expert consensus statement on the diagnosis and treatment of fulminant myocarditis显示文摘Fulminant myocarditis is primarily caused by infection with any number of a variety of viruses. It arises quickly, progresses rapidly, and may lead to severe heart failure or circulatory failure presenting as rapid-onset hypotension and cardiogenic shock,with mortality rates as high as 50%–70%. Most importantly, there are no treatment options, guidelines or an expert consensus statement. Here, we provide the first expert consensus, the Chinese Society of Cardiology Expert Consensus Statement on the Diagnosis and Treatment of Fulminant Myocarditis, based on data from our recent clinical trial(NCT03268642). In this statement, we describe the clinical features and diagnostic criteria of fulminant myocarditis, and importantly, for the first time,we describe a new treatment regimen termed life support-based comprehensive treatment regimen. The core content of this treatment regimen includes(i) mechanical life support(applications of mechanical respirators and circulatory support systems,including intraaortic balloon pump and extracorporeal membrane oxygenation),(ii) immunological modulation by using sufficient doses of glucocorticoid, immunoglobulin and(iii) antiviral reagents using neuraminidase inhibitor. | 4 | A targeted sequencing approach to find novel pathogenic genes associated with sporadic aortic dissection显示文摘Aortic dissection(AD) is a heterogeneous genetic disease of the aorta with high mortality and poor prognosis. However, only few genetic causes of AD have been explored till date. After conducting a broad literature review focused on identifying potential pathogenic pathways, we designed a panel containing 152 AD-associated genes to conduct massively parallel targeted nextgeneration sequencing of 702 sporadic aortic dissection patients and 163 matched healthy controls. After validation by Sanger sequencing, we identified 21 definitely pathogenic and 635 likely pathogenic variants in 61.25%(430/702) of patients. In these patients, 34.88%(150/430) harbored more than one variant that was either definitely or likely to be pathogenic. Among the candidate genes, we identified 546 likely pathogenic variants in 47.72%(335/702) of patients. Importantly, we identified 94 lossof-function(LOF) variants in 45 genes in AD patients, but only five LOF variants in the controls(P=1.34×10^(-4)). With a burden test, we highlighted RNF213 as an important new gene for AD pathogenesis. We also performed transcriptome sequencing of human aorta tissues to evaluate the expression levels of these newly identified genes. Our study has compiled a comprehensive genetic map of sporadic AD in the Han Chinese population. | 7 | Variants of genes encoding collagens and matrix metalloproteinase system increased the risk of aortic dissection显示文摘Aortic dissection(AD) is a devastating,heterogeneous condition of aorta.The homeostasis between collagens and matrix metalloproteases(MMPs)/tissue inhibitors of MMPs(TIMPs) system in the extracellular matrix plays an important role for structure and functions of aorta.However,our knowledge on association between variants of genes in this system and pathogenesis of AD is very limited.We analyzed all yet known coding human genes of collagens(45 genes),MMPs/TIMPs(27genes) in 702 sporadic AD patients and in 163 matched healthy controls,by using massively targeted next-generation and Sanger sequencing.To define the pathogenesis of potential disease-causing candidate genes,we performed transcriptome sequencing and pedigree co-segregation analysis in some genes and generated Col5a2 knockout rats.We identified 257 pathogenic or likely pathogenic variants which involved 88.89%(64/72) genes in collagens-MMPs/TIMPs system and accounted for 31.05%(218/702) sporadic AD patients.In them,84.86%patients(185/218) carried one variant,12.84%two variants and 2.30%more than two variants.Importantly,we identified 52 novel probably pathogenic loss-of-function(LOF) variants(20 nonsense,16 frameshift,14 splice sites,one stop-loss,one initiation codon) in 11.06%(50/452) AD patients,which were absent in 163controls(P=2.5×10^(-5)).Transcriptome sequencing revealed that identified variants induced dyshomeostasis in expression of collagens-TIMPs/MMPs systems.The Col5α2^(-/-) rats manifested growth retardation and aortic dysplasia.Our study provides a first comprehensive map of genetic alterations in collagens-MMPs/TIMPs system in sporadic AD patients and suggests that variants of these genes contribute largely to AD pathogenesis. | Zongzhe Li Chengming Zhou Lun Tan Peng Chen Yanyan Cao Chenze Li Xianqing Li Jiangtao Yan Hesong Zeng Dao-Wu Wang Dao-Wen Wang | 2017 | Science China(Life Sciences)2017,60,1: | 4 |

