1Clinical and laboratory survey of 65 Chinese patients with Leigh syndrome显示文摘Background Leigh syndrome is an inherited neurodegenerative disease that emerges in infancy and childhood and presents with a clinically heterogeneous variety of neuromuscular and non-neuromuscular disorders. It can result from the inheritance of mutations in either nuclear or mitochondrial DNA. In the current study, we performed a retrospective study in 65 patients in order to investigate the clinical and genetic characteristics of Leigh syndrome in Chinese patients. Methods Sixty-five unrelated cases (35 men and 30 women) who were hospitalized in the past 12 years were reviewed. Diagnosis was based on both the clinical presentation and the characteristic neuropathologic findings of bilateral symmetric necrotizing lesions in the basal ganglia and brain stem as detected using cranial computed tomography (CT) scan or magnetic resonance imaging (MRI). The differential diagnosis of organic acidurias and fatty acid ?-oxidation defects were performed. Specific point mutations and deletions in mitochondrial DNA (T8993G, T8993C, T9176C, A8344G, A3243G) were screened by PCR-restriction analysis and Southern blot. The SURF1 gene was sequenced. Skeletal muscle biopsies were performed in 17 (26.2%) of the patients. The diagnosis was confirmed by autopsy in 6 (9.2%) patients. Results The patients had various forms of metabolic encephalomyopathy. Fifty-nine (90.8%) of the patients had the typical neuroradiological features of Leigh syndrome, including symmetrical necrotizing lesions scattered within the basal ganglia, thalamus and brain stem. Twenty (30.8%) patients were confirmed by genetic, biochemical analysis and autopsy. Specific point mutations in mitochondrial DNA were found in 5 cases (7.7%). Of these, the A8344G mutation was detected in 2 patients. The T8993G, T8993C, and A3243G point mutations were identified in 3 other patients, respectively. SURF1 mutations associated with cytochrome c oxidase deficiency were identified in 8 (12.3%) families by DNA sequencing. A G604C mutation was identified in 6 (9.2%) patients. The genotypes of 52 patients remained unknown. Conclusions Leigh syndrome presents as a diverse array of clinical features and can result from specific mutations in nuclear or mitochondrial DNA. In this study, SURF1 mutations associated with cytochrome c oxidase deficiency were identified in 8 (12.3%) out of 65 patients with Leigh syndrome. It indicates that SURF1 mutations might be a common cause of Leigh syndrome in China. 2Decreased osteogenesis of adult mesenchymal stem cel s by reactive oxygen species under cyclic stretch: a possible mechanism of age related osteoporosis显示文摘Age related defect of the osteogenic differentiation of mesenchymal stem cells(MSCs) plays a key role in osteoporosis. Mechanical loading is one of the most important physical stimuli for osteoblast differentiation.Here, we compared the osteogenic potential of MSCs from young and adult rats under three rounds of 2 h of cyclic stretch of 2.5% elongation at 1 Hz on 3 consecutive days. Cyclic stretch induced a significant osteogenic differentiation of MSCs from young rats, while a compromised osteogenesis in MSCs from the adult rats.Accordingly, there were much more reactive oxygen species(ROS) production in adult MSCs under cyclic stretch compared to young MSCs. Moreover, ROS scavenger N-acetylcysteine rescued the osteogenic differentiation of adult MSCs under cyclic stretch. Gene expression analysis revealed that superoxide dismutase 1(SOD1) was significantly downregulated in those MSCs from adult rats. 6Factors predicting survival in patients with proximal gastric carcinoma involving the esophagus显示文摘AIM:To investigate the clinicopathologic features which predict surgical overall survival in patients with proximal gastric carcinoma involving the esophagus (PGCE).METHODS:Electronic pathology database established in the Department of Pathology of the Nanjing Drum Tower Hospital was searched for consecutive resection cases of proximal gastric carcinoma over the period from May 2004 through July 2009.Each