2Expenditure and financial burden for the diagnosis and treatment of colorectal cancer in China:a hospital.based,multicenter,cross-sectional survey显示文摘Background: The increasing prevalence of colorectal cancer(CRC) in China and the paucity of information about relevant expenditure highlight the necessity of better understanding the financial burden and effect of CRC diagnosis and treatment. We performed a survey to quantify the direct medical and non-medical expenditure as well as the resulting financial burden of CRC patients in China.Methods: We conducted a multicenter, cross-sectional survey in 37 tertiary hospitals in 13 provinces across China between 2012 and 2014. Each enrolled patient was interviewed using a structured questionnaire. All expenditure data were inflated to the 2014 Chinese Yuan(CNY; 1 CNY = 0.163 USD). We quantified the overall expenditure and financial burden and by subgroup(hospital type, age at diagnosis, sex, education, occupation, insurance type, household income, clinical stage, pathologic type, and therapeutic regimen). We then performed generalized linear modeling to determine the factors associated with overall expenditure.Results: A total of 2356 patients with a mean age of 57.4 years were included, 57.1 % of whom were men; 13.9% of patients had stage I cancer; and the average previous-year household income was 54,525 CNY.The overall average direct expenditure per patient was estimated to be 67,408 CNY, and the expenditures for stage Ⅰ, Ⅱ, Ⅲ, and Ⅳ disease were 56,099 CNY, 59,952 CNY, 67,292 CNY, and 82,729 CNY, respectively. Non-medical expenditure accounted for 8.3%of the overall expenditure. The 1-year out-of-pocket expenditure of a newly diagnosed patient was 32,649 CNY, which accounted for 59.9% of their previous-year household income and caused 75.0% of families to suffer an unmanageable financial burden. Univariate analysis showed that financial burden and overall expenditure differed in almost all subgroups(P < 0.05), except for sex. Multivariate analysis showed that patients who were treated in specialized hospitals and those who were diagnosed with adenocarcinoma or diagnosed at a later stage were likely to spend more,whereas those with a lower household income and those who underwent surgery spent less(all P < 0.05).Conclusions: For patients in China, direct expenditure for the diagnosis and treatment of CRC seemed catastrophic,and non-medical expenditure was non-ignorable. 4Unique cytologic features of thyroiditis caused by immune checkpoint inhibitor therapy for malignant melanoma显示文摘Blockade of immune checkpoint molecules to reverse cancer-induced immune suppression can improve anti-tumor immune responses in cancer patients.Monoclonal antibodies targeting two such molecules,Programmed cell death protein 1(PD-1)and cytotoxic T-lymphocyte associated protein 4(CTLA-4)have shown clinical benefit in the treatment of advanced malignancies,including metastatic melanoma.Adverse effects of these immune checkpoint inhibitors include immune-related adverse events(irAE),of which one of the most common is autoimmune thyroiditis.Though thyroiditis is increasingly recognized,there are no reports of the pathological findings that occur in immunotherapy-induced thyroiditis.We present a case of immunotherapy-induced thyroiditis demonstrating its unique cytopathologic features.A 51-year-old woman with metastatic melanoma was found to have a suppressed TSH and elevated free thyroxine concentration 14 days after starting treatment with nivolumab(PD-1 antagonist)plus ipilimumab(CTLA-4 antagonist)therapy.A thyroid biopsy was performed based on ultrasound findings and cytopathology revealed unique features including abundant clusters of necrotic cells,lymphocytes and CD163-positive histiocytes.This case reports cytopathologic features found in immune checkpoint inhibitor related thyroiditis.These appear to be unique findings and may help inform future research regarding the pathophysiology and mechanisms of this condition.Trevor E.Angell Le Min Tad J.Wieczorek F.Stephen Hodi 2018Genes & Diseases2018,5,1:13
5Validation of the chinese version of the EORTC QLQ-CR29 in patients with colorectal cancer显示文摘AIM To assess the validity and reliability of the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Colorectal Cancer 29(EORTC QLQ-CR29) in Chinese patients with colorectal cancer(CRC). METHODS From March 2014 to January 2015, 356 patients with CRC from four different hospitals in China were enrolled in the study, and all patients self-administered the EORTC QLQ-CR29 and the quality of life core questionnaire(EORTC QLQ-C30). Evaluation of the scores was based on the Karnofsky Performance Scale(KPS). The reliability and validity of the questionnaires were assessed by Cronbach's α coefficient, the Spearman correlation test and Wilcoxon rank sum test.RESULTS The EORTC QLQ-CR29 showed satisfactory reliability(α > 0.7), although the urinary frequency and blood and mucus in stool dimensions had only moderate reliability(α = 0.608). The multitrait scaling analyses showed good convergent(r > 0.4) and discriminant validity. Significant differences were obtained for each item in the different KPS subgroups(KPS ≤ 80; KPS > 80). body image and most single-item dimensions showed statistically significant differences in patients with a stoma compared with the rest of the patients. 