| 3 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 6 | Terrestrial carbon sinks in China and around the world and their contribution to carbon neutrality显示文摘Enhancing the terrestrial ecosystem carbon sink(referred to as terrestrial C sink) is an important way to slow down the continuous increase in atmospheric carbon dioxide(CO_(2)) concentration and to achieve carbon neutrality target.To better understand the characteristics of terrestrial C sinks and their contribution to carbon neutrality,this review summarizes major progress in terrestrial C budget researches during the past decades,clarifies spatial patterns and drivers of terrestrial C sources and sinks in China and around the world,and examines the role of terrestrial C sinks in achieving carbon neutrality target.According to recent studies,the global terrestrial C sink has been increasing from a source of (-0.2±0.9) Pg C yr^(-1)(1 Pg=1015g)in the 1960s to a sink of (1.9±1.1) Pg C yr^(-1) in the 2010s.By synthesizing the published data,we estimate terrestrial C sink of 0.20–0.25 Pg C yr^(-1) in China during the past decades,and predict it to be 0.15–0.52 Pg C yr^(-1) by 2060.The terrestrial C sinks are mainly located in the mid-and high latitudes of the Northern Hemisphere,while tropical regions act as a weak C sink or source.The C balance differs much among ecosystem types:forest is the major C sink;shrubland,wetland and farmland soil act as C sinks;and whether the grassland functions as C sink or source remains unclear.Desert might be a C sink,but the magnitude and the associated mechanisms are still controversial.Elevated atmospheric CO_(2) concentration,nitrogen deposition,climate change,and land cover change are the main drivers of terrestrial C sinks,while other factors such as fires and aerosols would also affect ecosystem C balance.The driving factors of terrestrial C sink differ among regions.Elevated CO_(2) concentration and climate change are major drivers of the C sinks in North America and Europe,while afforestation and ecological restoration are additionally important forcing factors of terrestrial C sinks in China.For future studies,we recommend the necessity for intensive and long-term ecosystem C monitoring over broad geographic scale to improve terrestrial biosphere models for accurately evaluating terrestrial C budget and its dynamics under various climate change and policy scenarios. | Yuanhe Yang Yue Shi Wenjuan Sun Jinfeng Chang Jianxiao Zhu Leiyi Chen Xin Wang Yanpei Guo Hongtu Zhang Lingfei Yu Shuqing Zhao Kang Xu Jiangling Zhu Haihua Shen Yuanyuan Wang Yunfeng Peng Xia Zhao Xiangping Wang Huifeng Hu Shiping Chen Mei Huang Xuefa Wen Shaopeng Wang Biao Zhu Shuli Niu Zhiyao Tang Lingli Liu Jingyun Fang | 2022 | Science China(Life Sciences)2022，65，5： | 32 |
| 7 | Add-on pegylated interferon augments hepatitis B surface antigen clearance vs continuous nucleos(t)ide analog monotherapy in Chinese patients with chronic hepatitis B and hepatitis B surface antigen≤1500 IU/mL:An observational study显示文摘BACKGROUND Nucleos(t)ide analog(NA)has shown limited effectiveness against hepatitis B surface antigen(HBsAg)clearance in chronic hepatitis B(CHB)patients.AIM To evaluate the efficacy and safety of add-on peginterferonα-2a(peg-IFNα-2a)to an ongoing NA regimen in CHB patients.METHODS In this observational study,195 CHB patients with HBsAg≤1500 IU/m L,hepatitis B e antigen(HBeAg)-negative(including HBeAg-negative patients or HBeAg-positive patients who achieved HBeAg-negative after antiviral treatment with NA)and hepatitis B virus-deoxyribonucleic acid<1.0×10^2 IU/mL after over 1 year of NA therapy were enrolled between November 2015 and December2018 at the Second Affiliated Hospital of Xi'an Jiaotong University,China.Patients were given the choice between receiving either peg-IFNα-2a add-on therapy to an ongoing NA regimen(add-on group,n=91)or continuous NA monotherapy(monotherapy group,n=104)after being informed of the benefits and risks of the peg-IFNα-2a therapy.Total therapy duration of peg-IFNα-2a was 48 wk.All patients were followed-up to week 72(24 wk after discontinuation of peg-IFNα-2a).The primary endpoint was the proportion of patients