retrieved pathology report was reviewed and the cases with tumors crossing the gastroesophageal junction line were selected as PGCE.Each tumor was re-staged,following the guidelines on esophageal adenocarcinoma,according to the 7th edition of the American Joint Commission on Cancer Staging Manual.All histology slides were studied along with the pathology report for a retrospective analysis of 13 clinicopathologic features,i.e.,age,gender,Helicobacter pylori (H.pylori ) infection,surgical modality,Siewert type,tumor Bormann's type,size,differentiation,histology type,surgical margin,lymphovascular and perineural invasion,and pathologic stage in relation to survival after surgical resection.Prognostic factors for overall survival were assessed with uniand multi-variate analyses.RESULTS:Patients' mean age was 65 years (range:47-90 years).The male:female ratio was 3.3.The 1-,3and 5-year overall survival rates were 87%,61% and 32%,respectively.By univariate analysis,age,male gender,H.pylori ,tumor Bormann's type,size,histology type,surgical modality,positive surgical margin,lymphovascular invasion,and pT stage were not predictive for overall survival;in contrast,perineural invasion (P = 0.003),poor differentiation (P = 0.0003),> 15 total lymph nodes retrieved (P = 0.008),positive lymph nodes (P = 0.001),and distant metastasis (P = 0.005) predicted poor post-operative overall survival.Celiac axis nodal metastasis was associated with significantly worse overall survival (P = 0.007).By multivariate analysis,≥ 16 positive nodes (P = 0.018),lymph node ratio > 0.2 (P = 0.003),and overall pathologic stage (P = 0.002) were independent predictors for poor overall survival after resection.CONCLUSION:Patients with PGCE showed worse overall survival in elderly,high nodal burden and advanced pathologic stage.This cancer may be more accurately staged as gastric,than esophageal,cancer.Yi-Fen Zhang Jiong Shi Hui-Ping Yu An-Ning Feng Xiang-Shan Fan Gregory Y Lauwers Hiroshi Mashimo Jason S Gold Gang Chen Qin Huang 2012World Journal of Gastroenterology2012,18,27:12

8Clinical risk score for invasive fungal diseases in patients with hematological malignancies undergoing chemotherapy:China Assessment of Antifungal Therapy in Hematological Diseases (CAESAR) study显示文摘Invasive fungal disease (IFD) is a major infectious complication in patients with hematological malignancies.In this study,we examined 4889 courses of chemotherapy in patients with hematological diseases to establish a training dataset (n=3500) by simple random sampling to develop a weighted risk score for proven or probable IFD through multivariate regression,which included the following variables: male patients,induction chemotherapy for newly diagnosed or relapsed disease,neutropenia,neutropenia longer than 10 days,hypoalbuminemia,central-venous catheter,and history of IFD.The patients were classified into three groups,which had low (0-10,~1.2%),intermediate (11-15,6.4%),and high risk (> 15,17.5%) of IFD.In the validation set (n=1389),the IFD incidences of the groups were ~1.4%,5.0%,and 21.4%.In addition,we demonstrated that antifungal prophylaxis offered no benefits in low-risk patients,whereas benefits were documented in intermediate (2.1% vs.6.6%,P=0.007) and high-risk patients (8.4% vs.23.3%,P=0.007).To make the risk score applicable for clinical settings,a pre-chemo risk score that deleted all unpredictable factors before chemotherapy was established,and it confirmed that anti-fungal prophylaxis was beneficial in patients with intermediate and high risk of IFD.In conclusion,an objective,weighted risk score for IFD was developed,and it may be useful in guiding antifungal prophylaxis.Ling Wang Ying Wang Jiong Hu Yuqian Sun He Huang Jing Chen Jianyong Li Jun Ma Juan Li Yingmin Liang Jianmin Wang Yan Li Kang Yu Jianda Hu Jie Jin Chun Wang Depei Wu Yang Xiao Xiaojun Huang 2019Frontiers of Medicine2019,13,3:10
