| 1 | Association of cancer prevention awareness with esophageal cancer screening participation rates: Results from a population-based cancer screening program in rural China显示文摘Objective: : China have implemented population-based esophageal cancer(EC) screening programs, however,the participant rates were relatively low. This study was conducted to examine the association between cancer prevention knowledge and EC screening participation rates(PRs).Methods: : Data in the analyses were obtained from a population-based cancer screening program in four provinces in China since 2007. In the course of 2007-2016, participants who were evaluated as high risk for EC and subsequently recommended for endoscopy examination were included in the final analysis. One-way analysis of variance test, Chi-square test and logistic regression analysis were applied.Results: : A total of 28,543 individuals assessed as high-risk population for EC were included in this study, with13,036 males(45.67%) and 15,507 females(54.33%). The prevalence rates of current smoking and alcohol drinking were higher in males(58.25% and 44.22%, respectively) than in females(5.35% and 4.05%, respectively).Participants of females, and those who had older age, lower income, as well as higher education level and cancer prevention knowledge level were more likely to undergo endoscopy. Multivariable analysis showed that higher cancer prevention knowledge was associated with higher PR for endoscopic screening [adjusted odd ratio(aORQ4/Q1) =1.511, 95% confidence interval(95% CI): 1.398-1.632] among our study subjects. This association between cancer prevention knowledge and compliance with endoscopic screening also tends to be strong within subgroups of males, females, aged below 60 years, aged 60 years and above, and lower level of education(illiterate individuals and those with junior high education or less).Conclusions: : Our results displayed a positive relationship between cancer prevention knowledge and PRs for endoscopic screening, implying cancer prevention awareness being an independent influence factor for compliance with EC screening. | 2 | Medical expenditures for colorectal cancer diagnosis and treatment: A 10-year high-level-hospital-based multicenter retrospective survey in China, 2002-2011显示文摘Objective: Colorectal cancer(CRC) causes a substantial burden of disease in China and the evidence of economic burden triggered is fundamental for priority setting. The aim of this survey was to quantify medical expenditures and the time trends for CRC diagnosis and treatment in China.Methods: From 2012 to 2014, a hospital-based multicenter retrospective survey was conducted in 13 provinces across China. For each eligible CRC patient diagnosed from 2002 to 2011, clinical information and expenditure data were extracted using a uniform questionnaire. All expenditure data were reported in Chinese Yuan(CNY)using 2011 values.Results: Of the 14,536 CRC patients included, the average age at diagnosis was 58.2 years and 15.8% were stageI cases. The average medical expenditure per patient was estimated at 37,902 CNY [95 % confidence interval(95%CI): 37,282-38,522], and the annual average increase rate was 9.2% from 2002 to 2011(P for trend <0.001), with a cumulative increase of 2.4 times(from 23,275 CNY to 56,010 CNY). The expenditure per patient in stages Ⅰ, Ⅱ, Ⅲ and Ⅳ were 31,698 CNY, 37,067 CNY, 38,918 CNY and 42,614 CNY, respectively(P<0.001). Expenditure significantly differed within various subgroups. Expenses for drugs contributed the largest proportion(52.6%).Conclusions: These conservative estimates illustrated that medical expenditures for CRC diagnosis and treatment in tertiary hospitals in China were substantial and increased rapidly over the 10 years, with drugs continually being the main expense by 2011. Relatively, medical expenditures are lower for CRC in the earlier stages. | 3 | No expenditure difference among patients with liver cancer at stageⅠ-ⅣV:Findings from a multicenter cross-sectional study in China显示文摘Objective:The number of liver cancer patients in China accounts for more than half of the world.However,China currently lacks national,multicenter economic burden data,and meanwhile,measuring the differences among different subgroups