1PREVALENCE AND RISK FACTORS OF PERIPHERAL ARTERIAL DISEASE IN DIABETIC PATIENTS OVER 50 YEARS OLD IN CHINA显示文摘Objective To investigate the prevalence of peripheral arterial disease (PAD) in China type 2 diabetic patients and to demonstrate the relationships between putative risk factors and PAD. Methods In total 1 397 type 2 diabetic patients aged 50 years and older were enrolled and determined ankle-brachial index (ABI) and brachial-ankle pulse wave velocity (baPWV) in 15 Class III Grade A hospitals in 7 major cities of China. Results Mean patient age was 63.7±8.2 years and mean duration of diabetes mellitus was 9.39±7.4 years. Two hundreds and seventy-two (19.47%) patients were diagnosed as PAD by ABI <0.9, 122 (18.37%) in male and 150 (20.46%) in female. PAD patients had a significantly longer duration of diabetes mellitus, higher hemoglobin Alc, and a significantly lower mean body mass index than non-PAD ones. Aging, smoking, and systolic blood pressure were found to be positively related with the prevalence of PAD. In terms of lipid profiles, no variable was found to relate with PAD. Notably, baPWV showed as the same significant guiding index for PAD, almost matched with ABI. Conclusions PAD is a common complication in China type 2 diabetic patients. 2Hepatectomy with primary closure of common bile duct for hepatolithiasis combined with choledocholithiasis显示文摘AIM: To evaluate the feasibility of hepatectomy and primary closure of common bile duct for intrahepatic and extrahepatic calculi. METHODS: From January 2008 to May 2013, anatomic hepatectomy followed by biliary tract exploration without biliary drainage(non-drainage group) was performed in 43 patients with intrahepatic and extrahepatic calculi. After hepatectomy, flexible choledochoscopy was used to extract residual stones and observe the intrahepatic bile duct and common bile duct(CBD) for determination of biliary stricture and dilatation. Function of the sphincter of Oddi was determined by manometry of the CBD. Primary closure of the CBD without T-tube drainage or bilioenteric anastomosis was performed when there was no biliary stricture or sphincter of Oddi dysfunction. Dexamethasone and anisodamine were intravenously injected 2-3 d after surgery to prevent postoperative retrograde infection due to intraoperative bile duct irrigation, and to maintain relaxation of the sphincter of Oddi, respectively. During the same period, anatomic hepatectomy followed by biliary tract exploration with biliary drainage(drainage group) was performed in 48 patients as the control group. Postoperative complications and hospital stay were compared between the two groups.RESULTS: There was no operative mortality in either group of patients. Compared to intrahepatic and extrabiliary drainage, hepatectomy with primary closure of the CBD(non-drainage) did not increase the incidenceof complications, including residual stones, bile leakage, pancreatitis and cholangitis(P > 0.05). Postoperative hospital stay and costs were nevertheless significantly less in the non-drainage group than in the drainage group. The median postoperative hospital stay was shorter in the non-drainage group than in the drainage group(11.2 ± 2.8 d vs 15.4 ± 2.1 d, P = 0.000). The average postoperative cost of treatment was lower in the non-drainage group than in the drainage group(29325.6 ± 5668.2 yuan vs 32933.3 ± 6235.1 yuan, P = 0.005). 3Expansion and differentiation of human hepatocyte-derived liver progenitor-like cells and their use for the study of hepatotropic pathogens显示文摘The study of pathophysiological mecha nisms in huma n liver disease has bee n constrained by the in ability to expa nd primary hepatocytes in vitro while maintaining proliferative capacity and metabolic function. We and others have previously shown that mouse mature hepatocytes can be con verted to liver progenitor-like cells in vitro with defined chemical factors. Here we describe a protocol achieving efficient conversion of human primary hepatocytes into liver progenitor-like cells (HepLPCs) through delivery of developmentally relevant cues, including NAD +-dependent deacetylase SIRT1 signaling. These HepLPCs could be expanded sign ifica ntly duri ng in vitro passage. The expa nded cells can readily be con verted back into metabolically functional hepatocytes in vitro and upon tran splantati on in vivo. Under three-dime nsional culture conditions, differentiated cells gen erated from HepLPCs regai ned the ability to support infect io n or reactivatio n of hepatitis B virus (HBV). Our work dem on strates the utility of the con versi on betwee n hepatocyte and liver proge nitor-like cells for studying HBV biology and an tiviral therapies. 