1Expenditure and financial burden for the diagnosis and treatment of colorectal cancer in China:a hospital.based,multicenter,cross-sectional survey显示文摘Background: The increasing prevalence of colorectal cancer(CRC) in China and the paucity of information about relevant expenditure highlight the necessity of better understanding the financial burden and effect of CRC diagnosis and treatment. We performed a survey to quantify the direct medical and non-medical expenditure as well as the resulting financial burden of CRC patients in China.Methods: We conducted a multicenter, cross-sectional survey in 37 tertiary hospitals in 13 provinces across China between 2012 and 2014. Each enrolled patient was interviewed using a structured questionnaire. All expenditure data were inflated to the 2014 Chinese Yuan(CNY; 1 CNY = 0.163 USD). We quantified the overall expenditure and financial burden and by subgroup(hospital type, age at diagnosis, sex, education, occupation, insurance type, household income, clinical stage, pathologic type, and therapeutic regimen). We then performed generalized linear modeling to determine the factors associated with overall expenditure.Results: A total of 2356 patients with a mean age of 57.4 years were included, 57.1 % of whom were men; 13.9% of patients had stage I cancer; and the average previous-year household income was 54,525 CNY.The overall average direct expenditure per patient was estimated to be 67,408 CNY, and the expenditures for stage Ⅰ, Ⅱ, Ⅲ, and Ⅳ disease were 56,099 CNY, 59,952 CNY, 67,292 CNY, and 82,729 CNY, respectively. Non-medical expenditure accounted for 8.3%of the overall expenditure. The 1-year out-of-pocket expenditure of a newly diagnosed patient was 32,649 CNY, which accounted for 59.9% of their previous-year household income and caused 75.0% of families to suffer an unmanageable financial burden. Univariate analysis showed that financial burden and overall expenditure differed in almost all subgroups(P < 0.05), except for sex. Multivariate analysis showed that patients who were treated in specialized hospitals and those who were diagnosed with adenocarcinoma or diagnosed at a later stage were likely to spend more,whereas those with a lower household income and those who underwent surgery spent less(all P < 0.05).Conclusions: For patients in China, direct expenditure for the diagnosis and treatment of CRC seemed catastrophic,and non-medical expenditure was non-ignorable. 3Deep-learning-based information mining from ocean remote-sensing imagery显示文摘With the continuous development of space and sensor technologies during the last 40 years,ocean remote sensing has entered into the big-data era with typical five-V(volume,variety,value,velocity and veracity)characteristics.Ocean remote-sensing data archives reach several tens of petabytes and massive satellite data are acquired worldwide daily.To precisely,efficiently and intelligently mine the useful information submerged in such ocean remote-sensing data sets is a big challenge.Deep learning-a powerful technology recently emerging in the machine-learning field-has demonstrated its more significant superiority over traditional physical-or statistical-based algorithms for image-information extraction in many industrial-field applications and starts to draw interest in ocean remote-sensing applications.In this review paper,we first systematically reviewed two deep-learning frameworks that carry out ocean remote-sensing-image classifications and then presented eight typical applications in ocean internal-wave/eddy/oil-spill/coastal-inundation/sea-ice/green-algae/ship/coral-reef mapping from different types of ocean remote-sensing imagery to show how effective these deep-learning frameworks are.Researchers can also readily modify these existing frameworks for information mining of other kinds of remote-sensing imagery.Xiaofeng Li Bin Liu Gang Zheng Yibin Ren Shuangshang Zhang Yingjie Liu Le Gao Yuhai Liu Bin Zhang Fan Wang 2020National Science Review2020,7,10:19