9Optical tuning of exciton and trion emissions in monolayer phosphorene显示文摘Monolayer phosphorene provides a unique two-dimensional(2D)platform to investigate the fundamental dynamics of excitons and trions(charged excitons)in reduced dimensions.However,owing to its high instability,unambiguous identification of monolayer phosphorene has been elusive.Consequently,many important fundamental properties,such as exciton dynamics,remain underexplored.We report a rapid,noninvasive,and highly accurate approach based on optical interferometry to determine the layer number of phosphorene,and confirm the results with reliable photoluminescence measurements.Furthermore,we successfully probed the dynamics of excitons and trions in monolayer phosphorene by controlling the photo-carrier injection in a relatively low excitation power range.Based on our measured optical gap and the previously measured electronic energy gap,we determined the exciton binding energy to be~0.3 eV for the monolayer phosphorene on SiO_(2)/Si substrate,which agrees well with theoretical predictions.A huge trion binding energy of~100 meV was first observed in monolayer phosphorene,which is around five times higher than that in transition metal dichalcogenide(TMD)monolayer semiconductor,such as MoS_(2).The carrier lifetime of exciton emission in monolayer phosphorene was measured to be,220 ps,which is comparable to those in other 2D TMD semiconductors.Our results open new avenues for exploring fundamental phenomena and novel optoelectronic applications using monolayer phosphorene.Jiong Yang Renjing Xu Jiajie Pei Ye Win Myint Fan Wang Zhu Wang Shuang Zhang Zongfu Yu Yuerui Lu 2015Light(Science & Applications)2015,4,1:8
10Induction of cytotoxic T lymphocyte respones in vivo after immunotherapy with dendritic cells in patients with nasopharyngeal carcinoma显示文摘The aim of the present study was to determine the efficacy of immunotherapy with dendritic cells to elicit EBV-specific CTL-immunity in advanced cases of EBV-positive patients with nasopharyngeal carcinoma (NPC) and to determine the safety and toxicity of this preparation. Nine cases of histologically confirmed patients with NPC undergoing treatment with radiological therapy were enrolled in this study. Dendritic cells, generated in vitro from blood monocytes of patients were cultured and matured with cytokines and then infected with recombinant adenovirus vaccine containing EBV-latent membrane protein-2 (Ad-LMP2). On 9 days' cultivation of cells, the matured DCs were harvested, irradiated with Co and then injected intradermally to patients with NPC. The injections were performed 3 times totally. After immunization, the CTL responses were assayed by means of cytotoxicity and epitope-specific IFN-γproduction. The results of this trial showed that all patients could tolerate this kind of treatment without any side effect, during which marked increase of LMP2-specific CTL-responses could be demonstrated in 5 patients of this group. And the level of IgA/VCA antibody decreased in 8 of 9 patients, thus accounting for a better prognosis for these patients. All patients will be followed up for another one year. At least, the present work shows that intradermal vaccination with autologous DCs infected with recombinant Ad-LMP2 adenovirus is a safe procedure in NPC patients, in which this procedure can enhance the LMP2-specific CTL responses in patients. 13Estimation of PM_(10) in the trafc-related atmosphere for three road types in Beijing and Guangzhou, China显示文摘The levels of roadside PM10in Beijing, China, were investigated in 2011 and 2012 on a seasonal basis to estimate the population exposure to particulates for three road types. The measurements of PM10 were also conducted in the southern Chinese megacity of Guangzhou for comparison purposes. The results showed that roadside PM10in Beijing correlated strongly with the PM10background in the urban atmosphere. The levels of PM10in street canyons were markedly higher than those along the open roads and in crossroad areas because of limited ventilation. An elevation of PM10was observed in April, which was possibly due to the sand storms that frequently occur in the spring. Based on these observations, roadside PM10in Beijing could have multiple origins and was to some extent dispersiongoverned. In Guangzhou, the roadside PM10did not closely relate to the background values. The PM10 pollution was greatly afected by local trafc conditions. The simulation of PM10 for diferent road types was completed during the study period using the Motor Vehicle Emissions Factor Model(MOBILE6.2) as an emission model and the California Line Source Dispersion Model(CALINE4) and Operational Street Pollution Model(OSPM) as dispersion models. 