| 1 | Traditional herbal medicine in preventing recurrence after resection of small hepatocellular carcinoma: a multicenter randomized controlled trial显示文摘BACKGROUND: Disease recurrence is a main challenge in treatment of hepatocellular carcinoma (HCC). There is no generally accepted method for preventing recurrence of HCC after resection. OBJECTIVE: To compare the efficacy of a traditional herbal medicine (THM) regimen and transarterial chemoembolization (TACE) in preventing recurrence in post-resection patients with small HCC. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS: This is a multicenter, open-label, randomized, controlled study, which was undertaken in five centers of China. A total of 379 patients who met the eligibility criteria and underwent randomization were enrolled in this trial. One hundred and eighty-eight patients were assigned to the THM group and received Cinobufacini injection and Jiedu Granule, and the other 191 patients were assigned to the TACE group and received one single course of TACE. MAIN OUTCOME MEASURES: Primary outcome measures were the annual recurrence rate and the time to recurrence. Incidence of adverse events was regarded as the secondary outcome measure. RESULTS: Among the 364 patients who were included in the intention-to-treat analysis, 67 patients of the THM group and 87 of the TACE group had recurrence, with a hazard ratio of 0.695 (P = 0.048). Median recurrence-free survival of the patients in the THM and TACE groups was 46.89 and 34.49 months, respectively. Recurrence rates at 1, 2 and 3 years were 17.7%, 33.0% and 43.5% for the THM group, and 28.8%, 42.5% and 54.0% for the TACE group, respectively (P = 0.026). Multivariate analysis indicated that the THM regimen had a big advantage for prolonging the recurrence-free survival. Adverse events were mild and abnormality of laboratory indices of the two groups were similar. CONCLUSION: In comparison with TACE therapy, the THM regimen was associated with diminished risk of recurrence of small-sized HCC after resection, with comparable adverse events. | 2 | Prevalence and evolution of drug resistance HIV-1 variants in Henan, China显示文摘To understand the prevalence and evolution of drug resistant HIV strains in Henan China after the implementation of free antiretroviral therapy for AIDS patients. 45 drug nave AIDS patients, 118 AIDS patients who received three months antiretroviral therapy and 124 AIDS patients who received six months antiretroviral treatment were recruited in the southern part of Henan province. Information on general condition, antiretroviral medicines, adherence and clinical syndromes were collected by face to face interview. Meanwhile, 14ml EDTA anticoagulant blood was drawn. CD4/CD8 T cell count, viral load and genotypic drug resistance were tested. The rates of clinical improvement were 55.1% and 50.8% respectively three months and six months after antiretroviral therapy. The mean CD4 cell count after antiretroviral therapy was significantly higher than in drug nave patients. The prevalence rate of drug resistant HIV strains were 13. 9%, 45.4% and 62.7% in drug nave patients, three month treatment patients and six month treatment patients, respectively. The number of resistance mutation codons and the frequency of mutations increased significantly with continued antiretroviral therapy. The mutation sites were primarily at the 103, 106 and 215 codons in the three-month treatment group and they increased to 15 codon mutations in the six-month treatment group. From this result, the evolution of drug resistant strains was inferred to begin with the high level NNRTI resistant strain, and then develop low level resistant strains to NRTIs. The HIV strains with high level resistance to NVP and low level resistance to AZT and DDI were highly prevalent because of the AZT+DDI+NVP combination therapy. These HIV strains were also cross resistant to DLV, EFV, DDC and D4T. Poor adherence to therapy was believed to be the main reason for the emergence and prevalence of drug resistant HIV strains. The prevalence of drug resistant HIV strains was increased with the continu- ation of antiretroviral therapy in the southern part of Henan province. | 4 | 2D DEM simulation of particle mixing in rotating drum: A parametric study显示文摘Mixing behaviors of equal-sized glass beads in a rotating drum were investigated by both DEM simulations and experiments. The experiments indicated that higher rotation speed can significantly enhance mixing. The particle profiles predicted by 2D DEM simulation were compared with the experimental results from a quasi-2D drum, showing inconsistency due to reduction of contacts in the single-layer 2D simulation which makes the driving friction weaker than that in the quasi-2D test, better results could be reached by specifying a higher frictional coefficient between the particles and