6Application and prospects of butylphthalide for the treatment of neurologic diseases显示文摘Objective: The 3-N-butylphthalide (NBP) comprises one of the chemical constituents of celery oil. It has a series of pharmacologic mechanisms including reconstructing microcirculation, protecting mitochondrial function, inhibiting oxidative stress, inhibiting neuronal apoptosis, etc. Based on the complex multi-targets of pharmacologic mechanisms of NBP, the clinical application of NBP is increasing and more clinical researches and animal experiments are also focused on NBP. The aim of this review was to comprehensively and systematically summarize the application of NBP on neurologic diseases and briefly summarize its application to non-neurologic diseases. Moreover, recent progress in experimental models of NBP on animals was summarized. Data sources: Literature was collected from PubMed and Wangfang database until November 2018, using the search terms including '3-N-butylphthalide,''microcirculation,''mitochondria,''ischemic stroke,''Alzheimer disease,''vascular dementia,''Parkinson disease,''brain edema,''CO poisoning,''traumatic central nervous system injury,''autoimmune disease,''amyotrophic lateral sclerosis,''seizures,''diabetes,''diabetic cataract,' and 'atherosclerosis.' Study selection: Literature was mainly derived from English articles or articles that could be obtained with English abstracts and partly derived from Chinese articles. Article type was not limited. References were also identified from the bibliographies of identified articles and the authors’ files. Results: NBP has become an important adjunct for ischemic stroke. In vascular dementia, the clinical application of NBP to treat severe cognitive dysfunction syndrome caused by the hypoperfusion of brain tissue during cerebrovascular disease is also increasing. Evidence also suggests that NBP has a therapeutic effect for neurodegenerative diseases. Many animal experiments have found that it can also improve symptoms in other neurologic diseases such as epilepsy, cerebral edema, and decreased cognitive function caused by severe acute carbon monoxide poisoning. Moreover, NBP has therapeutic effects for diabetes, diabetes-induced cataracts, and non-neurologic diseases such as atherosclerosis. Mechanistically, NBP mainly improves microcirculation and protects mitochondria. Its broad pharmacologic effects also include inhibiting oxidative stress, nerve cell apoptosis, inflammatory responses, and anti-platelet and anti-thrombotic effects. 8New incompatible pair of TCM:Epimedii Folium combined with Psoraleae Fructus induces idiosyncratic hepatotoxicity under immunological stress conditions显示文摘Epimedii Folium(EF)combined with Psoraleae Fructus(PF)is a common modern preparation,but liver injury caused by Chinese patent medicine preparations containing EF and PF has been frequently reported in recent years.Zhuangguguanjiewan pills(ZGW),which contain EF and PF,could induce immune idiosyncratic liver injury according to clinical case reports and a nonhepatotoxic dose of lipopolysaccharide(LPS)model.This present study evaluated the liver injury induced by EF or PF alone or in combination and investigated the related mechanism by using the LPS model.Liver function indexes and pathological results showed that either EF or PF alone or in combination led to liver injury in normal rats;however,EF or PF alone could lead to liver injury in LPS-treated rats.Moreover,EF combined with PF could induce a greater degree of injury than that caused by EF or PF alone in LPS-treated rats.Furthermore,EF or PF alone or in combination enhanced the LPS-stimulated inflammatory cytokine production,implying that IL-1β,which is processed and released by activating the NLRP3 inflammasome,is a specific indicator of EF-induced immune idiosyncratic hepatotoxicity.Thus,EF may induce liver injury through enhancing the LPS-mediated proinflammatory cytokine production and activating the NLRP3 inflammasome.In addition,the metabolomics analysis results showed that PF affected more metabolites in glycerophospholipid and sphingolipid metabolic pathways compared with EF in LPS model,suggesting that PF increased the responsiveness of the liver to LPS or other inflammatory mediators via modulation of multiple metabolic pathways.Therefore,EF and PF combination indicates traditional Chinese medicine incompatibility,considering that it induces idiosyncratic hepatotoxicity under immunological stress conditions.Yuan Gao Zhilei Wang Jinfa Tang Xiaoyi Liu Wei Shi Nan Qin Xiaoyan Wang Yu Pang Ruisheng Li Yaming Zhang Jiabo Wang Ming Niu Zhaofang Bai Xiaohe Xiao 2020Frontiers of Medicine2020,14,1:32
