Clinical research linking Traditional Chinese Medicine constitution types with diseases:a literature review of 1639 observational studies显示文摘OBJECTIVE:To analyze clinical studies on correlations between Traditional Chinese Medicine(TCM)body constitution types and diseases published in the past 10 years, and to provide an evidence base to support the use of such correlations for health maintenance and disease prevention.METHODS:We searched five databases for the period April 2009 to December 2019:China National Knowledge Infrastructure Database,Wanfang Database,China Science and Technology Journal Database,PubMed and Embase.Three types of observational studies on correlation between constitution types and diseases were included:cross-sectional,case-control and cohort studies.Descriptive statistical methods were employed for data analysis.RESULTS:A total of 1639 clinical studies were identified:1452(88.59%)cross-sectional studies,115(7.02%)case-control studies and 72(4.39%)cohort studies covering 30 regions of China and five other countries(Malaysia,South Korea,Singapore,Thailand and France). The collection of studies comprised 19 disease categories and 333 different diseases. The 10 most commonly studied diseases were hypertension, diabetes, stroke, coronary atherosclerotic heart disease(CAHD),sleep disorders, neoplasm of the breast, dysmenorrhea, fatty liver disease, chronic viral hepatitis B and dyslipidemia. We found high distributions for each biased constitution type in different patient populations as follows:Qi-deficiency constitution in stroke, diabetes, chronic obstructive pulmonary disease, acquired immunodeficiency syndrome and hypertension;Yang-deficiency constitution in female infertility, osteoporosis, irritable bowel syndrome, gonarthrosis and dysmenorrhea;Yin-deficiency constitution in hypertension, diabetes, constipation, female climacteric states and osteoporosis;phlegm-dampness constitution in hypertension, stroke, fatty liver disease, diabetes and metabolic syndrome;damp-heat constitution in acne, chronic gastritis, chronic viral hepatitis B,human papillomavirus infection and hyperuricemia;blood-stasis constitution in CAHD,endometriosis and stroke;Qi-stagnation constitution in hyperplasia and neoplasms of the breast, insomnia, depression and thyroid nodules;and inherited-special constitution in asthma and allergic rhinitis.CONCLUSION:Eight biased TCM constitutions were closely related to specific diseases, and could be used to guide individualized prevention and treatment.More rigorously designed studies are recommended to further verify the constitution-disease relationship.Liang Xue Wang Qi Jiang Zeqiang Li Zhuqing Zhang Mengxing Yang Peiying Wang Xin Wang Yanqiu Qin Yuehua Li Tianxing Zhang Tianyi Wang Yi Sun Jianxiang Li Yusi Luo Hui Li Lingru 2020Journal of Traditional Chinese Medicine2020,40,4:29
Gene expression signatures for phlegm-dampness constitution of Chinese Medicine显示文摘Dear Editor,The term'individualized medicine'was first used in Biochemistry Individuality in 1956 by Williams(Williams et al.,1956).With the emergence of the post-genome era in the21st century, individualized precision medicine has attracted increasing attention. In traditional Chinese medicine(TCM),there is also concept of individualized medicine, which is known as the Chinese constitutional theory, proposed in the1970s(Wang,2012).According to this theory, theLingru Li Juan Feng Haiqiang Yao Lan Xie Yu Chen Lingling Yang Shujuan Hou Shipeng Zhao Ranran Sun Yanling Wu Tongtong Bai Yingshuai Li Ruoxi Yu Ji Wang Qi Wang 2017Science China(Life Sciences)2017,60,1:13
Efficacy and safety of Yinchenwuling powder for hyperlipidemia: a systematic review and Meta-analysis显示文摘OBJECTIVE:To assess the clinical effectiveness and adverse effects of Yinchenwuling powder(YCWLP)in the treatment of hyperlipidemia using Meta-analysis.METHODS:Seven electronic databases were searched for randomized controlled trials designed to evaluate the clinical effectiveness of YCWLP for hyperlipidemia published in any language prior to February 2015.Two reviewers independently identified articles, extracted data, assessed quality, and cross-checked the results.Revman 5.3 was used to analyze the data.RESULTS:Only five randomized controlled trials with poor methodology were included in the analy-sis.The five trials compared YCWLP with conventional lipid-lowering drugs.Meta-analysis indicated that YCWLP was more