14Effect of quenching-partitioning treatment on the microstructure, mechanical and abrasive properties of high carbon steel显示文摘The present work employed the X-ray diffraction, scanning electron microscopy, electron backscattered diffraction, and electron probe microanalysis techniques to identify the microstructural evolution and mechanical and abrasive behavior of high carbon steel during quenching-partitioning treatment with an aim to enhance the toughness and wear resistance of high carbon steel.Results showed that, with the increase in partitioning temperature from 250 to 400℃, the amount of retained austenite(RA) decreased resulting from the carbide precipitation effect after longer partitioning times.Moreover, the stability of RA generally increased because of the enhanced degree of carbon enrichment in RA.Given the factors affecting the toughness of high carbon steel, the stability of RA associated with size, carbon content, and morphology plays a significant role in determining the toughness of high carbon steel.The analysis of the wear resistance of samples with different mechanical properties shows that hardness is the primary factor affecting the wear resistance of high carbon steel, and the toughness is the secondary one.Jian-ping Lai Jia-xin Yu Jiong Wang 2021International Journal of Minerals,Metallurgy and Materials2021,28,4:2
15A multicenter retrospective cohort study of ketogenic diet therapy in 481 children with infantile spasms显示文摘Background:Ketogenic diet(KD)therapy is one of the main treatments for drug-resistant epilepsy.However,the KD therapy has been applied in only a small number of infantile spasm cases.In this large multicenter study,we investigated the efficacy of KD therapy in the treatment of infantile spasms.Methods:In this retrospective,multicenter cohort study,clinical data from main epilepsy centers were analyzed.Patients were classified into different groups according to age,type of drug and whether glucocorticoid was used before initiation of KD.Results:From October 2014 to March 2020,481 patients(308 males and 173 females)with infantile spasms were treated with the KD therapy.The age of the patients ranged from 2 months to 20 years,with a mean age of 1 year and 10 months.The number of anti-seizure medications(ASMs)used before KD initiation ranged 0-6,with a median of 3.In different time from initiation(1,3,6,and 12 months),the rates of seizure freedom after KD were 6.9,11.6;16.0 and 16.8%,respectively(x^(2)=27.1772,P<0.0001).There was a significant difference in the rate of seizure freedom between 3 months and 1 month(x^(2)=6.5498,P=0.0105)groups,and 6 months and 3 months(x^(2)=3.8478,P=0.0498)groups,but not between 12 months and 6 months(x^(2)=0.1212,P=0.7278)groups.The rates of effectiveness were 44.7;62.8,49.1 and 32.0%(x^(2)=93.2674,P<0.0001),respectively.The retention rates were 94.0,82.5,55.7 and 33.1%(x^(2)=483.7551,P<0.0001),correspondingly.The rate of effectiveness and the retention rate of KD were significantly different among the 1,3,6 and 12 months.KD treatment was the first choice in 25 patients(5.2%),55 patients(11.4%)started KD after the failure of the first ASM,158 patients(32.8%)started KD after the failure of the second ASM,157 patients(32.6%)started KD after the failure of the third drug,and 86 patients(17.9%)started KD after the failure of the fourth and more.The KD effect was not related to the number of ASMs used before KD startup(P>0.05).Two hundred and eighteen patients(45.3%)failed to respond to corticotropin or glucocorticoid before initiation.There was no significant difference in the effectiveness rate at different time points between the group of KD therapy after glucocorticoid failure and