| 2 | Endoscopic features suggesting gastric cancer in biopsy-proven gastric adenoma with high-grade neoplasia显示文摘AIM:To elucidate the endoscopic features that predict the cancer following endoscopic submucosal dissection(ESD)in patients with high-grade neoplasia(HGN).METHODS:We retrospectively analyzed the medical records of patients who underwent ESD of gastric neoplasms from January 2007 to September 2010.ESD was performed in 555 cases involving 550 patients.A total of 112 lesions from 110 consecutive patients were initially diagnosed as HGN without cancer by forceps biopsy,and later underwent ESD.We classified lesions into two groups according to histologic discrepancies between the biopsy and ESD diagnosis.Gastric adenoma in the final diagnosis by ESD specimens were defined as adenoma group.Lesions with coexisting cancer after ESD were defined as cancer group.RESULTS:The mean age was 65.3 years,and 81 patients were male.There was no significant difference in the age or gender distribution between the adenoma(n=52)and cancer(n=60)groups.Thirty-six of these lesions(32.1%)showed histologic concordance between the forceps biopsy and ESD specimens,16(14.3%)showed a downgraded histology(low-grade neoplasia),and 60(53.6%)showed an upgraded histology(cancer).A red color change of the mucosal surface on endoscopy was found in 27/52(51.9%)of cases in the adenoma group and in 46/60(76.7%)of cases in the cancer group(P=0.006).Ulceration of the mucosal surface on endoscopy was found in 5(9.6%)of 52 lesions in the adenoma group and in 17(28.3%)of 60 lesions in the cancer group(P=0.013).In the multivariate analysis,a reddish surface color change and mucosal ulceration were significant predictive factors correlated with cancer after ESD of the HGN by forceps biopsy.CONCLUSION:HGN with a red color change or mucosal ulceration correlated with the presence of gastric cancer.These finding may help to guide the diagnosis and treatment. | Jung Ho Kim Yoon Jae Kim Jungsuk An Jong Joon Lee Jae Hee Cho Kyoung Oh Kim Jun-Won Chung Kwang An Kwon Dong Kyun Park Ju Hyun Kim | 2014 | World Journal of Gastroenterology2014,20,34: | 10 |
| 3 | Dexamethasone inhibits hypoxia-induced epithelial-mesenchymal transition in colon cancer显示文摘AIM:To elucidate the effects of dexamethasone on hypoxia-induced epithelial-to-mesenchymal transition(EMT) in colon cancer.METHODS:Human colon cancer HCT116 and HT29 cells were exposed to normoxic(21%) and hypoxic(1%) conditions. First,the effect of dexamethasone on cell viability was examined by MTT cell proliferation assay. In order to measure the expression levels of EMT markers(Snail,Slug,Twist,E-cadherin,and integrin αVβ6) and hypoxia-related genes [Hypoxia-inducible factor-1α(HIF-1α) and vascular endothelial growth factor(VEGF)] by dexamethasone,quantitative real-time polymerase chain reaction and western blot analysis were performed. Furthermore,the morphological changes of colon cancer cells and the expression pattern of E-cadherin by dexamethasone were detected through immunocytochemistry. Finally,the effects of dexamethasone on the invasiveness and migration of colon cancer cells were elucidated using matrigel invasion,migration,and wound healing migration assays.RESULTS:Under hypoxia,dexamethasone treatment inhibited HIF-1α protein level and its downstream gene,VEGF m RNA level in the colon cancer cell lines,HCT116 and HT29. In addition,the presence of dexamethasone down-regulated the m RNA levels of hypoxia-induced Snail,Slug,and Twist,all transcriptional factors of EMT,as well as hypoxia-induced integrin αVβ6 protein level,a well-known EMT marker for colon cancer cells. Furthermore,reduced E-cadherin in hypoxic condition was found to be recoverable by treating with dexamethasone in both colon cancer cell lines. | 4 | A gastrointestinal stromal tumor of the duodenum masquerading as a pancreatic head tumor显示文摘Gastrointestinal stromal tumor (GIST) represents the most