4SbHKT1;4, a member of the high‐affinity potassium transporter gene family from Sorghum bicolor, functions to maintain optimal Na^+/K^+ balance under Na^+ stress显示文摘In halophytic plants, the high‐affinity potassium transporter HKT gene family can selectively uptake Kt in the presence of toxic concentrations of Nat. This has so far not been well examined in glycophytic crops. Here, we report the characterization of SbHKT1;4, a member of the HKT gene family from Sorghum bicolor. Upon Nat stress, SbHKT1;4 expression was more strongly upregulated in salt‐tolerant sorghum accession, correlating with a better balanced Nat/ Kt ratio and enhanced plant growth. Heterogeneous expression analyses in mutants of Saccharomyces cerevisiae and Arabidopsis thaliana indicated that overexpressing SbHKT1;4 resulted in hypersensitivity to Nat stress, and such hypersensitivity could be alleviated with the supply of elevated levels of Kt, implicating that SbHKT1;4 may mediate Kt uptake in the presence of excessive Nat. Further electrophysiological evidence demonstrated that SbHKT1;4 could transport Nat and Kt when expressed in Xenopus laevis oocytes. 6Efficient spectral regulation in Ce:Lu_(3)(Al,Cr)_(5)O_(12) and Ce:Lu_(3)(Al,Cr)_(5)O_(12)/Ce:Y_(3)Al_(5)O_(12) transparent ceramics with high color rendering index for high-power white LEDs/LDs显示文摘Realizing a high color rendering index(CRI)in Ce:LuAG transparent ceramics(TCs)with desired thermal stability is essential to their applications in white LEDs/LDs as color converters.In this study,based on the scheme of configuring the red component by Cr^(3+) doping,an efficient spectral regulation was realized in Ce,Cr:LuAG TCs.A unilateral shift phenomenon could be observed in both photoluminescence(PL)and photoluminescence excitation(PLE)spectra of TCs.By constructing TC-based white LED/LD devices in a remote excitation mode,luminescence properties of Ce,Cr:LuAG TCs were systematically investigated.The CRI values of Ce:LuAG TC based white LEDs could be increased by a magnitude of 46.2%.Particularly,by combining the as fabricated Ce,Cr:LuAG TCs with a 0.5 at% Ce:YAG TC,surprising CRI values of 88 and 85.5 were obtained in TC based white LEDs and LDs,respectively.Therefore,Ce,Cr:LuAG TC is a highly promising color convertor for high-power white LEDs/LDs applied in general lighting and displaying.Tianyuan ZHOU Chen HOU Le ZHANG Yuelong MA Jian KANG Tao LI Rui WANG Jin HUANG Junwei LI Haidong REN Zhenxiao FU Farida ASELIM Ming LI Hao CHEN 2021Journal of Advanced Ceramics2021,10,5:7

8Population-level economic burden of lung cancer in China:Provisional prevalence-based estimations,2017-2030显示文摘Objective:Population-level economic burden is essential for prioritizing healthcare resources and healthcare budget making in the future.However,little is known about the economic burden of lung cancer in China.Methods:A prevalence-based approach was adopted to estimate the economic burden of lung cancer,including direct expenditure(medical and non-medical)and indirect cost(disability and premature death).Data on direct expenditure and work-loss days per patient in each year post-diagnosis were obtained from two primary surveys.Other parameters were obtained from literatures and official reports.Projections were conducted based on varying parameters.All expenditure data were reported in United States dollars(USD)using 2017 value(exchange rate:1 USD=6.760 CNY),with the discount rate of 3%.Results:The total economic burden of lung cancer was estimated to be 25,069 million USD in China in 2017(0.121%of gross domestic productivity,GDP).The estimated