1Prognostic value of dynamic electroencephalogram in comatose patients with different diseased regions显示文摘BACKGROUND: It has proved that dynamic electroencephalogram (EEG) is definite in judging the outcome of ischemic hypoxic comatose patients, EEG is more sensitive to the cortical affection, but not sensitive to the subcortical and brainstem affections, thus it is necessary to clarify the indications of this technique in the clinical application. OBJECTIVE: To observe and compare the prognostic value of dynamic EEG and Glasgow coma score in comatose patients with different diseased region. DESIGN: A clinical case-controlled observation. SETTING: Union Hospital of Fujian Medical University. PARTICIPANTS: Sixty-eight comatose patients were selected from the Union Hospital affiliated to Fujian Medical University from June 1998 to January 2005. The diseased regions were identified using cranial CT (n =43) or MR (n =25). According to different primarily diseased regions, the comatose patients were divided into two groups: ① brainstem affection group (n =23): 13 males and 10 females, 14–62 years of age; ② diffuse cortical affection group (n =45): 28 males and 17 females, 23–75 years of age. METHODS: The dynamic EEG and Glasgow coma score were examined in the 45 comatose patients with primarily cortical affection and 22 comatose patients with primarily brainstem affection at acute phase. The patients were followed-up for 3 months to observe the outcome, The termination of outcome judgment was 3 months after attack or the death. The clinical outcome was classified as complete rehabilitation, survived with disability, death or vegetative state. Correlations of dynamic EEG and Glasgow coma score with the outcome of patients were analyzed. The correlations of dynamic EEG grades and Glasgow coma scores with the outcome were analyzed, and the prognostic value of dynamic EEG grades was compared between the two groups. MAIN OUTCOME MEASURES: ① Correlations of dynamic EEG and Glasgow coma score with the outcome of patients; ② Comparison of the prognostic value of dynamic EEG grades between the two groups. RESULTS: All the 68 patients were involved in the analysis of results. ① Correlations of dynamic EEG grades and Glasgow scores and their correlation analysis: EEG grades had significant negative correlation with Glasgow coma scores in both the cortical affection group and brainstem affection group (r =–0.743, –0.564, P < 0.01, 0.05). In the cortical affection group, the Glasgow coma scores and dynamic EEG grades in the patients with the outcome of death or vegetative state were significantly different from those with the outcome of rehabilitation (P < 0.05–0.01). In the brainstem affection group, the Glasgow coma scores were only significantly different between the patients with outcome of rehabilitation and death (P < 0.05), and there was no significant difference in dynamic EEG grades among the three prognostic states (P > 0.05). ② Comparison of the prognostic value of dynamic EEG grades between comatose patients with cortical affection and brainstem affection: The sensitivity, specificity and accuracy were all higher (P < 0.05), while the error rate was lower (P < 0.05) in the cortical affection group than in the brainstem affection group. 3The role of corneal endothelium in macular corneal dystrophy development and recurrence显示文摘Macular corneal dystrophy(MCD)is a progressive,bilateral stromal dystrophic disease that arises from mutations in carbohydrate sulfotransferase 6(CHST6).Corneal transplantation is the ultimate therapeutic solution for MCD patients.Unfortunately,postoperative recurrence remains a significant challenge.We conducted a retrospective review of a clinical cohort comprising 102 MCD patients with 124 eyes that underwent either penetrating keratoplasty(PKP)or deep anterior lamellar keratoplasty(DALK).Our results revealed that the recurrence rate was nearly three times higher in the DALK group(39.13%,9/23 eyes)compared with the PKP group(10.89%,11/101 eyes),suggesting that surgical replacement of the corneal endothelium for treating MCD is advisable to prevent postoperative recurrence.Our experimental data confirmed the robust m RNA and protein expression of CHST6 in human corneal endothelium and the rodent homolog CHST5 in mouse endothelium.Selective knockdown of wild-type Chst5 in mouse corneal endothelium(AC^(siChst5)),but not in the corneal stroma,induced experimental MCD with similar extracellular matrix synthesis impairments and corneal thinning as observed in MCD patients.Mice carrying Chst5 point mutation also recapitulated clinical phenotypes of MCD,along with corneal endothelial abnormalities.Intracameral injection of wild-type Chst5 rescued the corneal impairments in AC^(siChst5)mice and retarded the disease progression in Chst5 mutant mice.Overall,our study provides new mechanistic insights and therapeutic approaches for MCD treatment by highlighting the role of corneal endothelium in MCD development.Bi-Ning Zhang Benxiang Qi Chunxiao Dong Bin Zhang Jun Cheng Xin Wang Suxia Li Xiaoyun Zhuang Shijiu Chen Haoyun Duan Dewei Li Sujie Zhu Guoyun Li Yihai Cao Qingjun Zhou Lixin Xie 2024Science China(Life Sciences)2024,67,2:0
