| 1 | Efficacy of fascia iliaca compartment nerve block as part of multimodal analgesia after surgery for femoral bone fracture显示文摘BACKGROUND: Fascia iliaca compartment nerve block(FICNB) has been an established technique for postoperative analgesia after surgery for femoral bone fracture. FICNB is technically easy, effective for postoperative pain control after operation for femoral bone fracture and decreases the complications induced by systemic analgesic drugs. The severity of postoperative pain is affected by genetics, cultural and social factors across the world. In this study we assessed the efficacy of fascia iliaca compartment nerve block when it is used as part of multimodal analgesia after surgery for femoral bone fracture.METHODS: An institution-based case control study was conducted from September, 2013 to May, 2014. All patients who had been operated on under spinal anesthesia for femoral bone fracture were included. The patients divided into a FICNB group(n=20) and a control group(n=20). The FICNB group was given 30 mL of 0.25% bupivacaine at the end of the operation. Postoperative pain was assessed within the f irst 24 hours, i.e. at 15 minutes, 2 hours, 6 hours, 12 hours and 24 hours using 100 mm visual analogue scale(VAS), total analgesic consumption, and the time for the f irst analgesic request.RESULTS: VAS pain scores were reduced within the f irst 24 hours after operation in the FICNB group compared wtih the control group. VAS scores at 2 hours were taken as median values(IQR) 0.00(0.00) vs.18.00(30.00), P=0.001; at 6 hours 0.00(0.00) vs. 34.00(20.75), P=0.000; at 24 hours 12.50(10.00) vs. 31.50(20.75), P=0.004; and at 12 hours(17.80±12.45) vs.(29.95±12.40), P=0.004, respectively. The total analgesic consumption of diclofenac at 12 and 24 hours was reduced in the FICNB group, and the time for the fi rst analgesic request was signifi cantly prolonged(417.50 vs. 139.25 minutes, P=0.000).CONCLUSIONS: A single injection for FICNB could lead to postoperative pain relief, reduction of total analgesic consumption and prolonged time for the fi rst analgesic request in the FICNB group after surgery for femoral bone fracture. | 3 | 紫茎泽兰对大鼠肠道结构和肠道黏膜免疫屏障的影响显示文摘为探究紫茎泽兰对动物肠道损伤及致毒机制,本试验选取16只7周龄雄性SD大鼠经7 d适应期后随机分为对照组(饲喂不含紫茎泽兰饲料,10 g·100 g^(-1)BW)与试验组(饲喂含30%紫茎泽兰饲料,10 g·100 g^(-1)BW),试验周期为14 d,试验结束后取各肠段(取十二指肠、空肠、回肠、盲肠、结肠和直肠)组织样品,观察结构损伤,计数免疫细胞,测定sIgA(secretory immunoglobulin A,sIgA)和炎性细胞因子分泌量。结果表明:与对照组相比,紫茎泽兰可使十二指肠绒毛出血及顶端轻度坏死脱落,肠绒毛高度、隐窝深度及其比值极显著增加(P<0.01);空肠充血并伴有绒毛顶端糜烂性坏死脱落,绒毛高度及绒毛高度与隐窝深度比值极显著增加(P<0.01);回肠大量肠绒毛顶端凝固性坏死,并伴有出血与炎性浸润,绒毛高度、隐窝深度及其比值极显著增加(P<0.01);盲肠水肿及充血;结肠淋巴细胞增多、直肠大量淋巴细胞浸润及淋巴细胞增生,其中以回肠和直肠机械损伤最为严重,回肠评分增加722.36%,直肠评分增加976.00%;试验组各肠段上皮内淋巴细胞(intestinal intraepithelial lymphocytes,IELs)、固有层淋巴细胞(lamina propria lymphocytes,LPLs)和杯状细胞(goblet cells,GCs)数量极显著增加(P<0.01),同时还可极显著增加sIgA的分泌量(P<0.01),极显著增加促炎因子IL-1β、IL-2、TNF-α和IFN-γ的表达量(P<0.01),极显著降低抑炎因子IL-4、IL-10的表达(P<0.01),激活肠道炎症反应,从而破坏肠道黏膜免疫屏障,造成肠道损伤。 | 崔玉晶 高佩 文娟 OKYERE Kumi Samuel 胡延春 | 2022 | 草业学报2022，31，6： | 3 |
| 4 | Effects of oral tacrolimus as a rapid induction therapy in ulcerative colitis显示文摘AIM:To determine the efficacy and safety of rapid induction therapy with oral tacrolimus without a meal in steroid-refractory ulcerative colitis(UC)patients.METHODS:This was a prospective,multicenter,observational study.Between May 2010 and August 2012,49 steroid-refractory UC patients(55 flare-ups)were consecutively enrolled.All patients were treated with oral tacrolimus without a meal at an initial dose of 0.1mg/kg per day.The dose was adjusted to maintain trough whole-blood levels of 10-15 ng/m L for the first 2 wk.Induction of remission at 2 and 4 wk after tacrolimus treatment initiation was evaluated using Lichtiger’s clinical activity index(CAI).RESULTS:The mean CAI was 12.6±3.6 at onset.Within the first 7 d,93.5%of patients maintained high trough levels(10-15 ng/m L).The CAI significantly decreased beginning 2 d after treatment initiation.At 2wk,73.1%of patients experienced clinical responses.After tacrolimus initiation,31.4%and 75.6%of patients achieved clinical remission at 2 and 4 wk,respectively.Treatment was well tolerated.CONCLUSION:Rapid induction therapy with oral tacrolimus shortened the time to achievement of appropriate trough levels and demonstrated a high remission rate 28 d after treatment initiation.Rapid induction therapy with oral tacrolimus appears to be a useful therapy for the treatment of refractory UC. | Ken Kawakami Takuya Inoue Mitsuyuki Murano Ken Narabayashi Sadaharu Nouda Kumi Ishida Yosuke Abe Koji Nogami Nobuyuki Hida Hirokazu Yamagami Kenji Watanabe Eiji Umegaki Shiro Nakamura Tetsuo Arakawa Kazuhide Higuchi | 2015 | World Journal of Gastroenterology2015，21，6： | 3 |