direct expenditure was 11,098 million USD,up to1.43%of total healthcare expenditure for China,covering 10,303 million USD and 795 million USD for medical and non-medical expenditure,respectively.The estimated indirect cost was 13,971 million,including 1,517 million USD due to disability and 12,454 million USD due to premature death.Under current assumptions,the projected total economic burden would increase to 30.1 billion USD,40.4 billion USD,and 53.4 billion USD in 2020,2025,and 2030,accounting for 0.121%,0.131%,and 0.146%of China's GDP,respectively.However,if China meets the United Nation sustainable development goal of reducing premature death from non-communicable diseases by one-third by 2030,the total economic burden in 2030 would be 31.9 billion USD,0.087%of China's GDP.Conclusions:The economic burden of lung cancer in China in 2017 is substantial and more likely to increase significantly in the future.Policy makers need to take urgent actions in budget making for health systems.The economic burden could be alleviated by reducing the disease burden of lung cancer via effective control and prevention actions.Chengcheng Liu Jufang Shi Hong Wang Xinxin Yan Le Wang Jiansong Ren Mark Parascandola Wanqing Chen Alin Dai 2021Chinese Journal of Cancer Research2021,33,1:6
9Oxaliplatin and 5-fluorouracil hepatic infusion with lipiodolized chemoembolization in large hepatocellular carcinoma显示文摘AIM: To investigate transarterial chemoembolization(TACE) with hepatic infusion of oxaliplatin and 5-fluorouracil and Lipiodol chemoembolization in large hepatocellular carcinoma(HCC).METHODS: In this retrospective study, 132 patients with unresectable HCCs larger than 10 cm were treated with hepatic infusion of oxaliplatin and 5-fluorouracil followed by Lipiodol chemoembolization. The primary endpoint was overall survival(OS). Sixteen-week disease-control rate, time to progression(TTP), and major complications were also studied. Univariate and multivariate analyses were performed to identify prognostic factors affecting OS and TTP.RESULTS: A total of 319 procedures were performed in the 132 patients. Eleven(8.3%) patients received radical resection following TACE treatment(median time to initial TACE 4.3 ± 2.3 mo). The median OS and TTP were 10.3 and 3.0 mo respectively, with a 50.0% 16-wk disease-control rate. Major complications were encountered in 6.0%(8/132) of patients following TACE and included serious jaundice in 1.5%(2/132) patients, aleukia in 1.5%(2/132), and hepatic failure in 3.0%(4/132). One patient died within one month due to serious hepatic failure and severe sepsis after receiving the second TACE. 1018β-Glycyrrhetinic acid protects against alpha-naphthylisothiocyanate- induced cholestasis through activation of the Sirt1/FXR signaling pathway显示文摘Cholestasis is a common feature of liver injury, which manifests as bile acid excretion and/or enterohepatic circulation disorders. However, very few effective therapies exist for cholestasis. Recently, 18β-Glycyrrhetinic acid (18b-GA), a major metabolic component of glycyrrhizin, which is the main ingredient of licorice, was reported to protect against alpha-naphthylisothiocyanate (ANIT)-induced cholestasis. However, its protective mechanism remains unclear. We hypothesized that 18b-GA may stimulate the signaling pathway of bile acid (BA) transportation in hepatocytes, resulting its hepatoprotective effect. According to the results, 18b-GA markedly attenuated ANIT-induced liver injury as indicated the hepatic plasma chemistry index and histopathology examination. In addition, the expression levels of nuclear factors, including Sirt1, FXR and Nrf2, and their target efflux transporters in the liver, which mainly mediate bile acid homeostasis in hepatocytes, signi?cantly increased. Furthermore, we first revealed that 18b-GA treatment significantly activated FXR, and which can be significantly reduced by EX-527 (a potent and selective Sirt1 inhibitor), indicating that 18b-GA activates FXR through Sirt1. 