9Anti-SARS-CoV-2 activities in vitro of Shuanghuanglian preparations and bioactive ingredients显示文摘Human infection with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)causes coronavirus disease 2019(COVID-19)and there is no cure currently.The 3CL protease(3CLpro)is a highly conserved protease which is indispensable for CoVs replication,and is a promising target for development of broad-spectrum antiviral drugs.In this study we investigated the anti-SARS-CoV-2 potential of Shuanghuanglian preparation,a Chinese traditional patent medicine with a long history for treating respiratory tract infection in China.We showed that either the oral liquid of Shuanghuanglian,the lyophilized powder of Shuanghuanglian for injection or their bioactive components dose-dependently inhibited SARS-CoV-23CLpro as well as the replication of SARS-CoV-2 in Vero E6 cells.Baicalin and baicalein,two ingredients of Shuanghuanglian,were characterized as the first noncovalent,nonpeptidomimetic inhibitors of SARS-CoV-23CLpro and exhibited potent antiviral activities in a cell-based system.Remarkably,the binding mode of baicalein with SARS-CoV-23CLpro determined by X-ray protein crystallography was distinctly different from those of known 3CLpro inhibitors.Baicalein was productively ensconced in the core of the substrate-binding pocket by interacting with two catalytic residues,the crucial S1/S2 subsites and the oxyanion loop,acting as a“shield”in front of the catalytic dyad to effectively prevent substrate access to the catalytic dyad within the active site.Overall,this study provides an example for exploring the in vitro potency of Chinese traditional patent medicines and effectively identifying bioactive ingredients toward a specific target,and gains evidence supporting the in vivo studies of Shuanghuanglian oral liquid as well as two natural products for COVID-19 treatment.Hai-xia Su Sheng Yao Wen-feng Zhao Min-jun Li Jia Liu Wei-juan Shang Hang Xie Chang-qiang Ke Hang-chen Hu Mei-na Gao Kun-qian Yu Hong Liu Jing-shan Shen Wei Tang Lei-ke Zhang Geng-fu Xiao Li Ni Dao-wen Wang Jian-ping Zuo Hua-liang Jiang Fang Bai Yan Wu Yang Ye Ye-chun Xu 2020Acta Pharmacologica Sinica2020,41,9:32
10Efficacy and safety of a novel anti-HER2 therapeutic antibody RC48 in patients with HER2-overexpressing,locally advanced or metastatic gastric or gastroesophageal junction cancer:a single-arm phase II study显示文摘Background:Current treatment options for human epidermal growth factor receptor 2(HER2)-overexpressing gastric cancer at third-line have shown limited clinical benefit.Further,there is no specific treatment for HER2 immunohistochemistry(IHC)2+and fluorescence in-situ hybridization-negative patients.Here,we report the efficacy and safety of a novel anti-HER2 antibody RC48 for patients with HER2-overexpressing,advanced gastric or gastroesophageal junction cancer.Methods:Patients with HER2-overexpressing(IHC 2+or 3+),locally advanced or metastatic gastric or gastroesophageal junction cancer who were under at least second-line therapy were eligible and received RC482.5 mg/kg alone every 2 weeks.The primary endpoint was the objective response rate(ORR)assessed by an independent review committee.Secondary endpoints included progressionfree survival(PFS),overall survival(OS),duration of response,time to progression,disease control rate,and safety.Results:Of 179 patients screened,125 were eligible and received RC48 treatment.The ORR was 24.8%(95%confidence interval[CI]:17.5%-33.3%).The median PFS and OS were 4.1 months(95%CI:3.7-4.9 months)and 7.9 