the group after non-hormone failure (x^(2)=0.8613,P=0.8348).The rate of adverse events of KD in 1,3,6,and 12 months after KD initiation were 22.3,21.7,16.8 and 6.9%,respectively.The adverse events mainly occurred during the first 3 months of KD,and the main adverse events were gastrointestinal disturbance and constipation.Conclusions:The efficacy of the KD treatment for infantile spasms was not affected by age,medication,and glucocorticoid use before initiation.KD is one of the effective treatments for infantile spasms.Yuanzhen Ye Dan Sun Hua Li Jianmin Zhong Rong Luo Baomin Li Dengna Zhu Dan Li Shaoping Huang Yuwu Jiang Nong Xiao Yucai Chen Yuqin Zhang Mei Yu Xiaoyun Shen Li Gao Guo Zheng Congmin Zhao Baoqiang Yuan Jianxiang Liao Jiong Qin CAAE KD group 2022Acta Epileptologica2022,5,1:2
16Intestinal dysbiosis exacerbates the pathogenesis of psoriasis-like phenotype through changes in fatty acid metabolism显示文摘The intestinal microbiota has been associated with host immunity as well as psoriasis;however,the mechanism of intestinal microbiota regulating psoriasis needs to be demonstrated systematically.Here,we sought to examine its role and mechanism of action in the pathogenesis of psoriasis.We found that the severity of psoriasis-like skin phenotype was accompanied by changes in the composition of the intestinal microbiota.We performed co-housing and fecal microbial transplantation(FMT)experiments using the K14-VEGF transgenic mouse model of psoriasis and demonstrated that the transfer of intestinal microbiota from mice with severe psoriasis-like skin phenotype exacerbated psoriasiform skin inflammation in mice with mild symptoms,including increasing the infiltration and differentiation of Th17,and increased the abundance of Prevotella,while decreasing that of Parabacteroides distasonis,in the colon.These alterations affected fatty acid metabolism,increasing the abundance of oleic and stearic acids.Meanwhile,gentamicin treatment significantly reduced the abundance of Prevotella and alleviated the psoriasis-like symptoms in both K14-VEGF mice and imiquimod(IMQ)-induced psoriasis-like mice.Indeed,administration of oleic and stearic acids exacerbated psoriasis-like symptoms and increased Th17 and monocyte-derived dendritic cell infiltration in the skin lesion areas in vivo,as well as increased the secretion of IL-23 by stimulating DCs in vitro.At last,we found that,treatment of PDE-4 inhibitor alleviated psoriasis-like phenotype of K14-VEGF mice accompanied by the recovery of intestinal microbiota,including the decrease of Prevotella and increase of Parabacteroides distasonis.Overall,our findings reveal that the intestinal microbiota modulates host metabolism and psoriasis-like skin inflammation in mice,suggesting a new target for the clinical diagnosis and treatment of psoriasis.Qixiang Zhao Jiadong Yu Hong Zhou Xiaoyan Wang Chen Zhang Jing Hu Yawen Hu Huaping Zheng Fanlian Zeng Chengcheng Yue Linna Gu Zhen Wang Fulei Zhao Pei Zhou Haozhou Zhang Nongyu Huang Wenling Wu Yifan Zhou Jiong Li 2023Signal Transduction and Targeted Therapy2023,8,2:2
17Preparation of Nano-porous Materials(Ⅰ) by Polymerization of Amphiphile Self-assemblies显示文摘The polymerization of amphiphilic self assemblies is a promising method to synthesize nano structured materials with novel properties. These materials have many attractive features for their application in biomedical area and materials science, such as catalysis, separation, surface modification, and therapeutics areas. A general review on the polymerization of lipids and surfactant self assemblies to amphiphilic self assemblies is given in this paper with 49 references. The polymerization and the subsequently resulted structure of lipids in different morphologies are summarized. The polymerization of polymerizable surfactants(surfmers) in emulsion and liquid crystalline phases are also discussed. 