11Reducing the yield asymmetry in Mg-5Li-3Al-2Zn alloy by hot-extrusion and multi-pass rolling显示文摘Reducing the yield asymmetry is very important concern for wrought Mg-Li alloys.In this study,Mg-5Li-3Al-2 Zn(LAZ532)alloy was successfully produced by hot-extrusion followed by multi-pass rolling at 573 K.Microstructure evolution,mechanical properties and yield asymmetry reducing of LAZ532 alloys at different rolling passes were studied.By observing microstructure using transmission electron microscopy showed that a small amount of ultra-fine Al Mg_(4)Zn_(11)and nano Li_(3)Al_(2) phases existed in the alloy.With the increasing of rolling passes,the grains of the alloys were obviously refined,and comprehensive mechanical properties were dramatically improved.Meanwhile,it also showed an excellent tension and compression yield symmetry(TYS/CYS was about 1).The results showed that the combined action of the weak{0001}basal lamellar texture,grain refinement and addition of Li element could effectively improve the yield symmetry.Furthermore,based on theoretical analysis,the yield strength in the alloys mainly depended on the strengthening contributions of LAGBs and HAGBs,and strengthening effect of HAGBs most(~50%)to the yield strength improvement.Xiaoqiang Li Liang Ren Qichi Le Lei Bao Peipeng Jin Ping Wang Chunlong Cheng Xiong Zhou Chenglu Hu 2021Journal of Magnesium and Alloys2021,9,3:4
12Investigation of Portevin–Le Chatelier effect in rolled α-phase Mg-Li alloy during tensile and compressive deformation显示文摘Avoiding the Portevin-Le Chatelier(PLC)effect is very important concern for wrought Mg-Li alloys.In this study,the special PLC effect was found in rolled Mg-5Li-3Al-2Zn(LAZ532)alloy during tensile and compressive deformation.By observing microstructure evolution of the alloy during tensile and compressive deformation,it was found that prismaticand pyramidalslips were activated during tensile deformation,resulting in plenty of dislocation accumulation.In the deformation process after compressive yielding,the deformations in coarse grains and fine grains were dominated by{1012}extension twinning and grain boundary slip,respectively.Based on experimental result analysis,the sudden appearance of PLC effect in the later stage of axial tensile deformation(along rolled direction)was caused by interaction between solute atoms and dislocations.In the process of axial compressive deformation(along rolled direction),PLC effect presented the complex and changeable phenomenon of appeared-disappeared-appeared,which was mainly caused by the continuous nucleation of twin in the material,the activation of grain boundary slip and the shear deformation of twin,respectively.Xiaoqiang Li Chunlong Cheng Qichi Le Lei Bao Peipeng Jin Ping Wang Liang Ren Hang Wang Xiong Zhou Chenglu Hu 2020Journal of Materials Science & Technology2020,52,17:3
13霉酚酸酯联合糖皮质激素诱导治疗过敏性紫癜性肾炎的回顾性研究(英文)显示文摘目的:合并中等量蛋白尿的过敏性紫癜性肾炎的诱导治疗尚无确切方案,本研究通过回顾性比较分析了霉酚酸酯联合糖皮质激素的疗效与安全性。创新点:首次对霉酚酸酯联合糖皮质激素诱导治疗过敏性紫癜性肾炎进行了回顾性研究。方法:回顾性分析2007年1月至2013年6月间在浙江大学附属第一医院肾脏病中心接受‘肾穿刺活检,且经过3个月以上血管紧张素转换酶抑制剂(ACEI)/血管紧张素受体拮抗剂(ARB)治疗后蛋白尿为1.0-3.5g/24h的过敏性紫癜性肾炎患者95例。根据治疗方案分为3组,霉酚酸酯组(33例)在原剂量ACEFARB的基础上加用霉酚酸酯联合低剂量糖皮质激素,糖皮质激素组(31例)在原剂量ACEFARB的基础上加用全剂量糖皮质激素,对照组(31例)维持单用ACEFARB治疗,但可提高其剂量。患者随访观察6~78月(中位观察时间28月),霉酚酸酯组、糖皮质激素组与对照组的蛋白尿缓解率分别为72.7%、64.5%与45.2%(图1),发生副作用分别为17例、30例与6例,糖皮质激素组高脂血症与高血压发生率较高(表3)。结论:霉酚酸酯联合低剂量糖皮质激素可有效诱导缓解过敏性紫癜性肾炎,其缓解率与全剂量糖皮质激素治疗相当,且副作用较少。Fei HAN Liang-liang CHEN Ping-ping REN Jing-yun LE Pei-jing CHOONG Hong-ju WANG Ying XU Jiang-hua CHEN 2015Journal of Zhejiang University-Science B(Biomedicine & Biotechnology)2015,16,9:3