months(95%CI:6.7-9.9 months),respectively.The most frequently reported adverse events were decreased white blood cell count(53.6%),asthenia(53.6%),hair loss(53.6%),decreased neutrophil count(52.0%),anemia(49.6%),and increased aspartate aminotransferase level(43.2%).Serious adverse events(SAEs)occurred in 45(36.0%)patients,and RC48-related SAEs were mainly decreased neutrophil count(3.2%).Seven patients had adverse events that led to death were not RC48-related.Conclusions:RC48 showed promising activity with manageable safety,suggesting potential application in patients with HER2-overexpressing,advanced gastric or gastroesophageal junction cancer who have previously received at least two lines of chemotherapy.Zhi Peng Tianshu Liu Jia Wei Airong Wang Yifu He Liuzhong Yang Xizhi Zhang Nanfeng Fan Suxia Luo Zhen Li Kangsheng Gu Jianwei Lu Jianming Xu Qingxia Fan Ruihua Xu Liangming Zhang Enxiao Li Yuping Sun Guohua Yu Chunmei Bai Yong Liu Jiangzheng Zeng Jieer Ying Xinjun Liang Nong Xu Chao Gao Yongqian Shu Dong Ma Guanghai Dai Shengmian Li Ting Deng Yuehong Cui Jianmin Fang Yi Ba Lin Shen 2021Cancer Communications2021,41,11:29
11Landscape Pattern Evolution Processes of Alpine Wetlands and Their Driving Factors in the Zoige Plateau of China显示文摘Zoige Plateau wetlands are located in the northeastern corner of the Qinghai-Tibet Plateau.The landscape pattern evolution processes in the Zoige Plateau and their driving factors were identified by analyzing the dynamic changes in landscape modification and conversion and their dynamic rates of alpine wetlands over the past four decades.The results showed that the landscape conversion between wetlands and non-wetlands mainly occurred during the period from 1966 to 1986.The marsh wetland area converted from lake and river wetlands was larger because of swamping compared to other wetland landscapes.Meanwhile,the larger area of marsh wetlands was also converted to lake wetlands more than other types of wetlands.The modification processes mainly occurred among natural wetland landscapes in the first three periods.Obvious conversions were observed between wetland and nonwetland landscapes(i.e.,forestland,grassland,and other landscapes) in the Zoige Plateau.These natural wetland landscapes such as river,lake and marsh wetlands showed a net loss over the past four decades,whereas artificial wetland landscapes(i.e.,paddy field and reservoir and pond wetlands) showed a net decrease.The annual dynamic rate of the whole wetland landscape was 0.72%,in which the annual dynamic rate of river wetlands was the highest,followed by lake wetlands,while marsh wetlands had the lowest dynamic rate.The integrated landscape dynamic rate showed a decreasing trend in the first three periods.The changes in wetland landscape patterns were comprehensively controlled by natural factors and human activities,especially human activities play an important role in changing wetland landscape patterns.BAI Jun-hong LU Qiong-qiong WANG Jun-jing ZHAO Qing-qing OUYANG Hua DENG Wei LI Ai-nong 2013Journal of Mountain Science2013,10,1:28

13Root microbiota shift in rice correlates with resident time in the field and developmental stage显示文摘Land plants in natural soil form intimate relationships with the diverse root bacterial microbiota. A growing body of evidence shows that these microbes are important for plant growth and health. Root microbiota composition has been widely studied in several model plants and crops; however, little is known about how root microbiota vary throughout the plant's life cycle under field conditions. We performed longitudinal dense sampling in field trials to track the time-series shift of the root microbiota from two representative rice cultivars in two separate locations in China. We found that the rice root microbiota varied dramatically during the vegetative stages and stabilized from the beginning of the reproductive stage, after which the root microbiota underwent relatively minor changes until rice ripening. Notably, both rice genotype and geographical location influenced the patterns of root microbiota shift that occurred during plant growth. The relative abundance of Deltaproteobacteria in roots significantly increased overtime throughout the entire life cycle of rice, while that of Betaproteobacteria, Firmicutes, and Gammaproteobacteria decreased. By a machine learning approach, we identified biomarker taxa and established a model to correlate root microbiota with rice resident time in the field(e.g., Nitrospira accumulated from 5 weeks/tillering in field-grown rice). 