with HBsAg clearance at week 72.RESULTS Demographic and baseline characteristics were comparable between the two groups.Intention-to-treatment analysis showed that the HBsAg clearance rate in the add-on group and monotherapy group was 37.4%(34/91)and 1.9%(2/104)at week 72,respectively.The HBsAg seroconversion rate in the add-on group was 29.7%(27/91)at week 72,and no patient in the monotherapy group achieved HBsAg seroconversion at week 72.The HBsAg clearance and seroconversion rates in the add-on group were significantly higher than in the monotherapy group at week 72(P<0.001).Younger patients,lower baseline HBsAg concentration,lower HBsAg concentrations at weeks 12 and 24,greater HBsAg decline from baseline to weeks 12 and 24 and the alanine aminotransferase≥2×upper limit of normal during the first 12 wk of therapy were strong predictors of HBsAg clearance in patients with peg-IFNα-2a add-on treatment.Regarding the safety of the treatment,4.4%(4/91)of patients in the add-on group discontinued peg-IFNα-2a due to adverse events.No severe adverse events were noted.CONCLUSION Peg-IFNα-2a as an add-on therapy augments HBsAg clearance in HBeAg-negative CHB patients with HBsAg≤1500 IU/m L after over 1 year of NA therapy. | Feng-Ping Wu Ying Yang Mei Li Yi-Xin Liu Ya-Ping Li Wen-Jun Wang Juan-Juan Shi Xin Zhang Xiao-Li Jia Shuang-Suo Dang | 2020 | World Journal of Gastroenterology2020，26，13： | 27 |
| 8 | Multicenter case-control study of the risk factors for ulcerative colitis in China显示文摘AIM:To evaluate potential risk factors in the development of ulcerative colitis(UC) in China.METHODS:A total of 1308 patients with UC and 1308 age-matched and sex-matched controls were prospectively studied in China.The UC cases were collected from 17 hospitals in China from April 2007 to April 2010.Uniform questionnaires were designed to investigate risk factors including smoking,appendectomy,stress,socio-economic conditions,nonsteroidal antiinflammatory drugs(NSAIDs),oral contraceptives,diet,breastfeeding,infections and family sanitary conditions.Group comparisons by each factor were done using simple logistic regression analysis.Conditional logistic regression was used for multivariate analysis.RESULTS:By univariate analysis,the variables predictive of UC included feeling stress,light and heavy alcoholic drinking,spicy food,sugar consumption and infectious diarrhea,while heavy tea intake and tap water consumption were protective against UC.On multivariate analysis,the protective factor for UC was tap water consumption [odds ratios(OR) = 0.424,95%CI:0.302-0.594,P < 0.001];while the potential risk factors for UC were heavy sugar consumption(OR = 1.632,95%CI:1.156-2.305,P < 0.001),spicy food(light intake:OR = 3.329,95%CI:2.282-4.857,P < 0.001;heavy intake:OR = 3.979,95%CI:2.700-5.863,P < 0.001),and often feeling stress(OR = 1.981,95%CI:1.447-2.711,P < 0.001).Other factors,such as smoking habit,appendectomy,breastfeeding,a history of measles,rural or urban residence,education,oral contraceptives,and NSAID use have not been found to have a significant association with the development of UC in the present study.CONCLUSION:Our study showed tap water consumption was a protective factor for UC,while spicy food,heavy sugar consumption and often feeling stress were risk factors for UC in this Chinese population. | Yu-Fang Wang Qin Ou-yang Bing Xia Li-Na Liu Fang Gu Kai-Fang Zhou Qiao Mei Rui-Hua Shi Zhi-Hua Ran Xiao-Di Wang Pin-Jin Hu Kai-Chun Wu Xin-Guang Liu Ying-Lei Miao Ying Han Xiao-Ping Wu Guo-Bing He Jie Zhong Guan-Jian Liu | 2013 | World Journal of Gastroenterology2013，19，11： | 24 |