4Prognosis of acute-on-chronic liver failure patients treated with artificial liver support system显示文摘AIM: To establish a new model for predicting survival in acute-on-chronic liver failure(ACLF) patients treated with an artificial liver support system. METHODS: One hundred and eighty-one ACLF patients who were admitted to the hospital from January 1, 2012 to December 31, 2014 and were treated with an artificial liver support system were enrolled in this retrospective study, including a derivation cohort(n = 113) and a validation cohort(n = 68). Laboratory parameters at baseline were analyzed and correlatedwith clinical outcome. In addition to standard medical therapy, ACLF patients underwent plasma exchange(PE) or plasma bilirubin adsorption(PBA) combined with plasma exchange. For the derivation cohort, KaplanMeier methods were used to estimate survival curves, and Cox regression was used in survival analysis to generate a prognostic model. The performance of the new model was tested in the validation cohort using a receiver-operator curve.RESULTS: The mean overall survival for the derivation cohort was 441 d(95%CI: 379-504 d), and the 90- and 270-d survival probabilities were 70.3% and 58.3%, respectively. The mean survival times of patients treated with PBA plus PE and patients treated with PE were 531 d(95%CI: 455-605 d) and 343 d(95%CI: 254-432 d), respectively, which were significantly different(P = 0.012). When variables with bivariate significance were selected for inclusion into the multivariate Cox regression model, number of complications, age, scores of the model for end-stage liver disease(MELD) and type of artificial liver support system were defined as independent risk factors for survival in ACLF patients. This new prognostic model could accurately discriminate the outcome of patients with different scores in this cohort(P < 0.001). The model also had the ability to assign a predicted survival probability for individual patients. 5Efficacy and Safety of Low Molecular Weight Heparin Prophylaxis for Venous Thromboembolism Following Lumbar Decompression Surgery显示文摘Objective To evaluate the efficacy and safety of low molecular weight heparin (LMWH) prophylaxis for venous thromboembolism (VTE) after lumbar decompression surgery. Methods Patients at high or the highest risk of VTE who underwent lumbar spine surgery in Peking Union Medical College Hospital from January 2004 to April 2011 were included in the present study. All the patients received a half dose of LMWH 6 hours after surgery followed by a full dose LMWH once per day until discharge. We recorded incidences of deep venous thrombosis (DVT), pulmonary embolism (PE), bleeding complications, and medication side effects. Results Seventy-eight consecutive patients were eligible and enrolled in this study. The mean hospital stat was 8.5±4.5 days. No symptomatic DVT, PE, or major bleeding events were observed. One patient developed wound ecchymosis, another developed wound bleeding, four had mild hepatic aminotransferase level elevation, and one developed a suspicious allergic reaction. 7Prevalence and Genotypes of Rotavirus A and Human Adenovirus among Hospitalized Children with Acute Gastroenteritis in Fujian, China, 2009-2017显示文摘Gastroenteritis is an infectious diarrhea that has been considered as an important cause of hospitalizations and death in children aged < 5 years, particularly in developing countries. Unsanitary water, contaminated food, poor hygiene, and inadequate disposal of waste and feces are all risk factors for gastroenteritis, resulting in the higher incidence in developing countries. Gastroenteritis is generally caused by viral infections, among which rotavirus (RV) infections have been reported to be the most common, especially among young children aged < 5 years with acute gastroenteritis in Asia and Africa[1]. Other viruses associated with acute gastroenteritis include human Adenovirus (HAdV), Norovirus, Sapovirus (SaV), human Astrovirus (HAstV), and Aichi virus. Recent research has reported that adenovirus types 40 and 41, belonging to species F, cause gastroenteritis and were therefore termed as enteric adenoviruses. 