14Curative effect of warming kidney and fortifying spleen recipe on diarrhea-predominant irritable bowel syndrome显示文摘OBJECTIVE:To observe the curative effect of a recipe for warming the kidney and fortifying the spleen on diarrhea-predominant irritable bowel syndrome(IBS-D).METHODS:This multi-center,double-blind,randomized,and controlled trial included 240 patients that met the inclusion criterion and were then divided into two groups of 120.Patients in the treatment group(group A)took modified Sishen Wan orally for warming the kidney and fortifying the spleen and patients in the control group(group B)took a placebo,Chao Maiya,for 4 weeks.28 days after withdrawal,there was a 6-month follow-up to observe patient recurrence condition.The total effective rate,curative effect,and recurrence rate were evaluated after treatment.RESULTS:There was statistical difference(P<0.01)between the two groups in total effective rate(92.24%in the treatment group and 49.07%in the control group),in curative effect of TCM syndrome(90.52%and 47.22%,respectively),and in the recurrence rate(15.79%and 56.86%,respectively)within6 months after treatment.CONCLUSION:Modified Sishen Wan,for warming the kidney and fortifying the spleen,can effectively treat IBS-D and better control its recurrence.Xiaolan Su Yanping Tang Jing Zhang Yuebin Dong Wei Wei Yibing Bai Yong Liu 2013Journal of Traditional Chinese Medicine2013,33,5:22
15Differential expression of serum miR-126,miR-141 and miR-21 as novel biomarkers for early detection of liver metastasis in colorectal cancer显示文摘Objective:MicroRNAs(miRNAs) have potential as diagnostic biomarkers in cancer.Evaluation of the association between miRNA expression patterns and early detection of liver metastasis in colorectal cancer(CRC) has not been reported.Methods:We investigated the expression of metastasis-associated miRs-31,335,206,141,126,200b,200c,21,Let7a,Let7b and Let7c in localized,liver-metastatic and other organ-metastatic CRC(OM-CRC).Expressions of target miRNAs in serum were evaluated in 116 consecutive localized CRC(L-CRC),72 synchronous liver-metastatic CRC(SLM-CRC) and 36 other OM-CRC by quantitative real-time PCR.Results:Seven of 11 tested miRNAs could be detected from serum.Four miRNAs,miR-126,Let-7a,miR-141 and miR-21 were identified as metastasis-associated miRNAs.Compared with L-CRC,significant upregulated expression was observed for miR-141 and miR-21 in SLM-CRC and OM-CRC,down-regulated expression was observed for miR-126 in SLM-CRC and OM-CRC,and up-regulated expression of Let-7a in OM-CRC.The receiver operating characteristic(ROC) curve showed serum miR-126 had a cut-off [log 10 relative quantity(log 10 RQ)=–0.2005] with 77.78% sensitivity and 68.97% specificity with an area under curve(AUC) of 0.7564,miR-141 had a cut-off(log 10 RQ=–0.2285) with 86.11% sensitivity and 76.11% specificity with an AUC of 0.8279,and miR-21 had a cut-off(log 10 RQ=–0.1310) with 73.61% sensitivity and 66.38% specificity with an AUC of 0.7479.Conclusions:We identified liver metastasis-associated miRNAs,suggesting serum miR-126,miR-141 and miR-21 may be novel biomarkers for clinical diagnosis of early stage liver-metastatic CRC.Jie Yin Zhigang Bai Jianning Song Yun Yang Jin Wang Wei Han Jun Zhang Hua Meng Xuemei Ma Yao Yang Tingting Wang Weirong Li Zhongtao Zhang 2014Chinese Journal of Cancer Research2014,26,1:22