effective at the levels of total cholesterol and triglycerides, while increasing the level of high-density lipoprotein cholesterol without serious adverse effects. However,it was not more effective than lipid-lowering drugs in reducing low-density lipoprotein cholesterol and improving hemorheology.CONCLUSION:YCWLP appeared to improve lipid levels. However, given the high risk of bias among the trials,we could not conclude that YCWLP was beneficial to patients with hyperlipidemia.More rigorous trials are required to provide stronger evidence for the conclusion.Yao Haiqiang Zhang Zengliang Wang Ji Zuo Jiacheng Chen Yu Zhu Libing Li Xiaoke Yang Zheng Wang Zisong Sun Ranran Xu Xuanxuan Li Changming Wu Yanling Li Lingru Wang Qi 2016Journal of Traditional Chinese Medicine2016,36,2:11
Expression profiling-based clustering of healthy subjects recapitulates classifications defined by clinical observation in Chinese medicine显示文摘Differences between healthy subjects and associated disease risks are of substantial interest in clinical medicine. Based on clinical presentations, Traditional Chinese Medicine(TCM) classifies healthy people into nine constitutions: Balanced, Qi, Yang or Yin deficiency, Phlegm-dampness, Damp-heat, Blood stasis,Qi stagnation, and Inherited special constitutions. In particular, Yang and Yin deficiency constitutions exhibit cold and heat aversion, respectively. However, the intrinsic molecular characteristics of unbalanced phenotypes remain unclear. To determine whether gene expression-based clustering can recapitulate TCM-based classification, peripheral blood mononuclear cells(PBMCs) were collected from Chinese Han individuals with Yang/Yin deficiency(n ? 12 each) and Balanced(n ? 8) constitutions, and global gene expression profiles were determined using the Affymetrix HG-U133 A Plus 2.0 array. Notably,we found that gene expression-based classifications reflected distinct TCM-based subtypes. Consistent with the clinical observation that subjects with Yang deficiency tend toward obesity, series-clustering analysis detected several key lipid metabolic genes(diacylglycerol acyltransferase(DGAT2), acyl-Co A synthetase(ACSL1), and ATP-binding cassette subfamily A member 1(ABCA1)) to be down-and upregulated in Yin and Yang deficiency constitutions, respectively. Our findings suggest that Yin/Yang deficiency and Balanced constitutions are unique entities in their m RNA expression profiles. Moreover, the distinct physical and clinical characteristics of each unbalanced constitution can be explained, in part,by specific gene expression signatures.Ruoxi Yu Dan Liu Yin Yang Yuanyuan Han Lingru Li Luyu Zheng Ji wang Yan Zhang Yingshuai Li Qian-Fei Wang Qi wang 2017Journal of Genetics and Genomics2017,44,4:9
Classification of traditional Chinese medicine constitution based on facial features in color images显示文摘Objective:To explore the possible correlation between traditional Chinese medicine(TCM)constitution and facial features in color images and to improve the accuracy of automated constitution classification. Methods:Color images were taken of 5150 individuals of different professions.Automated face detection and key point positioning were performed on the collected images, which were then transformed into a standard size. The relationship between facial features and TCM constitution based on the red, green, blue(RGB)pixel and the local binary pattern(LBP)texture features was explored. Results:The overall accuracy rate and robustness of TCM constitution classification based on RGB features were low.Classification results of the phlegm-dampness,damp-heat,blood stasis, and balance constitutions achieved high accuracy rates.Classification accuracy rate using the LBP texture feature was higher than that of the RGB feature, with the best accuracy observed for the balance constitution. Conclusion:Application of computer image acquisition and processing of facial features may serve as an adjunct to the TCM diagnostic method of inspection.Jian Zhang Shujuan Hou Ji Wang Lingru Li Pin Li Junwen Han Haiqiang Yao Ranran Sun Ziqing Li Zhen Lei Qi Wang 2016Journal of Traditional Chinese Medical Sciences2016,3,3:5