4The Spatial and Electronic Effects of Substituent Groups on the Thermal Curing of Bio-Based Benzoxazines显示文摘To explore the influence of substituent groups on thermally induced curing,eight new bio-based benzoxazines containing different substituent groups with different electron negativity and volumes were synthesized.The thermal curing of these bio-based benzoxazines was studied in detail.Combined with the curing reaction kinetics,simulation and calculation of Highest Occupied Molecular and Lowest Unoccupied Molecular values,the spatial and electronic effects of different substituent groups on the curing of benzoxazine was explored.It was found that when the substituent was located at the position directly connected to the N atom,the steric hindrance effect of the group was dominant.When the substituent group was located on the benzene ring connected to the O atom,both the electronic effect and the spatial effect influenced the curing of benzoxazine.When an electron-withdrawing group was connected ortho position to the O atom,the curing reaction was promoted due to the decreased electron cloud density of O-on the oxazine ring,making the C-O bond easier to break.When an electron-donating group was connected to the meta position of the O atom it also promoted the curing reaction,possibly because it increased the electron cloud density of the+CH2 reaction site and thereby facilitated electrophilic substitution via attack of+CH2 on the cross linking reaction centre.This work provides a deeper understanding of how spatial and electronic effects of substituents affect the curing of benzoxazine.Rumeng Li Guozhu Zhan Qi Ma Yunhe Yang Xiaoyun Liu Yitong Zhang Qixin Zhuang 2021Journal of Renewable Materials2021,9,12:0

7A Spider Monkey Optimization Algorithm Combining Opposition-Based Learning and Orthogonal Experimental Design显示文摘As a new bionic algorithm,Spider Monkey Optimization(SMO)has been widely used in various complex optimization problems in recent years.However,the new space exploration power of SMO is limited and the diversity of the population in SMO is not abundant.Thus,this paper focuses on how to reconstruct SMO to improve its performance,and a novel spider monkey optimization algorithm with opposition-based learning and orthogonal experimental design(SMO^(3))is developed.A position updatingmethod based on the historical optimal domain and particle swarmfor Local Leader Phase(LLP)andGlobal Leader Phase(GLP)is presented to improve the diversity of the population of SMO.Moreover,an opposition-based learning strategy based on self-extremum is proposed to avoid suffering from premature convergence and getting stuck at locally optimal values.Also,a local worst individual elimination method based on orthogonal experimental design is used for helping the SMO algorithm eliminate the poor individuals in time.Furthermore,an extended SMO^(3)named CSMO^(3)is investigated to deal with constrained optimization problems.The proposed algorithm is applied to both unconstrained and constrained functions which include the CEC2006 benchmark set and three engineering problems.Experimental results show that the performance of the proposed algorithm is better than three well-known SMO algorithms and other evolutionary algorithms in unconstrained and constrained problems.Weizhi Liao Xiaoyun Xia Xiaojun Jia Shigen Shen Helin Zhuang Xianchao Zhang 2023Computers, Materials & Continua2023,76,9:0
8Genomic instability and eye diseases显示文摘Background:Genetic information is stored in the bases of double-stranded DNA.However,the integrity of DNA molecules is constantly threatened by various mutagenic agents,including pollutants,ultraviolet light(UV),and medications.To counteract these environmental damages,cells have established multiple mechanisms,such as producing molecules to identify and eliminate damaged DNA,as well as reconstruct the original DNA structures.Failure or insufficiency of these mechanisms can cause genetic instability.However,the role of genome stability in eye diseases is still under-researched,despite extensive study in cancer biology.Main text:As the eye is directly exposed to the external environment,the genetic materials of ocular cells are constantly under threat.Some of the proteins essential for DNA damage repair,such as pRb,p53,and RAD21,are also key during