2Stenting for symptomatic intracranial arterial stenosis in China:1-year outcome of a multicentre registry study显示文摘background and purpose A multicentre prospective registry study of individually tailored stenting for a patient with symptomatic intracranial atherosclerotic stenosis(ICAS)combined with poor collaterals in China showed that the short-term safety and efficacy of stenting was acceptable.However,it remained uncertain whether the low event rate could be of a long term.We reported the 1-year outcome of this registry study to evaluate the long-term efficacy of individually tailored stenting for patients with severe symptomatic ICAS combined with poor collaterals.Methods Patients with symptomatic ICAS caused by 70%-99% stenosis located at the intracranial internal carotid,middle cerebral,intracranial vertebral or basilar arteries combined with poor collaterals were enrolled.Balloon-mounted stent or balloon plus self-expanding stent were selected based on the ease of vascular access and lesion morphology determined by the operators.The primary outcome was the rate of 30-day stroke,transient ischaemic attack and death,and 12-month ischaemic stroke within the same vascular territory,haemorrhagic stroke and vascular death after stenting.results From September 2013 to January 2015,300 patients(ages 58.3±9.78 years)were recruited.Among them,159 patients were treated with balloon-mounted stent and 141 with balloon plus self-expanding stent.During the 1-year follow-up,25 patients had a primary end point event.The probability of primary outcome at 1 year was 8.1%(95% CI 5.3% to 11.7%).In 76 patients with digital subtraction angiography follow-up,27.6%(21/76)had re-stenosis≥50% and 18.4%(14/76)had re-stenosis≥70%.No baseline characteristic was associated with the primary outcome.Conclusion The event rate remains low over 1 year of individually tailored stenting for patients with severe symptomatic ICAS combined with poor collaterals.Further randomised trial of comparing individually tailored stenting with best medical therapy is needed.Ning Ma Yong Zhang Jie Shuai Changchun Jiang Qiyi Zhu Kangning Chen Li Liu Baomin Li Xiangqun Shi Lianbo Gao Yajie Liu Feng Wang Yongli Li Tieyan Liu Hongbo Zheng Dapeng Mo Feng Gao Yilong Wang Yongjun Wang Lei Feng Zhongrong Miao 2018Stroke & Vascular Neurology2018,3,3:29
3Safety and efficacy of ciprofol vs.propofol for sedation in intensive care unit patients with mechanical ventilation:a multi-center,open label,randomized,phase 2 trial显示文摘Background:Ciprofol(HSK3486;Haisco Pharmaceutical Group Co.,Ltd.,Chengdu,China),developed as a novel 2,6-disubstituted phenol derivative showed similar tolerability and efficacy characteristics as propofol when applicated as continuous intravenous infusion for 12 h maintenance sedation in a previous phase 1 trial.The phase 2 trial was designed to investigate the safety,efficacy,and pharmacokinetic characteristics of ciprofol for sedation of patients undergoing mechanical ventilation.Methods:In this multicenter,open label,randomized,propofol positive-controlled,phase 2 trial,39 Chinese intensive care unit patients receiving mechanical ventilation were enrolled and randomly assigned to a ciprofol or propofol group in a 2:1 ratio.The ciprofol infusion was started with a loading infusion of 0.1-0.2 mg/kg for 0.5-5.0 min,followed by an initial maintenance infusion rate of 0.30 mg·kg^(-1)·h^(-1),which could be adjusted to an infusion rate of 0.06 to 0.80 mg·kg^(-1)·h^(-1),whereas for propofol the loading infusion dose was 0.5-1.0 mg/kg for 0.5-5.0 min,followed by an initial maintenance infusion rate of 1.50 mg·kg^(-1)·h^(-1),which could be adjusted to 0.30-4.00 mg·kg^(-1)·h^(-1)to achieve-2 to+1 Richmond Agitation-Sedation Scale sedation within 6-24 h of drug administration.Results:Of the 39 enrolled patients,36 completed the trial.The median(min,max)of the average time to sedation compliance values for ciprofol and propofol were 60.0(52.6,60.0)min and 60.0(55.2,60.0)min,with median difference of 0.00(95%confidence interval:0.00,0.00).In total,29(74.4%)patients comprising 18(69.2%)in the ciprofol and 11(84.6%)in the propofol group experienced 86 treatment emergent adverse events(TEAEs),the majority being of severity grade 1 or 2.Drug-and sedation-related TEAEs were hypotension(7.7%vs.23.1%,P=0.310)and sinus bradycardia(3.8%vs.7.7%,P=1.000)in the ciprofol and propofol groups,respectively.The plasma concentration-time curves for ciprofol and propofol were similar.Conclusions:ciprofol is comparable to propofol with good tolerance and efficacy for sedation of Chinese intensive care unit patients undergoing mechanical ventilation in the present study setting.Trial registration:ClinicalTrials.gov,NCT04147416.Yongjun Liu Xiangyou Yu Duming Zhu Jun Zeng Qinhan Lin Bin Zang Chuanxi Chen Ning Liu Xiao Liu Wei Gao Xiangdong Guan 2022Chinese Medical Journal2022,,9:19
