1Role of surgical resection for multiple hepatocellular carcinomas显示文摘AIM:To clarify the role of surgical resection for multiple hepatocellular carcinomas(HCCs)compared to transarterial chemoembolization(TACE)and liver transplantation(LT). METHODS:Among the HCC patients who were managed at Yonsei University Health System between January 2003 and December 2008,160 patients who met the following criteria were retrospectively enrolled:(1) two or three radiologically diagnosed HCCs;(2)no radiologic vascular invasion;(3)Child-Pugh class A;(4) main tumor smaller than 5 cm in diameter;and(5) platelet count greater than 50 000/mm3.Long-term outcomes were compared among the following three treatment modalities:surgical resection or combined radiofrequency ablation(RFA)(n=36),TACE(n=107), and LT(n=17).The survival curves were computed using the Kaplan-Meier method and compared with a log-rank test.To identify the patients who gained a survival benefit from surgical resection,we also investigated prognostic factors for survival following surgical resection.Multivariate analyses of the prognostic factors for survival were performed using the Cox proportional hazard model. RESULTS:The overall survival(OS)rate was significantly higher in the surgical resection group than in the TACE group(48.1%vs 28.9%at 5 years,P<0.005). LT had the best OS rate,which was better than that of the surgical resection group,although the difference was not statistically significant(80.2%vs 48.1%at 5 years,P=0.447).The disease-free survival rates were also significantly higher in the LT group than in the surgical resection group(88.2%vs 11.2%at 5 years, P<0.001).Liver cirrhosis was the only significant prognostic factor for poor OS after surgical resection. Clinical liver cirrhosis rates were 55.6%(20/36)in the resection group and 93.5%(100/107)in the TACE group.There were 19 major and 17 minor resections. En bloc resection was performed in 23 patients,multisite resection was performed in 5 patients,and combined resection with RFA was performed in 8 patients. In the TACE group,only 34 patients(31.8%)were recorded as having complete remission after primary TACE.Seventy-two patients(67.3%)were retreated with repeated TACE combined with other therapies. In patients who underwent surgical resection,the 16 patients who did not have cirrhosis had higher 5-year OS and disease-free survival rates than the 20 patients who had cirrhosis(80.8%vs 25.5%5-year OS rate, P=0.006;22.2%vs 0%5-year disease-free survival rate,P=0.048).Surgical resection in the 20 patients who had cirrhosis did not provide any survival benefit when compared with TACE(25.5%vs 24.7%5-year OS rate,P=0.225).Twenty-nine of the 36 patients who underwent surgical resection experienced recur-rence.Of the patients with cirrhosis,80%(16/20) were within the Milan criteria at the time of recurrence CONCLUSION:Among patients with two or three HCCs, no radiologic vascular invasion,and tumor diameters≤ 5 cm,surgical resection is recommended only in those without cirrhosis.Sung Hoon Choi Gi Hong Choi Seung Up Kim Jun Yong Park Dong Jin Joo Man Ki Ju Myoung Soo Kim Jin Sub Choi Kwang Hyub Han Soon Il Kim 2013World Journal of Gastroenterology2013,19,3:12
2Endoscopic treatment of duodenal fistula after incomplete closure of ERCP-related duodenal perforation显示文摘Endoscopic retrograde cholangiopancreatography(ERCP)is an important diagnostic and therapeutic modality for various pancreatic and biliary diseases.The most common ERCP-induced complication is pancreatitis,whereas hemorrhage,cholangitis,and perforation occur less frequently.Early recognition and prompt treatment of these complications may minimize the morbidity and mortality.One of the most serious complications is perforation.Although the incidence of duodenal perforation after ERCP has decreased to<1.0%,severe cases still require prolonged hospitalization and urgent surgical intervention,potentially leading to permanent disability or mortality.Surgery remains the mainstay treatment for perforations of the luminal organs of the gastrointestinal tract.However,evidence from case reports and case series support a beneficial role of endoscopic clipping in the closure of these defects.Duodenal fistulas are usually a result of sphincterotomies,perforated duodenal ulcers,or gastrectomy.Other causative factors include Crohn's disease,trauma,pancreatitis,and cancer.The majority of duodenal fistulas heal with nonoperative management.Those that fail to heal are best treated with gastrojejunostomy.Recently proposed endoscopic approaches for managing gastrointestinal leaks caused by fistulas include fibrin glue injection and positioning of endoclips.Our patient developed a secondary persistent duodenal fistula as a result of previous incomplete closure of duodenal perforation with hemoclips and an endoloop.The fistula was successfully repaired by additional clipping and fibrin glue injection.Dong Wook Yu Man Yong Hong Seung Goun Hong 2014World Journal of Gastrointestinal Endoscopy2014,6,6:10

