1Whole genome sequencing of 10K patients with acute ischaemic stroke or transient ischaemic attack: design, methods and baseline patient characteristics显示文摘Background and purpose Stroke is the second leading cause of death worldwide and the leading cause of mortality and long-term disability in China,but its underlying risk genes and pathways are far from being comprehensively understood.We here describe the design and methods of whole genome sequencing(WGS)for 10914 patients with acute ischaemic stroke or transient ischaemic attack from the Third China National Stroke Registry(CNSR-III).Methods Baseline clinical characteristics of the included patients in this study were reported.DNA was extracted from white blood cells of participants.Libraries are constructed using qualified DNA,and WGS is conducted on BGISEQ-500 platform.The average depth is intended to be greater than 30×for each subject.Afterwards,Sentieon software is applied to process the sequencing data under the Genome Analysis Toolkit best practice guidance to call genotypes of single nucleotide variants(SNVs)and insertion-deletions.For each included subject,21 fingerprint SNVs are genotyped by MassARRAY assays to verify that DNA sample and sequencing data originate from the same individual.The copy number variations and structural variations are also called for each patient.All of the genetic variants are annotated and predicted by bioinformatics software or by reviewing public databases.Results The average age of the included 10914 patients was 62.2±11.3 years,and 31.4%patients were women.Most of the baseline clinical characteristics of the 10914 and the excluded patients were balanced.Conclusions The WGS data together with abundant clinical and imaging data of CNSR-III could provide opportunity to elucidate the molecular mechanisms and discover novel therapeutic targets for stroke.Si Cheng Zhe Xu Yang Liu Jinxi Lin Yong Jiang Yilong Wang Xia Meng Anxin Wang Xinying Huang Zhimin Wang Guohua Chen Songdi Wu Zhengchang Jia Yongming Chen Xuerong Qiu Jun Wu Binbin Song Weizhong Ji Zhongping An Wenjun Xue Lili Zhao Yu Geng Hongyan Li Hao Li Yongjun Wang 2021Stroke & Vascular Neurology2021,6,2:8

3Incidence and prevalence of moyamoya disease in urban China: a nationwide retrospective cohort study显示文摘Background and objective Moyamoya disease(MMD)is an increasingly recognised cause of stroke,mainly described in East Asia.China is the largest nation in Asia,but few studies reported the epidemiology of MMD,especially at a national level.We aimed to estimate the incidence and prevalence of MMD in China.Methods We performed a population-based study using data from the national databases of Urban Basic Medical Insurance between 2013 and 2016,covering approximately 0.50 billion individuals.MMD cases were identified by diagnostic code(International Classification of Diseases,10th Revision I67.5)or related diagnostic text.Results A total of 1987 MMD patients(mean age 44.45±14.30 years,female-to male ratio 1.12)were identified,representing a national crude incidence of 0.59(95%CI:0.49 to 0.68)and a prevalence of 1.01(95%CI:0.81 to 1.21)per 100000 person-years in 2016.Rates were higher in females than in males for the incidence(0.66 vs 0.52)and prevalence(1.05 vs 0.90).And the age-specific rates showed a bimodal distribution,with the highest peak in middle-aged group and the second peak in child group.Conclusions Our results confirm that MMD is relatively common in East Asians,but the rates in China were lower than those in other East Asian countries such as Japan and Korea.The unique epidemiological features,including a relatively weak female predominance and a shift in the highest peak of incidence from children to adults,revealed new sight into MMD.Further research is expected to explore the potential pathogenesis of MMD.Yixin Sun Guoyu Zhou Jingnan Feng Lu Chen Guozhen Liu Jinxi Wang Qingliang Wang Junyou Yu Xiwang Yang Zheng Yang Pei Gao Shengfeng Wang Siyan Zhan 2021Stroke & Vascular Neurology2021,6,4:4

