1Small bowel ulcerative lesions are common in elderly NSAIDs users with peptic ulcer bleeding显示文摘AIM: To determine the frequency of small bowel ulcerative lesions in patients with peptic ulcer and define the significance of those lesions. METHODS: In our prospective study, 60 consecutive elderly patients with upper gastrointestinal bleeding from a peptic ulceration(cases) and 60 matched patients with a non-bleeding peptic ulcer(controls) underwent small bowel capsule endoscopy, after a negative colonoscopy(compulsory in our institution). Controls were evaluated for non-bleeding indications. Known or suspected chronic inflammatory conditions and medication that could harm the gut were excluded. During capsule endoscopy, small bowel ulcerative lesions were counted thoroughly and classified according to Graham classification. Other small bowellesions were also recorded. Peptic ulcer bleeding was controlled endoscopically, when adequate, proton pump inhibitors were started in both cases and controls, and Helicobacter pylori eradicated whenever present. Both cases and controls were followed up for a year. In case of bleeding recurrence upper gastrointestinal endoscopy was repeated and whenever it remained unexplained it was followed by repeat colonoscopy and capsule endoscopy.RESULTS: Forty(67%) cases and 18(30%) controls presented small bowel erosions(P = 0.0001), while 22(37%) cases and 4(8%) controls presented small bowel ulcers(P < 0.0001). Among non-steroidal antiinflammatory drug(NSAID) consumers, 39(95%) cases and 17(33%) controls presented small bowel erosions(P < 0.0001), while 22(55%) cases and 4(10%) controls presented small bowel ulcers(P < 0.0001). Small bowel ulcerative lesions were infrequent among patients not consuming NSAIDs. Mean entry hemoglobin was 9.3(SD = 1.4) g/d L in cases with small bowel ulcerative lesions and 10.5(SD = 1.3) g/dL in those without(P = 0.002). Cases with small bowel ulcers necessitate more units of packed red blood cells. During their hospitalization, 6(27%) cases with small bowel ulcers presented bleeding recurrence most possibly attributed to small bowel ulcers, nevertheless 30-d mortality was zero. Presence of chronic obstructive lung disease and diabetes was related with unexplained recurrence of hemorrhage in logistic regression analysis, while absence of small bowel ulcers was protective(relative risk 0.13, P = 0.05).CONCLUSION: Among NSAID consumers, more bleeders than non-bleeders with peptic ulcers present small bowel ulcers; lesions related to more severe bleeding and unexplained episodes of bleeding recurrence.Panagiotis Tsibouris Chissostomos Kalantzis Periklis Apostolopoulos Antonios Zalonis Peter Edward Thomas Isaacs Mark Hendrickse Georgios Alexandrakis 2014World Journal of Gastrointestinal Endoscopy2014,6,12:6

3Recommendations to quantify villous atrophy in video capsule endoscopy images of celiac disease patients显示文摘AIM To quantify the presence of villous atrophy in endoscopic images for improved automation.METHODS There are two main categories of quantitative descriptors helpful to detect villous atrophy:(1) Statistical and(2) Syntactic. Statistical descriptors measure the small intestinal substrate in endoscope-acquired images based on mathematical methods. Texture is the most commonly used statistical descriptor to quantify villous atrophy. Syntactic descriptors comprise a syntax, or set of rules, for analyzing and parsing the substrate into a set of objects with boundaries. The syntax is designed to identify and distinguish three-dimensional structures based on their shape.RESULTS The variance texture statistical descriptor is useful to describe the average variability in image gray level representing villous atrophy, but does not determine the range in variability and the spatial relationships between regions. Improved textural descriptors will incorporate these factors, so that areas with variability gradients and regions that are orientation dependent can be distinguished. The protrusion syntactic descriptor is useful to detect three-dimensional architectural components, but is limited to identifying objects of a certain shape. Improvement in this descriptor will require incorporating flexibility to the prototypical template, so that protrusions of any shape can be detected, measured, and distinguished.CONCLUSION Improved quantitative descriptors of villous atrophy are being developed, which will be useful in detecting subtle, varying patterns of villous atrophy in the small intestinal mucosa of suspected and known celiac disease patients.Edward J Ciaccio Govind Bhagat Suzanne K Lewis Peter H Green 2016World Journal of Gastrointestinal Endoscopy2016,8,18:3