8Genetic characterization and linkage disequilibrium mapping of resistance to gray leaf spot in maize(Zea mays L.)显示文摘Gray leaf spot(GLS),caused by Cercospora zeae-maydis,is an important foliar disease of maize(Zea mays L.)worldwide,resistance to which is controlled by multiple quantitative trait loci(QTL).To gain insights into the genetic architecture underlying the resistance to this disease,an association mapping population consisting of 161 inbred lines was evaluated for resistance to GLS in a plant pathology nursery at Shenyang in 2010 and 2011.Subsequently,a genome-wide association study,using 41,101 single-nucleotide polymorphisms(SNPs),identified 51 SNPs significantly(P<0.001)associated with GLS resistance,which could be converted into 31 QTL.In addition,three candidate genes related to plant defense were identified,including nucleotidebinding-site/leucine-rich repeat,receptor-like kinase genes similar to those involved in basal defense.Two genic SNPs,PZE-103142893 and PZE-109119001,associated with GLS resistance in chromosome bins 3.07 and 9.07,can be used for marker-assisted selection(MAS)of GLS resistance.These results provide an important resource for developing molecular markers closely linked with the target trait,enhancing breeding efficiency.Liyu Shi Xiangling Lv Jianfeng Weng Hanyong Zhu Changlin Liu Zhuanfang Hao Yu Zhou Degui Zhang Mingshun Li Xiaoke Ci Xinhai Li Shihuang Zhang 2014The Crop Journal2014,2,Z1:9
9Vulnerability of Water Resources and Its Spatial Heterogeneity in Haihe River Basin, China显示文摘To manage water resources effectively, a multiscale assessment of the vulnerability of water resources on the basis of political boundaries and watersheds is necessary. This study addressed issues on the vulnerability of water resources and provided a multiscale comparison of spatial heterogeneity under a climate change background. Using improved quantitative evaluation methods of vulnerability, the Theil index and the Shannon-Weaver index, we evaluated the vulnerability of water resources and its spatial heterogeneity in the Haihe River Basin in four scales, namely, second-class water resource regions(Class ⅡWRRs), third-class water resource regions(Class Ⅲ WRRs), Province-Class ⅡWRRs, and Province-Class Ⅲ WRRs. Results show that vulnerability enhances from the north to south in the different scales, and shows obvious spatial heterogeneity instead of moving toward convergence in multiscale assessment results. Among the Class Ⅱ WRRs, the Tuhai-Majia River is the most vulnerable area, and the vulnerability of the Luanhe River is lower than that of the north of the Haihe River Basin, which in turn is lower than that of the south of the Haihe River Basin. In the scales of Class Ⅲ WRRs and Province-Class Ⅲ WRRs, the vulnerability shows obvious spatial heterogeneity and diversity measured by the Theil index and the Shannon-Weaver index. 11Liuwei Dihuang pill suppresses metastasis by regulating the wnt pathway and disrupting β-catenin/T cell factor interactions in a murine model of triple-negative breast cancer显示文摘OBJECTIVE:To investigate if the Liuwei Dihuang pill(LWDHP)can inhibit metastasis to the liver and lungs in mice bearing triple-negative breast cancer(TNBC),and the molecular mechanism underpinning this action.METHODS:Ninety-nine TNBC bearing-mice were distributed randomly to five groups:control(Con),paclitaxel(PTX),low-dose LWDHP(LLP,2.3 g·kg^-1·d^-1),middle-dose LWDHP(MLP,4.6 g·kg^-1·d^-1)and high-dose LWDHP(HLP,9.2 g·kg^-1·d^-1).The LWDHP were administered(p.o.)to the agonal stage.The morphology of BC cells was observed by hematoxylin&eosin staining.Expression of axin-2,β-catenin,T cell factor(TCF),cyclin-D1 and vascular endothelial growth factor(VEGF)was detected by western blotting or immunofluorescence.β-catenin/TCF-1 interaction was measured using a co-immunoprecipitation assay.RESULTS:After LWDHP treatment,metastasis of BC cells to the lungs and liver was inhibited,expression of axin-2 was increased,expression of TCF-1,β-catenin,cyclin-D1 and VEGF was decreased,andβ-catenin/TCF-1 interaction was disrupted.CONCLUSION:The LWDHP could inhibit metastasis of BC cells to the liver and lungs.The molecular mechanism underlying this action may be regulation of protein expression andβ-catenin/TCF-1 interactions in the Wnt pathway.Zheng Lixiang Zheng Qing Yu Zhipeng Wang Jian Ren Xiaoying Gong Yan Yang Xue Hunag Ping Weng Meizhi Liu Hongning Liu Haizhou 2019Journal of Traditional Chinese Medicine2019,39,6:9