will be informative to formulate corresponding policies in liver cancer control.Thus,the aim of the study was to measure the economic burden of liver cancer by various subgroups.Methods:A hospital-based,multicenter and cross-sectional survey was conducted during 2012・2014,covering 39 hospitals and 21 project sites in 13 provinces across China.The questionnaire covers clinical information,sociology,expenditure,and related variables.All expenditure data were reported in Chinese Yuan(CNY)using 2014 values.Results:A total of 2,223 liver cancer patients were enrolled,of whom 59.61%were late-stage cases(III-IV),and 53.8%were hepatocellular carcinoma.The average total expenditure per liver cancer patient was estimated as 53,220 CNY,including 48,612 CNY of medical expenditures(91.3%)and 4,608 CNY of non-medical expenditures(8.7%).The average total expenditures in stage I,H,m and stage IV were 52,817 CNY,50,877 CNY,50,678 CNY and 54,089 CNY(P>0.05),respectively.Non-medical expenditures including additional meals,additional nutrition care,transportation,accommodation and hired informal nursing were 1,453 CNY,839 CNY,946 CNY,679 CNY and 200 CNY,respectively.The one-year out-of-pocket expenditure of a newly diagnosed patient was 24,953 CNY,and 77.2%of the patients suffered an unmanageable financial burden.Multivariate analysis showed that overall expenditure differed in almost all subgroups(P<0.05),except for sex,clinical stage,and pathologic type.Conclusions:There was no difference in treatment expenditure for liver cancer patients at different clinical stages,which suggests that maintaining efforts on treatment efficacy improvement is important but not enough.To fiirtherly reduce the overall economic burden from liver cancer,more effort should be given to primary and secondary prevention strategies. | Haike Lei Lin Lei Jufang Shi Yongzhong Wu Ling Liang Huiyao Huang Mei He Fangzhou Bai Maomao Cao Hui Qiu Yuting Wang Chengcheng Liu Jia Du Hong Wang Yan Zhang Mengdi Cao Ji Peng Ni Li Chunfeng Qu Min Dai Wanqing Chen Jie He | 2020 | Chinese Journal of Cancer Research2020,32,4: | 7 |
| 4 | Classifying risk level of gastric cancer: Evaluation of questionnaire-based prediction model显示文摘Objective: This study aimed at evaluating the efficacy of the questionnaire-based prediction model in an independent prospective cohort.Methods: A cluster-randomized controlled trial was conducted in Changsha, Harbin, Luoshan, and Sheyang in eastern China in 2015-2017. A total of 182 villages/communities were regarded as clusters, and allocated to screening arm or control arm randomly. Face-to-face interview through a questionnaire interview, including of relevant risk factors of gastric cancer, was administered for each subject. Participants were further classified into high-risk or low-risk groups based on their exposure to risk factors. All participants were followed up until December 31, 2019. Cumulative incidence rates from gastric cancer between high-risk and low-risk groups were calculated and compared using the log-rank test. Cox proportional hazard regression models were applied to estimate hazard ratio(HR) and 95% confidence interval(95% CI).Results: Totally, 89,914 residents were recruited with a mean follow-up of 3.47 years. And 42,015(46.73%)individuals were classified into high-risk group and 47,899(53.27%) subjects were categorized into low-risk group.Gastric cancer was diagnosed in 131 participants, of which 91 were in high-risk group. Compared with the low-risk participants, high-risk individuals were more likely to develop gastric cancer(adjusted HR=2.15, 95% CI,1.23-3.76). | 5 | Improved esophageal squamous cell carcinoma screening effectiveness by risk-stratified endoscopic screening:evidence from high-risk areas in China显示文摘Background:Risk-stratified endoscopic screening(RSES),which offers endoscopy to those with a high risk of esophageal cancer,has the potential to increase effectiveness and reduce endoscopic demands compared with the universal screening strategy(i.e.,endoscopic screening for all targets without risk prediction).Evidence of RSES in high-risk areas of China is limited.This study aimed to estimate whether RSES based on a 22-score esophageal squamous cell carcinoma(ESCC)risk prediction model