2Imatinib as preoperative therapy in Chinese patients with recurrent or metastatic GISTs显示文摘Objective: Imatinib has dramatically altered the options for management of patients with gastrointestinal stromal tumours. However, it has become clear that secondary resistance to the drug develops during long-term therapy. The purpose of our study was to retrospectively analyze safety and long-term outcomes in Chinese patients with recurrent or metastatic GISTs treated with imatinib preoperatively. Methods: Between June 2003 and June 2011, 22 patients underwent surgery for recurrent or metastatic GISTs after preoperative treatment with imatinib. Results: Complete resection was accomplished in 8 of the 10 responsive disease (RD) patients (80%), and in 3 of the 12 patients (25%) who had progression disease (PD). The amount of blood loss during the operation in PD patients was higher than in RD patients. There was 1 hospital death in PD group related to surgery, while the other patients recovered with conservative therapy because complications were mild. The difference in median PFS between patients with RD and those with PD was significant (24.8 vs. 2.81 months, P<0.001). The difference in 2-year OS rate between patients with RD and those with PD was not significant (100% vs. 87.5%, P>0.05). Conclusions: Our study indicates that surgical intervention can improve the PFS of Chinese patients with recurrent or metastatic GISTs responsive to imatinib, but does not prolong OS as well as in patients who develop imatinib resistance. 3An analytical biomarker for treatment of patients with recurrent B-ALL after remission induced by infusion of anti-CD19 chimeric antigen receptor T(CAR-T) cells显示文摘Anti-CD19 chimeric antigen receptor-modified T(CAR-T-19) cells have emerged as a powerful targeted immunotherapy for B-cell lineage acute lymphoblastic leukemia with a remarkable clinical response in recent trials. Nonetheless, few data are available on the subsequent clinical monitoring and treatment of the patients, especially those with disease recurrence after CAR-T-19 cell infusion. Here, we analyzed three patients who survived after our phase I clinical trial and who were studied by means of biomarkers reflecting persistence of CAR-T-19 cells in vivo and predictive factors directing further treatment. One patient achieved 9-week sustained complete remission and subsequently received an allogeneic hematopoietic stem cell transplant. Another patient who showed relapse after 20 weeks without detectable leukemia in the cerebrospinal fluid after CAR-T-19 cell treatment was able to achieve a morphological remission under the influence of stand-alone low-dose chemotherapeutic agents. The third patient gradually developed extensive extramedullary involvement in tissues with scarce immune-cell infiltration during a long period of hematopoietic remission after CAR-T-19 cell therapy. Long-term and discontinuous increases in serum cytokines(mainly interleukin 6 and C-reactive protein) were identified in two patients(Nos. 1 and 6) even though only a low copy number of CAR molecules could be detected in their peripheral blood. This finding was suggestive of persistent functional activity of CAR-T-19 cells. 14Identification of a mitochondria-targeting fluorescent small molecule for dual phototherapy显示文摘Phototherapy,mainly induding photodynamic therapy(PDT)and photothermal therapy(PTT),is a noninvasive and effective approach for cancer treatment.Since integration of PDT and PTT for simultaneous synergistic PDT/PTT treatment enables us to improve phototherapeutic eficacy significantly,it has been attracting a lot of investigations in current days.Here,we introduce IR-52,a new mitochondria-targeting near infrared(NIR)fAuorescent small molecule,which possesses structure inherent PTT and PDT synergistic photother apeutic effects wit hout conjugation to specific ligands.After NIR light irradiation(808 nm,2 W/cm^(2),5min),both the hyperthermia and excessive singlet oaxygen levels were determined when dis-solving IR-52 in aqueous sohutions.In vitro photoinduced cytotoxicity studies showed signifcant lower cell viabilities and higher necrotic/apoptotic rates when cancer cells were treated with IR-52 and irradiation,and its'mitochondrial localization in cancer cells would partially explain its high cytotoxicity.Further in vivo synergetic PDT and PTT effects were demonstrated by high tumor surface temperature and sigmificant inhibition of tumor growth.Our results strongly suggest that IR-52,which possesses excellent photosensitivity,may provide a promising strategy for tumor treatment with decreased side effects.Jie Chen Xu Tan Shenglin Luo Lei Long Lang Liu Zujuan Liu Yu Wang Chunmeng Shi 2018Journal of Innovative Optical Health Sciences2018,,4:0
