| 1 | Hepatectomy vs radiofrequency ablation for colorectal liver metastasis:A propensity score analysis显示文摘AIM:To compare outcomes from radiofrequency ablation(RFA) and hepatectomy for treatment of colorectal liver metastasis(CRLM).METHODS:From January 2000 to December 2009,408 patients underwent curative intent treatment for CRLM.We excluded patients using the criteria:size of CRLM > 3 cm,number of CRLM ≥ 5,percutaneous RFA,follow-up period < 12 mo,double primary cancer,or treatment with both RFA and hepatectomy.We matched 51 patients who underwent RFA with 102 patients who underwent hepatectomy by propensity scores.RESULTS:The median follow-up period was 45 mo(range,12 mo to 158 mo).Hepatic recurrence was more frequent in the RFA than the hepatectomy group(P = 0.021) although extrahepatic recurrence curves were similar(P = 0.716).Survival curves of hepatectomy group were better than that of RFA for multiple,large(> 2 cm) CRLM(P = 0.034).However,survival curves were similar for single or small(≤ 2 cm) CRLM(P = 0.714,P = 0.740).CONCLUSION:Hepatectomy is better than RFA for the treatment of CRLM.However,RFA might be suitable for selected patients with single,small(≤ 2 cm) CRLM. | Huisong Lee Jin Seok Heo Yong Beom Cho Seong Hyeon Yun Hee Cheol Kim Woo Yong Lee Seong Ho Choi Dong Wook Choi | 2015 | World Journal of Gastroenterology2015，21，11： | 14 |
| 2 | Endoscopic ultrasonography does not differentiate neoplastic from non-neoplastic small gallbladder polyps显示文摘AIM:To assess the ability of endoscopic ultrasonography(EUS) to differentiate neoplastic from non-neoplastic polypoid lesions of the gallbladder(PLGs) .METHODS:The uses of EUS and transabdominal ultrasonography(US) were retrospectively analyzed in 94 surgical cases of gallbladder polyps less than 20 mm in diameter.RESULTS:The prevalence of neoplastic lesions with a diameter of 5-10 mm was 17.2%(10/58) ;11-15 mm,15.4%(4/26) ,and 16-20 mm,50%(5/10) .The overall diagnostic accuracies of EUS and US for small PLGs were 80.9% and 63.9%(P < 0.05) ,respectively.EUS correctly distinguished 12(63.2%) of 19 neoplastic PLGs but was less accurate for polyps less than 1.0 cm(4/10,40%) than for polyps greater than 1.0 cm(8/9,88.9%) (P = 0.02) .CONCLUSION:Although EUS was more accurate than US,its accuracy for differentiating neoplastic from non-neoplastic PLGs less than 1.0 cm was low.Thus,EUS alone is not suffi cient for determining a treatment strategy for PLGs of less than 1.0 cm. | Young Koog Cheon Won Young Cho Tae Hee Lee Young Deok Cho Jong Ho Moon Joon Seong Lee Chan Sup Shim | 2009 | World Journal of Gastroenterology2009，15，19： | 12 |
| 3 | Plasma levels of acylated ghrelin in patients with functional dyspepsia显示文摘AIM:To investigate the relationship between plasma acylated ghrelin levels and the pathophysiology of functional dyspepsia.METHODS:Twenty-two female patients with functional dyspepsia and twelve healthy volunteers were recruited for the study.The functional dyspepsia patients were each diagnosed based on the Rome Ⅲ criteria.Eligible patients completed a questionnaire concerning the severity of 10 symptoms.Plasma acylated ghrelin levels before and after a meal were determined in the study participants using a commercial human acylated enzyme immunoassay kit;electrogastrograms were performed for 50 min before and after a standardized 10-min meal containing 265 kcal.RESULTS:There were no significant differences in plasma acylated ghrelin levels between healthy volunteers and patients with functional dyspepsia.However,in patients with functional dyspepsia,there was a negative correlation between fasting plasma acylated ghrelin levels and the sum score of epigastric pain(r =-0.427,P = 0.047) and a positive correlation between the postprandial/fasting plasma acylated ghrelin ratio and the sum score of early satiety(r = 0.428,P =0.047).Additionally,there was a negative correlation between fasting acylated ghrelin plasma levels and fasting normogastria(%)(r =-0.522,P = 0.013).Interestingly,two functional dyspepsia patients showed paradoxically elevated plasma acylated ghrelin levels after the meal.CONCLUSION:Abnormal plasma acylated ghrelin levels before or after a meal may be related to several of the dyspeptic symptoms seen in patients with functional dyspepsia. | Yeon Soo Kim Joon Seong Lee Tae Hee Lee Joo Young Cho Jin Oh Kim Wan Jung Kim Hyun Gun Kim Seong Ran Jeon Hoe Su Jeong | 2012 | World Journal of Gastroenterology2012，18，18： | 12 |
| 4 | Super-resolution localization photoacoustic microscopy using intrinsic red blood cells as contrast absorbers显示文摘Photoacoustic microscopy(PAM)has become a premier microscopy tool that can provide the anatomical,functional,and molecular information of animals and humans in vivo.However,conventional PAM systems suffer from limited temporal and/or spatial resolution.Here,we present a fast PAM system and an agent-free localization method based on a stable and commercial galvanometer scanner with a custom-made scanning mirror(L-PAM-GS).This novel hardware implementation enhances the temporal resolution significantly while maintaining a high signal-to-noise ratio(SNR).These improvements allow us to photoacoustically and noninvasively observe the microvasculatures of small animals and humans in vivo.Furthermore,the functional hemodynamics,namely,the blood flow rate in the microvasculature,is successfully monitored and quantified in vivo.More importantly,thanks to the high SNR and fast B-mode rate(500 Hz),by localizing photoacoustic signals from captured red blood cells without any contrast agent,unresolved microvessels are clearly distinguished,and the spatial resolution is improved by a factor of 2.5 in vivo.LPAM-GS has great potential in various fields,such as neurology,oncology,and pathology. | Jongbeom Kim Jin Young Kim Seungwan Jeon Jin Woo BAIK Seong Hee Cho Chulhong Kim | 2019 | Light(Science & Applications)2019，8，1： | 6 |