4Sol-gel preparation and characterization of Co_3O_4 nanocrystals显示文摘A new citrate acid-hydrazine sol-gel route for preparation of Co3O4 nanoparticles has been developed. Co3O4 nanoparticles with different particle-sizes and morphology were prepared at different heat-treatment temperatures and the pure cubic nanocrystals of Co3O4 were obtained at 600℃. The synthesis process was monitored by infrared spectroscopy (IR), thermal gravimetric and differential thermal analysis (TG-DTA). The structure and morphology of Co3O4 nanocrystals were characterized by X-ray powder diffraction (XRD), transmission electron microscopy (TEM) and X-ray-photoelectron spectroscopy (XPS). The infrared absorption bands blueshifted with particle size decreasing, which could be attributed to increasing surface effect. 7Altered expression of mitochondrial related genes in the native Tibetan placents by mitochondrial cDNA array analysis显示文摘Objective: To explore the mechanism of native Tibetan fetuses adaptation to hypoxia, we tried to find the different expression genes about mitochondrial function in the native Tibetan placents. Methods: In this study, the placents of native Tibetan and the high-altitude Han (ha-Han) were collected. After the total RNA extraction, the finally synthesized cDNAs were hybridized to mitochondrial array to find the altered expression genes between them. Then, the cytochrome c oxidase 17 (Cox17), dynactin 2 (DCTN2, also known as p50), and vascular endothelial growth factor receptor (VEGFR, also known as KDR) were chosen from the altered expression genes to further verify the array results using the SYBR Green real-time PCR. Because the altered expression genes (such as Cybb and Cox17) in the array results related to the activities of COXI and COXIV, the placental mitochondria activities of COXI and COXIV were measured to find their changes in the hypoxia. Results: By a standard of ≥1.5 or ≤0.67, there were 24 different expressed genes between the native Tibetan and the ha-Han placents, including 3 up-regulated genes and 21 down-regulated genes. These genes were related to energy metabolism, signal transduction, cell proliferation, electron transport, cell adhesion, nucleotide-excision repair. The array results of Cox17, DCTN2 and KDR were further verified by the real-time RT-PCR. Through the mitochondria respiration measurements, the activity of COXI in the native Tibetan placents were higher than that of ha-Han, there was no difference in COXIV activity between them. 8Estimating significant wave height from SAR imagery based on an SVM regression model显示文摘A new method for estimating significant wave height(SWH) from advanced synthetic aperture radar(ASAR) wave mode data based on a support vector machine(SVM) regression model is presented. The model is established based on a nonlinear relationship between σ0, the variance of the normalized SAR image, SAR image spectrum spectral decomposition parameters and ocean wave SWH. The feature parameters of the SAR images are the input parameters of the SVM regression model, and the SWH provided by the European Centre for Medium-range Weather Forecasts(ECMWF) is the output parameter. On the basis of ASAR matching data set, a particle swarm optimization(PSO) algorithm is used to optimize the input kernel parameters of the SVM regression model and to establish the SVM model. The SWH estimation results yielded by this model are compared with the ECMWF reanalysis data and the buoy data. The RMSE values of the SWH are 0.34 and 0.48 m, and the correlation coefficient is 0.94 and 0.81, respectively. The results show that the SVM regression model is an effective method for estimating the SWH from the SAR data. 11Ratiometric delivery of doxorubicin and berberine by liposome enables superior therapeutic index than Doxil?