| 8 | How does a real-world child psychiatric clinic diagnose and treat attention deficit hyperactivity disorder?显示文摘AIM: To investigate child and adolescent psychiatrists'(CAPs) attention deficit hyperactivity disorder(ADHD) and oppositional defiant disorder(ODD) diagnoses and treatments in real-world clinical practice. METHODS: The medical records of 69 ADHD children(mean age = 9.5 years), newly referred to the ADHD clinic, were reviewed for their scores of parent- and teacher-reported Vanderbilt ADHD Diagnostic Rating Scales(VADRSs), CAPs' diagnoses of ADHD and ODD, and CAPs' treatment recommendations. Among 63 ADHD subjects who completed both parent and teacher VADRSs, we examined the agreement of the parent and teacher VADRSs. We also examined the concurrent validity of CAPs' ODD diagnoses against the results from the VADRSs. In addition, we compared CAPs' treatment recommendations against established ADHD and ODD guidelines.RESULTS: Among 63 ADHD subjects, the majority of the subjects(92%) met full ADHD diagnostic criteria at least in one setting(parent or teacher) on the VADRSs. Nearly half of the patients met full ADHD diagnostic criteria in two settings(parent and teacher). Relatively low agreement between the parent and teacher VADRSs were found(95%CI:-0.33 to 0.14). For 29 children who scored positive for ODD on the rating scales, CAPs confirmed the ODD diagnosis in only 12 of these casepositives, which is considered as a fair agreement between CAPs and VADRSs(95%CI: 0.10-0.53). For 27 children with no ODD diagnosis made by either CAP or VADRS, more than half of them were recommended for medication only. In contrast, where CAPs made the diagnosis of ODD, or where the parent or teacher VADRS was positive for ODD, almost all of the patients received recommendations for medication and behavior therapy.CONCLUSION: CAPs' ADHD diagnoses have strong concurrent validity against valid rating scales, but ADHD's most common comorbid condition- ODD- may be underrecognized. | Kumi Yuki Jyoti Bhagia David Mrazek Peter S Jensen | 2016 | World Journal of Psychiatry2016，6，1： | 2 |
| 9 | Children's participatory design for sustainable development and community planning显示文摘The urban developments have been carried out by only professionals for a long time because of efficiency and safety.However,most residents are ordinary people.Therefore,it is necessary to change the awareness of sustainable living environments,not only on the construction side but also the residents.In recent years,the number of cases of residents' participation in planning,maintaining,and repairing increased.However,sometimes youths and children,who might spend a lifetime longer than adults in a city after the developments have been done,are not included in this 'Residents'.Concerning youth and children's participation,CFC (child friendly city) is one of the good practices for sustainable development.It was launched by UNICEF (United Nations International Children's Emergency Fund) and UN-Habitat in the Habitat II in 1996.City governments,especially in European countries,started to view CFC as their key concept for preservation and/or sustainable development.CFC means not only being 'Children' friendly but also 'All people' friendly.Various effects have been reported since the Historic Cities/districts included CFC in their city planning as a common concept,especially in education,community re-development,and operation and maintenance by the community.It is expected that better city planning in preservation and sustainable development can be achieved by adding CFC concept.In this research,an ideal way of city planning involving resident participation and the possibility in the future are analyzed based on case studies.Then a strategy of sustainable development and community planning involving youth and children's participation is proposed. | TASHIRO Kumi | 2012 | Journal of Chongqing University2012，11，1： | 2 |