19Prognostic Nomogram for Patients with Hepatitis E Virus-related Acute Liver Failure:A Multicenter Study in China显示文摘Background and Aims:Timely and effective assessment scoring systems for predicting the mortality of patients with hepatitis E virus-related acute liver failure(HEV-ALF)are urgently needed.The present study aimed to establish an effective nomogram for predicting the mortality of HEV-ALF patients.Methods:The nomogram was based on a cross-sectional set of 404 HEV-ALF patients who were identified and enrolled from a cohort of 650 patients with liver failure.To compare the performance with that of the model for end-stage liver disease(MELD)scoring and CLIF-Consortiumacute-on-chronic liver failure score(CLIF-C-ACLFs)models,we assessed the predictive accuracy of the nomogram using the concordance index(C-index),and its discriminative ability using time-dependent receiver operating characteristics(td-ROC)analysis,respectively.Results:Multivariate logistic regression analysis of the development set carried out to predict mortality revealed that γ-glutamyl transpeptidase,albumin,total bilirubin,urea nitrogen,creatinine,international normalized ratio,and neutrophil-to-lymphocyte ratio were independent factors,all of which were incorporated into the new nomogram to predict the mortality of HEV-ALF patients.The area under the curve of this nomogram for mortality prediction was 0.671(95%confidence interval:0.602-0.740),which was higher than that of the MELD and CLIF-C-ACLFs models.Moreover,the td-ROC and decision curves analysis showed that both discriminative ability and threshold probabilities of the nomogram were superior to those of the MELD and CLIF-C-ACLFs models.A similar trend was observed in the validation set.Conclusions:The novel nomogram is an accurate and efficient mortality prediction method for HEV-ALF patients.Jian Wu Cuifen Shi Xinyu Sheng Yanping Xu Jinrong Zhang Xinguo Zhao Jiong Yu Xinhui Shi Gongqi Li Hongcui Cao Lanjuan Li 2021Journal of Clinical and Translational Hepatology2021,9,6:2
20Dynamic changes of key metabolites during liver fibrosis in rats显示文摘BACKGROUND Fibrosis is the single most important predictor of significant morbidity and mortality in patients with chronic liver disease.Established non-invasive tests for monitoring fibrosis are lacking,and new biomarkers of liver fibrosis and function are needed.AIM To depict the process of liver fibrosis and look for novel biomarkers for diagnosis and monitoring fibrosis progression.METHODS CCl4 was used to establish the rat liver fibrosis model.Liver fibrosis process was measured by liver chemical tests,liver histopathology,and Masson’s trichrome staining.The expression levels of two fibrotic markers includingα-smooth muscle actin and transforming growth factorβ1 were assessed using immunohistochemistry and real-time polymerase chain reaction.Dynamic changes in metabolic profiles and biomarker concentrations in rat serum during liver fibrosis progression were investigated using ultra-performance liquid chromatography coupled to quadrupole time-of-flight mass spectrometry.The discriminatory capability of potential biomarkers was evaluated by receiver operating characteristic(ROC)curve analysis.RESULTS To investigate the dynamic changes of metabolites during the process of liver fibrosis,sera from control and fibrosis model rats based on pathological results were analyzed at five different time points.We investigated the association of liver fibrosis with 21 metabolites including hydroxyethyl glycine,L-threonine,indoleacrylic acid,β-muricholic acid(β-MCA),cervonoyl ethanolamide(CEA),phosphatidylcholines,and lysophosphatidylcholines.Two metabolites,CEA andβ-MCA,differed significantly in the fibrosis model rats compared to controls(P<0.05)and showed prognostic value for fibrosis.ROC curve analyses performed to calculate the area under the curve(AUC)revealed that CEA andβ-MCA differed significantly in the fibrosis group compared to controls with AUC values exceeding 0.8,and can clearly differentiate early stage from late stage fibrosis or cirrhosis.CONCLUSION This study identified two novel biomarkers of fibrosis,CEA andβ-MCA,which were effective for diagnosing fibrosis in an animal model.Jiong Yu Jian-Qin He De-Ying Chen Qiao-Ling Pan Jin-Feng Yang Hong-Cui Cao Lan-Juan Li 2019World Journal of Gastroenterology2019,25,8:2
