1Is tailored therapy based on antibiotic susceptibility effective?A multicenter,open-label,randomized trial显示文摘An effective eradication therapy of Helicobacter pylori(H.pylori)should be used for the first time.In this study,we assessed whether tailored therapy based on antibiotic susceptibility testing is more effective than traditional therapy.We also evaluated the factors that cause treatment failure in high-resistance areas.For this multicenter trial,we recruited 467 H.pylori-positive patients.The patients were randomly assigned to receive tailored triple therapy(TATT),tailored bismuth-containing quadruple therapy(TABQT),or traditional bismuthcontaining quadruple therapy(TRBQT).For the TATT and TABQT groups,antibiotic selection proceeded via susceptibility testing using an agar-dilution test.The patients in the TRBQT group were given amoxicillin,clarithromycin,esomeprazole,and bismuth.Successful eradication was defined as a negative 13C-urea breath test at least eight weeks after the treatment ended.Susceptibility testing was conducted using an agar-dilution test.The eradication rate was examined via intention-to-treat(ITT)and per-protocol(PP)analyses.The clarithromycin,levofloxacin,and metronidazole resistance rates were 26.12%,28.69%,and 96.79%,respectively.Resistance against amoxicillin and furazolidone was rare.The eradication rates for TATT,TRBQT,and TABQT were 67.32%,63.69%,and 85.99%in the ITT analysis(P<0.001)and 74.64%,68.49%,and 91.22%in the PP analysis(P<0.001),respectively.The efficacy of TABQTwas affected by clarithromycin resistance,and bismuth exerted a direct influence on TATT failure.TABQT was the most efficacious regimen for use in high-resistance regions,especially among clarithromycin-susceptible patients.Jie Pan Zhengchao Shi Dingsai Lin Ningmin Yang Fei Meng Lang Lin Zhencheng Jin Qingjie Zhou Jiansheng Wu Jianzhong Zhang Youming Li 2020Frontiers of Medicine2020,14,1:10
2Reductive transformation and detoxification mechanism of 2,4-dinitrochlorobenzene in combined zero valent iron and anaerobic-aerobic process显示文摘A combined zero valent iron (ZVI) and anaerobic-aerobic process was adopted for the treatment of 2,4-dinitrochlorobenzene (DNCB)-containing wastewater. The transformation pathway, reduction of acute toxicity and enhancement of biodegradability were investigated. After pretreatment by ZVI, DNCB in wastewater could be completely converted into 2,4-diaminochlorobenzene (DACB). The ratio of BOD5/COD increased from 0.005±0.001 to 0.168±0.007, while EC 50, 48 hr (V/V) increased from 0.65% to 5.20%, indicating the enhancement of biodegradability and reduction of acute toxicity with the pretreatment by ZVI. DACB was further dechlorinated to m-phenylenediamine during the anaerobic process using methanol as electron donor, with EC50, 48 hr increasing from 5.20% to 48.2%. After the subsequent anaerobic-aerobic process, m-phenylenediamine was degraded completely, with effluent COD of 67.5±10.8 mg/L. This effluent of the subsequent anaerobic-aerobic process was not toxic to zebrafish. 3Basic and clinical study of increased effect of partial anti-tumor agents by infusing sodium bicarbonate through target artery显示文摘Objective: To observe the influence of pH value on the proliferation of LAK cells and on the killing effect of rIL-2, IFN-ɑ2b, TNF-α, LAK cells and doxorubicin on malignant tumor cells, and investigate the possibility of increasing the efficacy of rIL-2 or IFN-ɑ2b and doxorubicin by infusing sodium bicarbonate (NaHCO3) through target arteries. Methods: Separating single nucleus cells from peripheral blood of healthy men, and observing the influence of pH on the activation of single nucleus cells by rIL-2. MTT assay was used to measure the killing effect of rIL-2, IFN-ɑ2b and TNF-α on 7404 cells and the increased effect of doxorubicin on rIL-2 and IFN-ɑ2b, the cytotoxity of LAK cells in different pH. Forty-two patients with advanced primary liver cancer were obtained by stratified random, NaHCO3, rIL-2/IFN-ɑ2b and doxorubicin were infused through target arteries. The efficacy was estimated after two cycles. Results: The conditions of pH 7.3 and pH 7.6 in vitro helped the proliferation of LAK cells and the killing effect of rIL-2, IFN-ɑ2b and LAK cells on 7404 cells. In the condition of pH 6.8 there was almost no killing effect for LAK cells. In the condition of pH 7.0, 7.2, 7.4 and 7.6, the killing rate of TNF-α to 7404 cells increased by degrees, and in pH 7.4 the killing effect was the optimum. After two cycles treatments in the 42 patients with advanced primary liver cancer, the response rate (CR+PR) was 88% (37/42). The median overall response and median overall survival were increased , and no complication associated with infusing sodium bicarbonate was observed. 