| 1 | Differential diagnosis of left-sided abdominal pain:Primary epiploic appendagitis vs colonic diverticulitis显示文摘AIM:To investigate the clinical characteristics of left primary epiploic appendagitis and to compare them with those of left colonic diverticulitis.METHODS:We retrospectively reviewed the clinical records and radiologic images of the patients who presented with left-sided acute abdominal pain and had computer tomography(CT) performed at the time of presentation showing radiological signs of left primary epiploic appendagitis(PEA) or left acute colonic diverticulitis(ACD) between January 2001 and December 2011. A total of 53 consecutive patients were enrolled and evaluated. We also compared the clinical characteristics,laboratory findings,treatments,and clinical results of left PEA with those of left ACD.RESULTS:Twenty-eight patients and twenty-five patients were diagnosed with symptomatic left PEA and ACD,respectively. The patients with left PEA had focal abdominal tenderness on the left lower quadrant(82.1%). On CT examination,most(89.3%) of the patients with left PEA were found to have an oval fatty mass with a hyperattenuated ring sign. In cases of left ACD,the patients presented with a more diffuse abdominal tenderness throughout the left side(52.0%vs 14.3%; P = 0.003). The patients with left ACD had fever and rebound tenderness more often than those with left PEA(40.0% vs 7.1%,P = 0.004; 52.0% vs14.3%,P = 0.003,respectively). | 2 | Proportion and clinical features of never-smokers with non-small cell lung cancer显示文摘Background: The proportion of never?smokers with non?small cell lung cancer(NSCLC) is increasing, but that in Korea has not been well addressed in a large population. We aimed to evaluate the proportion and clinical features of never?smokers with NSCLC in a large single institution.Methods: We analyzed clinical data of 1860 consecutive patients who were newly diagnosed with NSCLC between June 2011 and December 2014.Results: Of the 1860 NSCLC patients, 707(38.0%) were never?smokers. The proportions of women(83.7% vs. 5.6%) and adenocarcinoma(89.8% vs. 44.9%) were higher among never?smokers than among ever?smokers. Significantly more never?smokers were diagnosed at a younger median age(65 vs. 68 years, P < 0.001) and earlier stage(stage I–II, 44.5% vs. 38.9%, P < 0.001) a= 0.015) compared with ever?smokers. Epidermal growth factor receptor mutations(57.8% vs. 24.4%, Pnd anaplastic lymphoma kinase rearrangements(7.8% vs. 2.8%, P < 0.001) were more common in never?smokers, whereas Kirsten rat sarcoma viral oncogene homolog mutations(5.8% vs. 9.6%, P ntly encountered in never?smokers than in ever?smokers. Never?smokers showed longer su= 0.021) were less frequervival after adjust?ing for the favorable effects of younger age, female sex, adenocarcinoma histology, better performance status, early stage disease, being asymptomatic at diagnosis, received antitumor treatment, and the presence of driver mutations(hazard ratio, 0.624; 95% confidence interval, 0.460–0.848; P = 0.003).Conclusions: More than one?third of the Korean patients with NSCLC were never?smokers. | 13 | Hemorrhagic hemangioma in the liver:A case report显示文摘Hemangioma is the most common type of benign tumor that arises in the liver. Although rupture and hemorrhage of hepatic hemangioma are rare complications, they can be the cause of mortality. The authors report a case of hemorrhagic hepatic hemangioma: in a 54-yearold woman who was admitted with epigastric pain.She had taken oral contraceptives several weeks prior.The results of a blood examination were normal. An abdominal computed tomography scan revealed a tumor in hepatic segment 4, and a hemorrhage inside the cystic mass was suspected. The mass was removed laparoscopically to confirm the tumor properties and control the hemorrhage. The pathologic findings of the resected mass were consistent with hepatic hemangioma with intratumoral hemorrhage. | 17 | Acupuncture for treating uremic pruritus in patients with end stage renal disease:a systematic review显示文摘Objective This review aimed to systematically summarize and critically evaluate the clinical evidence for or against the effectiveness of acupuncture as a symptomatic therapy for uremic pruritus (UP) in end-stage renal disease (ESRD) patients. Materials and method: We searched 16 electronic databases from their inceptions to November 2009. All prospective clinical studies of needle acupuncture for uremic pruritus in hemodialysis patients with end-stage renal disease were included regardless of their design. Risk of bias of the included studies were assessed using the Cochrane criteria. Results Of 464 initially located articles, 458 studies were excluded. Three randomized controlled trials (RCTs), one non-randomized controlled trial (CCT) and two uncontrolled observational studies were included. In three RCTs, acupuncture was compared to sham-acupuncture (n=1), oral antihistamine plus topical ointment (n=1) and oral calcitriol (n=1). In one CCT, electroacupuncture (EA) was compared to superfi cial electrical stimulation. In two observational studies, EA (n=1) or manual acupuncture (n=1) were employed. Four of 6 included studies mentioned mild or no occurrence of adverse events. All of included trials reported benef icial effects of acupuncture. However, most of the studies showed high risk of bias, which leave their reports unconvincing. Conclusion The current evidence is insuffi cient to show that acupuncture is an effective treatment for uremic pruritus in patients with end-stage renal disease due to suboptimal quality and lack of methodological rigor of included studies.