6Biomarker and genomic analyses reveal molecular signatures of non-cardioembolic ischemic stroke显示文摘Acute ischemic stroke(AIS)is a major cause of disability and mortality worldwide.Non-cardioembolic ischemic stroke(NCIS),which constitutes the majority of AIS cases,is highly heterogeneous,thus requiring precision medicine treatments.This study aimed to investigate the molecular mechanisms underlying NCIS heterogeneity.We integrated data from the Third China National Stroke Registry,including clinical phenotypes,biomarkers,and whole-genome sequencing data for 7695 patients with NCIS.We identified 30 molecular clusters based on 63 biomarkers and explored the comprehensive landscape of biological heterogeneity and subpopulations in NCIS.Dimensionality reduction revealed fine-scale subpopulation structures associated with specific biomarkers.The subpopulations with biomarkers for inflammation,abnormal liver and kidney function,homocysteine metabolism,lipid metabolism,and gut microbiota metabolism were associated with a high risk of unfavorable clinical outcomes,including stroke recurrence,disability,and mortality.Several genes encoding potential drug targets were identified as putative causal genes that drive the clusters,such as CDK10,ERCC3,and CHEK2.We comprehensively characterized the genetic architecture of these subpopulations,identified their molecular signatures,and revealed the potential of the polybiomarkers and polygenic prediction for assessing clinical outcomes.Our study demonstrates the power of large-scale molecular biomarkers and genomics to understand the underlying biological mechanisms of and advance precision medicine for NCIS.Lingling Ding Yu Liu Xia Meng Yong Jiang Jinxi Lin Si Cheng Zhe Xu Xingquan Zhao Hao Li Yongjun Wang Zixiao Li 2023Signal Transduction and Targeted Therapy2023,8,6:0
7Effects of Shenlian formula(参连方)on microbiota and inflammatory cytokines in adults with type 2 diabetes:a double-blind randomized clinical trial显示文摘OBJECTIVES:To observe the efficacy of Shenlian formula(SL formula,参连方),which consist of Huanglian(Rhizoma Coptidis)and Renshen(Radix Ginseng),in the treatment of type 2 diabetes mellitus(T2DM)and explore the effects on gut microbiota and serum inflammatory cytokines.METHODS:In a double-blind,randomized,placebocontrolled parallel-group clinical trial,31 adults with T2DM were randomly allocated to receive the SL formula or placebo for 12 weeks.Body mass index(BMI),blood lipid indices,glycemic biomarkers including glycated hemoglobin(HbA1C),fasting plasma glucose(FPG),postprandial blood glucose(PBG),fasting insulin levels(FIL),fasting C-peptide(C-P),homoeostasis model assessment for insulin resistance(HOMA-IR)and inflammatory cytokines were assessed at baseline and 12 weeks.The contents of gut microbiota were determined by pyrosequencing of the V3-V4 regions of 16S rRNA genes.RESULTS:Sixteen cases were allocated in the treatment group and 15 in the placebo group.Compared with the placebo,SL formula resulted in a higher significant reduction in PBG[(-1.318±0.772)vs(-0.008±1.404)mmol/L,P=0.003],BMI[(-0.611±0.524)vs(0.957±2.212)kg/m2,P=0.01],FIL[(-1.627±6.268)vs(3.976±6.85)μIU/mL,P=0.02],HOMA-IR[(-0.530±1.461)vs(1.511±2.288),P=0.006],and C-reactive protein(CRP)[(-1.307±0.684)vs(0.828±0.557)mg/L,P=0.04].In terms of gut microbiota,compared with the placebo,the SL formula resulted in a significant decrease in species richness and evenness.CONCLUSIONS:The SL formula showed the efficacy to improve postprandial blood glucose,insulin resistance,BMI and CRP levels.In addition,it could reduce the total number,richness and evenness of species,meanwhile increase the abundance of probiotics to modulate the structure of gut microbiota in patients with T2DM.However,further studies are required for exploring the deeper mechanism of TCM on gut microbiota.JIANG Li FU Qiang WANG Shidong ZHAO Jinxi CHEN Yu LI Jiayue XIAO Yonghua HUANG Weijun SUN Ruixi XIAO Yao SHEN Aijia WANG Junheng LIU Jiangteng FU Xiaozhe LI Yuanyuan ZHAO Yu XUE Taiqi 2023Journal of Traditional Chinese Medicine2023,43,4:0