common kind of mesenchymal tumor that arises from the alimentary tract. GIST is currently defined as a gastrointestinal tract mesenchymal tumor showing CD117 (c-kit protein) positivity at immunohistochemistry. Throughout the whole length of the gastrointestinal tract, GIST arises most commonly from the stomach followed by the small intestine, the colorectum, and the esophagus. Only 3%-5% of GISTs occur in the duodenum, and especially, if GIST arises from the C loop of the duodenum, it can be difficult to differentiate from the pancreas head mass because of its anatomical proximity. | 5 | Endoscopic stenting for recurrence-related colorectal anastomotic site obstruction:Preliminary experience显示文摘AIM:To evaluate the efficacy of stents in treating patients with anastomotic site obstructions due to cancer recurrence following colorectal surgery.METHODS:The medical records of patients who underwent endoscopic self-expanding metal stents(SEMS)insertion for colorectal obstructions between February2004 and January 2014 were retrospectively reviewed.During the study period,a total of 218 patients underwent endoscopic stenting for colorectal obstructions.We identified and examined the patients who underwent endoscopic stenting for obstructions caused by cancer recurrence at the anastomotic site following colorectal surgeries for primary colorectal cancer.RESULTS:Five consecutive patients[mean age,56.4years(range:39-82 years);4 women,1 man]underwent endoscopic stenting for obstructions caused by cancer recurrence at the anastomotic site following colorectal surgeries for primary colorectal cancer.Technical and clinical success was achieved in all 5 patients,without any early complications.During follow-up,3patients did not need further intervention,prior to their death,after the first stent insertion;thus,the overall success rate was 3/5(60%).Perforations occurred in 2patients who required a second SEMS insertion due to re-obstruction;none of the patients experienced stent migration.CONCLUSION:SEMS placement is a promising treatment option for patients who develop obstructions of their colonic anastomosis sites due to cancer recurrence. | Jung Ho Kim Jong Joon Lee Jae Hee Cho Kyoung Oh Kim Jun-Won Chung Yoon Jae Kim Kwang An Kwon Dong Kyun Park Ju Hyun Kim | 2014 | World Journal of Gastroenterology2014,20,38: | 4 |
| 6 | Effects of exogenous recombinant human bone morphogenic protein-7 on the corneal epithelial mesenchymal transition and fibrosis显示文摘AIM:To evaluate the effect of exogenous recombinant human bone morphogenic protein-7(rhBMP-7)on transforming growth factor-β(TGF-β)-induced epithelial mesenchymal cell transition(EMT)and assessed its antifibrotic effect via topical application.METHODS:The cytotoxic effect of rhBMP-7 was evaluated and the EMT of human corneal epithelial cells(HECEs)was induced by TGF-β. HECEs were then cultured in the presence of rhBMP-7 and/or hyaluronic acid(HA). EMT markers,fibronectin,E-cadherin,α-smooth muscle actin(α-SMA),and matrix metaloproteinase-9(MMP-9),were evaluated. The level of corneal fibrosis and the reepithelization rate were evaluated using a rabbit keratectomy model. Expression of α-SMA in keratocytes were quantified following treatment with different concentrations of rhBMP-7.RESULTS:Treatment with rhBMP-7 attenuated TGF-β-induced EMT in HECEs. It significantly attenuated fibronectin secretion(31.6%; P<0.05),the α-SMA protein level(72.2%; P<0.01),and MMP-9 expression(23.6%,P<0.05)in HECEs compared with cells grown in the presence of TGF-β alone. E-cadherin expression was significantly enhanced(289.7%; P<0.01)in the presence of rhBMP-7. Topical application of rhBMP-7 combined with 0.1% HA significantly reduced the amount of α-SMA^+ cells by 43.18%(P<0.05)at a concentration of 2.5 μg/mL and by 47.73%(P<0.05)at 25 μg/mL,compared with the control group,without disturbing corneal reepithelization.CONCLUSION:rhBMP-7 attenuates TGF-β-induced EMT in