Potential mechanism of Huatan Qushi decoction on improving phlegm-dampness constitution using microRNA array and RT-qPCR targeting on hsa-miR-1237e3p显示文摘Background:Metabolic diseases pose considerable burden on the healthcare system worldwide, indicating the significance of prevention and treatment. In constitution theory of traditional Chinese medicine,phlegm-dampness constitution(PDC)is the common basis of metabolic diseases. In clinical practice,Huatan Qushi(HTQS)decoction targeting on PDC can effectively improve metabolic indicators. However, its underlying biochemical mechanism still remains unclear. Methods:Eight PDC participants received HTQS decoction for three months.Their blood was collected at baseline and 1 and 3 months after intervention started.Related biomedical indicators were detected.High-throughput sequencing and RT-qPCR were used for validation.Due to the missing data, repeated measures with missing values in mixed models were used. Results:After 3-month treatment,HDL-C level increased(P<.001)and FBG,FINS,and HbA1c all showed decreasing trend at different time points(all P<.05).After miRNA high-throughput sequencing, compared with the baseline, differential miRNAs at 1 and 3 months were screened, and target gene prediction and KEGG pathway enrichment analysis were performed. The results displayed that metabolic disease-related pathways mainly included pathways in cancer,PI3K-Akt signaling pathway,etc.Further,RT-qPCR showed that hsa-miR-1237e3p differed statistically(P=.008).Then we validated the target genes of hsa-miR-1237e3p in the“Pathways in Cancer”pathway including SDF1,AC,CRK,and HGF,also known as upstream target genes of PI3K/AKT pathway. The results showed that two indicators of CRK and HGF were in statistical significance(P=.045 and P=.036,respectively).Conclusion:PDC serves as a common basis for various metabolic diseases.Through adjusting PDC,HTQS decoction can improve biomedical indicators including blood glucose,HbA1c,insulin,and HDL-C.The target pathway is“Pathways in cancer”.Specifically,HTQS decoction acts on targets of CRK and HGF by regulating hsa-miR-1237e3p,and probably exerts effects on their downstream PI3K/AKT pathway.Xiuping Zhang Ji Wang Baoxing Liu Haiqiang Yao Yu Chen Yuqing Yin Xuejing Yang Lingru Li 2021Journal of Traditional Chinese Medical Sciences2021,8,1:3

Association between metabolic syndrome and body constitution of traditional Chinese medicine:A systematic review and meta-analysis显示文摘Background:The constitution theory of traditional Chinese medicine(TCM)states that body constitution and disease are associated;therefore, the identification of body constitution can provide reference for disease prevention and treatment.A series of clinical studies evaluating the association between TCM constitution and metabolic syndrome(MetS)have been conducted in recent years. The aim of this systematic review is to summarize and evaluate the clinical evidence of the relationship between constitution and MetS.Methods:The PubMed,Embase,China National Knowledge Infrastructure(CNKI),Chongqing VIP,and Wanfang Data electronic databases were searched from April 2009 to July 2020.The search terms included“metabolic syndrome,”“constitution,”and“Chinese medicine.”Observational studies evaluating the relationship between body constitution of TCM and MetS were included. The primary outcome included the distribution of constitutional types in the MetS population, and odds ratio(OR)or relative risk(RR)between constitution and MetS.Effect size was expressed as proportion or OR with 95%confidence interval(CI).Methodological quality of included studies was evaluated by the NewcastleeOttawa Scale(NOS)or the American Agency for Healthcare Research and Quality(AHRQ)criteria. Results:Three caseecontrol and 26 cross-sectional studies involving 30435 cases were included.Phlegm-dampness,qi-deficiency,and damp-heat were the main constitution types in patients with MetS,accounting for 29%(22%e39%),18%(13%e24%),and 12%(9%e17%)of the population, respectively;the risk of MetS in people with phlegm-dampness and qi-deficiency constitution was 1.74[1.27,2.38]and 1.29[1.01,1.65]times higher than that in those with other constitutions. There were sex-and age-related differences in the distribution of TCM constitution in patients with MetS.Conclusion:Phlegm-dampness and qi-deficiency were associated with the occurrence of MetS.Welldesigned cohort and caseecontrol studies are needed to provide evidence for the use of TCM to prevent and treat MetS.Hui Luo Lingru Li Tianxing Li Xing Liao Qi Wang 2020Journal of Traditional Chinese Medical Sciences2020,7,4:3
