| 3 | Application of internal fixation of steel-wire limited loop in early Achilles tendon rupture显示文摘Objective:To explore the clinical effect and safety of internal fixation of steel-wire limited loop in early Achilles tendon rupture.Methods:Seventy-six patients respectively with early transected and avulsed types of Achilles tendon rupture were selected and treated with internal fixation of steel-wire limited loop.The patients began to take exercise for their lower limbs through continous passive motion as early as possible after surgical repair,and the loops were removed after 3-5 months.Six months later,the condition of complications including Achilles tendon re-rupture,wound fistula,wound infection and skin necrosis,cutaneous sensation in sural nerve dominance region,time back to preinjury work or learning as well as time to physical activities were observed.One year later,the therapeutic effect was evaluated,and the maximum circumferences of bilateral legs and raptured plane circumferences of Achilles tendon were measured.Results:The wound of all patients healed well,no complications like Achilles tendon re-rupture,wound fistula,wound infection and skin necrosis occured,and the cutaneous sensation in sural nerve dominance region was normal.The mean time back to preinjury work or learning as well as to pysical activities of all patients were respectively 10 and 22 weeks.Seventy out of 76 patients(92.1%) achieved an excellent effect,and 6(7.9%) good effect.The excellent and good rate came up to 100%.The maximum circumference in the affected leg decreased to 2 mm averagely compared with the offside,while the ruptured plane circumferences of Achilles tendon in the affected side increased to 2.2 mm compared with the offside.Conclusions:Kor early Achilles tendon rupture,internal fixation of steel-wire limited loop can recover the ankle function better,return to the preinjury state in the shortest lime,and has few complications. | Zhe Chen Jia-Sen Wei Zhao-Yang Hou Jiong Hu Yan-Guang Cao Qi-Xin Chen | 2013 | Asian Pacific Journal of Tropical Medicine2013，6，11： | 10 |

| 6 | Factors associated with upstaging in patients preoperatively diagnosed with ductal carcinoma in situ by core needle biopsy显示文摘Objective: Patients preoperatively diagnosed with ductal carcinoma in situ(DCIS) by core needle biopsy(CNB) exhibit a significant risk for upstaging on final pathology, which leads to major concerns of whether axillary staging is required at the primary operation. The present study aimed to identify clinicopathological factors associated with upstaging in patients preoperatively diagnosed with DCIS by CNB.Methods: The present study enrolled 604 patients(cN0 M0) with a preoperative diagnosis of pure DCIS by CNB, who underwent axillary staging between August 2006 and December 2015, at Fudan University Shanghai Cancer Center(Shanghai, China).Predictive factors of upstaging were analyzed retrospectively.Results: Of the 604 patients, 20.03%(n = 121) and 31.95%(n = 193) were upstaged to DCIS with microinvasion(DCISM) and invasive breast cancer(IBC) on final pathology, respectively. Larger tumor size on ultrasonography(> 2 cm) was independently associated with upstaging [odds ratio(OR) 1.558, P = 0.014]. Additionally, patients in lower breast imaging reporting and data system(BI-RADS) categories were less likely to be upstaged(4 B vs. 5: OR 0.435, P = 0.002;4 C vs. 5: OR 0.502, P = 0.001). Overall,axillary metastasis occurred in 6.79%(n = 41) of patients. Among patients with axillary metastasis, 1.38%(4/290), 3.31%(4/121)and 17.10%(33/193) were in the DCIS, DCISM, and IBC groups, respectively.Conclusions: For patients initially diagnosed with DCIS by CNB, larger tumor size on ultrasonography(> 2 cm) and higher BIRADS category were independent predictive factors of upstaging on final pathology. | 7 | Prognostic Nomogram for Patients with Hepatitis E Virus-related Acute Liver Failure:A Multicenter Study in China显示文摘Background and Aims:Timely and effective assessment scoring systems for predicting the mortality of patients with hepatitis E virus-related acute liver failure(HEV-ALF)are urgently needed.The present study aimed to establish an effective nomogram for predicting the mortality of HEV-ALF patients.Methods:The nomogram was based on a cross-sectional set of 404 HEV-ALF patients who were identified and enrolled from a cohort of 650 patients with liver failure.To compare the performance with that of the model for end-stage liver disease(MELD)scoring and CLIF-Consortiumacute-on-chronic liver failure score(CLIF-C-ACLFs)models,we assessed the predictive accuracy of the nomogram using the concordance index(C-index),and its discriminative ability using time-dependent receiver operating characteristics(td-ROC)analysis,respectively.Results:Multivariate logistic regression analysis of the development set carried out to predict mortality revealed that γ-glutamyl transpeptidase,albumin,total bilirubin,urea