| 3 | Methods for the extraction and RNA profiling of exosomes显示文摘AIM:To develop protocols for isolation of exosomes and characterization of their RNA content.METHODS:Exosomes were extracted from He La cell culture media and human blood serum using the Total exosome isolation(from cell culture media)reagent,and Total exosome isolation(from serum)reagent respectively.Identity and purity of the exosomes was confirmed by Nanosight?analysis,electron microscopy,and Western blots for CD63 marker.Exosomal RNA cargo was recovered with the Total exosome RNA and protein isolation kit.Finally,RNA was profiled using Bioanalyzer and quantitative reverse transcriptionpolymerase chain reaction(q RT-PCR)methodology.RESULTS:Here we describe a novel approach for robust and scalable isolation of exosomes from cell culture media and serum,with subsequent isolation and analysis of RNA residing within these vesicles.The isolation procedure is completed in a fraction of the time,compared to the current standard protocols utilizing ultracentrifugation,and allows to recover fully intact exosomes in higher yields.Exosomes were found tocontain a very diverse RNA cargo,primarily short sequences 20-200 nt(such as mi RNA and fragments of m RNA),however longer RNA species were detected as well,including full-length 18S and 28S r RNA.CONCLUSION:We have successfully developed a set of reagents and a workflow allowing fast and efficient extraction of exosomes,followed by isolation of RNA and its analysis by q RT-PCR and other techniques. | Emily Zeringer Mu Li Tim Barta Jeoffrey Schageman Ketil Winther Pedersen Axl Neurauter Susan Magdaleno Robert Setterquist Alexander V Vlassov | 2013 | World Journal of Methodology2013，3，1： | 5 |

| 6 | Puncturing the myths of acupuncture显示文摘Acupuncture is a widely practiced system of medicine that has been in place for thousands of years. Consumer interest and use of acupuncture are becoming increasingly popular in the United States, as it is used to treat a multitude of symptoms and disease processes as well as to maintain health and prevent illness. A growing body of evidence increasingly validates the practice of acupuncture. Further developing scientific data will play an important role in the future of acupuncture and other complementary and alternative medicines in public health. Acupuncture is commonly used concurrently with conventional medicine. Although acupuncture is embraced by consumers and medical professionals, misconceptions abound. We have explored and dispelled ten misconceptions common to the practice of acupuncture, utilizing an evidence-based approach. As the trend of merging conventional medical care with acupuncture treatment grows, it is important to develop a conceptual model of integrative medicine. Using a scientific evidence approach will create a structure from which to begin and grow confidence among conventional medical providers. Acupuncture is a safe and effective modality when performed properly by trained professionals. Educating both the consumer and medical community is important to enable appropriate and evidence-based applications of acupuncture and integration with conventional medicine for high-quality patient care. | Molly J. | 11 | 体重调整固定剂量普通肝素与低分子肝素治疗急性静脉血栓栓塞的比较显示文摘背景:普通肝素用于治疗急性静脉血栓栓塞时,通常在监测血凝下静脉输注给药,患者需要住院治疗。然而,体重调整后皮下注射固定剂量普通肝素可能既适用于住院也适用于门诊静脉血栓栓塞患者的治疗。
目的:确定皮下注射体重调整固定剂量普通肝素治疗静脉血栓栓塞是否与低分子肝素同样安全有效。设计、地点及患者:随机、开标、裁定者盲法、非劣效试验。708例急性静脉血栓栓塞患者来自加拿大和新西兰6所大学附属临床中心,年龄≥18岁。试验于1998年9月至2004年2月进行。在随机分组的患者中,11例从疗效分析中排除,8例从安全性分析中排除。
干预:普通肝素皮下注射初始剂量为333U/kg,以后给予固定剂量250U/kg,每12小时1次(n=345)。低分子肝素(依诺肝素或达肝素)按100IU/kg剂量皮下注射,每12小时1次(n=352)。两种治疗均可院外进行,配合华法林治疗3个月。
主要观测指标:随机分组后3个月内复发性静脉血栓栓塞事件及10天内严重出血情况。结果:普通肝素组有13例患者(3.8%)发生静脉血栓栓塞复发,而低分子肝素组有12例患者复发(3.4%;绝对差值,0.4%;95%可信区间,-2.6%~3.3%)。开始治疗后10天内普通肝素组有4例患者(1.1%)发生严重出血,而低分子肝素组有5例(1.4%;绝对差值,-0.3%;95%可信区间,-2.3%~1.7%)。院外治疗患者在普通肝素组占72%,低分子肝素组占68%。