could optimize the universal endoscopic screening strategy for ESCC screening in high-risk areas of China.Methods:Eight epidemiological variables in the ESCC risk prediction model were collected retrospectively from 26,618 individuals aged 40-69 from three high-risk areas of China who underwent endoscopic screening betweenMay 2015 and July 2017.The model’s performance was estimated using the area under the curve(AUC).Participants were categorized into a high-risk group and a low-risk group with a cutoff score having sensitivities of both ESCC and severe dysplasia and above(SDA)at more than 90.0%.Results:The ESCC risk prediction model had an AUC of 0.80(95% confidence interval:0.75-0.84)in this external population.We found that a score of 8(ranging from 0 to 22)had a sensitivity of 94.2% for ESCC and 92.5% for SDA.The RSES strategy using this threshold score would allow 50.6% of endoscopies to be avoided and save approximately US$0.59 million compared to universal endoscopic screening among 26,618 participants.In addition,a higher prevalence of SDA(1.7%vs.0.9%),a lower number need to screen(60 vs.111),and a lower average cost per detected SDA(US$3.22 thousand vs.US$5.45 thousand)could have been obtained by the RSES strategy.Conclusions:The RSES strategy based on individual risk has the potential to optimize the universal endoscopic screening strategy in ESCC high-risk areas of China. | He Li Chao Ding Hongmei Zeng Rongshou Zheng Maomao Cao Jiansong Ren Jufang Shi Dianqin Sun Siyi He Zhixun Yang Yiwen Yu Zhe Zhang Xibin Sun Guizhou Guo Guohui Song Wenqiang Wei Wanqing Chen Jie He | 2021 | Cancer Communications2021,41,8: | 3 |
| 6 | Sex disparity of lung cancer risk in non-smokers:a multicenter population-based prospective study based on China National Lung Cancer Screening Program显示文摘Background: Non-smokers account for a large proportion of lung cancer patients, especially in Asia, but the attention paid to them is limited compared with smokers. In non-smokers, males display a risk for lung cancer incidence distinct from the females-even after excluding the influence of smoking;but the knowledge regarding the factors causing the difference is sparse. Based on a large multicenter prospective cancer screening cohort in China, we aimed to elucidate the interpretable sex differences caused by known factors and provide clues for primary and secondary prevention.Methods: Risk factors including demographic characteristics, lifestyle factors, family history of cancer, and baseline comorbidity were obtained from 796,283 Chinese non-smoking participants by the baseline risk assessment completed in 2013 to 2018. Cox regression analysis was performed to assess the sex difference in the risk of lung cancer, and the hazard ratios (HRs) that were adjusted for different known factors were calculated and compared to determine the proportion of excess risk and to explain the existing risk factors.Results: With a median follow-up of 4.80 years, 3351 subjects who were diagnosed with lung cancer were selected in the analysis. The lung cancer risk of males was significantly higher than that of females;the HRs in all male non-smokers were 1.29 (95% confidence interval [CI]: 1.20-1.38) after adjusting for the age and 1.38 (95% CI: 1.28-1.50) after adjusting for all factors, which suggested that known factors could not explain the sex difference in the risk of lung cancer in non-smokers. Known factors were 7% (|1.29-1.38|/1.29) more harmful in women than in men. For adenocarcinoma, women showed excess risk higher than men, contrary to squamous cell carcinoma;after adjusting for all factors, 47% ([1.30-1.16]/[1.30-1]) and 4% ([7.02-6.75]/[7.02-1])) of the excess risk was explainable in adenocarcinoma and squamous cell carcinoma. The main causes of gender differences in lung cancer risk were lifestyle factors, baseline comorbidity, and family history.Conclusions: Significant gender differences in the risk of lung cancer were discovered in China non-smokers. | 15 | Effects of diethyldithiocarbamate on myelin basic protein expression in the rat lateral olfactory tract显示文摘BACKGROUND:Dithiocarbamates can cause demyelination of axons in the peripheral nervous system.Its derivate,diethyldithiocarbamate,is cytotoxic,and causes olfactory