the cylinder wall. In order to explore the influences of physical properties (density, size or friction) on mixing behavior, numerical 2D simulations were carried out systematically, in which one examined specific property being examined was exaggerated while the others were kept the same as that in the control group. The DEM simulations reveal that particle density and size are the dominating factors affecting mixing behaviors, while the effect of frictional coefficient is less significant. | 5 | Exosomes derived from bone marrow mesenchymal stem cells protect the injured spinal cord by inhibiting pericyte pyroptosis显示文摘Mesenchymal stem cell(MSC)transplantation is a promising treatment strategy for spinal cord injury,but immunological rejection and possible tumor formation limit its application.The therapeutic effects of MSCs mainly depend on their release of soluble paracrine factors.Exosomes are essential for the secretion of these paracrine effectors.Bone marrow mesenchymal stem cell-derived exosomes(BMSC-EXOs)can be substituted for BMSCs in cell transplantation.However,the underlying mechanisms remain unclear.In this study,a rat model of T10 spinal cord injury was established using the impact method.Then,30 minutes and 1 day after spinal cord injury,the rats were administered 200μL exosomes via the tail vein(200μg/mL;approximately 1×106 BMSCs).Treatment with BMSC-EXOs greatly reduced neuronal cell death,improved myelin arrangement and reduced myelin loss,increased pericyte/endothelial cell coverage on the vascular wall,decreased bloodspinal cord barrier leakage,reduced caspase 1 expression,inhibited interleukin-1βrelease,and accelerated locomotor functional recovery in rats with spinal cord injury.In the cell culture experiment,pericytes were treated with interferon-γand tumor necrosis factor-α.Then,Lipofectamine 3000 was used to deliver lipopolysaccharide into the cells,and the cells were co-incubated with adenosine triphosphate to simulate injury in vitro.Pre-treatment with BMSC-EXOs for 8 hours greatly reduced pericyte pyroptosis and increased pericyte survival rate.These findings suggest that BMSC-EXOs may protect pericytes by inhibiting pyroptosis and by improving blood-spinal cord barrier integrity,thereby promoting the survival of neurons and the extension of nerve fibers,and ultimately improving motor function in rats with spinal cord injury.All protocols were conducted with the approval of the Animal Ethics Committee of Zhengzhou University on March 16,2019. | Yan Zhou Lu-Lu Wen Yan-Fei Li Kai-Min Wu Ran-Ran Duan Yao-Bing Yao Li-Jun Jing Zhe Gong Jun-Fang Teng Yan-Jie Jia | 2022 | Neural Regeneration Research2022,17,1: | 15 |
| 6 | Epidemiological investigation of Helicobacter pylori infection in elderly people in Beijing显示文摘BACKGROUND The Helicobacter pylori(H.pylori)infection rate in China is approximately 50%.H.pylori is a pathogenic factor of peptic ulcer and chronic gastritis.In addition,H.pylori infection may also be associated with a variety of cardiovascular diseases in elderly people,such as arteriosclerosis,coronary heart disease,and cerebral infarction,having deleterious effect on their health.With the aging of the population,the disease characteristics of the elderly population have been increasingly valued by the whole society.We conducted an epidemiological survey of H.pylori infection among elderly people in Beijing to provide a basis for health management of H.pylori infection.AIM To understand the epidemiological characteristics of H.pylori infection in elderly people in Beijing.METHODS A total of 1090 elderly people aged more than 60 years from different parts of Beijing(urban and rural areas)were selected using the random cluster sampling method.Structured questionnaires were completed during home visits and the 13C-urea breath test was conducted for H.pylori detection.RESULTS The prevalence of H.pylori infection was 46.5%(507/1090).The infection rate in men was 51.8%,which was significantly higher than that in women(42.5%;P<0.05).The H.pylori infection rate in illiterate people was significantly higher than that in literate persons(53.5%vs 44.8%,P<0.05).The total infection rate of H.pylori gradually increased with age and the difference was statistically significant(P<0.01).The H.pylori infection rate in smokers was significantly higher than that in non-smokers and those who had quit smoking(P<0.05).CONCLUSION The prevalence of H.pylori infection among elderly people is 46.5%and the infection rate gradually increases with age.Sex,education level,age,and smoking were determined to be H.pylori infection risk factors.The relationship of H.pylori infection with region,occupation,drinking,and diet structure needs to be further studied. | Hong-Ming Zhu Bang-Yi Li Zhe Tang Jing She Xue-Ying Liang Li-Kou Dong Mei Zhang | 2020 | World Journal of Clinical Cases2020,8,11: | 11 |