16Management of cytokine release syndrome related to CAR-T cell therapy显示文摘Chimeric antigen receptor T (CAR-T) cell therapy is a novel cellular immunotherapy that is widely used to treat hematological malignancies, including acute leukemia, lymphoma, and multiple myeloma. Despite its remarkable clinical effects, this therapy has side effects that cannot be underestimated. Cytokine release syndrome (CRS) is one of the most clinically important and potentially life-threatening toxicities. This syndrome is a systemic immune storm that involves the mass cytokines releasing by activated immune cells. This phenomenon causes multisystem damages and sometimes even death. In this study, we reported the management of a patient with recurrent and refractory multiple myeloma and three patients with acute lymphocytic leukemia who suffered CRS during CAR-T treatment. The early application of tocilizumab, an anti-IL-6 receptor antibody, according to toxicity grading and clinical manifestation is recommended especially for patients who suffer continuous hyperpyrexia, hypotensive shock, acute respiratory failure, and whose CRS toxicities deteriorated rapidly. Moreover, low doses of dexamethasone (5-10 mg/day) were used for refractory CRS not responding to tocilizumab. 19Upregulation of Chemokine CXCL12 in the Dorsal Root Ganglia and Spinal Cord Contributes to the Development and Maintenance of Neuropathic Pain Following Spared Nerve Injury in Rats显示文摘Emerging evidence indicates that CXCL12/CXCR4 signaling is involved in chronic pain. However, few studies have systemically assessed its role in direct nerve injury-induced neuropathic pain and the underlying mechanism. Here, we determined that spared nerve injury(SNI)increased the expression of CXCL12 and its cognate receptor CXCR4 in lumbar 5 dorsal root ganglia(DRG)neurons and satellite glial cells. SNI also induced longlasting upregulation of CXCL12 and CXCR4 in the ipsilateral L4–5 spinal cord dorsal horn, characterized by CXCL12 expression in neurons and microglia, and CXCR4 expression in neurons and astrocytes. Moreover, SNIinduced a sustained increase in TNF-a expression in the DRG and spinal cord. Intraperitoneal injection(i.p.) of the TNF-a synthesis inhibitor thalidomide reduced the SNI-induced mechanical hypersensitivity and inhibited the expression of CXCL12 in the DRG and spinal cord.Intrathecal injection(i.t.) of the CXCR4 antagonist AMD3100, both 30 min before and 7 days after SNI,reduced the behavioral signs of allodynia. Rats given an i.t.or i.p. bolus of AMD3100 on day 8 of SNI exhibited attenuated abnormal pain behaviors. The neuropathic pain established following SNI was also impaired by i.t. administration of a CXCL12-neutralizing antibody. Moreover,repetitive i.t. AMD3100 administration prevented the activation of ERK in the spinal cord. The mechanical hypersensitivity induced in na?¨ve rats by i.t. CXCL12 was alleviated by pretreatment with the MEK inhibitor PD98059. 20Anus-preserving rectectomy via telescopic colorectal mucosal anastomosis for low rectal cancer: Experience from a Chinese cohort显示文摘AIM: To investigate the safety and efficacy of anus-preserving rectectomy via telescopic colorectal mucosal anastomosis (TCMA) for low rectal cancer. METHODS: From August 1993 to October 2012, 420 patients including 253 males and 167 females with low rectal cancer underwent transabdominal and transanal anterior resection, followed by TCMA. The distance be-tween the anus and inferior margin of the tumor ranged from 5 to 7 cm, and was 5 cm in 6 patients, 6 cm in 127, and 7 cm in 287 patients. Tumor-node-metastasis staging showed that 136 patients had stage Ⅰ, 252 had stage Ⅱ and 32 had stage Ⅲ. Fifty-six patients with T3 or over received preoperative neoadjuvant chemoradio-therapy. RESULTS: The postoperative follow-up rate was 91.9% (386/420) with a median time of 6.4 years. All 420 pa-tients underwent radical resection. No postoperativedeath occurred. Postoperative complications included anastomotic leakage in 13 (3.1%) patients and anas-tomotic stenosis in 7 (1.6%). The local recurrence rate after surgery was 6.2%, the hepatic metastasis rate was 13.2% and the pulmonary metastasis rate was 2.3%. The 5-year survival rate was 74.0% and the disease-free survival rate was 71.0%. Kirwan classification showed that continence was good in 94.4% of patients with stage I when scored 12 mo after resection.