5Transcriptome analysis of near-isogenic lines provides molecular insights into starch biosynthesis in maize kernel显示文摘Starch is the major component in maize kernels,providing a stable carbohydrate source for humans and livestock as well as raw material for the biofuel industry.Increasing maize kernel starch content will help meet industry demands and has the potential to increase overall yields.We developed a pair of maize near-isogenic lines(NILs) with different alleles for a starch quantitative trait locus on chromosome 3(q HS3), resulting in different kernel starch content. To investigate the candidate genes for q HS3 and elucidate their effects on starch metabolism, RNA-Seq was performed for the developing kernels of the NILs at 14 and 21 d after pollination(DAP). Analysis of genomic and transcriptomic data identified 76 genes with nonsynonymous single nucleotide polymorphisms and 384 differentially expressed genes(DEGs) in the in trogressed fragment, including a hexokinase gene, Zm HXK3 a, which catalyzes the conversion of glucose to glucose-6-phosphate and may play a key role instarch metabolism. The expression pattern of all DEGs in starch metabolism shows that altered expression of the candidate genes for q HS3 promoted starch synthesis,with positive consequences for kernel starch content. 6Association of human leukocyte antigen-DR-DQ-DP haplotypes with the risk of hepatitis B virus-related hepatocellular carcinoma显示文摘Aim:Genetic polymorphisms of human leukocyte antigen(HLA)class II molecules are associated with chronic hepatitis B virus(HBV)infection.We aimed to investigate the impacts of HLA-II haplotypes on viral evolution and the risks of HBV-caused liver diseases.Methods:HLA-DR-DQ-DP haplotypes were estimated in 1210 healthy controls,296 HBV clearance subjects,301 asymptomatic hepatitis B surface antigen carriers,770 chronic hepatitis B patients,443 HBV-related liver cirrhosis(LC)patients,and 1037 HBV-related hepatocellular carcinoma(HCC)patients.HBV mutations were determined by sequencing.The associations of HLA-DR-DQ-DP haplotypes with viral mutations and the risks of liver diseases were assessed by multivariate logistic regression.Results:Compared to HBV-free subjects,the haplotypes CCAACG,CCGACG,TCAATA,and TCGATA were associated with decreased HCC risk,with an odds ratio(OR)[95%confidence interval(CI)]of 0.62(0.40-0.95),0.60(0.39-0.92),0.73(0.54-0.98),and 0.58(0.42-0.78),respectively.CCAACG,CCGACG,and TCAATA were significantly associated with decreased frequencies of the HCC-risk HBV mutations:preS1 deletion,APOBECsignature HBV mutations in the core promoter and preS regions,A51C/T,G104C/T,and G146C/T.TCGATA and TTAACG were associated with increased LC risk,with an OR(95%CI)of 1.54(1.03-2.30)and 2.23(1.50-3.33),respectively.However,TCGATA and TTAACG were not consistently associated with the cirrhosis-risk HBV mutations.Conclusion:CCAACG,CCGACG,and TCAATA are inversely associated with HCC risk,possibly because they are involved in creating an immune microenvironment attenuating the generation of HCC-risk HBV mutations.TCGATA and TTAACG might predispose the polarity of immunity towards Th17 isotype related to LC.Yifan Chen Jiansheng Lin Yang Deng Wenbin Liu Zishuai Li Xinyu Zhou Shiliang Cai Rui Pu Jianhua Yin Xiaojie Tan Jun Zhao Xue Han Guangwen Cao 2022Hepatoma Research2022,8,1:2

8Characterization of SARS-CoV-2 worldwide transmission based on evolutionary dynamics and specific viral mutations in the spike protein显示文摘Background:The coronavirus disease 2019(COVID-19)caused by severe acute respiratory syndrome-related coronavirus-2(SARS-CoV-2)is pandemic.However,the origins and global transmission pattern of SARS-CoV-2 remain largely unknown.We aimed to characterize the origination and transmission of SARS-CoV-2 based on evolutionary dynamics.Methods:Using the full-length sequences of SARS-CoV-2 with intact geographic,demographic,and temporal information worldwide from the GISAID database during 26 December 2019 and 30 November 2020,we constructed the transmission tree to depict the evolutionary process by the R package'outbreaker'.The affinity of the mutated receptor-binding region of the spike protein to angiotensin-converting enzyme 2(ACE2)was predicted using mCSM-PPI2 software.Viral infectivity and antigenicity were tested in ACE2-transfected HEK293T cells by pseudovirus transfection and neutralizing antibody test.Results:From 26 December 2019 to 8 March 