显示文摘Although the appearance of Doxil alleviated the cardiotoxicity of DOX, the progression-free survival of patients was not prolonged compared with traditional medication regimens, and side effects such as hand-foot syndrome has occurred. In order to solve this dilemma, we have designed a novel co-delivery strategy to construct a co-loaded liposome of berberine(BER) and doxorubicin(DOX), which was called Lipo Be Do. The optimal synergistic ratio of the two drugs was screened by cell cytotoxicity experiments in vitro, and the optimal attenuation ratio was further determined by in vivo cardiac H&E staining pathological sections. The optimal combination treatment caused a robust increase in apoptotic cells of 4T1, as compared to drug alone treatment. The prepared co-loaded liposome, Lipo Be Do, had high encapsulation efficiency and good stability. The nanoliposome carrier controlled the biological fate of the drugs and maintained a pre-defined optimal ratio in vivo. The Lipo Be Do significantly inhibited tumor growth in 4T1 murine mammary carcinoma model compared with Doxil(P < 0.05), and completely overcame the myocardial rupture toxicity caused by Doxil in mice. 13Dynamic Modeling and Experiment of a New Type of Parallel Servo Press Considering Gravity Counterbalance显示文摘The large capacity servo press is traditionally realized by means of redundant actuation,however there exist the over-constraint problem and interference among actuators,which increases the control difficulty and the product cost.A new type of press mechanism with parallel topology is presented to develop the mechanical servo press with high stamping capacity.The dynamic model considering gravity counterbalance is proposed based on the virtual work principle,and then the effect of counterbalance cylinder on the dynamic performance of the servo press is studied.It is found that the motor torque required to operate the press is a lot less than the others when the ratio of the counterbalance force to the gravity of ram is in the vicinity of 1.0.The stamping force of the real press prototype can reach up to 25 MN on the position of 13 mm away from the bottom dead center.The typical deep-drawing process with 1200 mm stroke at 8 strokes per minute is proposed by means of five order polynomial.On this process condition,the driving torques are calculated based on the above dynamic model and the torque measuring test is also carried out on the prototype.It is shown that the curve trend of calculation torque is consistent to the measured result and that the average error is less than 15%.The parallel mechanism is introduced into the development of large capacity servo press to avoid the over-constraint and interference of traditional redundant actuation,and its dynamic characteristics with gravity counterbalance are presented.HE Jun GAO Feng BAI Yongjun WU Shengfu 2013Chinese Journal of Mechanical Engineering2013,26,6:4
14Staged angioplasty versus regular carotid artery stenting in patients with carotid artery stenosis at high risk of hyperperfusion:a randomised clinical trial显示文摘Background and purpose Hyperperfusion(HP)is a devastating complication associated with carotid artery stenting(CAS)or endarterectomy.The efficacy and safety of staged angioplasty(SAP)in patients with CAS at high risk of HP remains unclear.We sought to determine whether SAP is superior to regular CAS in patients with high risk of HP.Methods A randomised,multicentre open-label clinical trial with blinded outcome assessment(STEP)was conducted.Patients with severe carotid stenosis at high risk of HP were randomly assigned(1:1)to the SAP or regular CAS group.The primary endpoint was hyperperfusion syndrome(HPS)and intracerebral haemorrhage(ICH)within 30 days after the procedure.Results From November 2014 to January 2017,a total of 64 patients were enrolled in 11 centres.33 patients were allocated to the SAP group and 31 to the regular CAS group.At 30 days,the rate of primary endpoint was 0.0%(0/33)in the SAP group and 9.7%(3/31)in the regular CAS group(absolute risk reduction(ARR),9.7%;95%CI−20.1% to 0.7%;p=0.11).As one of the secondary endpoints,the incidence of HP phenomenon(HPP)was lower in the SAP group than the regular CAS group(0.0%vs 22.6%,ARR,−22.6%;95% CI−36.8%to−10.2%;p=0.04).Conclusion The rate of HPS and ICH was not significantly lower in SAP group;the extended secondary endpoint of HPP,however,significantly reduced,which suggested that SAP may be a safe and effective carotid revascularisation procedure to prevent HP.Dapeng Mo Baixue Jia Huaizhang Shi Yaxuan Sun Qingan Liu Chengzhe Fan Jianping Deng Jinglin Yuan Wei Wu Changchun Jiang Guilian Zhang Hanjun Du Ning Ma Feng Gao Xuan Sun Ligang Song Lian Liu Guangge Peng Yongjun Wang Yilong Wang Zhongrong Miao the STEP Study Group 2021Stroke & Vascular Neurology2021,6,1:3