9Assessment of the cropland classifications in four global land cover datasets: A case study of Shaanxi Province, China显示文摘Accurate and reliable cropland surface information is of vital importance for agricultural planning and food security monitoring. As several global land cover datasets have been independently released,an inter-comparison of these data products on the classification of cropland is highly needed. This paper presents an assessment of cropland classifications in four global land cover datasets,i.e.,moderate resolution imaging spectrometer(MODIS) land cover product,global land cover map of 2009(Glob Cover2009),finer resolution observation and monitoring of global cropland(FROM-GC) and 30-m global land cover dataset(Globe Land30). The temporal coverage of these four datasets are circa 2010. One of the typical agricultural regions of China,Shaanxi Province,was selected as the study area. The assessment proceeded from three aspects: accuracy,spatial agreement and absolute area. In accuracy assessment,506 validation samples,which consist of 168 cropland samples and 338 non-cropland ones,were automatically and systematically selected,and manually interpreted by referencing high-resolution images dated from 2009 to 2011 on Google Earth. The results show that the overall accuracy(OA) of four datasets ranges from 61.26 to 80.63%. Globe Land30 dataset,with the highest accuracy,is the most accurate dataset for cropland classification. The cropland spatial agreement(mainly located in the plain ecotope of Shaanxi) and the non-cropland spatial agreement(sparsely distributed in the south and middle of Shaanxi) of the four datasets only makes up 33.96% of the whole province. FROM-GC and Globe Land30,obtaining the highest spatial agreement index of 62.40%,have the highest degree of spatial consistency. In terms of the absolute area,MODIS underestimates the cropland area,while Glob Cover2009 significantly overestimates it. These findings are of value in revealing to which extent and on which aspect that these global land cover datasets may agree with each other at small scale on each ecotope region. 11Comprehensive evaluation on the ecological function of groundwater in the Shiyang River watershed显示文摘With an arid climate and shortage of water resources,the groundwater dependent ecosystems in the oasis-desert ecotone of the Shiyang River Watershed has been extremely damaged,and the water crisis in the oasis has become a major concern in the social and the scientific community.In this study,the degene-ration characteristics of the groundwater ecological function was identified and comprehensive evaluated,based on groundwater depth data,vegetation quadrat and normalized difference vegetation index(NDVI)from Landsat program.The results showed that(1)the suitable groundwater depth for sustainable ecology in the Shiyang River Watershed is about 2-4 m;(2)the terms of degenerative,qualitative and disastrous stages of the groundwater ecological function are defined with the groundwater depths of about 5 m,7 m and 10 m;(3)generally,the groundwater ecological function in the oasis-desert ecotone of the lower reaches of Shiyang River Watershed is weak with an area of 1397.9 km2 identified as the severe deterioration region,which accounted 74.7%of the total area.In the meantime,the percentages of the good,mild and moderate deterioration areas of groundwater ecological function are 3.5%,5.5%and 16.3%,respectively,which were mainly distributed in the Qingtu lake area and the southeastern area of the Shoucheng town;(4)the degradation and shrinkage of natural oasis could be attributed to the dramatic groundwater decline,which is generally caused by irrational use of water and soil resources.This study could provide theoretical basis and scientific support for the decision-making in environmental management and ecological restoration of the Shiyang River Watershed.Min Liu Zhen-long Nie Le Cao Li-fang Wang Hui-xiong Lu Zhe Wang Pu-cheng Zhu 2021Journal of Groundwater Science and Engineering2021,9,4:5

15A new loss-of-function allele 28y reveals a role of ARGONAUTE1 in limiting asymmetric division of stomatal lineage ground cell显示文摘In Arabidopsis thaliana L., stomata are produced through a series of divisions including asymmetric and symmetric divisions. Asymmetric entry division of meristemoid mother cell produces two daughter cells, the smaller meristemoid and the larger sister cell, a stomatal lineage ground cell(SLGC). Stomatal lineage ground cells can differentiate into epidermal pavement cells but have the potential to divide asymmetrically, spacing divisions, to create satellite meristemoids. Peptide ligands and TOO MANY MOUTHS(TMM) and ERECTA family receptors regulate the initiation of stomatal lineages, activity, and orientation of spacing divisions. Here, we reported that a natural mutant 28 y displayed an increased stomatal density and index. Using map-based cloning, we identified mutation in ARGONAUTE1(AGO1)as the cause of 28 y phenotypes. Time-lapse tracing of stomatal lineage cells reveals that stomatal overproduction in 28 y is caused by the excessive asymmetric spacing division of SLGCs.Further genetic results demonstrated that AGO1 acts downstream of TMM and negatively regulates the SPCH transcripts,but in a brassinosteroid-independent manner. 