| 11 | Indoor Allergen Levels and Household Distributions in Nine Cities Across China显示文摘Objective Chinese allergic subjects have high levels of sensitization to house dust mite(HDM) and other indoor allergens. This study quantifies common indoor allergen levels in Chinese households. Methods Dust samples were collected from nine cities. Major allergens Der p 1 and Der f 1 from Dermatophagoides pteronyssinus and D. farinae, and specific antigens of Blomia tropicalis, Tyrophagus putrescentiae, Acarus siro, and cockroach species Blattella germanica and Periplaneta americana were measured by ELISA. Results HDM allergens were found in dust samples from bedding in 95% of the Chinese households. The median levels varied from <0.006 to 9.2 μg/g of dust, depending on the city. The percentages of households having HDM allergen levels associated with the risk of developing allergy sensitization and asthma were 65% and 25%, respectively. Specific antigens of the storage mite and cockroach were only found in samples from the southern and tropical regions of China. Levels of mite allergens were generally higher in samples from bedding compared to samples from the living room, even for storage mites, whereas levels of cockroach antigens were higher in the living room samples. Conclusion HDM allergens are present in bedding dust samples from most Chinese households. Cities in southern and central China have relatively high levels of HDM major allergens compared to cities in northern and western China. | 15 | Correlation between gene expression of CCK-A receptor and emptying dysfunction of the gallbladder in patients with gallstones and diabetes mellitus显示文摘BACKGROUND: The incidence of gallstone is higher in patients with diabetes mellitus than in general population because of hypomotility and lowered emptying function. The aim of this study was to investigate the gene expression of cholecystokinin-A ( CCK-A) receptor in patients with gallstones and diabetes mellitus and its correlation with the hypomotility of the gallbladder. METHODS: Smooth muscle was taken from the gallbladder after cholecystectomy and total RNA was extracted from it by Trizol reagent to compose cDNA. CCK-A receptor was enlarged by reverse transcription-polymerase chain reaction (RT-PCR) and identified by electrophoresis. Based on the analysis of images, the relative expression of the CCK-A re- ceptor was obtained after comparison of the CCK-A recep- tor and β-action. The emptying function of the gallbladder was measured with B ultrasound before operation. RESULTS: Gallbladder ejetion fraction ( GBEF2 ) (%) (24.9±12.7) decreased more significantly in patients with gallstones and diabetes mellitus than in those with simple gallstone (61.5±8.5) (P <0.01). In the two groups, a frac- tion of 540bp of CCK-A receptor could be testified respec- tively, but the relative expression was different. Compared to the expression of β-action, it was 0. 400 ±0.068 in pa- tients with gallstones and diabetes mellitus, but 0. 622 ± 0.070 in those with gallstone only (F =7.169, P <0.01). The result was consistent with the decreased contractility. CONCLUSIONS: The gene expression of CCK-A receptor on the smooth muscle of the gallbladder may decrease more significantly in patients with gallstone and diabetes mellitus than in those with gallstone only; this result is con- sistent with the decreased motih’ty. Gastrointestinal transition time wasrecorded simultaneously. Serum concentration of nitrcoxide (NO) was determined by the kinetic cadmiunreduction and colorimetric methods. ImmunohistochemicalSABC method was used to observe the expression anddistribution of three types of nitric oxide synthase (NOS)isoforms in the mt gastrointestinal tract. Western blot wasused to detect expression of gastrointestinal NOS isoforms.RESULTS: Half gastric emptying time and trans-gastrointestinal time were significantly prolonged( 124.0 ± 26.4min; 33.7± 8.9min;72.1 ± 15.3 min; P<0.01), (12.4±0.5h; 9.5±0.3 h; 8.2±0.8 h; P<0.01), 2h residual rate wasraised in cirrhotic rots than in controls and cirrhotic ratstreated with L-NAME(54.9± 7.6 % ,13.7 ± 3.2 %, 34.9± 10.3%, P< 0.01). Serum concentration of NO was significantlyincreased in cirrhotic rots than in the other groups (8.20 ± 2.48)μmol@L-1, (5.94± 1.07) μmol@L-1 ,and control (5.66± 1.60) tμmol@L-1, P< 0.01. NOS staining intensities which weremainly located in the gastrointestinal tissues were markedlylower in cirrhotic rats than in the controls and cirrhotic ratsafter treated with L- NAME.CONCLUSION: Gastrointestinal motility was remarkablyinhibited in cirrhotic rats, which could he alleviated by L-NAME. | 18 | Human placenta-derived mesenchymal stem cells loaded on linear ordered collagen scaffold improves