Consistency between traditional Chinese medicine constitution-based classification and genetic classification显示文摘Background:We studied the consistency between two classification systems for categorizing patients:traditional Chinese medicine(TCM)constitution-based methods, versus genetic clustering.Genetic classification in constitutional identification was also evaluated. Methods:A TCM physician evaluated the constitution of each patient, according to four examinations(inspection,auscultation-olfaction,interrogation,and palpation). Those who met the criteria for Yang-deficient,Yin-deficient,and balanced constitutions were enrolled in the study.Peripheral blood samples were obtained from the participants, and peripheral blood mononuclear cells were separated from the samples within 2 hours.Total RNA extraction from the white blood cells was performed;and an Affymetrix HG-U133 Plus2.0 array was used to determine the peripheral blood gene expression profiles. The samples were classified using a support vector machine genetic classifier, and the“leave-one-out”method was used for validation. Results:The global gene expression profiles of 32 samples were grouped into three categories, and the samples in each of the gene categories corresponded with the three constitution categories. The three constitution types were distinguished using the genetic classifier with 165 genes. The accuracy of the prediction classification was greater than 95%using mathematical method. Conclusions:Participants with Yin-deficient,Yang-deficient,and balanced constitutions have varying physical characteristics and gene expression patterns.Additionally, the results from TCM constitution classification matched those obtained by genetic classification. Finally, our preliminary gene classifier distinguishes among Yin-deficient,Yang-deficient,and balanced constitutions, and provides a methodological basis for identifying the different constitutions.Ruoxi Yu Xiaohong Zhao Lingru Li Cheng Ni Yin Yang Yuanyuan Han Ji Wang Yan Zhang Qi Wang 2015Journal of Traditional Chinese Medical Sciences2015,2,4:2
Proteomic analysis of overweight/obesity and related abnormal glucose and lipid metabolism caused by phlegm-dampness retention显示文摘Objective:To investigate the proteomic characteristics of overweight/obesity and related abnormal glucose and lipid metabolism caused by phlegm-dampness retention to identify related biomarkers.Methods:Seventy-one subjects were enrolled in the study. We assessed blood glucose, blood lipids, body mass index(BMI),and phlegm-dampness pattern, which was confirmed by a traditional Chinese medicine clinician.Of the participants,we included healthy participants with normal weight(NW,n=23),overweight/obese participants with normal metabolism(ONM,n=19),overweight/obese participants with pre-diabetes(OPD,n=12),and overweight/obese participants with marginally-elevated blood lipids(OML,n=17).Among them, the ONM,OPD,and OML groups were diagnosed with phlegmdampness pattern. The data-independent acquisition(DIA)method was first used to analyze the plasma protein expression of each group, and the relevant differential proteins of each group were screened. The co-expressed proteins were evaluated by Venn analysis. The pathway analyses of the differential proteins were analyzed using Ingenuity Pathway Analysis(IPA)software.Parallel reaction monitoring(PRM)was used to verify the differential and common proteins in each group. Results:After comparing ONM,OPD,and OML groups with NW group,we identified the differentially expressed proteins(DEPs).Next,we determined the DEPs among OPD,OML,and ONM groups.Using Venn analysis of the DEPs in each group,24 co-expressed proteins were screened. Two co-expressed proteins were verified by PRM.IPA analysis showed that pathways including LXR/RXR activation, acute phase response signaling, and FXR/RXR activation were common to all three groups of phlegmdamp overweight/obesity participants. However, the activation or inhibition of these pathways was different among the three groups. Conclusion:Participants with overweight/obesity have similar proteomic characteristics, though each type shows specific proteomic characteristics. Two co-expressed proteins,VTN and ORM1,are potential biomarkers for glucose and lipid metabolism diseases with overweight/obesity caused by phlegmdampness retention.Jiayi Ma Shuxian Sun Cheng Ni Lingru Li Jing Xia Houqin Li Huirong Song Xujun Heng Dandan Hu Yuanyuan Li 2021Journal of Traditional Chinese Medical Sciences2021,8,3:1