16Immune-related endocrine disorders in novel immune checkpoint inhibition therapy显示文摘Immune checkpoint inhibition against advance malignancies was named breakthrough discovery by the science magazine in 2013.In numerous clinical studies,monoclonal antibodies against the immune checkpoints,CTLA4,PD1 and PD1 ligand PDL1 have shown promising tumor response in different type of metastatic malignancies.The adverse events are autoimmune-related.The endocrine disorders,hypophysitis and thyroiditis are among the most common side effects associated with immune checkpoint inhibition treatment.Hypophysitis,a very rare endocrine disorder occurs in about one tenth of the patients receiving anti-CTLA4 treatment.Thyroiditis,on the other hand,is more commonly seen in patients receiving anti-PD1 treatment.In addition,both thyroiditis and hypophysitis are common in patients receiving combination treatment with anti-CTLA4 and anti-PD1 treatment.The time to onset of hypophysitis and thyroiditis is short.Most of the endocrine disorders occur within 12 weeks after initiation of the immune checkpoint inhibition therapy.Hypophysitis can manifest as total anterior pituitary hormone deficiency or isolated pituitary hormone deficiency.Diabetes insipidus is rare.TSH and gonadotropin deficiencies may be reversible but ACTH deficiency appears permanent.Thyroiditis can present as hypothyroidism or thyrotoxicosis followed by hypothyroidism.Hypothyroidism appears irreversible.Early identifying the onset of hypophysitis and thyroiditis and proper management of these endocrine disorders will improve the quality of the life and the outcome of this novel immunotherapy.Le Min 2016Genes & Diseases2016,3,4:3
17Mitochondrial Oxidative Stress Enhances Vasoconstriction by Altering Calcium Homeostasis in Cerebrovascular Smooth Muscle Cells under Simulated Microgravity显示文摘Objective Exposure to microgravity results in postflight cardiovascular deconditioning in astronauts.Vascular oxidative stress injury and mitochondrial dysfunction have been reported during this process.To elucidate the mechanism for this condition,we investigated whether mitochondrial oxidative stress regulates calcium homeostasis and vasoconstriction in hindlimb unweighted(HU)rat cerebral arteries.Methods Three-week HU was used to simulate microgravity in rats.The contractile responses to vasoconstrictors,mitochondrial fission/fusion,Ca^(2+) distribution,inositol 1,4,5-trisphosphate receptor(IP3 R)abundance,and the activities of voltage-gated K+channels(KV)and Ca^(2+)-activated K+channels(BKCa)were examined in rat cerebral vascular smooth muscle cells(VSMCs).Results An increase of cytoplasmic Ca^(2+) and a decrease of mitochondrial/sarcoplasmic reticulum(SR)Ca^(2+) were observed in HU rat cerebral VSMCs.The abundance of fusion proteins(mitofusin 1/2[MFN1/2])and fission proteins(dynamin-related protein 1[DRP1]and fission-mitochondrial 1[FIS1])was significantly downregulated and upregulated,respectively in HU rat cerebral VSMCs.The cerebrovascular contractile responses to vasoconstrictors were enhanced in HU rats compared to control rats,and IP3 R protein/mRNA levels were significantly upregulated.The current densities and open probabilities of KV and BKCa decreased and increased,respectively.Treatment with the mitochondrial-targeted antioxidant mitoTEMPO attenuated mitochondrial fission by upregulating MFN1/2 and downregulating DRP1/FIS1.It also decreased IP3 R expression levels and restored the activities of the KV and BKCa channels.MitoTEMPO restored the Ca^(2+) distribution in VSMCs and attenuated the enhanced vasoconstriction in HU rat cerebral arteries.Conclusion The present results suggest that mitochondrial oxidative stress enhances cerebral vasoconstriction by regulating calcium homeostasis during simulated microgravity.LIU Zi Fan WANG Hai Ming JIANG Min WANG Lin LIN Le Jian ZHAO Yun Zhang SHAO Jun Jie ZHOU Jing Jing XIE Man Jiang LI Xin ZHANG Ran 2021Biomedical and Environmental Sciences2021,34,3:3
18Anti-oxidant and anti-inflammatory activities of ultrasonicassistant extracted polyphenol-rich compounds from Sargassum muticum显示文摘Polypehnol is an important,potentially bioactive component of Sargassum muticum.In this study,ultrasonic assisted extraction of polyphenol-rich substances was performed using a 38%ethanol solution at a solid:liquid ratio of 1:30 at 68℃ for 32min,determined by single-factor and response surface methodology(RSM)optimization.The content of polyphenol was 5.66mg/g in the crude extract.Further extraction showed that the polyphenol mainly distributed in ethyl acetate(SKEE)and water phases(SKEW).The anti-oxidation test by electron spin resonance(ESR)spectrum showed that the SKEE had the strongest scavenging activity on DPPH(1,1-diphenyl-2-picrylhydrazyl)and alkyl radicals.SKEE was shown noncytotoxic but could inhibit the generation of cellular ROS,showing protective effects in H2O2 and AAPHinduced Vero cells and UV-B irradiated HaCaT cells.SKEE also signifi cantly inhibited the release of NO of LPS-induced RAW 264.7 cells.Therefore,the polyphenol-rich extracts in ethanol and ethyl acetate showed excellent anti-oxidant and anti-infl ammatory activities,which is beneficial to the development of high-value bio-substances.YU Yuan WANG Le FU Xiaoting WANG Lei FU Xiaodan YANG Min HAN Zhenlian MOU Haijin JEON You-Jin 2019Journal of Oceanology and Limnology2019,37,3:3