11Optimal control algorithm of fertigation system in greenhouse based on EC model显示文摘Two new control algorithms based on MSP430 microcontroller unit(MCU)were developed to improve the performance of a fertigation system controlled by the electrical conductivity(EC)value of an irrigation nutrient solution in a greenhouse.The first algorithm is incremental proportional-integral-derivative(PID),and the second one is a two-stage combination algorithm(PID+fuzzy).With an improved multi-line mixing Venturi tube,several sets of experiments were conducted for a fertilizer absorption test under two conditions,namely,various suction lines and different EC target values settings.Under the first condition,with an EC target value of 2.0 mS/cm and opening of various suction pipes,the steady-state times are 186 s,172 s,134 s,and 122 s corresponding to the opening of one to four suction pipes,respectively,for PID+fuzzy control.The corresponding values are 220 s,196 s,158 s,and 148 s for incremental PID control.Under the second condition,four suction pipes are opened with different target EC values of 1.5 mS/cm,2.0 mS/cm,and 2.5 mS/cm,and the shortest response time and the minimum overshoot were obtained for PID+fuzzy control when the target EC value is 1.5 mS/cm,which are 96 s and 0.18 mS/cm,respectively.While the corresponding values are 112 s and 0.4 mS/cm,respectively for incremental PID control.The two control strategies can adjust the EC value to the target value for real-time control,but the combination control algorithm can be implemented more rapidly,accurately,and steadily with a small overshoot compared with incremental PID control.The combination algorithm(PID+fuzzy)control strategy also possesses better properties for automatic fertigation control in greenhouses than the incremental PID control strategy,the combination algorithm provides an optimal way of water and fertilizer management for crops in greenhouses which will contribute to water and fertilizer saving.Yong Wu Li Li Shuaishuai Li Hongkang Wang Man Zhang Hong Sun Nikolaos Sygrimis Minzan Li 2019International Journal of Agricultural and Biological Engineering2019,12,3:1

16Simulation of a soft-gamma-ray polarimeter on board a microsatellite显示文摘Gamma-ray polarimetry is a new and prospective tool for studying extremely high-energy celestial objects and is of great significance for the field of astrophysics.With the rapid development of microsatellite technology,the advantages of space exploration have become increasingly apparent.Therefore,we simulated a soft-gamma-ray polarimeter for a microsatellite based on the Compton scattering principle.We performed detailed Monte Carlo simulations using monoenergetic gamma-ray linear-polarization sources and Crab-like sources in the energy range of 0.1-10 MeV considering the orbital background.The polarimeter exhibited excellent polarization detection performance.The modulation factor was 0.80±0.01,and the polarization angles were accurate within an error of 0.2°at 200 keV for on-axis incidence.For the Crab-like sources for on-axis incidence,the polarization degrees were consistent with the set values within the error tolerance,the modulation factor was 0.76±0.01,and the minimum detectable polarization reached 2.4%at 3σfor an observation time of10^(6) s.Additionally,the polarimeter exhibited recoil electron tracking,imaging,and powerful background suppression in a large field of view(FoV;∼2πsr).The proposed polarimeter meets the requirements of a space soft-gamma-ray polarization detector and has promising research prospects.Xiang‑Man Liu Shu‑Wen Tang Wei Wang Yu‑Hong Yu Zhi‑Yu Sun Yong‑Jie Zhang Fang Fang Duo Yan Shi‑Tao Wang Xue‑Heng Zhang Shu‑Ya Jin Bi‑Tao Hu 2023Nuclear Science and Techniques2023,34,11:0

19The discovery of 64 luminous infrared galaxies in the LAMOST Complete Spectroscopic Survey of Pointing Area at the Southern Galactic Cap显示文摘We report the discovery of 64 luminous infrared galaxies, based on new observations of 20 square degrees from the LAMOST Complete Spectroscopic Survey of Pointing Area at the Southern Galactic Cap and the WISE 22 μm catalog from the AllW ISE Data Release. Half of them are classified as late-type spirals and the others are classified as peculiar/compact galaxies. The peculiar/compact galaxies tend to exhibit higher luminosities and lower stellar masses. We also separate AGNs from HII galaxies in a simple way by examining LAMOST spectra.