2020,early stage of the COVID-19 pandemic,SARS-CoV-2 strains identified worldwide were mainly composed of three clusters:the Europe-based cluster including two USA-based subclusters;the Asia-based cluster including isolates in China,Japan,the USA,Singapore,Australia,Malaysia,and Italy;and the USA-based cluster.The SARS-CoV-2 strains identified in the USA formed four independent clades while those identified in China formed one clade.After 8 March 2020,the clusters of SARS-CoV-2 strains tended to be independent and became'pure'in each of the major countries.Twenty-two of 60 mutations in the receptor-binding domain of the spike protein were predicted to increase the binding affinity of SARS-CoV-2 to ACE2.Of all predicted mutants,the number of E484K was the largest one with 86585 sequences,followed by S477N with 55442 sequences worldwide.In more than ten countries,the frequencies of the isolates with E484K and S477N increased significantly.V367F and N354D mutations increased the infectivity of SARS-CoV-2 pseudoviruses(P<0.001).SARS-CoV-2 with V367F was more sensitive to the S1-targeting neutralizing antibody than the wild-type counterpart(P<0.001).Conclusions:SARS-CoV-2 strains might have originated in several countries simultaneously under certain evolutionary pressure.Travel restrictions might cause location-specific SARS-CoV-2 clustering.The SARS-CoV-2 evolution appears to facilitate its transmission via altering the affinity to ACE2 or immune evasion.Jiluo Liu Xi Chen Yan Liu Jiansheng Lin Jiaying Shen Hongwei Zhang Jianhua Yin Rui Pu Yibo Ding Guangwen Cao 2021Infectious Diseases of Poverty2021,10,4:2

13Study on the Microscopic Network Model of PVA Hydrogel Based on the Tensile Behavior显示文摘Hydrogel is an emerging material widely used in the biological field.The tensile properties of hydrogel mainly depend on its polymer network,but due to the complex microstructure of the network,some properties of the hydrogel have yet to be qua nt itatively analyzed.In this article,we built a three-dimensional random fiber net work to characterize the micros tructure of a hydrogel.The fibers were simplified as elastic beams,and the cross-links were regarded as'hinges.'Combined with the macroscopic experimental results,the influence of the change of fiber length and the relationship between the polymer content and the cross-link density were studied.The model was verified by the quasi-static tensile test results of real PVA hydrogel materials with different fiber contents.The results show that the model can well match the tensile stress-strain curve and tensile strength of the PVA hydrogels with different fiber contents.The change in micros true ture of the fiber network induces nonlinear behaviors,and although the cross-link number increases with the increase in polymer content,the cross-link density(per unit PVA chain length)of the PVA hydrogel slightly decreases as the content increases.Kejia Xu Liqun Tang Yongrou Zhang Rong Ding Xinyuan Wang Dafei Huang Jiansheng Lin 2019Acta Mechanica Solida Sinica2019,32,5:1

20Clinical Study on the Treatment of Low Anal Fistula in Infants and Young Children by Anal Gland Excision and Virtual Hanging Procedure显示文摘Objective:To compare the efficacy of anal adenectomy with virtual hanging wire and anal fistulotomy in the treatment of low anal fistula in infants and children.Methods:60 children with low anal fistula who were admitted to our hospital from October 2021 to March 2022 and met the inclusion criteria were randomly divided into two groups of 30 cases each;the treatment group was treated with anal adenectomy and virtual hanging wire surgery,and the control group was treated with anal fistula resection.The clinical efficacy after treatment was compared.Results:The total effective rate of both groups was 96.67%and the difference between the two groups was not statistically significant(P>0.05).The postoperative pain score of the treatment group was lower than that of the control group(P<0.05).The length of hospitalization and healing time of the treatment group was lower than that of the control group(P<0.05).The anal function of the patients in both groups was normal,and there was no adverse reaction.Conclusion:Anal gland excision and virtual hanging surgery for the treatment of low anal fistula in infants and children have the advantages of mild pain,reduced length of hospitalization,short healing time,and better patient experience as compared to anal fistula excision.Hongbo Su Linmei Sun Yimiao Liang Jiansheng Hu Yongli Zhang Ni Wei Chaoyang Li Lin Tang 2024Journal of Clinical and Nursing Research2024,8,3:0