15Significance of VEGF and NF-κB Expression in Thyroid Carcinoma显示文摘OBJECTIVE To investigate the significance of vascular endothelial growth factor (VEGF) and nuclear factor-kappaB (NF-κB) expression in thyroid carcinoma. METHODS The expression of NF-κB and VEGF was determined by im- munohistochemistry in formalin-fixed and paraffin-embedded specimens obtained from 10 normal thyroid tissues (NT), 12 cases of thyroid adeno- ma (TA) and 68 cases of thyroid carcinoma (TC).Differences in expres- sion between NT, TA and TC were statistically analyzed. In addition, in cases of TC, the relationship of NF-κB and VEGF expression with various clinicopathological factors, including histological typing, clinical staging and lymph node metastasis, as well as the correlation between N F - κ B and VEGF expression was examined. RESULTS In contrast to the negative immunoreactivity for VEGF in NT, there was a significantly higher positive incidence (PI) in TA (41.7%, P= 0.040) and TC (75.0%, P<0.001), and a significant difference between TA and TC (P=0.036). Immunoreactivity for NF-κB in NT was negative and significantly higher in TC (63.2%, P<0.001),but not in TA (16.7%, P= 0.481). However the PI difference between TA and TC (P=0.003) was significant. Between the histological types of TC, a significantly higher PI was found in undifferentiated thyroid carcinoma (UTC),namely,100% for VEGF and 90.0% for NF-κB. We also found significant positive relation- ships of VEGF and N F - κ B expression with the clinical stage and lymph node metastasis. Furthermore, a significant positive correlation between VEGF and NF-κB expression in TC was observed. CONCLUSION Our data showed that the expression of VEGF and NF- ΚB/P65 was greater in TC and UTC, and documented their significant positive correlations with the clinical stage and lymph node metastasis in TC. 16Ambulatory blood pressure profile and stroke recurrence显示文摘Objectives To establish a new ambulatory blood pressure(ABP)parameter(24-hour ABP profile)and evaluated its performance on stroke outcome in ischaemic stroke(IS)or transient ischaemic attack(TIA)patients.Methods The prospective cohort consisted of 1996 IS/TIA patients enrolled for ABP monitoring and a 3-month follow-up for stroke recurrence as outcome.Profile groups of systolic blood pressure(SBP)were identified via an advanced functional clustering method,and the associations of the profile groups and conventional ABP parameters with stroke recurrence were examined in a Cox proportional hazards model.Results Three discrete profile groups(n=604,781 and 611 in profiles 1,2 and 3,respectively)in 24-hour ambulatory SBP were identified.Profile 1 resembled most to the normal diurnal blood pressure pattern;profile 2 also dropped at night,but climbed earlier and with higher morning surge;while profile 3 had sustained higher nocturnal SBP without significant nocturnal SBP decline.The incidence of stroke recurrence was 2.9%,3.9%and 5.5%in profiles 1,2 and 3,respectively.After adjustment for covariates,profile 3 was significantly associated with higher risk of stroke recurrence with profile 1 as reference(HR 1.76,95%CI:1.00 to 3.09),while no significant difference was observed between profiles 2 and 1(HR 1.22,95%CI:0.66 to 2.25).None of conventional ABP parameters showed significant associations with the outcome.Conclusions Ambulatory 24-hour SBP profile is associated with short-term stroke recurrence.Profiles of ABP may help improve identification of stroke recurrence by capturing the additive effects of individual ABP parameters.Jie Xu Fei Jiang Anxin Wang Hui Zhi Yuan Gao Junping Tian Jinglin Mo Zimo Chen An-Ding Xu Benyan Luo Bo Hu Yuqing Zhang Xingquan Zhao Yilong Wang Hao Li Haipeng Shen Yongjun Wang 2021Stroke & Vascular Neurology2021,6,3:3

Qomolangma (Everest) (ER) (28.02°N, 86.96°E, 6536 m a.s.l.), the Zhadang Glacier on Mt. Nyainqentanglha (NQ) (30.47°N, 90.65°E, 5800m a.s.l.), and the Guoqu Glacier on Mt. Geladaindong (GL) (33.95°N, 91.28°E, 5823m a.s.l.) over the Tibetan Plateau (TP). Variations of microparticle and major ions (e.g. Mg2+, Ca2+) concentrations in snowpits show that the values of the microparticles and ions in the non-monsoon seasons are much higher than those in the monsoon seasons. Annual flux of microparticle deposition at ER is lower than those at NQ and GL, which could be attributed to the long distance away from the possible dust source regions as well as the elevation for ER higher than the others. Compared with other remote areas, microparticle concentrations in the southern TP are much lower than those in the northern TP, but still much higher than those in Greenland and Antarctica. The seasonal and spatial microparticle variations are clearly related to the variations of atmospheric circulation according to the air mass 5-day backward trajectory analyses of HYSPLIT Model. Resultingly, the high microparticle values in snow are mainly attributed to the westerlies and the strong dust storm outbreaks on the TP, while the monsoon circulation brings great amount of precipitation from the Indian Ocean, thus reducing in the aerosol concentrations.ZHANG Yulana 2010Journal of Mountain Science2010,7,1:3