functional recovery after completely transected spinal cord injury in canine显示文摘Traumatic spinal cord injury(SCI) is a major challenge in the clinic. In this study, we sought to examine the synergistic effects of linear ordered collagen scaffold(LOCS) and human placenta-derived mesenchymal stem cells(hPMSCs) when transplanted into completely transected beagle dogs. After 36 weeks observation, we found that LOCS+hPMSCs implants promoted better hindlimb locomotor recovery than was observed in the non-treatment(control) group and LOCS group. Histological analysis showed that the regenerated tissue after treatment was well integrated with the host tissue, and dramatically reduced the volume of cystic and chondroitin sulfate proteoglycans(CSPGs) expression. Furthermore, the LOCS+hPMSCs group also showed more neuron-specific βIII-tubulin(Tuj-1)-and NeuN-positive neurons in the lesion area, as well as axonal regeneration, remyelination and synapse formation in the lesion site. Additionally, dogs in the LOCS+hPMSCs group experienced enhanced sprouting of both ascending(CGRP-positive) sensory fibers and descending(5-HT-and TH-positive) motor fibers at the lesion area. All these data together suggested that the combined treatment had beneficial effects on neuronal regeneration and functional improvement in a canine complete transection model. | 19 | Ultrasonic diagnosis of biliary atresia: A retrospective analysis of 20 patients显示文摘AIM: To investigate the clinical value of ultrasono-graphic diagnosis of biliary atresia (BA), a retrospective analysis of the sonogram of 20 children with BA was undertaken. METHODS: Ultrasonography (US) was performed in 20 neonates and infants with BA, which was confirmed with cholangiography by operation or abdominoscopy. The presence of triangular cord, the size and echo of liver, the changes in empty stomach gallbladder and postprandial gallbladder were observed and recorded. RESULTS: The triangular cord could be observed at the porta hepatis (thickness: 0.3-0.6 cm) in 10 cases. Smaller triangular cord (0.2-0.26 cm) can be observed in 3 cases. The gallbladder was not observed in 2 cases, and 1 case showed a streak gallbladder without capsular space. The gallbladders of 15 cases were flat and small. The gallbladders of 2 cases were of normal size and appearance, however, there was no postprandial contraction. The livers of all cases showed hepatomegaly and hetreogeneous echogenicity. Statistical analysis was performed to compare the hepatomegaly and hetreogeneous echogenicity and the stage of hepatic fibrosis. CONCLUSION: The presence of the triangular cord atthe porta hepatis is specific. However, it is not the only diagnostic criterion, since flat and small gallbladder and poor contraction are also of important diagnostic and differential diagnostic significance. | 20 | Genetic pattern of belt-wide petroliferous phenomenon in the eastern Pearl River Mouth Basin and its practical application显示文摘The Pearl River Mouth Basin(PRMB) covers an area of approximately 20×104 km2. However, oil-gas fields detected in this area thus far are highly concentrated and controlled predominantly by second-order structural belts, the seven largest of which aggregate proved oil reserves of 7.7×108 m3, accounting for 86% of the total discovered reserve in the basin. These second-order structures have one common phenomenon: oil is contained in all traps present in them. In other words, they are all belt-wide petroliferous reservoirs. Research has identified eight types of second-order structural belts under two categories in the eastern PRMB. Their petroliferous properties are subject to three typical constraints: petroliferous properties of subsags hosting these structural belts, locations of these belts in the petroleum system, and availability of traps prior to the hydrocarbon expulsion and migration. The formation and distribution of oil reservoirs in these belts are characterized by subsag–belt integration and 'three-in-one'. The former indicates that sags and the second-order structural belts within the supply range of the sags constitute the basic units of hydrocarbon accumulations and are therefore inseparable.