| 9 | DC-CIK联合化疗治疗非小细胞肺癌的临床疗效评价(英文)显示文摘Objective: The aim of the study was to evaluate the safety and therapeutic effects of autologous dendritic cells co-cultured with cytokine-induced killer cells (DC-CIK) combined with chemotherapy in advanced non-small cell lung cancer (NSCLC) patients. Methods: Fifty patients with advanced NSCLC (stages III to IV), who had received therapies in our Center (Department of Biotherapy, Affiliated to Cancer Hospital of Shanxi Medical University, Taiyuan, China) from August 2008 to January 2010, were treated by DC-CIK + chemotherapy as the combined treatment group; fifty advanced NSCLC patients treated with chemotherapy at the same time served as controls. The immunologic function, short-term therapeutic effects, the 1-year survival rate, the life quality, the chemotherapy side effects were compared between the two groups, the safety and therapeutic effects of DC-CIK cells therapy were observed too. Results: There was no obvious change of subsets of T cells in peripheral blood before and after therapy in DC-CIK + chemotherapy group, and IFN-γ was improved after therapy in this group (P < 0.05); in chemotherapy alone group, the ratios of CD3+CD4+, CD3+CD8+, CD3-CD56+ cells and the secretion of IL-2, TNF-α decreased significantly after therapy (P < 0.05); the ratios of CD3+CD8+, CD3+CD56+ were improved after cell culture (P < 0.05). The disease control rate (DCR) of DC-CIK + chemotherapy group was higher than that in the chemotherapy alone group (78.0% vs 56.0%, P < 0.05); the 1-year survival rates of DC-CIK + chemotherapy group and chemotherapy alone group were 50% and 44% respectively, had no significant difference. Compared with chemotherapy alone group, the occurrence of chemotherapy side effects (including bone marrow suppression, nausea and vomiting, peripheral nerve toxicity) was less in the DC-CIK + chemotherapy group (P < 0.05). The physical and appetite were better in DC-CIK + chemotherapy group after therapy. | 12 | An imputed whole-genome sequence-based GWAS approach pinpoints causal mutations for complex traits in a specific swine population显示文摘Sequencing-based genome-wide association studies(GWAS) have facilitated the identification of causal associations between genetic variants and traits in diverse species. However, it is cost-prohibitive for the majority of research groups to sequence a large number of samples. Here, we carried out genotype imputation to increase the density of single nucleotide polymorphisms in a large-scale Swine F;population using a reference panel including 117 individuals, followed by a series of GWAS analyses. The imputation accuracies reached 0.89 and 0.86 for allelic concordance and correlation, respectively. A quantitative trait nucleotide(QTN) affecting the chest vertebrate was detected directly, while the investigation of another QTN affecting the residual glucose failed due to the presence of similar haplotypes carrying wild-type and mutant allelesin the reference panel used in this study. A high imputation accuracy was confirmed by Sanger sequencing technology for the most significant loci. Two candidate genes,CPNE5 and MYH3, affecting meat-related traits were proposed. However, high power density and long cycle life are still challenges for the current LIHSs due to the imbalance of charge-storage capacity and electrode kinetics between capacitor-type cathode and battery-type anode. Therefore, great efforts have been made on designing novel cathode materials with high storage capacity and anode material with enhanced kinetic behavior for LIHSs. With unique two-dimensional form and numerous appealing properties, for the past several years, the rational designed graphene and its composites materials exhibit greatly improved electrochemical performance as cathode or anode for LIHSs. Here, we summarized and discussed the latest advances of the stateof-art graphene-based materials for LIHSs applications. The major roles of graphene are highlighted as(1) a superior active material,(2) ultrathin 2D flexible support to remedy the sluggish reaction of the metal compound anode, and(3) good 2D building blocks for constructing macroscopic 3D porous carbon/graphene hybrids. In addition, some high performance aqueous LIHSs using graphene as electrode were also summarized. | 19 | Claw-shaped flexible and low-impedance conductive polymer electrodes for EEG recordings: Anemone dry electrode显示文摘Electroencephalogram(EEG) signals provide important brain information in healthcare settings. Wet electrodes have several limitations, including their time-consuming preparation and no long-term acquisition, and dry electrodes have become a promising candidate. However, dry electrodes face several challenges related to signal quality, comfort, conductivity and cost.This study aims to develop and evaluate a novel dry electrode for EEG recordings. The electrode material properties are characterized by electrochemical, mechanical force, and skin measurements. The electrode performance is evaluated by comparing the impedance, signal-to-noise ratio(SNR), and EEG signal features of the proposed electrode with those of a wet electrode and two commercial dry electrodes. The dry electrode design is based on the biological structure of the sea anemone and demonstrates an improved scalp fit and reduced signal noise and motion artifacts. The electrode materials include advanced conducting polymers combined with thermoplastic elastomers(TPEs) and carbon nanotubes(CNTs), which exhibit good electrical conductivity, mechanical properties, and safety. A low-cost injection molding fabrication method is proposed. The dry electrodes show a scalp contact impedance of 7 kΩ at 20 Hz, which is lower than that of wet and commercial dry electrodes.Resting-state EEG and event-related potential signals collected by the anemone dry electrode achieved more than 90% similarity with signals acquired by wet electrodes. Thus, a low-cost, comfortable anemone dry electrode that exhibits excellent EEG recording performance is presented.