the ocular disease development.In this review,we discuss five ocular diseases that are associated with genomic instability.Retinoblastoma and pterygium are linked to abnormal cell cycles.Fuchs’corneal endothelial dystrophy and age-related macular degeneration are related to the accumulation of DNA damage caused by oxidative damage and UV.The mutation of the subunit of the cohesin complex during eye development is linked to sclerocornea.Conclusions:Failure of DNA damage detection or repair leads to increased genomic instability.Deciphering the role of genomic instability in ocular diseases can lead to the development of new treatments and strategies,such as protecting vulnerable cells from risk factors or intensifying damage to unwanted cells.Hongyan Liu Jun Cheng Xiaoyun Zhuang Benxiang Qi Fenfen Li Bining Zhang 2023Advances in Ophthalmology Practice and Research2023,3,3:0
9Chaotic electrical stimulation of the subthalamic nucleus-mossy fiber sprouting, epileptic seizures, and brain electrical activity in pentylenetetrazol-kindled rats显示文摘BACKGROUND: Previous studies have demonstrated that appropriate interventions can alter brain electrical activity of epileptic patients prior to and during a seizure, leading to maintenance of a highly chaotic state, thereby inhibiting abnormal epileptic discharges, and eventually controlling epileptic seizure. OBJECTIVE: This study was designed to observe the effects of chaotic electrical stimulation to the subthalamic nucleus on mossy fiber sprouting, epileptic seizures, and electrical discharges, and to summarize the most suitable intervention. DESIGN, TIME AND SETTING: This randomized grouping, neuroelectrophysiological study was performed at the Laboratory of Neurology, Union Hospital Affiliated to Fujian Medical University in September 2007. MATERIALS: Fifty-five healthy, male, Sprague Dawley rats were subjected to an epileptic model by an intraperitoneal injection of pentylenetetrazol. The YC-2 programmed electrical stimulator was provided by Chengdu Instrument Factory, China; the video electroencephalographic system (KT-88-2400) and 24-hour active electroencephalographic system were products of Contec Medical System Co., Ltd., China; pentylenetetrazol was purchased from Sigma, USA. METHODS: The present interventional method consisted of electrical stimulation to the subthalamic nucleus with an intensity of 500 μA, pulse width 0.05 ms, frequency 30 Hz, and a duration of 20 minutes for 14 successive days. Fifty-five rats were divided into 6 groups: (1) pre-stimulation (n = 10), pentylenetetrazol was administered and 30 minutes later, chaotic electrical stimulation was performed; (2) synchronous stimulation (n = 10), rats received pentylenetetrazol and chaotic electrical stimulation concurrently; (3) post-administration stimulation (n = 10), after pentylenetetrazol administration, chaotic electrical stimulation was performed immediately after cessation of a seizure; (4) sham-stimulation (n = 10), following pentylenetetrazol administration, an electrode was connected to the stimulator, but electrical stimulation was not performed; (5) control (n = 10), pentylenetetrazol administration, but no electrode was implanted; (6) blank control (n = 50), administration of the same amount of physiological saline and chaotic electrical stimulation. MAIN OUTCOME MEASURES: Timm-stained granule change was scored. Simultaneously, electroencephalography was performed to acquire seizure counts and time course of epileptic discharge within 24 hours. RESULTS: Timm scores were lower in the electrically stimulated rats than in the non-stimulated rats (P < 0.01). Timm scores were lowest in the synchronous stimulation group. When the rats suffered from tonic clonic seizure, the electroencephalogram primarily showed a persistent spike-slow wave and sharp wave. For the electrically stimulated rats, the mean values of seizure counts and time course of epileptic discharge during each hour were noticeably decreased compared with the non-stimulated rats. The synchronous stimulation group, however, had the lowest seizure counts and the shortest time course, followed by the pre-stimulation group, and lastly the post-administration stimulation group. Significant differences existed among the groups (P < 0.01). Compared with the pre-stimulation group and the post-administration stimulation group, the latent period of grades Ⅰ and Ⅳ epileptic seizures was significantly prolonged, and the time course of tonic clonic seizure, as well as total time course, were significantly shortened in the synchronous stimulation group (P < 0.01). CONCLUSION: Simultaneous administration of pentylenetetrazol together with chaotic electrical stimulation produced the greatest inhibitory effects on epileptic seizures.