1213-Methyltetradecanoic acid mitigates cerebral ischemia/reperfusion injury显示文摘13-Methyltetradecanoic acid can stabilize cell membrane and have anti-inflammatory,antioxidant and anti-apoptotic effects.Previous studies mainly focused on peripheral nerve injury,but seldom on the central nervous system.We investigated whether these properties of 13-methyltetradecanoic acid have a neuroprotective effect on focal cerebral ischemia/reperfusion injury,and detected the expression of basic fibroblast growth factor and vascular endothelial growth factor.This study established rat models of middle cerebral artery occlusion/reperfusion injury by ischemia for 2 hours and reperfusion for 24 hours.At the beginning of reperfusion,13-methyltetradecanoic acid 10,40 or 80 mg/kg was injected into the tail vein.Results found that various doses of 13-methyltetradecanoic acid effectively reduced infarct volume,mitigate cerebral edema,and increased the m RNA and protein expression of basic fibroblast growth factor and vascular endothelial growth factor at 24 hours of reperfusion.The effect was most significant in the 13-methyltetradecanoic acid 40 and 80 mg/kg groups.The findings suggest that 13-methyltetradecanoic acid can relieve focal ischemia/reperfusion injury immediately after reperfusion,stimulate the upregulation of basic fibroblast growth factor and vascular endothelial growth factor to exert neuroprotective effects.Juan Yu Li-nan Yang Yan-yun Wu Bao-hua Li Sheng-mei Weng Chun-lan Hu Yong-ling Han 2016Neural Regeneration Research2016,11,9:8

15Additive effects of alkaline-earth metals and nickel on the performance of Co/γ-Al_2O_3 in methane catalytic partial oxidation显示文摘Nano-sized γ-alumina (γ-Al2O3) was first prepared by a precipitation method. Then, active component of cobalt and a series of alkaline- earth metal promoters or nickel (Ni) with different contents were loaded on the γ-Al2O3 support. The catalysts were characterized by N2 adsorption-desorption, X-ray diffraction (XRD) and thermogravimetry analysis (TGA). The activity and selectivity of the catalysts in catalytic partial oxidation (CPO) of methane have been compared with Co/γ-Al2O3, and it is found that the catalytic activity, selectivity, and stability are enhanced by the addition of alkaline-earth metals and nickel. The optimal loadings of strontium (Sr) and Ni were 6 and 4 wt%, respectively. 17Anatomic resection of liver segments 6-8 for hepatocellular carcinoma显示文摘AIM:To report the devised anatomic liver resection of segments 6,7 and 8 to improve the resection rate for patients with right liver tumors.METHODS:We performed anatomic liver resection of segments 6,7 and 8 to guarantee the maximum preservation of the remaining normal liver tissue.Segment 5 was determined by two steps of Glissonean pedicle occlusion.And a'┏┛'shaped broken resection line was marked upon the diaphragmatic surface of the liver.Selective right hemihepatic inflow occlusion was used to reduce blood loss during parenchymal transection between segments 6 and 5 and between segments 8 and 5.If needed,total hepatic Glissonean pedicle occlusion was used during parenchymal transection between segment 8 and the left liver.RESULTS:Compared to right hemihepatectomy,the percentage of future liver remnant volume was increased by an average of 13.9%if resection of segments 6,7 and 8 was performed.Resection of segments 6,7 and 8 was completed uneventfully.After hepatectomy,the inflow and outflow of segment 5were maintained.There was no perioperative mortality,postoperative abdominal bleeding or bile leakage in this group.Alpha-fetoprotein(AFP)returned to the normal range within 2 mo after the operation in all the patients.One patient died 383 d postoperatively due to obstructive suppurative cholangitis.One patient suffered from severe liver dysfunction shortly after surgery and had intrahepatic recurrence 4 mo postoperatively.Postoperative lung metastasis was found in one patient.No tumor recurrence was found in the other patients and the parameters including liver function and AFP level were in the normal range.CONCLUSION:Anatomic liver resection of segments6,7 and 8 can be a conventional operation to improve the overall resection rate for hepatocellular carcinoma.Chang-Ku Jia Jie Weng You-Ke Chen Yu Fu 2014World Journal of Gastroenterology2014,20,15:8