| 8 | Progressive LiDAR Adaptation for Road Detection显示文摘Despite rapid developments in visual image-based road detection, robustly identifying road areas in visual images remains challenging due to issues like illumination changes and blurry images. To this end, LiDAR sensor data can be incorporated to improve the visual image-based road detection,because LiDAR data is less susceptible to visual noises. However,the main difficulty in introducing LiDAR information into visual image-based road detection is that LiDAR data and its extracted features do not share the same space with the visual data and visual features. Such gaps in spaces may limit the benefits of LiDAR information for road detection. To overcome this issue, we introduce a novel Progressive LiDAR adaptation-aided road detection(PLARD) approach to adapt LiDAR information into visual image-based road detection and improve detection performance. In PLARD, progressive LiDAR adaptation consists of two subsequent modules: 1) data space adaptation, which transforms the LiDAR data to the visual data space to align with the perspective view by applying altitude difference-based transformation; and 2) feature space adaptation, which adapts LiDAR features to visual features through a cascaded fusion structure. Comprehensive empirical studies on the well-known KITTI road detection benchmark demonstrate that PLARD takes advantage of both the visual and LiDAR information, achieving much more robust road detection even in challenging urban scenes. | 9 | Nitrous oxide emissions following seasonal freeze-thaw events from arable soils in Northeast China显示文摘Seasonal soil freeze-thaw events may enhance soil nitrogen transformation and thus stimulate nitrous oxide(N_2O)emissions in cold regions.However,the mechanisms of soil N_2O emission during the freeze-thaw cycling in the field remain unclear.We evaluated N_2O emissions and soil biotic and abiotic factors in maize and paddy fields over 20 months in Northeast China,and the structural equation model(SEM)was used to determine which factors affected N_2O production during non-growing season.Our results verified that the seasonal freeze-thaw cycles mitigated the available soil nitrogen and carbon limitation during spring thawing period,but simultaneously increased the gaseous N_2O-N losses at the annual time scale under field condition.The N_2O-N cumulative losses during the non-growing season amounted to 0.71 and 0.55 kg N ha^(–1) for the paddy and maize fields,respectively,and contributed to 66 and 18%of the annual total.The highest emission rates(199.2–257.4μg m^(–2) h^(–1))were observed during soil thawing for both fields,but we did not observe an emission peak during soil freezing in early winter.Although the pulses of N_2O emission in spring were short-lived(18 d),it resulted in approximately80%of the non-growing season N_2O-N loss.The N_2O burst during the spring thawing was triggered by the combined impact of high soil moisture,flush available nitrogen and carbon,and rapid recovery of microbial biomass.SEM analysis indicated that the soil moisture,available substrates including NH_4^+and dissolved organic carbon(DOC),and microbial biomass nitrogen(MBN)explained 32,36,16 and 51%of the N_2O flux variation,respectively,during the non-growing season.Our results suggested that N_2O emission during the spring thawing make a vital contribution of the annual nitrogen budget,and the vast seasonally frozen and snow-covered croplands will have high potential to exert a positive feedback on climate change considering the sensitive response of nitrogen biogeochemical cycling to the freeze-thaw disturbance. | CHEN Zhe YANG Shi-qi ZHANG Ai-ping JING Xin SONG Wei-min MI Zhao-rong ZHANG Qing-wen WANG Wen-ying YANG Zheng-li | 2018 | Journal of Integrative Agriculture2018,17,1: | 8 |
| 10 | Richness of sputum microbiome in acute exacerbations of eosinophilic chronic obstructive pulmonary disease显示文摘Background:The eosinophilic chronic obstructive pulmonary disease(COPD)is known to be more sensitive to corticosteroid.The sputum microbiome has been shown to affect COPD prognosis,but its role in acute exacerbations of eosinophilic COPD is unclear.This study aimed to investigate the dynamic changes of the airway microbiome in patients with acute exacerbations of eosinophilic COPD.Methods:Fifty-seven patients with acute exacerbations of COPD from the First Affiliated Hospital of Guangxi Medical University between June 2017 and June 2018 were divided into two groups.Patients with eosinophils≥300 cells/mL in the peripheral venous blood were assigned to the eosinophilic group(Eos)and the rest to the non-eosinophilic group(Noneos).All patients received similar treatment including inhaled budesonide according to the guidelines.The induced sputum microbiome was analyzed on the 1st and 7th day of treatment using the 16S ribosomal