7Implementation of a polling protocol for predicting celiac disease in videocapsule analysis显示文摘AIM: To investigate the presence of small intestinal villous atrophy in celiac disease patients from quantitative analysis of videocapsule image sequences.METHODS: Nine celiac patient data with biopsy-proven villous atrophy and seven control patient data lacking villous atrophy were used for analysis. Celiacs had biopsy-proven disease with scores of Marsh Ⅱ-Ⅲ C except in the case of one hemophiliac patient. At four small intestinal levels (duodenal bulb, distal duodenum, jejunum, and ileum), video clips of length 200 frames (100 s) were analyzed. Twenty-four measurements were used for image characterization. These measurements were determined by quantitatively processing the videocapsule images via techniques for texture analysis, motility estimation, volumetric reconstruction using shape-from-shading principles, and image transformation. Each automated measurement method, or automaton, was polled as to whether or not villous atrophy was present in the small intestine, indicating celiac disease. Each automaton's vote was determined based upon an optimized parameter threshold level, with the threshold levels being determined from prior data. A prediction of villous atrophy was made if it received the majority of votes (≥ 13), while no prediction was made for tie votes (12-12). Thus each set of images was classified as being from either a celiac disease patient or from a control patient. RESULTS: Separated by intestinal level, the overall sensitivity of automata polling for predicting villous atrophy and hence celiac disease was 83.9%, while the specificity was 92.9%, and the overall accuracy of automata-based polling was 88.1%. The method of image transformation yielded the highest sensitivity at 93.8%, while the method of texture analysis using subbands had the highest specificity at 76.0%. Similar results of prediction were observed at all four small intestinal locations, but there were more tie votes at location 4 (ileum). Incorrect prediction which reduced sensitivity occurred for two celiac patients with Marsh type Ⅱ pattern, which is characterized by crypt hyperplasia, but normal villous architecture. Pooled from all levels, there was a mean of 14.31 ± 3.28 automaton votes for celiac vs 9.67 ± 3.31 automaton votes for control when celiac patient data was analyzed (P<0.001). Pooled from all levels, there was a mean of 9.71 ± 2.8128 automaton votes for celiac vs 14.32 ± 2.7931 automaton votes for control when control patient data was analyzed (P<0.001). CONCLUSION: Automata-based polling may be useful to indicate presence of mucosal atrophy, indicative of celiac disease, across the entire small bowel, though this must be confirmed in a larger patient set. Since the method is quantitative and automated, it can potentially eliminate observer bias and enable the detectionof subtle abnormality in patients lacking a clear diagnosis. 8Neural Network-Based Second Order Reliability Method(NNBSORM)for Laminated Composite Plates in Free Vibration显示文摘Monte Carlo Simulations(MCS),commonly used for reliability analysis,require a large amount of data points to obtain acceptable accuracy,even if the Subset Simulation with Importance Sampling(SS/IS)methods are used.The Second Order Reliability Method(SORM)has proved to be an excellent rapid tool in the stochastic analysis of laminated composite structures,when compared to the slower MCS techniques.However,SORM requires differentiating the performance function with respect to each of the random variables involved in the simulation.The most suitable approach to do this is to use a symbolic solver,which renders the simulations very slow,although still faster than MCS.Moreover,the inability to obtain the derivative of the performance function with respect to some parameters,such as ply thickness,limits the capabilities of the classical SORM.In this work,a Neural Network-Based Second Order Reliability Method(NNBSORM)is developed to replace the finite element algorithm in the stochastic analysis of laminated composite plates in free vibration.Because of the ability to obtain expressions for the first and second derivatives of the NN system outputs with respect to any of its inputs,such as material properties,ply thicknesses and orientation angles,the need for using a symbolic solver to calculate the derivatives of the performance function no longer exists.The proposed approach is accordingly much faster,and easily allows for the consideration of ply thickness uncertainty.The present analysis showed that dealing with ply thicknesses as random variables results in 37%increase in the laminate’s probability of failure.Mena E.Tawfik Peter L.Bishay Edward E.Sadek 2018Computer Modeling in Engineering & Sciences2018,,4:3

16Hybrid coronary revascularization vs. percutaneous coronary interventions for multivessel coronary artery disease显示文摘OBJECTIVE Hybrid coronary revascularization(HCR)combines a minimally invasive surgical approach to the left anterior descending(LAD)artery with percutaneous coronary intervention(PCI)for non-LAD diseased coronary arteries.It is associated with shorter hospital lengths of stay and recovery times than conventional coronary artery bypass surgery,but there is little information comparing it to isolated PCI for multivessel disease.Our objective is to compare long-term outcomes of HCR and PCI for patients with multivessel disease.METHODS This cohort study used data from New York’s cardiac surgery and PCI registries in 2010−2016 to examine mortality and repeat revascularization rates for patients with multivessel coronary artery disease who underwent HCR and PCI.Cox proportional hazards methods were used to reduce selection bias.Patients were followed for a median of four years.RESULTS There was a total of 335 HCR patients(1.2%)and 25,557 PCI patients(98.8%)after exclusions.There was no difference in 6-year risk adjusted survival between HCR and PCI patients 83.17%vs.81.65%,adjusted hazard ratio(aHR)=0.90(95%CI:0.67−1.20).However,HCR patients were more likely to be free from repeat revascularization in the LAD artery(91.13%vs.83.59%,aHR=0.51(95%CI:0.34−0.77)).CONCLUSIONS For patients with multi-vessel coronary artery disease,HCR is rarely performed.There are no differences in mortality rates after four years,but HCR is associated with lower repeat revascularization rates in the LAD artery,presumably due to better longevity in left arterial mammary grafts.Edward L.Hannan Yi-Feng WU Kimberly Cozzens Jacqueline Tamis-Holland Frederick S.K.Ling Alice K.Jacobs Ferdinand J.Venditti Peter B.Berger Gary Walford Spencer B.KingⅢ 2021Journal of Geriatric Cardiology2021,18,3:2

19Data-Driven Discovery in Mineralogy: Recent Advances in Data Resources, Analysis, and Visualization显示文摘Large and growing data resources on the diversity, distribution, and properties of minerals are ushering in a new era of data-driven discovery in mineralogy. The most comprehensive international mineral database is the IMA database, which includes information on more than 5400 approved mineral species and their properties, and the mindat.org data source, which contains more than 1 million species/locality data on minerals found at more than 300 000 localities. Analysis and visualization of these data with diverse techniques—including chord diagrams, cluster diagrams, Klee diagrams, skyline diagrams, and varied methods of network analysis—are leading to a greater understanding of the co-evolving geosphere and biosphere. New data-driven approaches include mineral evolution, mineral ecology, and mineral network analysis—methods that collectively consider the distribution and diversity of minerals through space and time. These strategies are fostering a deeper understanding of mineral co-occurrences and, for the first time, facilitating predictions of mineral species that occur on Earth but have yet to be discovered and described.Robert M.