mucosal damage and atrophy of the olfactory bulb.However,it is still unclear whether the myelin sheath of the lateral olfactory tract is affected by diethyldithiocarbamate. OBJECTIVE:To investigate the effects of diethyldithiocarbamate on the myelin sheath of the rat lateral olfactory tract.This was done by examining changes in myelin basic protein expression after diethyldithiocarbamate treatment. DESIGN,TIME AND SETTING:A randomized,controlled,animal study was performed at the Laboratory of the Department of Human Anatomy and Neurobiology,Xiangya School of Medicine, Central South University,China from July to November 2007. MATERIALS:A total of 72 Sprague Dawley rats were randomly assigned into a diethyldithiocarbamate group(n = 32),a solvent control group(n = 32),and a blank control group (n = 8).The diethyldithiocarbamate and solvent control groups were separately divided into 3-d, 7-d,14-d and 28-d survival subgroups,with eight rats in each.Diethyldithiocarbamate(Sigma, USA) and goat anti-myelin basic protein polyclonal antibody(Santa Cruz,USA) were used in this study. METHODS:Rats in the diethyldithiocarbamate and solvent control groups were subcutaneously injected with diethyldithiocarbamate(600 mg/kg) and 0.01 mol/L phosphate buffered saline (600 mg/kg) at the posterior neck,respectively.Rats in the blank control group received no treatment. MAIN OUTCOME MEASURES:Immunohistochemical staining and Western blot assay were used to measure myelin basic protein expression in the rat lateral olfactory tract. RESULTS:Following immunohistochemical staining,myelin basic protein was uniformly distributed in the rat lateral olfactory tract in the blank control and solvent control groups.Western blot assay showed 21.5,18,17 and 14 ku positive bands.No significant difference was found in myelin basic protein distribution and blot pattern,in the rat lateral olfactory tract,in the diethyldithiocarbamate group,following immunohistochemical staining and Western blot assay. Myelin basic protein expression gradually decreased at day 3,reached the lowest level at day 7, and gradually increased again at days 14 and 28. | 19 | Reduced Firing of Nucleus Accumbens Parvalbumin Interneurons Impairs Risk Avoidance in DISC1 Transgenic Mice显示文摘A strong animal survival instinct is to approach objects and situations that are of benefit and to avoid risk.In humans,a large proportion of mental disorders are accompanied by impairments in risk avoidance.One of the most important genes involved in mental disorders is disrupted-in-schizophrenia-1(DISC1),and animal models in which this gene has some level of dysfunction show emotion-related impairments.However,it is not known whether DISC1 mouse models have an impairment in avoiding potential risks.In the present study,we used DISC1-N terminal truncation(DISC1-N^(TM))mice to investigate risk avoidance and found that these mice were impaired in risk avoidance on the elevated plus maze(EPM)and showed reduced social preference in a three-chamber social interaction test.Following EPM tests,c-Fos expression levels indicated that the nucleus accumbens(NAc)was associated with risk-avoidance behavior in DISC1-N^(TM)mice.In addition,in vivo electrophysiological recordings following tamoxifen administration showed that the firing rates of fast-spiking neurons(FS)in the NAc were significantly lower in DISC1-N^(TM)mice than in wild-type(WT)mice.In addition,in vitro patch clamp recording revealed that the frequency of action potentials stimulated by current injection was lower in parvalbumin(PV)neurons in the NAc of DISC1-N^(TM)mice than in WT controls.The impairment of risk avoidance in DISC1-N^(TM)mice was rescued using optogenetic tools that activated NAcPV neurons.Finally,inhibition of the activity of NAcPV neurons in PV-Cre mice mimicked the risk-avoidance impairment found in DISC1-N^(TM)mice during tests on the elevated zero maze.Taken together,our findings confirm an impairment in risk avoidance in DISC1-N^(TM)mice and suggest that reduced excitability of NAc^(PV) neurons is responsible. | Xinyi Zhou Bifeng Wu Wenhao Liu Qian Xiao Wei He Ying Zhou Pengfei Wei Xu Zhang Yue Liu Jie Wang Jufang He Zhigang Zhang Weidong Li Liping Wang Jie Tu | 2021 | Neuroscience Bulletin2021,37,9: | 0 |