RNA(rRNA)method.The levels of interleukin(IL)-6 and IL-8 were measured in the plasma and the sensitivity to corticosteroids was determined in isolated peripheral blood mononuclear cells.Quantitative data were compared between the two groups using the independent samples t test or Mann-Whitney U test.Categorical data were evaluated using Chi-squared test or Fisher’s exact test.Results:Twenty-six patients were classified into Eos group and 31 patients were classified into Noneos group.Prior to treatment,the alpha diversity(Shannon index)(2.65±0.63 vs.2.56±0.54,t=0.328,P=0.747)and the structure of the sputum microbiome were similar in the Eos group and the Noneos group.After 7 days of treatment,alpha diversity increased in both groups,while the microbiome richness(Ace index)was significantly lower in the Eos group(561.87±109.13 vs.767.88±148.48,t=-3.535,P=0.002).At the same time,IL-6(12.09±2.85 pg/mL vs.15.54±2.45 pg/mL,t=-4.913,P<0.001)and IL-8(63.64±21.69 pg/mL vs.78.97±17.13 pg/mL,t=-2.981,P=0.004)decreased more significantly in the Eos group,and the percentages of inhibition of IL-8 at dexamethasone concentrations 10-8 to 10-6 mol/L were significantly higher in the Eos group than those in the Noneos group(all P<0.05).Conclusions:The induced sputum microbiome richness decreased more significantly following treatment in the Eos patients compared to the Noneos patients.The lower plasma inflammatory factor levels and the higher percentage of inhibition of IL-8 might be due to higher corticosteroid sensitivity in Eos patients. | Yu-Jing Qi Xue-Jiao Sun Zhe Wang Yan-Fei Bin Ying-Hua Li Xiao-Ning Zhong Jing Bai Jing-Min Deng Zhi-Yi He | 2020 | Chinese Medical Journal2020,,5: | 8 |
| 11 | Protection of Mice from Lethal Endotoxemia by Chimeric Human BPI-Fcγ1 Gene Delivery显示文摘To evaluate the potentiality of applying gene therapy to endotoxemia in high-risk patients, we investigated the effects of transferring an adeno-associated virus serotype 2 (AAV2)-mediated BPI-Fcγ1 gene on protecting mice from challenge of lethal endotoxin. The chimeric BPI-Fcγ1 gene consists of two parts, one encods functional N-terminus (1 to 199 amino acidic residues) of human BPI, which is a bactericidal/permeability-increasing protein, and the other encodes Fc segment of human immunoglobulin G1(Fcγ1). Our results indicated that the target protein could be expressed and secreted into the serum of the gene-transferred mice. After lethal endotoxin challenge, the levels of endotoxin and TNF-αin the gene-transferred mice were decreased. The survival rate of the BPI-Fcγ1 gene-transferred mice was markedly increased. | 12 | Characteristics of fecal microbial communities in patients with non-anastomotic biliary strictures after liver transplantation显示文摘AIM To explore the possible relationship between fecal microbial communities and non-anastomotic stricture(NAS) after liver transplantation(LT).METHODS A total of 30 subjects including 10 patients with NAS, 10 patients with no complications after LT, and 10 nonLT healthy individuals were enrolled. Fecal microbial communities were assessed by the 16 S r RNA gene sequencing technology.RESULTS Different from the uncomplicated and healthy groups, unbalanced fecal bacterium ratio existed in patients with NAS after LT. The results showed that NAS patients were associated with a decrease of Firmicutes and Bacteroidetes and an increase of Proteobacteria at the phylum level, with the proportion-ratio imbalance between potential pathogenic families including Enterococcaceae, Streptococcaceae, Enterobacteriaceae, Pseudomonadaceae and dominant families including Bacteroidaceae. | 15 | Modified Xiaochaihu Decoction for gastroesophageal reflux disease:A randomized double-simulation controlled trial显示文摘BACKGROUND Gastroesophageal reflux disease(GERD)has a high prevalence worldwide,and its incidence is increasing annually.Modified Xiaochaihu Decoction(MXD)could relieve the symptoms of GERD,but the effects of MXD on GERD manifestations and relapse prevention need to be further explained.Therefore,we performed a prospective,double-blind,and double-simulation study.AIM To verify the efficacy of MXD for GERD and its effect on esophageal motility.METHODS Using randomization,double-blinding,and a simulation design,288 participants with GERD were randomized to the treatment group and control group and received herbs(MXD)plus omeprazole simulation and omeprazole plus herbs simulation,respectively,for 4 wk.The GERD-Q scale score and esophageal manometry were measured at baseline,after treatment,and at 1 mo and 3 mo follow-up visits when medication was complete to evaluate recurrence indicators.RESULTS The GERD-Q scale score in both groups decreased significantly compared to those before treatment(P<0.01).However,no significant difference was observed between the two groups(P>0.05).Esophageal manometry showed that participants with