vitro,and topical application of rhBMP-7 reduces keratocyte myodifferentiation during the early wound healing stages in vivo without hindering reepithelization. | 7 | Dietary fat-associated osteoarthritic chondrocytes gain resistance to lipotoxicity through PKCK2/STAMP2/FSP27显示文摘Free fatty acids(FFAs), which are elevated with metabolic syndrome, are considered the principal offender exerting lipotoxicity. Few previous studies have reported a causal relationship between FFAs and osteoarthritis pathogenesis. However, the molecular mechanism by which FFAs exert lipotoxicity and induce osteoarthritis remains largely unknown. We here observed that oleate at the usual clinical range does not exert lipotoxicity while oleate at high pathological ranges exerted lipotoxicity through apoptosis in articular chondrocytes. By investigating the differential effect of oleate at toxic and nontoxic concentrations, we revealed that lipid droplet(LD) accumulation confers articular chondrocytes, the resistance to lipotoxicity. Using high fat diet-induced osteoarthritis models and articular chondrocytes treated with oleate alone or oleate plus palmitate, we demonstrated that articular chondrocytes gain resistance to lipotoxicity through protein kinase casein kinase 2(PKCK2)—six-transmembrane protein of prostate 2(STAMP2)—and fat-specific protein 27(FSP27)-mediated LD accumulation. We further observed that the exertion of FFAs-induced lipotoxicity was correlated with the increased concentration of cellular FFAs freed from LDs, whether FFAs are saturated or not. In conclusion, PKCK2/STAMP2/FSP27-mediated sequestration of FFAs in LD rescues osteoarthritic chondrocytes. | 8 | Effect of external beam radiotherapy on patency of uncovered metallic stents in patients with inoperable bile duct cancer显示文摘BACKGROUND: Although biliary decompression with metallic stenting is the preferred treatment for inoperable bile duct cancer(BDC), maintenance of patency is still unsatisfactory.We tried to assess the effectiveness and safety of external beam radiotherapy(EBRT) for prolonging stent patency in patients having uncovered metallic stents.METHOD: We retrospectively reviewed 50 patients who received endoscopic stenting, of whom 18 received EBRT(RT group) and 32 did not(non-RT group).RESULTS: No difference was found in baseline characteristics between the two groups. Although stent patency was longer in the RT group than that in the non-RT group(140.7±51.3 vs136.4±34.9 days, P=0.94), the difference was not statistically significant. There were a lower rate of stent occlusion(27.8% vs50.0% of patients, P=0.12) and a longer overall survival(420.1 ±73.2 vs 269.1±41.7 days, P=0.11) in the RT group than in the non-RT group, and the difference again was not statistically significant. The development of adverse reactions did not differ(55.6% vs 53.1% of patients, P=0.91). There was no serious adverse reaction in both groups(P=0.99).CONCLUSIONS: EBRT did not significantly improve stent patency in patients with inoperable BDC having uncovered metallic stents. However, EBRT was safe. | 10 | Distant skeletal muscle metastasis from intrahepatic cholangiocarcinoma presenting as Budd-Chiari syndrome显示文摘Intrahepatic cholangiocarcinoma is a malignant neoplasm arising from the biliary epithelium, which frequently invades adjacent organs or metastasizes to other visceral organs such as the lungs, bones, adrenals, and brain. However, distant skeletal muscle metastasis of cholangiocarcinoma has never been described before to the best of our knowledge and, furthermore, Budd-Chiari syndrome secondary to intrahepatic cholangiocarcinoma is also extremely rare. Here we present the first case overall of distant muscle metastasis from intrahepatic cholangiocarcinoma presenting as Budd-Chiari syndrome. A 44-year-old man admitted to the hospital with complaints of abdominal distension, edema of both legs, back pain and anorexia of 30 d' duration.