nitrogen,creatinine,international normalized ratio,and neutrophil-to-lymphocyte ratio were independent factors,all of which were incorporated into the new nomogram to predict the mortality of HEV-ALF patients.The area under the curve of this nomogram for mortality prediction was 0.671(95%confidence interval:0.602-0.740),which was higher than that of the MELD and CLIF-C-ACLFs models.Moreover,the td-ROC and decision curves analysis showed that both discriminative ability and threshold probabilities of the nomogram were superior to those of the MELD and CLIF-C-ACLFs models.A similar trend was observed in the validation set.Conclusions:The novel nomogram is an accurate and efficient mortality prediction method for HEV-ALF patients. | Jian Wu Cuifen Shi Xinyu Sheng Yanping Xu Jinrong Zhang Xinguo Zhao Jiong Yu Xinhui Shi Gongqi Li Hongcui Cao Lanjuan Li | 2021 | Journal of Clinical and Translational Hepatology2021，9，6： | 2 |
| 8 | Dynamic changes of key metabolites during liver fibrosis in rats显示文摘BACKGROUND Fibrosis is the single most important predictor of significant morbidity and mortality in patients with chronic liver disease.Established non-invasive tests for monitoring fibrosis are lacking,and new biomarkers of liver fibrosis and function are needed.AIM To depict the process of liver fibrosis and look for novel biomarkers for diagnosis and monitoring fibrosis progression.METHODS CCl4 was used to establish the rat liver fibrosis model.Liver fibrosis process was measured by liver chemical tests,liver histopathology,and Masson’s trichrome staining.The expression levels of two fibrotic markers includingα-smooth muscle actin and transforming growth factorβ1 were assessed using immunohistochemistry and real-time polymerase chain reaction.Dynamic changes in metabolic profiles and biomarker concentrations in rat serum during liver fibrosis progression were investigated using ultra-performance liquid chromatography coupled to quadrupole time-of-flight mass spectrometry.The discriminatory capability of potential biomarkers was evaluated by receiver operating characteristic(ROC)curve analysis.RESULTS To investigate the dynamic changes of metabolites during the process of liver fibrosis,sera from control and fibrosis model rats based on pathological results were analyzed at five different time points.We investigated the association of liver fibrosis with 21 metabolites including hydroxyethyl glycine,L-threonine,indoleacrylic acid,β-muricholic acid(β-MCA),cervonoyl ethanolamide(CEA),phosphatidylcholines,and lysophosphatidylcholines.Two metabolites,CEA andβ-MCA,differed significantly in the fibrosis model rats compared to controls(P<0.05)and showed prognostic value for fibrosis.ROC curve analyses performed to calculate the area under the curve(AUC)revealed that CEA andβ-MCA differed significantly in the fibrosis group compared to controls with AUC values exceeding 0.8,and can clearly differentiate early stage from late stage fibrosis or cirrhosis.CONCLUSION This study identified two novel biomarkers of fibrosis,CEA andβ-MCA,which were effective for diagnosing fibrosis in an animal model. | Jiong Yu Jian-Qin He De-Ying Chen Qiao-Ling Pan Jin-Feng Yang Hong-Cui Cao Lan-Juan Li | 2019 | World Journal of Gastroenterology2019，25，8： | 2 |

| 20 | PSPC1 regulates CHK1 phosphorylation through phase separation and participates in mouse oocyte maturation显示文摘Liquid–liquid phase separation (LLPS) underlies the formation of membraneless compartments in mammal cells. However, there are few reports that focus on the correlation of mouse oocyte maturation with LLPS. Previous studies have reported that paraspeckle component 1 (PSPC1) is related to the occurrence and development of tumors, but whether PSPC1 functions in mouse oocyte maturation is still unclear. Sequence analysis of PSPC1 protein showed that it contains a prion-like domain (PrLD) that is required for phase separation of proteins. In this study, we found that PSPC1 could undergo phase separation. Moreover, the loss of PrLD domain of PSPC1 could greatly weaken its phase separation ability. The immunofluorescence assays showed that PSPC1 is present in mouse oocytes in the germinal vesicle (GV) stage. Knockdown of PSPC1 significantly impeded the maturation of mouse oocytes in vitro. CHK1 has been reported to play important roles in the GV stage of mouse oocytes. Co-IP experiment revealed that PSPC1 could interact with phosphatase serine/threonine-protein phosphatase 5 (PPP5C), which regulates CHK1 phosphorylation. Western blot analysis revealed that PSPC1 could regulate the phosphorylation of CHK1 through PPP5C;however, PSPC1 without PrLD domain was inactive, suggesting that the lack of phase separation ability led to the abnormal function of PSPC1 in regulating CHK1 phosphorylation. Thus, we conclude that PSPC1 may undergo phase separation to regulate the phosphorylation level of CHK1 via PPP5C and participate in mouse oocyte maturation.