| 5 | Predicting tumor response after preoperative chemoradiation using clinical parameters in rectal cancer显示文摘AIM: To evaluate the clinical parameters and identify a better method of predicting pathological complete response (pCR). METHODS: We enrolled 249 patients from a database of 544 consecutive rectal cancer patients who underwent surgical resection after preoperative chemoradiation therapy (PCRT). A retrospective review of morphological characteristics was then performed to collect data regarding rectal examination findings. A scoring model to predict pCR was then created. To validate the ability of the scoring model to predict complete regression.RESULTS: Seventy patients (12.9%) achieved a pCR. A multivariate analysis found that pre-CRT movability (P = 0.024), post-CRT size (P = 0.018), post-CRT morphology (P = 0.023), and gross change (P = 0.009) were independent predictors of pCR. The accuracy of the scoring model was 76.8% for predicting pCR with the threshold set at 4.5. In the validation set, the accuracy was 86.7%. CONCLUSION: Gross changes and morphological findings are important predictors of pathological response. | 6 | Does immunohistochemical staining have a clinical impact in early gastric cancer conducted endoscopic submucosal dissection?显示文摘AIM: To evaluate clinicopathologic parameters and the clinical significance related lymphovascular invasion (LVI) by immunohistochemical staining (IHCS) in endoscopic submucosal dissection (ESD). METHODS: Between May 2005 and May 2010, a total of 348 lesions from 321 patients (mean age 63 ± 10 years, men 74.6%) with early gastric cancer (EGC) who met indication criteria after ESD were analyzed retrospectively. The 348 lesions were divided into the absolute (n = 100, differentiated mucosal cancer without ulcer ≤ 20 mm) and expanded (n = 248) indica-tion groups after ESD. The 248 lesions were divided into four subgroups according to the expanded ESD indication. The presence of LVI was determined by factor Ⅷ-related antigen and D2-40 assessment. We compared LVI IHCS-negative group with LVI IHCSpositive in each group. RESULTS: LVI by hematoxylin-eosin staining (HES) and IHCS were all negative in the absolute group, while was observed in only the expanded groups. The positive rate of LVI by IHCS was higher than that of LVI by HES (n = 1, 0.4% vs n = 11, 4.4%, P = 0.044). LVI IHCS-positivity was observed when the cancer invaded to the mucosa 3 (M3) or submucosa 1 (SM1) levels, with a predominance of 63.6% in the subgroup that included only SM1 cancer (P < 0.01). In a univariate analysis, M3 or SM1 invasion by the tumor was significantly associated with a higher rate of LVI by IHCS, but no factor was significant in a multivariate analysis. There were no cases of tumor recurrence or metastasis during the median 26 mo follow-up. CONCLUSION: EGCs of the absolute group are immunohistochemically stable. | 7 | Repeat hepatic resection in patients with colorectal liver metastases显示文摘AIM:To investigate the survival outcomes of secondaryhepatectomy for recurrent colorectal liver metastases(CRLM).METHODS:From October 1994 to December 2009,patients with CRLM who underwent surgical treatment with curative intent were investigated.Patients were divided into two groups:patients who underwent primary hepatectomy(Group 1)and those who underwent secondary hepatectomy for recurrent CRLM(Group 2).RESULTS:Survival and prognostic factors were analyzed.A total of 461 patients were included:406patients in Group 1 and 55 patients in Group 2.After a median 39-mo(range,3-195 mo)follow-up,there was a significant difference between Groups 1 and 2in terms of disease-free survival(P=0.029)although there was no significant difference in overall survival(P=0.206).Secondary hepatectomy was less effective in patients with multiple recurrent CRLM than primary hepatectomy for initial CRLM(P=0.008).Multiple CRLM and radiofrequency ablation therapy were poor prognostic factors of secondary hepatectomy in multivariate Cox regression analysis(P=0.006,P=0.004,respectively).CONCLUSION:Secondary hepatectomy for single recurrent CRLM is as effective as primary surgical treatment for single recurrent CRLM.However,secondary hepatectomy for multiple recurrent CRLM is less effective than that for single recurrent CRLM. | Huisong Lee Seong Ho Choi Yong Beom Cho Seong Hyeon Yun Hee Cheol Kim Woo Yong Lee Jin Seok Heo Dong Wook Choi Kyung Uk Jung Ho-Kyung Chun | 2015 | World Journal of Gastroenterology2015，21，7： | 4 |
| 8 | A case of intussuscepted Meckel’s diverticulum显示文摘We report colonoscopic features of an intussuscepted Meckel's diverticulum, presenting with hematochezia. A 35-year-old woman presented to the emergency room with acute onset, transient, sharp, severe epi-gastric pain that began 6 h earlier. Colonoscopy re-vealed a reddish, soft, fi st-sized polypoid lesion in the terminal ileum. The lesion was misinterpreted as a hematoma by an inexperienced endoscopist. The pa-tient began to complain of intermittent, severe peri-umbilical pain following the colonoscopic examination. Subsequent computed tomography showed an enteric intussusception. An exploratory laparotomy revealed an intussuscepted Meckel's diverticulum, with trans-mural infarction. Colonoscopy was of little use in as-sessing the intussusception. However, colonoscopic examination may be performed initially, especially in an intussuscepted Meckel's diverticulum present-ing with hematochezia.
