| 1 | Prediction of median survival time in sepsis patients by the SOFA score combined with different predictors显示文摘Background:Sepsis is the leading cause of intensive care unit(ICU)admission.The purpose of this study was to explore the prognostic value of the Sequential Organ Failure Assessment(SOFA)score,the Acute Physiological and Chronic Health Evaluation Ⅱ(APACHE Ⅱ)score,and procalcitonin(PCT),albumin(ALB),and lactate(LAC)levels in patients with sepsis.Methods:Consecutive adult patients with suspected or documented sepsis at ICU admission were recruited.Their basic vital signs and related auxiliary examinations to determine their PCT and ALB levels and APACHE Ⅱ score were recorded at ICU admission,and their LAC levels and SOFA scores were recorded for one week after admission.The influence of these variables on hospital mortality was evaluated.Logistic regression was used to derive the Sepsis Hospital Mortality Score(SHMS),a prediction equation describing the relationship between predictors and hospital mortality.The median survival time was calculated by the Kaplan–Meier method.In the validation group,the kappa value was calculated to evaluate the stability of the derived formula.Results:This study included 894 sepsis patients admitted to 18 ICUs in 16 tertiary hospitals.Patients were randomly assigned to an experimental group(626 cases)and validation group(258 cases).In addition,a nonsurvival group(248 patients)of the experimental group was established according to the outcome at the time of discharge.The hospital mortality rate in the experimental group was 39.6%(248/626).Univariate and multivariate regression analyses revealed that the APACHE Ⅱ score(odds ratio[OR]=1.178),△SOFA(OR=1.186),△LAC(OR=1.157),and SOFA mean score(OR=1.086)were independently associated with hospital mortality.The SHMS was calculated as logit(p)=4.715–(0.164×APACHE Ⅱ)–(0.171×△SOFA)–(0.145×△LAC)–(0.082×SOFA mean).A receiver operating characteristic curve was constructed to further investigate the accuracy of the SHMS,with an area under the curve of 0.851(95%confidence interval[CI]0.821–0.882;p<0.001)for hospital mortality.In the low-risk group and high-risk groups,the corresponding median survival times were 15 days and 11 days,respectively.Conclusion:The APACHE Ⅱ score,△SOFA,△LAC and SOFA mean score were independently associated with hospital mortality in sepsis patients and accurately predicted the hospital mortality rate and median survival time.Data on the median survival time in sepsis patients could be provided to clinicians to assist in the rational use of limited medical resources by facilitating prudent resource allocation.Trial registration:ChiCTR-ECH-13003934,retrospectively registered on August 03,2013. | Wen Li Meiping Wang Bo Zhu Yibing Zhu Xiuming Xi | 2020 | Burns & Trauma2020,8,1: | 7 |
| 2 | Negative regulation of gamma-aminobutyric acid type A receptor on free calcium ion levels following facial nerve injury显示文摘Previous studies have demonstrated that muscarinic,and nicotinic receptors increase free Ca2+ levels in the facial nerve nucleus via various channels following facial nerve injury.However,intracellular Ca2+ overload can trigger either necrotic or apoptotic cell death.Gamma-aminobutyric acid(GABA),an important inhibitory neurotransmitter in the central nervous system,exists in the facial nerve nucleus.It is assumed that GABA negatively regulates free Ca2+ levels in the facial nerve nucleus.The present study investigated GABA type A(GABAA) receptor expression in the facial nerve nucleus in a rat model of facial nerve injury using immunohistochemistry and laser confocal microscopy,as well as the regulatory effects of GABAA receptor on nicotinic receptor response following facial nerve injury.Subunits α1,α3,α5,β1,β2,δ,and γ3 of GABAA receptors were expressed in the facial nerve nucleus following facial nerve injury.In addition,GABAA receptor expression significantly inhibited the increase in nicotinic receptor-mediated free Ca2+ levels in the facial nerve nucleus following facial nerve injury in a concentration-dependent fashion.These results suggest that GABAA receptors exhibit negative effects on nicotinic receptor responses following facial nerve injury. | Fugao Zhu Dawei Sun Yanqing Wang Rui Zhou Junfeng Wen Xiuming Wan Yanjun Wang Banghua Liu | 2010 | Neural Regeneration Research2010,5,22: | 1 |
| 3 | Rational construction of metal–base synergetic sites on Au/Mg-beta catalyst for selective aerobic oxidation of 5-hydroxymethylfurfural显示文摘The selective aerobic oxidation of biomass-derived 5-hydroxymethylfurfural to 2,5-furandicarboxylic acid (FDCA, a potential renewable substitution of fossil-based terephthalic acid to produce polyethylene 2,5-furandicarboxylate plastic) is an appealing transformation for constructing eco-friendly and sustainable chemical processes. Au supported catalysts have showed encouraging performances for this well-received conversion, whose catalytic behavior was greatly affected by the adopted support derived from the existence of metal-support interactions. Herein, a series of Mg-Beta zeolites were hydrothermally synthesized via developed structural reconstruction, which were employed as basic supports for Au catalysts to construct bifunctional catalysts. The relationship between structure (Au particle size, basicity within zeolites and Auδ+ contents) and FDCA yield was concretely established. The conclusion was made that the utilization of Mg-Beta zeolites with strong basicity as the support could not only improve the FDCA yield but also decrease the amount of additional base. Furthermore, the possible reaction mechanism was also proposed via tracking time-dependent variations of corresponding organics and controlled experiment. In addition, γ-aminobutyric acid A (GABAA) receptors have been shown to negatively regulate free calcium ion levels in the facial nucleus by inhibiting nicotine receptors. The present study investigated the influence of GABAA, γ-aminobutyric acid B (GABAB) and C (GABAC) receptors on muscarinic receptors in rats with facial nerve injury by confocal laser microscopy. GABAA and GABAB receptors exhibited significant dose-dependent inhibitory effects on increased muscarinic receptor-mediated free calcium ion levels following facial nerve injury. | 11 | Characteristics,classification and KNN-based evaluation of paleokarst carbonate reservoirs:A case study of Feixianguan Formation in northeastern Sichuan Basin,China显示文摘The Feixianguan Formation reservoirs in northeastern Sichuan are mainly a suite of carbonate platform deposits.The reservoir types are diverse with high heterogeneity and complex genetic mechanisms.Pores,vugs and fractures of different genetic mechanisms and scales are often developed in association,and it is difficult to classify reservoir types merely based on static data such as outcrop observation,and cores and logging data.In the study,the reservoirs in the Feixianguan Formation are grouped into five types by combining dynamic and static data,that is,karst breccia-residual vuggy type,solution-enhanced vuggy type,fractured-vuggy type,fractured type and matrix type(non-reservoir).Based on conventional logging data,core data and formation microscanner image(FMI)data of the Qilibei block,northeastern Sichuan Basin,the reservoirs are classified in accordance with fracture-vug matching relationship.Based on the principle of cluster analysis,K-Nearest Neighbor(KNN)classification templates are established,and the applicability of the model is verified by using the reservoir data from wells uninvolved in modeling.Following the analysis of the results of reservoir type discrimination and the production of corresponding reservoir intervals,the contributions of various reservoir types to production are evaluated and the reliability of reservoir type classification is verified.The results show that the solution-enhanced vuggy type is of high-quality sweet spot reservoir in the study area with good physical property and high gas production,followed by the fractured-vuggy type,and the fractured and karst breccia-residual vuggy types are the least promising. | Yang Ren Wei Wei Peng Zhu Xiuming Zhang Keyong Chen Yisheng Liu | 2023 | Energy Geoscience2023,4,3: | 0 |