lower esophageal sphincter pressure reduction and the proportion of ineffective swallowing(more than 50%)improved in both groups from baseline(P<0.01),especially in the treatment group(P<0.05).The percentage of small intermittent contractions,large intermittent contractions,and increased pre-phase contractions in the treatment group significantly improved compared with baseline(P<0.05)but did not improve in the control group(P>0.05).There was no significant difference between the groups after treatment(P>0.05).The percentage of weak esophageal contractility(distal contractile integral<450 mmHg·s·cm),improved in both groups(P<0.01),but no significant difference was observed between the groups after treatment(P>0.05).The relapse rate in the treatment group was lower than that in the control group at the 1 mo(P<0.01)and 3 mo follow-up(P<0.05).CONCLUSION MXD has a similar therapeutic effect to omeprazole in mild-to-moderate GERD.The therapeutic effect may be related to increased pressure in the lower esophageal sphincter and reduced ineffective swallowing. | Zhe Li Lin Tao Sheng-Sheng Zhang Xiao-Hong Sun Su-Ning Chen Jing Wu | 2021 | World Journal of Gastroenterology2021,27,28: | 6 |

The effect of traditional antibiotic therapy is hindered by drug-resistant bacteria and unneutralizable endotoxin. Few effective methods can protect high risk patients from bacterial infection. This study explored the protection of adeno-associated virus 2 (AAV2)-bacteriacidal permeability increasing protein 700 (BPI700) -fragment crystallizable gamma one 700 (Fc(1700) chimeric gene transferred mice against the minimal lethal dose (MLD) of E.coli and application of gene therapy for bacterial infection . Methods After AAV2-BPI700-Fc(1700 virus transfection,dot blotting and Western blotting were used to detect the target gene products in Chinese hamster ovary-K1 cells (CHO-K1cells). Reverse transcription-polymerase chain reaction and immunohistochemical assay were carried out to show the target gene expression in mice. Modified BPI-enzyme linked immunosorbent assay was used to identify the target gene products in murine serum. The protection of BPI700-Fc(1700 gene transferred mice was examined by survival rate after MLD E. coli challenge. Colony forming unit (CFU) count, limulus amebocyte lysate kit and cytokine kit were used to quantify the bacteria, the level of endotoxin, and proinflammatory cytokine. Results BPI1-199-Fc(1 protein was identified in the CHO-K1 cell culture supernatant, injected muscles and serum of the gene transferred mice. After MLD E. coli challenge, the survival rate of AAV2-BPI700-Fc(1700 gene transferred mice (36.7%) was significantly higher than that of AAV2-enhanced green fluorescent protein (AAV2- EGFP) gene transferred mice (3.3%) and PBS control mice (5.6%). The survival rate of AAV2-BPI700-Fc(1700 gene transferred mice treated with cefuroxime sodium was 65.0%. The bacterium number in main viscera, the levels of endotoxin and proinflammatory cytokine (tumor necrcsis factor-α and interleukin-1β) in serum of the AAV2-BPI700-Fc(1700 gene transferred mice were markedly lower than that of PBS control mice (P<0.01). Conclusions AAV2-BPI700-Fc(1700 gene transferred mice can resist MLD E. coli infection through expressing BPI1-199-Fc(1 protein. | 18 | Pirfenidone ameliorates silica-induced lung inflammation and fibrosis in mice by inhibiting the secretion of interleukin-17A显示文摘Silicosis is a global occupational disease characterized by lung dysfunction, pulmonary inflammation, and fibrosis, for which there is a lack of effective drugs. Pirfenidone has been shown to exert anti-inflammatory and anti-fibrotic properties in the lung. However, whether and how pirfenidone is effective against silicosis remains unknown. Here, we evaluated the efficacy of pirfenidone in the treatment of early and advanced silicosis in an experimental mouse model and explored its potential pharmacological mechanisms. We found that pirfenidone alleviated silica-induced lung dysfunction, secretion of inflammatory cytokines (TNF-α, IL-1β, IL-6) and deposition of fibrotic proteins (collagen I and fibronectin) in both early and advanced silicosis models. Moreover, we observed that both 100 and 200 mg/kg pirfenidone can effectively treat early-stage silicosis, while 400 mg/kg was recommended for advanced silicosis. Mechanistically, antibody array and bioinformatic analysis showed that the pathways related to IL-17 secretion, including JAK-STAT pathway, Th17 differentiation, and IL-17 pathway, might be involved in the treatment of silicosis by pirfenidone. Further in vivo experiments confirmed that pirfenidone reduced the production of IL-17A induced by silica exposure via inhibiting STAT3 phosphorylation. Neutralizing IL-17A by anti-IL-17A antibody improved lung function and reduced pulmonary inflammation and fibrosis in silicosis animals.