19Infection within 2 weeks before liver transplantation closely related to prognosis of posttransplant infection:A single-center retrospective observational study in China显示文摘Background:Infections still represent the main factors influencing morbidity and mortality following liver transplantation.This study aimed to evaluate the incidence and risk factors for infection and survival after liver transplantation.Methods:We retrospectively examined medical records in 210 liver recipients who underwent liver transplantation between April 2015 and October 2017 in our hospital.Clinical manifestations and results of pathogen detection test were used to define infection.We analyzed the prevalence,risk factors and prognosis of patients with infection.Results:The median follow-up was 214 days;the incidence of infection after liver transplantation was 46.7%(n=98)which included pneumonia(43.4%),biliary tract infection(21.9%),peritonitis(21.4%)and bloodstream infection(7.6%).Among the pathogens in pneumonia,the most frequently isolated was Acinetobacter baumanii(23.5%)and Klebsiella pneumoniae(21.2%).Model for end-stage liver disease(MELD)score(OR=1.083,95%CI:1.045–1.123;P<0.001),biliary complication(OR=4.725,95%CI:1.119–19.947;P=0.035)and duration of drainage tube(OR=1.040,95%CI:1.007–1.074;P=0.017)were independent risk factors for posttransplant infection.All-cause mortality was 11.0%(n=23).The prognostic factors for postoperative infection in liver recipients were prior-transplant infection,especially pneumonia within 2 weeks before transplantation.Kaplan-Meier curves of survival showed that recipients within 2 weeks prior infection had a significantly lower cumulative survival rate compared with those without infection(65.2%vs.90.0%;hazard ratio:4.480;P<0.001).Conclusions:Infection,especially pneumonia within 2 weeks before transplantation,complication with impaired renal function and MELD score after 7 days of transplantation was an independent prognostic factor for postoperative infection in liver transplant recipients.Yue Ying Rui-Dong Li Jing-Wen Ai Yi-Min Zhu Xian Zhou Yi-Yi Qian Xin-Chang Chen Xu-Yang Wang Hao-Cheng Zhang Yang Li Shan-Shan Weng Yi-Qi Yu Yi-Feng Tao Yu-Xian Huang Zheng-Xin Wang Wen-Hong Zhang 2020Hepatobiliary & Pancreatic Diseases International2020,19,4:7
20Chondrogenesis mediates progression of ankylosing spondylitis through heterotopic ossification显示文摘Ankylosing spondylitis(AS)is chronic inflammatory arthritis with a progressive fusion of axial joints.Anti-inflammatory treatments such as anti-TNF-αantibody therapy suppress inflammation but do not effectively halt the progression of spine fusion in AS patients.Here we report that the autoimmune inflammation of AS generates a microenvironment that promotes chondrogenesis in spine ligaments as the process of spine fusion.Chondrocyte differentiation was observed in the ligaments of patients with earlystage AS,and cartilage formation was followed by calcification.Moreover,a large number of giant osteoclasts were found in the inflammatory environment of ligaments and on bony surfaces of calcified cartilage.Resorption activity by these giant osteoclasts generated marrow with high levels of active TGF-β,which induced new bone formation in the ligaments.Notably,no Osterix+osteoprogenitors were found in osteoclast resorption areas,indicating uncoupled bone resorption and formation.Even at the late and maturation stages,the uncoupled osteoclast resorption in bony interspinous ligament activates TGF-βto induce the progression of ossification in AS patients.Osteoclast resorption of calcified cartilage-initiated ossification in the progression of AS is a similar pathologic process of acquired heterotopic ossification(HO).Our finding of cartilage formation in the ligaments of AS patients revealed that the pathogenesis of spinal fusion is a process of HO and explained why anti-inflammatory treatments do not slow ankylosing once there is new bone formation in spinal soft tissues.Thus,inhibition of HO formation,such as osteoclast activity,cartilage formation,or TGF-βactivity could be a potential therapy for AS.Tao Yu Jianguo Zhang Wei Zhu Xiao Wang Yun Bai Bin Feng Qianyu Zhuang Chang Han Shengru Wang Qimiao Hu Senbo An Mei Wan Shiwu Dong Jianzhong Xu Xisheng Weng Xu Cao 2021Bone Research2021,9,2:7