18Doxorubicin-loaded zein in situ gel for interstitial chemotherapy of colorectal cancer显示文摘The aim of this research was to evaluate doxorubicin(DOX)-loaded zein in situ gels,a new drug delivery system in which a liquid state drug can be transformed into semi-solid after intratumoral injection.In vitro release of DOX-loaded zein was investigated and the pharmacokinetics,biodistribution and therapeutic efficacy of these DOX-loaded zein formulations were investigated using BAI B/c nude tumor-bearing mice.In vitro release of DOX from the gels extended up to 7 days.Efficient accumulation of DOX in the tumor with lower drug concentration in blood and normal organs was obtained resulting in effective inhibition of tumor growth and fewer off-target side effects.In conclusion,a DOX-loaded in situ gel was developed with sustained release,enhanced anti-cancer efficacy for colorectal cancer in vivo,and especially with reduced off-target side effects.Ning Shen Jie Hu Linan Zhang Li Zhang Yongjun Sun Yinghua Xie Shaomei Wu Lei Liu Zibin Gao 2012Acta Pharmaceutica Sinica B2012,2,6:3
19ROS-responsive capsules engineered from EGCG-Zinc networks improve therapeutic angiogenesis in mouse limb ischemia显示文摘The successful treatment of limb ischemia requires that promote angiogenesis along with microenvironment improvement.Zinc ions have been reported to stimulate angiogenesis,but application was limited to the toxicity concerns.We hypothesized that zinc based metal-EGCG capsule(EGCG/Zn Ps)can achieve sustained release Zn2+resulting in reduced toxicity and improve angiogenesis as well as the improvement of microenvironment by ROS scavenging of EGCG.The surface morphology,zeta potential,infrared absorbance peaks and zinc ion release profile of the EGCG/Zn Ps were measured.In vitro,EGCG/Zn showed significantly antioxidant,antiinflammatory and induced cell migration effect.In addition,EGCG/Zn Ps enabled the sustained release of zinc ions,which reduced cytotoxicity and enhanced the secretion of vascular endothelial growth factor(VEGF)in vitro and in vivo.In mouse models of limb ischemia,EGCG/Zn Ps promoted angiogenesis and cell proliferation in ischemic tissues.Moreover,EGCG/Zn Ps group exhibited the most significant recovery of limb ischemic score,limb temperature and blood flow than other groups.In conclusion,EGCG/Zn Ps is a safe and promising approach to combine the merit of Zn2+and EGCG,thus enabling the direct application to limb ischemia.Zuoguan Chen Jianwei Duan Yongpeng Diao Youlu Chen Xiaoyu Liang Huiyang Li Yuqing Miao Qing Gao Liang Gui Xiaoli Wang Jing Yang Yongjun Li 2021Bioactive Materials2021,6,1:3
20Acute Ischaemic Stroke Cooperation Group of Endovascular Treatment (ANGEL) registry: study protocol for a prospective, multicentre registry in China显示文摘Background and purpose Endovascular treatment could improve functional outcomes and reduce mortality in patients with intracranial large artery occlusion. This registry aims to evaluate the endovascular treatment delivery and to improve endovascular treatment algorithm in clinical practice for patients with stroke in China. Methods and analysis This multicentric, nationwide, prospective registry plans to include 20 stroke centres and recruit 900 consecutive AIS patients with large- artery occlusion under endovascular treatment. This registry will enrol acute large vessel occlusion patients suitable for endovascular treatment and the inclusion and exclusion criteria. In this study, 90 days functional independence (modified Rankin Scale score ≤2) is the primary efficacy endpoint. The procedural efficacy endpoint of this registry is target artery recanalisation defined by modified Thrombolysis in Cerebral Infarction score 2b or 3 after endovascular therapy. Symptomatic intracranial haemorrhage with 24±3 hours after the procedure is the primary safety endpoint of this registry. Ethics and dissemination Beijing Tiantan Hospital’s Ethics committee and all other participating centres approved the protocol and data collection of Acute Ischaemic Stroke Cooperation Group of Endovascular Treatment registry.
